39歳女性.患者は, 検診の上部消化管造影検査にて右腎の異常石灰化を指摘され, CTにて右腎上極に径8cmの著明な石灰化を伴う充実性腫瘍が認められた.MRIではT2強調画像で腎実質より軽度高信号を呈し, 18F-fluorodeoxyglucoseを用いたPETは陰性であった.以上より, 腎癌の診断で根治的腎摘除術を施行した結果, 組織学的には典型的な神経鞘腫で免疫染色にてS-100蛋白陽性であった, Schwannoma arising from the kidney is a rare benign tumor, with only 20 cases reported in the English literature. We encountered a histopathologically typical Schwannoma of the kidney in a 39-year-old woman, which was characterized by marked calcification. Radical nephrectomy was performed under the diagnosis of renal cell carcinoma. The 21 reported cases of schwannoma of the kidney, including the present case, are reviewed in detail.

大気の寒冷と相関のあるといわれる精巣捻転症39例について, 発症時の気温を検討した。14例は春季に, 13例が秋季に, 12例は冬季に起こっていた。発症時間は4:00p.m.から深夜が29例(54%), 深夜から8:00a.m.が13例(35.1)で日中は僅か3例(10.8%)であった。患者の住む消防署の気象データから, 気温は29例(76.3%)が当県の平均気温15°C以下で発症していた。発症頻度は温度と相関した, A study on correlation between testicular torsion and atmospheric temperature was made based on 39 patients observed at Tokai University Hospital. Testicular torsion did not occur in any of the 39 patients in the summer (June to August), but occurred in 14 patients in spring (March to May), in 13 patients in autumn (September to November), and in 12 patients in winter (December to February). The onset hour was between 04:00 p.m. and midnight in 20 patients (54.1%), between midnight and 08:00 a.m. in 13 patients (35.1%), and between 08:00 a.m. and 04:00 p.m. (in the daytime) in only 3 patients (10.8%). In each of 38 patients, the atmospheric temperature at the onset hour was investigated on the basis of meteorological observations made by the fire department located in the region of the patient's residence. The data indicated that testicular torsion in 29 patients (76.3%) occurred below 15 degrees C (below the average temperature in Kanagawa Prefecture). Furthermore, a correlation (Spearman's coefficient above 95%) between reduction in temperature and onset frequency was noted. Evidently these findings suggest that the incidence of testicular torsion tends to increase with reduction in temperature.

慢性細菌性前立腺炎19症例についてTFLXの薬効評価を行うとともに, 分離菌のMICの比較, 再発症例, 安全性について検討を加えた。1)感受性:EPSからの分離菌18菌種27株のMICはTFLXがCPFX, NFLX, OFLX, CCLに比較して最も低く, 特にGPCと嫌気性菌に対する抗菌活性が優れている傾向を認めた。2)細菌学的効果:薬効評価基準に合致した19症例からの分離菌は14菌種19株で, 除菌率は17/19であった。3)薬効評価:白血球の正常化率は11/19, 細菌の陰性化率は16/19であり, 総合臨床効果は18/19であった。4)主治医判定:有効率13/19をえた。5)再発症例の検討:TFLX投与終了1ヵ月後の再発を3例で検討したが, 全例とも再発はなかった。6)安全性:自覚的副作用は消化器症状を3例に認めたが重篤なものはなく, 臨床検査値の異常変動もなかった, In a multicenter trial at 17 institutions, the usefulness of an oral dose of 300 mg of tosufloxacin (TFLX) twice a day for 14 days were assessed in the management of chronic bacterial prostatitis (CBP) diagnosed by Meares and Stamey method. Nineteen patients who were evaluated by the UTI criteria were chosen out of the 27 patients diagnosed with CBP. The isolated bacteria from EPS were 18 species and 27 strains. In terms of MICs of isolates, TFLX gave lower MICs than any of the control drugs (CPFX, NFLX, OFLX, CCL). In particular, this tendency was clear for gram-positive coccus and anaerobes. The bacteriological elimination rate obtained for 14 strains and 19 species was 89.5% (17/19) [GPC; 75.0% (6/8), GNR; 100% (7/7), Anaerobes; 100% (4/4)]. The overall clinical efficacy by the UTI criteria was documented in 94.7% (18/19). According to the doctor's evaluation, the overall clinical efficacy rate was 68.4%. Safety evaluation revealed the development of gastrointestinal symptoms in 3 cases, which were not serious, with an incidence rate of 4.0% (3/75), while no laboratory abnormalities were observed. Transference of TFLX into the prostate is slightly lower than in other new quinolones. However, from this study we considered that the higher antibacterial activity of TFLX is one of the reasons for its usefulness for CBP.

