AIMS: To assess tolerability and optimal time point for initiation of sacubitril/valsartan in patients stabilised after acute heart failure (AHF). METHODS AND RESULTS: TRANSITION was a randomised, multicentre, open-label study comparing two treatment initiation modalities of sacubitril/valsartan. Patients aged ≥ 18 years, hospitalised for AHF were stratified according to pre-admission use of renin-angiotensin-aldosterone system inhibitors and randomised (n = 1002) after stabilisation to initiate sacubitril/valsartan either ≥ 12-h pre-discharge or between Days 1-14 post-discharge. Starting dose (as per label) was 24/26 mg or 49/51 mg bid with up- or down-titration based on tolerability. The primary endpoint was the proportion of patients attaining 97/103 mg bid target dose after 10 weeks. Median time of first dose of sacubitril/valsartan from the day of discharge was Day -1 and Day +1 in the pre-discharge group and the post-discharge group, respectively. Comparable proportions of patients in the pre- and post-discharge initiation groups met the primary endpoint [45.4% vs. 50.7%; risk ratio (RR) 0.90; 95% confidence interval (CI) 0.79-1.02]. The proportion of patients who achieved and maintained for ≥ 2 weeks leading to Week 10, either 49/51 or 97/103 mg bid was 62.1% vs. 68.5% (RR 0.91; 95% CI 0.83-0.99); or any dose was 86.0% vs. 89.6% (RR 0.96; 95% CI 0.92-1.01). Discontinuation due to adverse events occurred in 7.3% vs. 4.9% of patients (RR 1.49; 95% CI 0.90-2.46). CONCLUSIONS: Initiation of sacubitril/valsartan in a wide range of heart failure with reduced ejection fraction patients stabilised after an AHF event, either in hospital or shortly after discharge, is feasible with about half of the patients achieving target dose within 10 weeks. Bu çalışmanın amacı; 2002-2014 yılları arasında Türkiye-Mısır İlişkilerinin incelenmesidir.2000'lerin başlarından itibaren Mısır ve Türkiye ekonomik ilişkiler bakımından altın çağını yaşamaktadır. Türkiye Devleti; Mısır devletinin ticaret ortakları arasında dördüncü sırada yerini almaktadır. Türkiye ile Mısır arasındaki ticaret hacim oranı 5 milyar Amerikan doları düzeyindedir. Ticaret hacminde düşme görülse bile, Mısır bölge çerçevesinde ülkemizin en fazla ihracat gerçekleştirdiği ülkedir. 2016-2017 döneminde Ekonomi Bakanlığımız Mısır'ı önceliği olan yatırım devletlerinden birisi olarak saptamıştır. Türkiye Mısır Ortak Toplantısı 15. kez bir araya geldiğinde daha fazla istihdam elde edilmesi için iki ülke arasında gerçekleştirilen yatırımların oluşturulması gerektiğine işaret ederken, son yıllarda Türkiye'nin diğer ülkelerle birlikte maruz bırakıldığı bir takım koruyucu politikaların mısır hükümeti tarafından esneklik yapılmasının gerekli olduğu vurgusu yapılmıştır.1980'nin ardından gelen süreçte Arap dünyasının Türkiye'ye gösterdiği ilgi bir kat daha artış göstermiştir. 1979 yılında gerçekleşen İran Devrimi'nin bir tehdit olduğu düşüncesi ve İsrail ile Mısır arasında yapılan barış neticesinde bölgesel politikalardan çekilme gibi riskli unsurlar Türkiye'ye olan ilginin artmasında etkili olmuştur. WOS: 000401951700010, PubMed ID: 28559660, Background: In this study, we investigated the relationship between the mean platelet volume (MPV) with mortality and heart failure (HF)-related hospitalization in stable chronic HF outpatients with reduced ejection fraction (HFrEF) and with sinus rhythm (SR). Methods: This retrospective cohort study included 197 consecutive stable chronic HFrEF outpatients with SR, who were admitted to our cardiology outpatient clinics for examination between January 2014 and January 2015. According to the receiver-operating characteristic curve analysis, the optimal cut-off value of MPV to predict HF-related hospitalization was > 9.1 fL. Patients were classified into two categories according to threshold MPV levels, as group I with MPV 9.1 fL. Results: The mean age of patients was 65 13 years. The mean follow-up duration was 10 3 months, and 44 patients (22%) succumbed to cardiovascular (CV) death. The rate of CV mortality was similar between the two groups (21% vs. 24%, p = 0.649). However, the rate of patients who experienced HF-related hospitalization was lower in group I compared with group II (41% vs. 87%, p < 0.001, respectively). Univariate analysis demonstrated associations of many clinical factors in addition to increased MPV > 9.1 fL with HF-related hospitalization; however, In the multivariate Cox proportional-hazards model, only increased MPV > 9.1 fL (HR: 2.895, 95% CI: 1.774-4.724, p < 0.001), systolic pulmonary artery pressure level (HR: 1.018, 95% CI: 1.001-1.036, p = 0.048) and pre-admission beta blocker use (HR: 0.517, 95% CI: 0.305-0.877, p = 0.014) remained associated with a risk of HF-related hospitalization. Conclusions: The mean platelet volume might be a useful parameter for risk stratification with regard to HF-related hospitalization in HFrEF outpatients with SR.

