Abstract

International audience; Introduction: We aimed to describe patients with coexisting infective endocarditis (IE) and bacterial meningitis (BM).Methods: We merged two large prospective cohorts, an IE cohort and a BM cohort, with only cases of definite IE and community-acquired meningitis. We compared patients who had IE and BM concurrently to patients with IE only and BM only.Results: Among the 1030 included patients, we identified 42 patients with IE-BM (4.1%). Baseline characteristics of patients with IE-BM were mostly similar to those of patients with IE, but meningitis was the predominant presentation at admission (39/42, 92.3%). Causative pathogens were predominantly Streptococcus pneumoniae (18/42, 42.9%) and Staphylococcus aureus (14/42, 33.3%). All pneumococcal IE were associated with BM (18/18). BM due to oral and group D streptococci, Streptococcus agalactiae, and S. aureus were frequently associated with IE (14/30, 46.7%). Three-month mortality was 28.6% in patients with IE-BM, 20.5% in patients with IE, and 16.6% in patients with BM.Conclusions: Patients with pneumococcal IE or altered mental status during IE must be investigated for BM. Patients with S. aureus, oral and group D streptococcal or enterococcal BM, or unfavorable outcome in pneumococcal meningitis would benefit from an echocardiography. Patients with the dual infection have the worst prognosis. Their identification is mandatory to initiate appropriate treatment.

17. Reading Comprehension Quiz Generation using Generative Pre-trained Transformers

Abstract

Recent advances in AI have resulted in large pre-trained language models with superior performance on text generation tasks, prompting the question of whether we can use them to generate educationally useful text completions. This holds the potential to generate relevant quizzes for any educational text, greatly complementing current formative and summative tests from education professionals. We explore pre-trained language models for quiz generation on reading comprehension texts and propose EduQuiz, an end-to-end quiz generator based on a GPT-3 model fine-tuned on text-quiz pairs, able to generate a complete multiple-choice question, with the correct and distractor answers. We observed that the majority of generated quizzes is reasonable, and that generation of high-quality distractors is more challenging than question and answer generation. More generally, while it may be too early to replace manually generated tests for summative feedback and grading with automatic quiz generation, EduQuiz already has potential value for formative feedback and to increase engagement during the learning phase by enhancing textbooks with assessments.

Abstract

Additive manufacturing, commonly referred to as 3D printing, is currently one of the most dominant developments in the manufacturing sector. Consumer demands for more customized goods and services, in conjunction with the emergence of advanced manufacturing technologies, are influencing changes in the scope and distribution of manufacturing. In this work, the function of digital fabrication, one of these advanced manufacturing process technologies, is examined. The Digital Manufacturing and even other advanced manufacturing technologies have a significant footprint in all future manufacturing endeavours because of the robustness and mass customization that they provide. The Fused Deposition Modelling (FDM) is a sustainable method used extensively for the development of prototypes for toys. The present study investigates the potential of FDM process as a method for prototype development. Additionally the advantages of FDM process from the viewpoint of sustainable manufacturing integrating innovativeness and do-it-yourself approach at home driven by consumer demand for products such as toys will also be investigated. 3D printing technology such as FDM process, where objects such as toy prototypes are constructed by adding materials layer by layer has been in the forefront of various Digital Manufacturing techniques as it reduces material wastage thereby optimizing resource utilization.

Abstract

Three major inhibitors of the NF-κB/Rel family of transcription factors, IκBα, IκBβ, and IκBε, have been described. To examine the in vivo role of the most recently discovered member of the IκB family, IκBε, we generated a null allele of the murine IκBε gene by replacement of all coding sequences with nlslacZ. Unlike IκBα nullizygous mice, mice lacking IκBε are viable, fertile, and indistinguishable from wild-type animals in appearance and histology. Analysis of β-galactosidase expression pattern revealed that IκBε is mainly expressed in T cells in the thymus, spleen, and lymph nodes. Flow cytometric analysis of immune cell populations from the bone marrow, thymus, spleen, and lymph nodes did not show any specific differences between the wild-type and the mutant mice, with the exception of a reproducible 50% reduction of the CD44−CD25+ T cell subspecies. The IκBε-null mice present constitutive up-regulation of IgM and IgG1 Ig isotypes together with a further increased synthesis of these two isotypes after immunization against T cell-dependent or independent Ags. The failure of observable augmentation of constitutive nuclear NF-κB/Rel-binding activity is probably due to compensatory mechanisms involving IκBα and IκBβ, which are up-regulated in several organs. RNase-mapping analysis indicated that IL-1α, IL-1β, IL-1Ra, and IL-6 mRNA levels are constitutively elevated in thioglycolate-elicited IκBε-null macrophages in contrast to GM-CSF, G-CSF, and IFN-γ, which remain undetectable.

