Amaç: Bu çalışmanın amacı; akciğer kanserli hastalarda pozitron emisyon tomografisi (18F-FDG PET/BT) görüntülerinden elde edilen, tümör metabolizmasını yansıtan semikantitatif parametrelerin tümörün klinik evresi ve histopatolojik özellikleri ile ilişkisinin araştırılmasıdır. Gereç ve Yöntem: Çalışmaya 27’si adenokarsinom, 35’i epidermoid karsinom tanısı alan toplam 62 KHDAK’li hasta (E/K: 38/24; yaş ortalaması 61,29±9,5) dahil edildi. Hastaların tamamına kemo/radyoterapi öncesi F-18 FDG PET/ BT çekimi yapıldı ve SUVmax, SUVmean, metabolik tümör volümü (MTV) ve total lezyon glikolizis (TLG) değerlerinden oluşan metabolik semikantitatif parametreler hesaplandı. F-18 FDG PET/ BT metabolik parametreleri ile tümörün klinik evresi ve histopatolojik özellikleri (tümör alttipi, ki-67 indeksi) arasında bir ilişki olup olmadığı istatistiksel olarak değerlendirildi. Bulgular: Tümör evrelerine göre tüm gruplar arası SUVmax, SUVmean, MTV ve TLG değerlerinde anlamlı farklılık gözlenmedi. Tümör alt tiplerine göre yapılan analizde adenokarsinomlu hastaların SUVmax, , SUVmean, MTV ve TLG değerlerinin epidermoid karsinomlu hastalara göre istatistiksel olarak anlamlı oranda daha düşük olduğu gözlendi (p=0,003, p=0,002, p=0,002, p=0,002). Ayrıca 40 hastadan oluşan subgrupta yaptığımız histopatolojik incelemede ki-67 proliferasyon indeksi ile TLG arasında orta derecede korelasyon gözlendi (p=0.4, p=0.03). Sonuç: Küçük hücreli dışı akciğer kanserli hastalarda, F-18 FDG PET/BT metabolik parametreleri ile tümörün histopatolojik özellikleri korelasyon göstermekte olup hastalığın klinik sonuçlarını noninvaziv olarak öngörmede bu parametreler yardımcı rol oynayabilir.

ÖZETAmaç: Bu çalışmanın amacı, erken evre uterin serviks kanserli hastalarda pelvik ve paraaortik lenf nodumetastazlarının saptanmasında 18F-florodeoksiglukoz Pozitron Emisyon tomografisi / Bilgisayarlı Tomografi(PET/BT) 'nin tanısal değerini belirlemekti.Gereç ve Yöntem: Radikal histerektomi ve sistemik pelvik ve paraaortik lenf nodu diseksiyonu öncesi PET/BT görüntülemesi yapılan erken evre uterin serviks kanserli 18 hasta çalışmaya dahil edildi. Lenf nodlarınınhistopatolojik değerlendirmesi tanısal standart olarak kabul edildi ve PET/BT bulguları histopatolojik bulgularlakarşılaştırıldı.Bulgular: 18 hastadan toplam 807 (577 pelvik - 230 paraaortik) lenf nodu örneklendi ve histopatolojikincelemede 5 (% 28) hastada 10 (%1,2) nod (8 pelvik - 2 paraaortik) pozitif bulundu. Genel nod bazlı PET/BT'nin duyarlılık, özgüllük, pozitif prediktif değer (PPV), negatif prediktif değer (NPV) ve doğruluğu sırasıyla %10 (1/10), % 100 (797/797), %100 (1/1) % 99 (797/806) ve % 99 (798/807) idi.Genel hasta bazlı PET/BT'nin duyarlılık, özgüllük, PPV, NPV ve doğruluğu sırasıyla %20 (1/5), %100 (13/13), %100 (1/1), % 76 (13/17), %78 (14/18) idi.Sonuç: Erken evre uterin serviks kanserli hastaların yönetiminde, mikrometastatik lenf nodu olan hastalarınPET / BT'de yanlış negatif olarak rapor edilebileceği akılda tutulmalıdır.Anahtar Kelimeler: Erken evre uterin serviks kanseri; Preoperatif FDG PET/BT; Pelvik ve paraaortik lenf nodumetastazı.ABSTRACTObjective: The aim of this study was to determine the diagnostic value of 18F-fluorodeoxyglucose PositronEmission Tomography / Computed Tomography (PET / CT) in detection of pelvic and paraaortic lymph nodemetastases in patients with early stage uterine cervix cancer.Materials and Methods: 18 patients with early stage uterine cervix cancer who underwent PET / CT imagingbefore radical hysterectomy and systemic pelvic and paraaortic lymph node dissection were included in thestudy. Histopathological evaluation of lymph nodes was accepted as the diagnostic standard and PET/CTfindings were compared with histopathological findings.Results: Of the total 807 (577 