Bu araştırmanın amacı, matematik derslerinde gerçek hayat bağlantılarının kullanımı konusunda ortaokul matematik öğretmenlerinin görüşlerini araştırmaktır. Bu çalışma nitel bir çalışmadır. Bu çalışmanın örneklemini; 2013-2014 Eğitim-Öğretim yılında, Uşak ilinde görev yapan 56 matematik öğretmeni oluşturmaktadır. Araştırmanın verileri `Matematik Eğitiminde Anket Çalışması` kullanılarak toplanmıştır. Verilerin analizinde betimsel analiz yöntemi kullanılmıştır. Veriler kategorik olarak ayrılmış ve öğretmenlerin görüşleri çoktan aza doğru belirlenmiştir. Araştırma sonucunda, öğretmenler bilgilerin kalıcılığını arttırdığı, öğrenci başarısına katkı sağladığını, matematiğe karşı olumlu tutumu ve problem çözme becerisini geliştirdiği, derslerde uygun örnek bulmada güçlük yaşadıklarını ve GME' nin zaman alıcılığı olduğunu ifade etmişlerdir. Ayrıca araştırmaya katılan öğretmenler gerçek hayat bağlantılarının derslerde soyut genellemelerden önce kullanılması gerektiğini söylemişlerdir. Son olarak da öğretmenlerin çoğunun gerçek hayat bağlantılarını kullanma konusunda yeteri kadar donanımlı olmadığı görülmüştür. Bu çalışmanın sonuçlarının öğretmenlere, akademik çalışma yapan bilim insanlarına, üniversitelere, müfredat geliştiricilere ve Milli Eğitim Bakanlığı'na yol göstermesi hedeflenmektedir. The purpose of this research is to explore the views of secondary school mathematics teachers' on the use of real-life connections in mathematics classes. This work is a qualitative study. In the academic year of 2013-2014, 56 mathematics teachers working in the Uşak province form the sample of this study. The data of the study were collected using the `Mathematics Education Questionnaire`. In the analysis of the data, a descriptive analysis method was used. The data were categorized and the views of the teachers were determined accordingly. As a result of the research, teachers express that using real life connections in mathematics lessons increased the permanence of information, contributed to the students' success, helped them develop a positive attitude towards mathematics and problem solving, have diffuculty on finding suitable examples and RME is time consuming. Also, teachers state that during the courses, real-life connections should be used before abstract generalization. Finally, it is seen that most teachers are not as well-equipped to use real life connections. It is aimed that the results of this study will lead teachers, academicians, universities, curriculum developers and the Ministry of National Education. 147

WOS: 000458034100002, Background Cancer antigen-125 (Ca-125) is traditionally recognised as a tumour marker and its role in cardiovascular diseases has been studied only in recent years. Whether Ca-125 is elevated in patients with atrial fibrillation (AF) and its levels predict the risk of AF remains controversial. Therefore, we conducted a systematic review and meta-analysis of the association between Ca-125 levels and AF. Methods PubMed and EMBASE databases were searched until 1 June 2017 for studies that evaluated the association between Ca-125 and AF. Inclusion criteria included studies that compare Ca-125 in patients with and without AF, or those reporting HRs/ORs for risk of AF stratified by Ca-125 levels. Results A total of 39 entries were retrieved from the databases, of which 10 studies were included in the final meta-analysis. Ca-125 was significantly higher in patients with AF compared with those in sinus rhythm (mean difference=16 U/mL, 95% CI 2 to 30 U/mL, P, Croucher Foundation of Hong Kong [81570298]; National Natural Science Foundation of China, GT and SHW are supported by the Croucher Foundation of Hong Kong. CONTEXT AND OBJECTIVE: The location of embolism is associated with clinical findings and disease severity in cases of acute pulmonary embolism. The level of gamma-glutamyl transferase increases under oxidative stress-related conditions. In this study, we investigated whether gamma-glutamyl transferase levels could predict the location of pulmonary embolism. DESIGN AND SETTING: Hospital-based cross-sectional study at Cumhuriyet University, Sivas, Turkey. METHODS : 120 patients who were diagnosed with acute pulmonary embolism through computed tomography-assisted pulmonary angiography were evaluated. They were divided into two main groups (proximally and distally located), and subsequently into subgroups according to thrombus localization as follows: first group (thrombus in main pulmonary artery; n = 9); second group (thrombus in main pulmonary artery branches; n = 71); third group (thrombus in pulmonary artery segmental branches; n = 34); and fourth group (thrombus in pulmonary artery subsegmental branches; n = 8). RESULTS : Gamma-glutamyl transferase levels on admission, heart rate, oxygen saturation, right ventricular dilatation/hypokinesia, pulmonary artery systolic pressure and cardiopulmonary resuscitation requirement showed prognostic significance in univariate analysis. The multivariate logistic regression model showed that gamma-glutamyl transferase level on admission (odds ratio, OR = 1.044; 95% confidence interval, CI: 1.011-1.079; P = 0.009) and pulmonary artery systolic pressure (OR = 1.063; 95% CI: 1.005-1.124; P = 0.033) remained independently associated with proximally localized thrombus in pulmonary artery. CONCLUSIONS : The findings revealed a significant association between increased existing embolism load in the pulmonary artery and increased serum gamma-glutamyl transferase levels. RESUMO CONTEXTO E OBJETIVO : A localização da embolia está associada com os resultados clínicos e a gravidade da doença do embolismo pulmonar agudo (EPA). O nível de gama-glutamil transferase (GGT) aumenta em condições relacionadas com estresse oxidativo. Investigou-se se os níveis de GGT podem prever a localização do EPA. TIPO DE ESTUDO E LOCAL : Estudo observacional transversal na Universidade Cumhuriyet, Sivas, Turquia. MÉTODOS : Avaliamos 120 pacientes diagnosticados com EPA após a realização de angiografia pulmonar assistida por tomografia computadorizada. Eles foram divididos em dois grupos principais (localização proximal e distal) e depois em subgrupos de acordo com a localização do trombo da seguinte forma: primeiro grupo (trombo na artéria pulmonar [AP] principal, n = 9); segundo (trombo no ramo da AP principal; n = 71); terceiro grupo (trombo na segmentar da AP; n = 34); quarto grupo (trombo na subsegmentar da AP; n = 8). RESULTADOS : Na análise univariada, os níveis de GGT tiveram significado prognóstico em relação à admissão, pulsação arterial, saturação de oxigênio, dilatação do ventrículo direito/hipocinesia, pressão sistólica da artéria pulmonar (PSAP) e necessidade de ressuscitação cardiopulmonar. O modelo de regressão logística multivariada demonstrou que o nível de GGT na admissão (razão de possibilidades, OR: 1,044; 95% intervalo de confiança, CI: 1,011-1,079; P = 0,009) e PSAP (OR: 1,063, 95% CI: 1,005-1,124; P = 0,033) permaneceram independentemente associados com trombo localizado proximalmente na AP. CONCLUSÕES : Os resultados demonstraram associação significativa entre aumento da carga existente de embolia da AP e aumento dos níveis séricos da GGT.

Objective: In heart failure (HF) patients, functional capacity has been demonstrated to be a marker of poor prognosis, independent of left ventricular ejection fraction (EF). Lymphocyte count is currently recognized in certain risk stratification scores for chronic HF, and severe HF is associated with lymphocytopenia. However, no data exists on the association between lymphocyte count and functional capacity in patients with stable HF. This study aimed to assess the relationship between lymphocyte count and New York Heart Association (NYHA) functional capacity in systolic HF outpatients.

A considerable percentage of electrical injuries occur as a result of work activities. Electrical injury can lead to various cardiovascular disorders: acute myocardial necrosis, myocardial ischemia, heart failure, arrhythmias, hemorrhagic pericarditis, acute hypertension with peripheral vasospasm, and anomalous, nonspecific ECG alterations. Ventricular fibrillation is the most common arrhythmia resulting from electrical injury and is the leading cause of death in electrical (especially low voltage alternating current) injury cases. Asystole, premature ventricular contractions, ventricular tachycardia, conduction disorders (various degrees of heart blocks, bundle-brunch blocks), supraventricular tachycardia, and atrial fibrillation are the other arrhythmic complications of electrical injury. Complete atrioventricular block has rarely been reported and permanent pacemaker was required for the treatment in some of these cases. Herein, we present a case of reversible complete atrioventricular block due to low voltage electrical injury in a young electrical technician.
