OBJECTIVE The goal of this article was to assess the effects and consequences of strict measures in radiation oncology centers in northeastern Turkey during the coronavirus disease-2019 pandemic and to ensure that the reorganization of programs occurred without interruption to radiotherapy (RT). METHODS We retrospectively reviewed the records of 1063 patients (median age 63 years; range 3-93 years) who were treated in RI centers from March 2020 to March 2021. The study cohort was divided into two groups: Group A consisted of patients who received RT in the first 6 months and Group B comprised patients who received RT in the second 6 months. RESULTS The age distribution was similar in both groups. The analysis of the intent of treatment showed that 585 (55%) patients were treated with a radical intent in Group A, whereas only 478 (45%) were treated with a radical intent in Group B. Lung, breast, and genitourinary cancers were the most common cancer types in both periods. There was a significant decline in the number of female patients in Group B. Test results of 31 (2.9%) patients, 8 (25.8%) women and 23 (74.2%) men, who had nasopharyngeal swabs, were positive. CONCLUSION This study shows that oncology patients in our region are more worried about disease progression than the pandemic and insist on going to RT, especially during the first 6-month period when the pandemic was thought to be temporary. As a result of careful precautions, treatments in the clinics continued without contamination.

Amaç: Farklı malignitelerin tedavisinde kullanılan bir antitümör ilaç olan bleomisin ile ilişkili pulmoner toksisite, önemli bir sorundur ve bu toksisiteyi önleyecek molekülleri bulmak için çalışmalar devam etmektedir. Bu deneysel çalışmada, ratlarda, bleomisinle indüklenen akciğer fibrozisinde telmisartan ve tadalafilin etkinliğini araştırmayı amaçladık.Yöntemler: 32 erkek rat dört gruba ayrıldı: Kontrol grubu, Bleomisin grubu, Bleomisin+Tadalafil grubu, Bleomisin+Telmisartan grubu. Akciğer fibrozisi intratrakeal bleomisin uygulaması ile oluşturulurken aynı prosedür kontrol grubunda bleomisin yerine salin kullanılarak uygulandı. Tadalafil ve telmisartan gruplara göre 14 gün boyunca orogastrik kateter vasıtasıyla verildi. Doku MDA düzeyleri kalorimetrik yöntemler kullanılarak ölçüldü. Masson trikrom boyama uygulanarak histolojik değerlendirme yapıldı.Bulgular: Bleomisin+tadalafil ve bleomisin+telmisartan gruplarının fibrozis skorları bleomisin grubuna göre daha düşüktü. Gruplar arasında MDA düzeyleri açısından farklılık görülmedi.Sonuç: Tadalafil ve telmisartanın bleomisine bağlı artan fibrozis skorlarını düşürdüğü ve pulmoner toksisitenin birden fazla süreçle ilişkili olduğu ve önlenebilir olduğu kanısındayız., Aim: Pulmonary toxicity related to bleomycin, an antitumor drug used in the treatment of several malignancies, is a challenge, and studies to find out molecules to prevent it are ongoing. In this study, we aimed to investigate the effectiveness of telmisartan and tadalafil in an experimental rat model of bleomycin-induced lung fibrosis.Methods: A total of 32 male rats were divided into four groups: Control group, Bleomycin group, Bleomycin-plus-Tadalafil group and Bleomycin-plus-Telmisartan group. Lung fibrosis was achieved by intratracheal administration of bleomycin, and the same procedure was performed to the control group, but saline was substituted for bleomycin. Tadalafil and telmisartan were administered with an orogastric catheter for 14 days. Tissue malondialdehyde levels (MDA) were determined using colorimetric methods. Masson’s trichrome staining was used in the histological examination of the tissue samples.Results: The fibrosis scores of bleomycin-plus-tadalafil and bleomycin-plus-telmisartan groups were lower than that of the bleomycin group (P=0.007 and P=0.007, respectively). MDA levels did not differ among study groups.Conclusion: Tadalafil and telmisartan were found to decrease fibrosis scores, which were increased with bleomycin, concluding that pulmonary toxicity was related to multiple processes and preventable.