pelvic ve 230 para- aortic) lymph nodes sampled from 18 patients, and 10(1,2%) nodes (8 pelvik ve 2 paraaortic) in 5 (28%) patients were positive for metastasis at histopathologicexamination. The overall node-based sensitivity, specificity, positive predictive value (PPV), negative predictivevalue (NPV), and accuracy of PET/CT were 10% (1of 10), 100% (797 of 797), 100% (1 of 1), 99% (797 of 806),and 99% (798 of 807), respectively. Meanwhile, the overall patient-based sensitivity, specificity, PPV, NPV,and accuracy of PET/CT were 20% (1 of 5), 100% (13 of 13), 100% (1 of 1), 76% (13 of 17), and 78% (14 of 18),respectively.Conclusions: In the management of patients with early stage uterine cervix cancer, it should be kept in mindthat patients with micro-metastatic lymph nodes can be reported as false negative in PET / CT.Key words: Early stage uterine cervix cancer; Preoperative PET / CT; Pelvic and paraaortic lymph nodemetastasis

Amaç: Fluorine-18 2-floro-2-deoksi-D-glukoz (FDG) pozitron emisyon tomografisi (PET)/ BT görüntüleme tekniği, lokalizasyonun ve fonksiyonel görüntülemenin avantajlarını birleştirdiğinden, endometrial kanserli hastalarda lenf nodu (LN) metastazlarının tespiti için tercih edilen bir yöntem haline gelmiştir. Bu çalışmada 18F-FDG PET/BT görüntüleme sonuçlarının, endometrial kanserin lenf nodu metastazının belirlenmesindeki etkinliğini araştırmayı amaçladık. Yöntemler: Bu çalışma 2009-2015 yılları arasında yapılan retrospektif kohort bir çalışmadır. Çalışma grubu kliniğimize başvuran ve endometriyal kanser tanısı alan tüm kadınlardan (n=135) oluşmaktaydı. Çalışmaya, ameliyat öncesi 18F-FDG PET/BT çekilen ve LN diseksiyonu yapılan hastaları dahil edildi. Bu yöntemin lend nodu metastazını belirlemede etkinliği değerlendirildi. Değerlendirmeler ve karşılaştırmalar altın standard patolojik tanı yöntemleriyle yapıldı. Bulgular: Lenf nodu metastazlarının tanımlanmasında 18F-FDG PET/BT yönteminin %76 duyarlılık, %80 özgüllük, %46.34 pozitif prediktif değeri ve %93.62 negatif prediktif değeri gösterdiği bulundu. Genel tanısal doğruluk ise %79,26 idi.Sonuç: Analizlerimiz, 18F-FDG PET/BT'nin, endometriyal kanser hastalarında lenf nodu metastazı açısından yüksek negatif prediktif değere sahip olduğunu ve bu hastalarda lenf nodu metastazı araştırmada uygulanabilir bir görüntüleme yöntemi olduğunu göstermiştir., Aim: Fluorine-18 2-fluoro-2-deoxy-D-glucose (FDG) positron emission tomography (PET)/CT imaging technique combines the advantages of localization and functional imaging, and thus has become a preferred method for detection of metastases to the lymph nodes (LN) of patients with endometrial cancer. The aim of this study was to evaluate the efficacy of 18F-FDG PET/CT imaging results in the detection of metastasis of endometrial cancer to lymph nodes.Methods: This is a retrospective cohort study conducted between 2009 and 2015. The study group consisted of females who were admitted to our clinic and diagnosed with endometrial cancer (n=135). We included patients who underwent LN dissection with pre-operative 18F-FDG PET / CT imaging, and evaluated the capability of this method in terms of its effectiveness in determining metastases to the lymph nodes. Assessments and comparisons were performed with gold-standard pathological methods. Results: The 18F-FDG PET/CT method for identification of lymph node metastases had a sensitivity of 76%, a specificity of 80%, a positive predictive value of 46.34% and a negative predictive value of 93.62%. The overall diagnostic accuracy was 79.26%.Conclusion: Our analysis suggests that 18F-FDG PET/CT has a high negative predictive value with regards to lymph node metastasis in endometrial cancer and may be used effectively in evaluating lymph node metastasis in this cohort.

