Abstract

Although the exact cause of asthma remains unknown, recent studies have suggested that specific white blood cells known as T lymphocytes may be involved. When these cells become activated they make proteins called cytokines (interleukin, interferon) that cause tissue damage and inflammation. It has been suggested that T lymphocytes may contribute to the inflammation that occurs in the airways of patients with asthma. Studies have shown that patients with severe asthma have abnormally high levels of activated T lymphocytes in their blood. Many patients with asthma are treated with glucocorticoids, such as prednisolone and dexamethasone. Glucocorticoids reduce tissue inflammation and inhibit the actions of T lymphocytes. However, glucocorticoids are not effective in treating all patients with asthma; such patients are said to have glucocorticoid-resistant asthma. The T lymphocytes from these patients are not inhibited by treatment with glucocorticoids. To further investigate these findings, 37 patients with severe asthma were treated with prednisolone. Some of the patients showed improvement in their asthma following treatment and others did not. The patients who did not show improvement following treatment with prednisolone had higher levels of activated T lymphocytes in their blood. When T lymphocytes from these patients were grown in culture, it was discovered that glucocorticoids did not inhibit the activity of the T lymphocytes. However, cyclosporin A (a drug used to prevent tissue rejection following organ transplant) was able to inhibit the activity of T lymphocytes from the patients with glucocorticoid-resistant asthma. These results suggest that the ability of glucocorticoids to treat asthma may be related to their ability to inhibit the activity of T lymphocytes, and that cyclosporin A may be effective in treating glucocorticoid-resistant asthma. Abstract

To understand what causes asthma, research has focused on identifying the physiological changes that occur in the lungs during an asthma attack. Previous studies have shown that during an attack neutrophils (a type of white blood cell) enter into the lungs. It has been suggested that these cells may play a role in asthma since they can make enzymes and proteins that cause inflammation and can damage lung tissue. Although neutrophils have been found in the lungs during an attack, it is not known what causes these cells to leave the blood and enter into the lungs (a process called chemotaxis). To investigate this further, blood samples from 20 patients with acute severe asthma and samples from 10 patients without asthma were analyzed. Monocytes and lymphocytes (two types of white blood cells) were isolated from the blood and grown in culture. The cells from the patients with asthma produced larger amounts of a new protein that attracted neutrophils (a protein with neutrophil chemotactic activity, NCA) and the amount of the protein made was related to the severity of the asthma. Following seven days of drug therapy and rest, the patients' symptoms improved and the levels of this particular protein were reduced. A biochemical analysis of this protein indicated that it was different from previously identified proteins (such as interleukin-8) that are known to attract neutrophils. These findings indicate that blood cells (monocytes and lymphocytes) from patients with asthma make larger amounts of a protein that may be responsible for causing the neutrophils to enter the lungs, which leads to inflammation and tissue damage. 42. Immunotherapy Is Allergen-Specific: A Double-Blind Trial of Mite or Timothy Extract in Mite and Grass Dual-Allergic Patients.

Abstract

Background: One hundred years ago, Noon [Lancet 1911;1:1572-1573], using conjunctival provocation testing (CPT), was the first to demonstrate the effectiveness of subcutaneous immunotherapy (SCIT) in grass-allergic subjects with hay fever. In this centenary year, we present data that, by use of CPT and allergen-specific IgG, replicate this observation and additionally confirm the allergen specificity of SCIT by using a double-blind design employing either grass or mite SCIT in dual grass- and mite-allergic individuals. Methods: Twenty adults (11 females) with perennial rhinoconjunctivitis and exacerbation of symptoms during the grass pollen season and in the autumn had immediate skin and conjunctival sensitivity and raised specific IgE to both Dermatophagoides farinae and Phleum pratense. Participants were randomly assigned to either timothy or D. farinae immunotherapy for 3 years. CPT and specific IgG tests to both allergens were performed annually. After 3 years, subjects gave their blinded overall evaluation. Results: Six mild-to-moderate general reactions occurred in 2 timothy- and 4 mite-treated patients. Four of these patients and 2 other patients withdrew from the study. Seven patients in each group completed the study. After 3 years of immunotherapy, the timothy CPT threshold concentration had increased 16- fold in timothy-treated patients (p < 0.05; between-group change, p < 0.05). The increase in the mite CPT threshold in mite- compared to grass-treated patients was 31-fold (p < 0.05). The overall assessment of conjunctival sensitivity was highly significant in favour of treatment (p < 0.015), as was that of allergen-specific IgG (p < 0.0001). Conclusions: Allergen immunotherapy is allergen species-specific, as judged by decreased conjunctival sensitivity and changes in allergen-specific IgG concentrations. Copyright © 2011 S.