Oldukça geniş kıyı alanlarına sahip Ülkemizde, Anayasamızla korunma altına alınmış olan kıyı alanlarında uygulama yapılabilmesi için KKÇ tespiti bir zorunluluktur. Her ne kadar KKÇ tespitleri Ülkemizde büyük ölçüde tamamlanmış olsa da yargı mercilerinde bu tespitlere oldukça fazla itiraz yapılmaktadır. Bu çalışmayla KKÇ tespiti yapan Valilik Kıyı Kenar Çizgisi Tespit Komisyonları ile Mahkeme süreçlerinde görev alan bilirkişi heyetlerinin KKÇ tespit işlemleri sırasında kullanacakları bir konumsal veri altyapısı tasarımı yapılmıştır. Bunun için öncelikle gereksinim analizi yapılarak hangi verilere ihtiyaç duyulduğu, bu verilerin hangi kurumlardan temin edileceği ve sistemde yer alacak sorguların neler olduğu vb. belirlenmiştir. Uygulama, söz konusu verilerin kamu kurum ve kuruluşlarının web servislerinden çekilmesi üzerine tasarlanmıştır. Bu sayede KVA' nın temelini oluşturan veri paylaşımı gerçekleştirilerek konu ile ilgili hızlı, ekonomik ve doğru çözümler üretebilmek söz konusu olacaktır. Ancak Ülkemiz koşulları dikkate alındığında uygulamada bazı eksikliklerin bulunduğu görülmektedir. Bu eksikliklerin giderilebilmesi ve KKÇ iş ve işlemleri ile ilgili bir KVA oluşturabilmek için öncelikle sistemde veri sağlaması beklenen kamu kurum ve kuruluşlarının verileri bilgisayar ortamına aktararak veri paylaşımı yapması gerekmektedir. İhtiyaç duyulan veriler tamamlandığında bilgisayar ortamında bu veriler değerlendirilerek daha doğru, hassas ve objektif bir tespit işlemi gerçekleştirilecek ve tasarımı yapılan sistemdeki sorguların gerçekleştirilmesi mümkün olacaktır. In our country with quite large coastal areas, coastal edge line determination is a necessity in order to be able to application in coastal areas which are protected by our Constitution. Although the coastal edge line fixation has been completed in our country in large quantities, the judicial authorities have made a lot of objections to these findings. In this study, a spatial data infrastructure design was made to be used by the governorship coastal edge line determination commissions and expert committees working in court processes during coastal edge line determination procedures. For this purpose, first of all, it was identified what data is needed by performing requirement analysis, from which institutions this data will be supplied, and what queries will take place in the system, etc. The application is designed to take the data from the web services of public institutions and organizations. In this way, it will be possible to create fast, economical and accurate solutions for the subject by realizing the data sharing which is the basis of the spatial data infrastructures. However, given the conditions of our country, there are some deficiencies in practice. In order to overcome these deficiencies and to create a spatial data infrastructures related to coastal edge line work and transactions, public institutions and organizations that are expected to provide data in the system must transfer the data to the computer environment and share the data. When the required data is completed, the data will be evaluated in the computer environment and a more accurate, accurate and objective detection process will be performed and the queries in the design system will be realized. 81

Systemic chemotherapy, genotype-based targeted therapies and immunotherapy are widely used in the treatment of advanced non small-cell lung cancer (NSCLC). Among the targeted therapies, the agents targeting the epidermal growth factor receptor (EGFR), the echinoderm microtubule-associated protein-like 4 anaplastic lymphoma kinase (EML4-ALK) rearrangement and C-ros oncogene 1 (ROS-1) have currently become standard treatment for cases presenting such molecular anomalies. Of them, cases with ALK-rearrangement displayed dramatic results with a first generation ALK-inhibitor crizotinib; however, most of the patients develop a resistance in a few years. Especially the central nervous system relapses pose the most common clinical problem. Next generation ALK-inhibitors are promising with a high level of effectiveness in this resistance, in which various molecular mechanisms take part. Also, it gains increasing importance to re-perform a biopsy in the progression stage and reveal the mechanisms causing the secondary resistance in those patients.