Primary malignant melanoma occurs most often in the skin and much less frequently in the choroid layer of the eyes, in the leptomeninges, oral cavity, nasal mucosa, pharynx, esophagus, bronchus, under the nail and vaginal or anorectal mucosa. Primary melanoma of the gastrointestinal tract has been confirmed for lesions occurring in the esophagus, stomach, small bowel, and anorectum through several published reports, as these are the areas where melanocytes normally exist. The occurrence of primary malignant melanoma in the colon is relatively rare, because melanocytes are embryologically absent in the large bowel. Herein we report a patient whose colonic malignant melanoma was diagnosed and disseminated metastatic lesions were revealed with F-18-FDG PET/CT scan. There were multiple nodular lesions showing increased F-18-FDG uptake in both lungs. There was a soft tissue lesion with slightly increased F-18-FDG uptake, which extended to the intraluminal region of the thoracic esophagus. Increased metabolic activity was detected in the asymmetric stomach wall thickening site and in a soft tissue lesion located on the gall bladder wall that was filling the lumen. Multiple hypodense/hyper-metabolic lesions were identified in the liver. Multiple hyper-metabolic polypoid soft tissue lesions were visualized in almost the entire colonic segments. Multiple hyper-metabolic peritoneal implants were noted in all abdominal quadrants. Increased F-18-FDG uptake was detected at the right surrenal gland soft tissue lesion. There was a hyper-metabolic soft tissue lesion on the posterior wall of the rectum. Hyper-metabolic lytic lesions were seen at the thoracal and lumbar vertebrae, left scapula, left iliac bone, sacrum and left femur. There was no evidence of F-18-FDG avid skin lesions in both attenuation corrected and non-corrected images.

In this study, our goal was to perform tumor staging and subtype determination automatically using different texture analysis approaches for a very common cancer type, i.e., non-small cell lung carcinoma (NSCLC). Especially, we introduced a texture analysis approach, called Law’s texture filter, to be used in this context for the first time. The 18F-FDG PET images of 42 patients with NSCLC were evaluated. The number of patients for each tumor stage, i.e., I-II, III or IV, was 14. The patients had ~45% adenocarcinoma (ADC) and ~55% squamous cell carcinoma (SqCCs). MATLAB technical computing language was employed in the extraction of 51 features by using first order statistics (FOS), gray-level co-occurrence matrix (GLCM), gray-level run-length matrix (GLRLM), and Laws’ texture filters. The feature selection method employed was the sequential forward selection (SFS). Selected textural features were used in the automatic classification by k-nearest neighbors (k-NN) and support vector machines (SVM). In the automatic classification of tumor stage, the accuracy was approximately 59.5% with k-NN classifier (k=3) and 69% with SVM (with one versus one paradigm), using 5 features. In the automatic classification of tumor subtype, the accuracy was around 92.7% with SVM one vs. one. Texture analysis of FDG-PET images might be used, in addition to metabolic parameters as an objective tool to assess tumor histopathological characteristics and in automatic classification of tumor stage and subtype., {"references":["K. Kemik yapınındeğerlendirilmesinde tüm iskelet sistemi grafileri çekilmektedir. Kafa ve iç organhasarlarının tespiti için bilgisayarlı tomografi BT , manyetik rezonans görüntüleme MRG ve ultrasonografi US uygulanmaktadır. Teknolojide hızlı gelişmelerdendolayı nükleer tıp tanı yöntemleri de istismar ve ihmalin tanısında kullanılmayabaşlanmıştır. İstismar ve ihmalin saptanması, doğru