A case report of primary osteosarcoma of the bladder is herein presented. A 75-year-old man presented with asymptomatic macrohematuria. Cystoscopic examination revealed a large tumor with a broad base arisen on the anterior wall. Pelvic computed tomographic scan showed multiple calcifications in the tumor itself. Total cystectomy and ileal conduit were performed. Histopathological diagnosis was osteosarcoma. The patient died 10 weeks after surgery. Metastasis to lung, heart, lymphnode, ureter and peritoneum were identified by autopsy. Primary osteosarcoma of the bladder is a rare tumor, only 25 cases have been reported previously. The prognosis of this tumor is very poor. Nineteen of these patients died within 6 months.

Operations on inpatients during the 15-year period from 1975 to 1989 were analyzed statistically. A total of 3, 791 operations were performed at our Department of Urology, including 717 operations on the kidney, 316 operations on the ureter, 583 operations on the bladder, 811 operations on the prostate, 136 operations on the urethra, 1, 097 operations on the scrotum, 43 operations on the adrenal gland, 19 operations on the retroperitoneal space and 69 operations on other organs.

We examined bacteria isolated from urine of patients in our urological department from 1985 through 1987. A total of 2, 566 strains were isolated from outpatients. They were 834 strains of gram-positive coccus (32.5%). E. coli (20.8%) was isolated most frequently, followed by Proteus sp. (12.6%), Enterococcus (9.6%), P. aeruginosa (6.7%), S. epidermidis (5.5%), and K. pneumoniae (3.7%) and others. A total of 784 strains were isolated from inpatients. They were 313 strains of gram-positive coccus (39.9%). Enterococcus (22.6%) was isolated most frequently, followed by P. aeruginosa (10.1%), S. marcescens (9.7%), E. coli (8.2%), S. epidermidis (7.9%), and K. pneumoniae (4.3%) and others. Gram-positive bacterium occupied a larger proportion of the total-isolates from both outpatients and inpatients, than in our previous examination. Fewer P. aeruginosa and Enterococcus were isolated from outpatients, and fewer P. aeruginosa from inpatients than previously. More S. marcescens were isolated from inpatients than previously. S. marcescens, E. cloacae, C. freundii, S. epidermidis, and S. aureus all isolated from outpatients showed a higher sensitivity to all drugs we tested than those isolated from inpatients.

A case of contralateral ureteral metastasis from renal cell carcinoma is reported. A 52-year-old man underwent left nephrectomy for renal cell carcinoma of November 20, 1986. He was clinically asymptomatic for 4 years 8 months after the operation. He was admitted on August 14, 1991, because of right loin pain. Right retrograde pyelography and percutaneous pyelography showed a filling defect in the right ureter at the level of L3. After the right ureter was explored, the tumor lesion of ureter was resected and end to end anastomosis of the ureter was performed. Histopathologic examination showed a metastatic clear cell carcinoma consistent with a renal primary. The contralateral ureteral metastasis from renal cell carcinoma is very rare and only 15 cases have been reported previously.

Schwannoma arising from the kidney is a rare benign tumor, with only 20 cases reported in the English literature. They were 241 strains of gram-positive bacteria (29.8%). E. coli (32.0%) was isolated most frequently, followed by Enterococcus spp. (9.9%), Proteus group (7.4%), S. epidermidis (6.2%), P. aeruginosa (5.1%) and others. A total of 412 strains were isolated from the inpatients. They consisted of 173 strains of gram-positive bacteria (42.0%). Enterococcus spp. (17.0%) was isolated most frequently, followed by P. aeruginosa (12.6%), S. epidermidis (9.7%), E. coli (7.3%), S. aureus (6.6%) and others. Fewer P. aeruginosa, S. marcescens and Proteus group were isolated from the outpatients and fewer S. marcescens, E. coli and K. pneumoniae from inpatients than previously. More S. aureus were isolated abruptly from the inpatients and more E. coli were isolated from the outpatients than previously. E. cloacae, S. epidermidis, S. aureus and Corynebacterium spp. all isolated from inpatients showed lower antimicrobial sensitivity than those isolated from the outpatients.

Among congenital renal anomalies, crossed renal ectopia is relatively rare. We report two cases of crossed renal ectopia without fusion. One patient was a 26-year-old woman, admitted to our hospital with complaint of infertility. She was diagnosed to have hydrosalpinx on hysterosalpingography. Intravenous pyelography and renoscintigraphy disclosed crossed renal ectopia without fusion. We confirmed it on the operation of hydrosalpinx. The other was a 28-year-old man with complaint of hypospadia. Intravenous pyelography, retrograde pyelography and abdominal aortography disclosed crossed renal ectopia without fusion. These patients had subjectively no complaints, and the renal functions were unimpaired. So we decided to follow them up. In Japan there have been 30 reports of crossed renal ectopia including these cases. We performed a statistical study of the renal ectopia without fusion.