Recently, conditional Renyi’s divergence of order α and Kerridge’s inaccuracy measures are studied by Navarro et al. (2014). In the present article, a generalized dynamic conditional Kerridge’s inaccuracy measure is introduced, which can be represented as the sum of conditional Renyi’s divergence and Renyi’s entropy. Some useful bounds are obtained using the concept of likelihood ratio order. The results are extended to weighted distributions. Sufficient conditions are obtained for the monotonicity of the proposed measure. Characterizations for bivariate exponential conditional distribution are presented based on the proposed measure. We now report 41 patients with complete IL-12Rbeta1 deficiency from 17 countries. The only opportunistic infections observed, in 34 patients, were of childhood onset and caused by weakly virulent Salmonella or Mycobacteria (Bacille Calmette-Guerin -BCG- and environmental Mycobacteria). Three patients had clinical tuberculosis, one of whom also had salmonellosis. Unlike salmonellosis, mycobacterial infections did not recur. BCG inoculation and BCG disease were both effective against subsequent environmental mycobacteriosis, but not against salmonellosis. Excluding the probands, seven of the 12 affected siblings have remained free of case-definition opportunistic infection. Finally, only five deaths occurred in childhood, and the remaining 36 patients are alive and well. Thus, a diagnosis of IL-12Rbeta1 deficiency should be considered in children with opportunistic mycobacteriosis or salmonellosis; healthy siblings of probands and selected cases of tuberculosis should also be investigated. The overall prognosis is good due to broad resistance to infection and the low penetrance and favorable outcome of infections. Unexpectedly, human IL-12 is redundant in protective immunity against most microorganisms other than Mycobacteria and Salmonella. Moreover, IL-12 is redundant for primary immunity to Mycobacteria and Salmonella in many individuals and for secondary immunity to Mycobacteria but not to Salmonella in most.

Abstract

Three major inhibitors of the NF-kappaB/Rel family of transcription factors, IkappaBalpha, IkappaBbeta, and IkappaBepsilon, have been described. To examine the in vivo role of the most recently discovered member of the IkappaB family, IkappaBepsilon, we generated a null allele of the murine IkappaBepsilon gene by replacement of all coding sequences with nlslacZ. Unlike IkappaBalpha nullizygous mice, mice lacking IkappaBepsilon are viable, fertile, and indistinguishable from wild-type animals in appearance and histology. Analysis of beta-galactosidase expression pattern revealed that IkappaBepsilon is mainly expressed in T cells in the thymus, spleen, and lymph nodes. Flow cytometric analysis of immune cell populations from the bone marrow, thymus, spleen, and lymph nodes did not show any specific differences between the wild-type and the mutant mice, with the exception of a reproducible 50% reduction of the CD44-CD25+ T cell subspecies. The IkappaBepsilon-null mice present constitutive up-regulation of IgM and IgG1 Ig isotypes together with a further increased synthesis of these two isotypes after immunization against T cell-dependent or independent Ags. The failure of observable augmentation of constitutive nuclear NF-kappaB/Rel-binding activity is probably due to compensatory mechanisms involving IkappaBalpha and IkappaBbeta, which are up-regulated in several organs. RNase-mapping analysis indicated that IL-1alpha, IL-1beta, IL-1Ra, and IL-6 mRNA levels are constitutively elevated in thioglycolate-elicited IkappaBepsilon-null macrophages in contrast to GM-CSF, G-CSF, and IFN-gamma, which remain undetectable.

46. Actinomyces in chronic granulomatous disease: an emerging and unanticipated pathogen

Abstract

BACKGROUND: Chronic granulomatous disease (CGD) is a rare inherited disease of the phagocyte NADPH oxidase system that causes defective production of toxic oxygen metabolites, impaired bacterial and fungal killing, and recurrent life-threatening infections, mostly by catalase-producing organisms. We report for the first time, to our knowledge, chronic infections with Actinomyces species in 10 patients with CGD. Actinomycosis is a chronic granulomatous condition that commonly manifests as cervicofacial, pulmonary, or abdominal disease, caused by slowly progressive infection with oral and gastrointestinal commensal Actinomyces species. Treatment of actinomycosis is usually simple in immunocompetent individuals, requiring long-term, high-dose intravenous penicillin, but is more complicated in those with CGD because of delayed diagnosis and an increased risk of chronic invasive or debilitating disease. METHODS: Actinomyces was identified by culture, staining, 16S ribosomal DNA polymerase chain reaction, and/or a complement fixation test in 10 patients with CGD. RESULTS: All 10 patients presented with a history of fever and elevated inflammatory signs without evident focus. Diagnosis was delayed and clinical course severe and protracted despite high-dose intravenous antibiotic therapy and/or surgery. These results suggest an unrecognized and unanticipated susceptibility to weakly pathogenic Actinomyces species in patients with CGD because these are catalase-negative organisms previously thought to be nonpathogenic in CGD. CONCLUSIONS: Actinomycosis should be vigorously sought and promptly treated in patients with CGD presenting with uncommon and prolonged clinical signs of infection. Actinomycosis is a catalase-negative infection important to consider in CGD.