tanı ve tedavinin yanında etik,ahlaki ve hukuki yükümlülükler taşımaktadır. Nükleer tıp yöntemlerinin çocuk istismar ve ihmalinde kullanılmasının adli olaylarda nesnel kanıtların ortaya konularak,davaların çözülmesi ve adil yargılamanın gerçekleşmesine önemli katkılar sağlayabileceği düşünülmektedir. Ayrıca konuyla ilgili uzmanlara objektif sonuçlar vermesi açısından yararlı olacağını ümit ediyorum, Child abuse and neglect are detrimental to a child’s health, physical and psychosocial development and result from inappropriate behavior or inattentionon the part of those responsible for the child’s care and protection. Childabuse/neglect is a serious public problem encompassing medical, ethical andlegal aspects. Imaging methods play an important role in investigating and documenting child abuse and neglect. Radiological imaging methods have particular priority. X-rays of the whole skeletal system are obtained for evaluationof the bone structure. Computerized tomography CT , magnetic resonance imaging MRI , and ultrasonography US are used for the detection of cranial andinternal organ damage. Due to the rapid developments in technology, diagnosticmethods using nuclear medicine have also been initiated for use in identifyingabuse and neglect. Detection of abuse and neglect carries ethical, judicial andmoral liabilities as well as a responsibility for accurate diagnosis and treatment.Implementation of diagnostic methods of nuclear medicine in determining childabuse and neglect might contribute significantly in resolving court cases by providing objective evidence in medicolegal cases for realization of fair trials andin facilitating substantial conclusions. It is hoped that nuclear medicine methodswill be helpful in terms of providing objective evidence for the experts

Functional dyspepsia is a syndrome characterized by symptoms such as epigastric pain, discomfort, early satiety, postprandial fullness and nausea that originate from the gastroduodenal region. The aim of our study is to evaluate the gastric emptying and regional distribution of food in patients with functional dyspepsia. Materials and Method; Seventeen patients with functional dyspepsia (F/M 10/7) and 12 healthy volunteers (F/M 6/6) were enrolled into the study. Following an 8-hour fast, one minute scintigraphic images were obtained in the anterior and posterior projection immediately, 30, 60, 90 and 120 min after a 500mCi (18MBq) Tc99m sulphur colloid-labeled egg, 2 slices of bread and 150 mL of orange juice ingestion. The per cent of radioactivity was calculated by dividing the gastric counts at each time period for total, proximal and distal stomach by the total counts at time zero. The gastric half emptying time (T1/2) was calculated from time activity curves. Results; Total gastric retention at initial and 30 min was not significantly different between patients with FD and controls, whereas a significant difference was observed in the total gastric emptying at 60, 90 and 120 minutes between groups. Although proximal gastric retention was higher in patients with FD compared with the normal subjects at initial, subsequently the ratio of the radioactivity in proximal stomach of FD patients decreased to a level significantly lower than that of controls after 60 min. Distal gastric retention was not significantly different at initial and 60 and 90 min between groups or even lower in FD patients compared to the normal subjects, whereas the values at 30 and 120 min. were even lower in the patients than that of controls. In conclusion, impaired gastric accommodation or early proximal emptying may be associated with symptoms in patients with functional dyspepsia. But early antral filling does not seem to be related to the