In order to examine effectiveness, safety and usefulness of, KW-l062, a new aminoglycoside antibiotic, it was compared with gentamicin (GM) for treatment of chronic complicated urinary tract infection in a double-blind trial. KW-I062 was administered to hospitalized adult patients at the dosage of 120 mg b.i.d. for five days. GM was administered to hospitalized adult patients at the dosage of 60 mg b.i.d. for five days. 今日, 膣トリコモナスの臨床材料からの検出には, 市販の培地が使用されているが, その検出感度は必ずしも満足できるものではない。感度のよい培地には保存性がわるいなどの欠点がある。泌尿器科的立場からより好ましい培地を追求し, 今回新しく肝エキスを除いた培地について従来の日水培地, 浅見培地と比較し, この2種の培地の中間に位置する検出感度であることをたしかめた。, For demonstration of Trichomonas vaginalis, commercial liquid media commonly used in most clinical laboratories but none has proven to be fully satisfactory in respect of presrvation, trichomonad detect ability and price. Experiments were performed to assess usefulness of a trial preparation of liver extract-free medium. The results indicate that the new medium is superior to the conventional media with respect to cost and preservability but there is no appreciable difference in trichomonad detectability between them.

A case of ureterocele with ureteral stone and bladder tumor is reported. The patient, a 35-year-old man, presented with the complaint of terminal miction pain. IVP revealed the cobra-head appearances of a left-sided ureterocele, ureteral stone and hydroureter. The stone in the ureterocele and the tumor arising from the ureterocele were confirmed by cystoscopic examination. Transurethral biopsy was carried out and followed cystolithotomy, partial cystectomy, partial ureterectomy and ureteroneocystostomy. Histopathological examination of surgical specimens revealed the same feature of transitional cell carcinoma developed from the ureterocele.

The following results were obtained from the three-year (1976-1978) statistics of bacterial florae isolated from the urinary tract and of their drug sensitivity. During the past three years, 3534 strains were isolated and 2827 strains (80%) were gram-negative bacilli. The number of strains which had been isolated from outpatients were 1945 consisting of 37.5% E. coli, 10.9% Enterococci, 10.8% Proteus group, 7.7% Klebsiella pneumoniae, 7.3% Pseudomonas aeruginosa, 7.1 % Staphylococcus epidermidis and others. There were 1589 strains isolated from hospitalized patients consisting of 16.3% P. aeruginosa, 14.7% Enterococci, 12.0% E. coli, 11.9% Proteus group, 9.5% E. cloacae and others. The increasing tendency has been seen in Serratia marcescens and Proteus spp. in the strains isolated from the outpatients and in S. marcescens and S. epidermidis in those from the inpatients. The 9 main strains isolated from outpatients were more susceptible to the antimicrobial agents (except nalidixic acid) than those from inpatients. The effect of colistin and chloramphenicol, antibiotics not routinely used, against isolates from the outpatients showed little difference from that against those from the inpatients. The reason why the effect of nalidixic acid against isolates from the inpatients was superior to that against those from the outpatients is probably that it has been mainly used in the outpatient clinic.

The clinical effectiveness of Fosmicin-S, a new antibiotic, was studied in 30 patients with genitourinary tract infections (9 acute simple cases and 21 chronic complicated cases). The drug was given in doses of 2 g, once or twice a day by intravenous injection (11 cases), or by intravenous drip infusion (19 cases) for 3 to 7 days. For the judgment of clinical efficacy, Case 4 in the acute case group and Cases 10 and 29 in the chronic case group were excluded from the analysis. The reasons were that the urine sample collected from Case 4 before treatment gave a negative culture, while the latter two patients received the drug for only one day. The overall efficacy rates for the analyzed cases were 75% (6 out of8 cases) for acute simple genitourinary tract infections and 47.4% (9 out of 19 cases) for chronic complicated genito-urinary tract infections. Adverse reactions were observed in 3 cases, taking the form of diarrhea, rise in GOT and GPT, and transient blurred vision in one case each. Bacteriologically, Pseudomonas aeruginosa was isolated from 7 cases, and complete elimination of the organism was observed in only one case. This suggested that the doses employed in this trial were too small to produce satisfactory results. There have been no reports on this possibility. The authors thus felt it necessary to determine the urinary excretion rate, as well as the serum concentration, following administration of relatively higher doses. For this purpose, 3 different higher doses of Fosmicin-S were given to 3 healthy adult volunteers using the crossover method: (1) one hour i.v. infusion of 4g dissolved in 100 ml of water for injection, (2) two-hour infusion of 4g dissolved in 300 ml of 5% glucose solution, and (3) two-hour infusion of 6 g dissolved in 500 ml of 5% glucose solution.

Two cases of torsion of the spermatic cord occurring in early infancy are reported herein. One was 4-day-old newbron baby. Edema and hyperemia developed on the left half of the scrotum during the 2nd day of life. The testicle was removed 2 days later. In the other case, 5-month-old infant, the diagnosis of torsion of the right spermatic cord was made about 1 hour later to onset of the disease. The testis was saved by untwisting the cord. The torsion was extravaginal in both of them.

3種の経口的に使用できる抗膣トリコモナス剤について，男子尿性器由来保容膣トリコモナス株48株を用いてMIC分布を測定した. ニトロフラントインは8-10γ/ml，メニトロニダゾールは0.8-0.9γ/ml，チニダゾールは0.3-0.9γ/mlのMICが得られた。相乗効果について4種の薬剤の組合わせで検討したところ，メトロニダゾールとニトロフラントイン，チニダゾールとニトロフラントインの組合わせで軽度にみとめられた。

Two cases of retroperitoneal liposarcoma are reported. Case I was a 71-year-old-male presenting with a mass in the left abdomen. On X-ray examination, a large extrarenal tumor was found in the left retroperitoneal space. The tumor appeared to invade the psoas muscle and the aorta. Exploratory laparotomy was carried out. The histological diagnosis of the tumor was myxoid-type liposarcoma. He died of the disease. Case 2 was a 44-year-old-male who presented with a mass in the left abdomen. On X-ray examination, a large tumor was found in the left retroperitoneal space, and left kidney and the descending colon were markedly displaced by the tumor. Excisional surgery was carried out. The tumor was almost completely removed. The histological diagnosis was myxoid-type liposarcoma. A combination chemotherapy (actinomycin D, vincristine, cyclophosphamide) was started. but soon switched to radiation therapy because of liver dysfunction. Four months following the operation, there is no evidence of residual disease. One hundred sixty three cases of retroperitoneal liposarcoma reported in Japan are reviewed.

A total of 794 operations were performed at the Department of Urology, Tokai University Hospital between January, 1982 and March, 1984, including 5 operations on the adrenal gland, 64 operations on the kidney, 48 operations on the ureter, 93 operations on the urinary bladder, 124 operations on the prostate, 25 operations on the urethra, 124 operations on the penis, 190 operations on the scrotum and 19 operations on other organs.

P. aeruginosaによる複雑性尿路感染症に対するCPMの治療効果と副作用を評価する目的で, CPM 2g/日とCFS 2g/日の二重盲検法による比較試験をおこなった.総投与例数は274例で, うち効果を判定し得たのはCPM群136例, CFS群118例であった.1) P. aeruginosaを含む感染症では, CPMあるいはCFSを1日2g, 5日間投与時の効果では, 前者の有効率が有意に高かった.2) P. aeruginosaのみを対象にした場合も同様であった.3) P. aeruginosaを含む感染で細菌尿の陰性化率はCPM群はCFS群より有意に高かった.4)交代菌の出現は, CFS群でS. faecalisの出現率が有意に高く, 全体的にもCFS群の投与後の細菌出現率が有意に高かった, 5)薬剤投与開始前の分離菌のMICでは, CPM群の方がCFS群よりもMICの低いものが多かった.6)副作用発現率, 臨床検査値の異常化は両群間に差はなかった, The therapeutic efficacy and safety of Cefpiramide (CPM, SM-1652) at a 2 g/day dose were objectively compared with those of Cefsulodin (CFS) at a 2 g/day dose in patients with chronic complicated urinary tract infections (UTI) by P. aeruginosa in a double-blind study at 46 institutions in Japan. The results are as follows: The therapeutic efficacy was analyzed in 254 patients (136 cases administered CPM and 118 cases administered CFS) after excluding 20 cases as drop-out. Among 254 cases, the number of patients with infection of P. aeruginosa was 190 cases (100 cases administered CPM and 90 cases administered CFS), while that with infection of organisms other than P. aeruginosa was 64 cases (36 cases administered CPM and 28 cases administered CFS). By the administration of a 2 g/day dose for 5 days, the overall clinical effective rate of CPM was significantly higher than that of CFS in total patients. When the patients were classified into 2 groups with respect to causative organisms (P. aeruginosa and others), the clinical effective rate of CPM in patients with infections of P. aeruginosa was significantly higher than that of CFS, while the clinical effective rate of CPM in patients with infection of other organisms than P. aeruginosa was the same as that of CFS. As to the bacteriological effect on bacteriuria, the eradication rate of CPM was significantly higher than that of CFS not only against all causative organisms but also against P. aeruginosa. The rate of replacement by S. faecalis was significantly higher in the CFS-treated group than in the CPM-treated group. The same result was obtained on the rate of replacement by other organisms. The MIC values of CPM for isolated organisms before drug administration were lower than those of CFS. The incidence rates of side effects and the abnormal findings of clinical laboratory tests were the same for the CPM- and CFS-treated groups. From the results, it was concluded that CPM is a useful drug for the treatment of patients with chronic complicated urinary tract infections caused by P. aeruginosa.