symptoms., Fonksiyonel dispepsi; orijini üst gastrointestinal sistem olan, abdominal ağrı, huzursuzluk, erken doyma, postprandial doygunluk ve bulantı ile seyreden bir sendromdur. Çalışmamızın amacı fonksiyonel dispepsili hastalarda, mide boşalma zamanı ve bölgesel dağılımı değerlendirmektir. Materyal ve Metod: Çalışmamıza fonksiyonel dispepsi tanısı alan 17 hasta (E/K: 7/10) ve sağlıklı 12 (E/K:6/6) gönüllü dahil edildi. Hastalar 8 saat açlıktan sonra, 500 mCi (18 MBq) Tc99m sülfür kolloid ile işaretli yumurta 2 dilim ekmek ve 150 ml portakal suyu ile yedikten hemen sonra, her biri 1 dakika olacak şekilde 0, 30, 60, 90 ve 120 inci dakikalarda anterior ve posterior projeksiyonlardan sayımlar alındı. Her bir zamanın Total, Proksimal ve Distal gastrik sayımları sıfır anındaki sayıma oranlanarak yüzdeleri hesaplandı. Zaman aktivite eğrisi kullanılarak mide boşalma zamanı (T1/2) hesap edildi. Bulgular: Total GR değerleri, 0. ve 30. dakikalardaki normal bireylerden anlamlı farklılık göstermemesine karşın, 60. 90. ve 120. dakikalarda normal bireylerden anlamlı azalma göstermiştir. Proksimal GR, 0 dakikada hasta grubunda kontrol grubuna oranla anlamlı yüksek bulunmasına rağmen diğer zamanlarda düşük bulundu. 30. dakikadaki azalma anlamlı olmamasına rağmen diğer zamanlardaki azalma istatistiksel olarak anlamlı idi. Hasta grubunda kontrole göre başlangıç ve 60-90. dakikalarda distal mide retansiyonu anlamlı olarak farklı değilken, 30. ve 120. dakikalardaki değerler anlamlı düşük bulundu. Sonuç: Bozulmuş gastrik uyum veya erken proksimal boşalma dispepsi ile ilişkili fakat erken antral dolma fonksiyonel dispepsi ile ilişkisizdir.

ÖZET Çalışmamızın amacı, 99mTc MIBI GATED SPET ile Non-Q miyokard infarktüsü tanısı konulan hastalarda erken dönem prognozu belirlemekti. Çalışmaya Non Q Mİ geçiren, yaş ortalaması 60.7±2 olan, 18'i erkek, 11 'i kadın 29 hasta alındı. Hastalara infarktüs sonrası ilk 48 saat içerisinde, ortalama 30.7± 2.3 saatte 99mTc MIBI GATED SPET çalışması yapılarak mryokardiyal perfüzyon ve sol ventrikül fonksiyonunu yansıtan parametreler kaydedildi. Prognoz belirlemede kullanılabilecek klinik ve laboratuvar bulgulan kaydedildi Geç dönem Gated SPET çalışması, infarktüs sonrası 28-37. günler arasında, ortalama 30.65 ± 0.49 günde yapıldı. Beş aylık takipte 5 hasta KABG, 6 hasta da PTKA ile revaskülarize edildi, 22 hastaya ise medikal tedavi uygulandı, erken ve geç dönem istirahatteki sol ventrikül fonksiyonlarında istatiksel farklılık izlenmedi. Erken dönem EF'u ile geç dönem dobutaminli stres EF'da fark gözlendi (p=0.044). Perfüzyon defektini gösteren ES'u ortalaması erken dönemde 147.517±15.710 pixel iken, geç dönemde 131.793±14.885 pixel olarak hesaplanması istatiksel fark göstermedi ancak tedavi etkinliği olarak değerlendirildi (p= 0.094). Sonuç olarak, AMI sonrası risk ve prognoz belirlemede önemli yere sahip miyokardial perfüzyon ve ventrikül fonksiyonlarım eş zamanlı değerlendirebilen 99mTc MIBI Gated SPET'in, noninvaziv, kolay uygulanabilir bir yöntem olarak kullanılabileceği kanaatine varıldı. 40 52

Aim: Chronic hepatitis B virus (HBV) infection can induce nephropathy and major HBV antigens (HBcAg, HBeAg, HBsAg) and immune complexes responsible for its pathogenesis. The purpose of our study was to assess the renal function of patients with HBV infection by means of technetium diethylenetriamine pentaacetic acid (Tc99m DTPA) scintigraphy, in order to detect any early renal dysfunction, which might be related to nephropathy associated with HBV infection. Methods: Nineteen patients (10 male and 9 female with a mean age of 37.11±11.34 years, age range 25−62 years) with the diagnosis of HBV infections, and 16 healthy controls (8 male and 8 female with a mean age of 33.50±11.56 years, age range 