1)各種尿性器疾患患者の血清フコースを測定した.i)担癌患者のStageの進行したもの, ii)腎盂腎炎, 前立腺炎などの炎症の著明なもの, iii)術後の患者, などでは正常より高値を示した.2)癌性悪液質に陥っているものはいちじるしい高値を示したが, 前立腺癌では病期の進展は高度でも正常値のものがみとめられた.3)血清フコース値と血沈値は相関した, Serum fucose is known to increase in malignant diseases. We measured the serum fucose of the patients with urogenital diseases, and found that the level of serum fucose was elevated beyond the normal limit in patients having progressed stage of tumors, pyelonephritis, marked inflammation s11ch as prostatitis and in patients within 1 month postoperation. Patients having cancerous cachexia had distinctly elevated serum fucose while patients having prostatic cancer sometimes had normal serum fucose even in the progressive stage of cancer. In general, the elevation of serum fucose correlated with that of the erythrocyte sedimentation rate.

男子尿性器由来の陸トリコモナス保存株を経代培養すると，薬剤に対する感受性が変化し，経代を重ねるほどMICが低下する。つまり薬剤が効きやすくなることが判明した。 またin vitroでの薬剤効果と臨床効果にも差のあることが判明した。したがって，膣トリコモナスの薬剤感受性を測定し， それを臨床的に利用しようとするには， 1. 経代数については，なるべく代数を重ねない株がよい。2. 培地，虫体数などは，常に一定する, ということが必要であることがわかっているが, さらに, 3. 臨床無効例より検出した虫体の感受性の検討，および誌験管内感受性との間の差がなぜ起こるかについての検討。 4. 耐性獲得という現象が本当にあるのか? 5. 男子尿性器へ薬剤が如何なる濃度で分布するか，また膣トリコモナスが，どこに感染するか? 等についての追及が必要となろう。 それには，資料として，男子TV感染者の発見と，その株の保存が大切である。本論文の要旨は第22回日本化学療法学会総会において発表した。

膣トリコモナス虫体内のdehydrogenase各種を染色法で検討してみた。LDH，Aldolase，AlcDH，M DH，の活性は虫体内にみとめられたが，αKeto DH，SDH, ICDH にはあまり活性がみとめられなかった。

Presented are five cases in which computed tomography was quite useful as one of the diagnostic tools. A discussion was made how we should apply computed tomography in urological practice. It is recommended to use computed tomography next to IVP or DIP rather routinely whenever we want to know the extension of malignant neoplasia in the retroperitoneal or intrapelvic space. It is not so helpful for diagnosis of adrenal diseases.

1981年および82年に尿から分離されたS. marcescensについて検討した.この感染は宿主側要因が低下し, カテーテルを留置しているという例に多いが, それに対する生体反応は微弱で, 発熱や膿尿をきたさぬことが多い.複数菌感染として存在することが多く, その時の混在菌にはP. aeruginosa, S. faecalisがとくに多い.S. marcescensが生命に危険をおよぼすような症状を呈することはまれであるが, 逆に予後の悪いような症例にはS. marcescens感染が多い.薬剤感受性はGM, DKB, ST合剤などが高かった, A study was made of the actual status of urinary tract infection induced by Serratia marcescens on a total of 88 patients who were admitted to our University Hospital in 1981 approximately 1982, and in whose urines not less than 10(4)/ml of S. marcescens were found. An investigation into the background factors of these patients revealed that indwelling catheter, operation for malignancy and use of antibiotics were the major factors responsible for the infection. S. marcescens was found to coexist with P. aeruginosa in not a few cases. The pathogens were highly susceptible to ST compound preparations, DKB and GM, and the pathogens in only 7 of the patients were tolerant to all of these antibiotics. None of the patients died of S. marcescens infection.