24-55 years) were enrolled into the study. All subjects had no history of renal disease and treatment of HBV infection including interferon-alfa, pegylated interferon-alfa, lamivudine and adefovir. Blood urea nitrogen (BUN), urea and creatinine were analyzed. The renal perfusion, concentration and excretion were evaluated by Tc99m DTPA scintigraphy. The time to peak (TTP), peak activity (counts/second) (PA), clearance half time (T1/2) and percent contribution of each kidney to total function (differential function) were calculated. The glomerular filtration rate (GFR) was determined simultaneously by 3 methods; gamma camera uptake method (Gates, GFR), predicted creatinine clearance method (Cockcroft-Gault, CG-GFR), and Modification of Diet in Renal Disease (MDRD). The variables were compared between groups. Results: The mean values of patients’ parameters were as follows; right TTP:5.73±3.14 min, left TTP:5.52±2.09 min, right PA:884.78±214.85 cts/sec, left PA:889.52±252.68 cts/sec and right T1/2:17.84±6.89 min, left T1/2:14.51±8.59 min. Differential functions were 50.27±3.39% on the right side and 49.72±3.39% on the left side. In study group, Gates GFR value was 125.63±25.84 ml/min, CG-GFR:124.05±24.32 ml/min, MDRDGFR:113.15±24.70 ml/min/1.73 m2, BUN:14.57±5.05 mg/dl, urea: 27.37±10.24 mg/dl, and creatinine:0.73±0.18 mg/dl. The mean values of control group were as follows; right TTP:4.05±1.87 min, left TTP:2.93±1.14 min, right PA:819.25±260.48 cts/sec, left PA:796.75±176.94 cts/sec, and right T1/2:12.25±3.19 min, left T1/2:12.25±5.49 min. In control group, Gates GFR value was 109.75±39.03 ml/min, CG-GFR:104.62±39.67 ml/min, MDRD-GFR:89.68±34.15 ml/min/1.73 m2, BUN:11.37±4.47 mg/dl, urea:24.75±5.54 mg/dl, and creatinine:0.78±0.17 mg/dl. Aim: Fibrous dysplasia is a slow growing skeletal developmental anomaly in which particularly long tubular bones, ribs, skull and facial bones are involved. We aimed to present two cases diagnosed as fibrous dysplasia in our neurosurgey clinic. Case report: The first case was 11 year-old, male and had a craniofacial involvement with a rarely seen humerus involvement polyostotic fibrous dysplasia. The second case was 14 year-old, male and had only craniofacial involvement monostotic fibrous dysplasia. While medical treatment is used to reduce the symptomps of the disease, the surgical treatment is used to correct deformities in fibrous dysplasia. Cranioectomy was carried out for cosmetic purposes in case I, and due to prediagnosis of “osteom” in the Case II. The two cases were diagnosed as fibrous dysplasia as a result of clinical, histopathological and radiological examinations. Follow-ups for both cases were done. Result: Craniofacial involvement with humerus involvement is rarely seen in polyostotic fibrous dysplasia. We evaluated these two cases under the light of literature, Amaç: Fibröz displazi, benign, yavaş ilerleyen, özellikle kraniyofasiyal, uzun kemikler ve kosta tutulumu gösteren iskelet lezyonudur. Kliniğimizde fibröz displazi tanısı koyduğumuz 2 olguyu sunmayı amaçladık. Olgu Sunumu: İlk olgu 11 yaşında kraniyofasiyal tutulumla birlikte humerus tutulumu olan polyostotik fibröz displazi. İkinci olgu 14 yaşında sadece kraniyal tutulumu olan monostotik fibröz displazi. Fibröz displazide medikal tedavi semptomları azaltmak için kullanılır iken cerrahi tedavi deformiteleri düzeltmek amacıyla uygulanır. İlk olguya kozmetik amaçlı kraniektomi, ikinci olguya ise osteom ön tanısıyla kraniektomi yapıldı. Klinik, histopatolojik ve radyolojik inceleme sonucu olgulara fibröz displazi tanısı konuldu. Her iki olguda klinik olarak takibe alındı. Sonuç: Kraniyofasiyal tutulum ile birlikte humerus tutulumu olan polyostotik fibröz displazi oldukça nadir görülmektedir
