Abstract

Reactive oxygen species, such as hydrogen peroxide and superoxide anion, cause a contraction of capacitance and hepatic resistance vessels, and or sinusoidal endothelial cells. The time for the interaction of hepatocytes with circulating substrates decreases due to contraction, which also reduces the vascular bed in the liver. Bromosulfophthalein transport, vascular volume of hepatic sinusoid and the variations in portal pressure due to oxidase are partially inhibited by the addition of superoxide dismutase or catalase to the perfusion medium.

Abstract

This paper presents a sensitivity-based approach for structural control of electric power networks using FACTS (Flexible AC Transmission System) devices i order to improve transient stability margin for one controlling unstable equilibrium point (u.e.p.). It is comprised of the sensitivity analysis of critical energy of TEF (Transient Energy Function), network control strategy an evaluation. A related practical application to a 39-bus test system follows and shows that the structural control of power networks is effective for the enhancement of power system transient stability.

Abstract

Sliding mode control can deal with nonlinearities of control systems and is robust. However, it has drawbacks, such as high control gain effect and control chattering. This paper proposes a method of nonlinear feedback control introducing fuzzy inference into sliding mode control. The fuzzy inference is introduced to treat nonlinearities of control systems and to reduce the control chattering. Furthermore, the stability of the system is discussed by the fuzzy stability theory based on Lyapunov's direct method. The proposed method is applied numerically to the stabilizing control of an electric power system as a nonlinear system and is shown to give a good control effect.

Abstract

Background Irinotecan plus S-1 (IRIS) is the only oral fluoropyrimidine-based regimen reported to be non-inferior to FOLFIRI and widely used in clinical practice for metastatic colorectal cancer (mCRC) patients. However, the combination of IRIS plus an anti-EGFR agent has not been evaluated previously. This study aimed to investigate the feasibility and efficacy of IRIS with panitumumab as second-line therapy for wild-type KRAS mCRC. Methods Main inclusion criteria were patients with wild-type KRAS mCRC refractory to one prior chemotherapy regimen for mCRC, ECOG PS 0-2, and age ≥ 20 years. Patients received panitumumab (6mg/kg) and irinotecan (100mg/m2) on days 1 and 15 and S-1 (40-60 mg according to body surface area) twice daily for 2 weeks, repeated every 4 weeks. The primary endpoint was the feasibility of the therapy. The secondary endpoints were response rate (RR), progression-free survival (PFS), and overall survival (OS). Results A total of 36 patients received protocol treatment in eight centers. Of these, 23 patients (63.9%) completed protocol treatment, demonstrating achievement of the primary endpoint. The most frequent grade 3/4 toxicities were diarrhea (16.7%), acne-like rash (13.9%), and neutropenia (11.1%). The overall RR was 33.3% (12/36). Of these 4 five underwent conversion surgery. Median PFS and OS were 9.5 months (95% CI 3.5-15.4 months) and 20.1 months (95% CI 16.7-23.2 months), respectively. Conclusion IRIS plus panitumumab has an acceptable toxicity profile and a promising efficacy in patients with previously treated wild-type KRAS mCRC. Accordingly, this regimen can be an additional treatment option for second-line chemotherapy in wild-type KRAS mCRC.

32. Experimental Studies on Protective Effects of FK506 Against Hepatic Ischemia-Reperfusion Injury

Abstract

Purposes ; FK506 (strong immunosuppressive agent) was investigated experimentally whether to protect the hepatic IRI. Methods ; Warm ischemic experiment using pigs and rats were performed and examined whether FK506 is effective. Results ; The results obtained are as follows. 1. Warm ischemia allowed time of the pigs without FK506 was 150 minutes, but as for that of FK506 group, the extension of 30 minutes was got in 180 minutes. 2. Biliary excretion rate of BSP after reperfusion were better in the group of 180 minutes ischemia with FK506 than in without FK506 group. 3. Chemiluminescence intensity in the peripheral neutrophils and adhered and infiltrated leukocytes in the liver were suppressed markedly by FK506. 4. The vascular endothelium with the scanning electron microscope was relatively preserved in the FK506 group comparing to the placebo group on 30 minutes after reperfusion. 5. Stress gastric ulcer was controlled and myeloperoxidase activity in the gastric mucosa was suppressed by FK506. Conclusion ; Based on the results of theses experiments, it was suggested that FK506 has a protective effect on IRI by suppressing : the impairment of sinusoidal endothelial cells ; the activation of KCs ; the disturbance of micro-circulation ; oxidative stress ; inflammation ; and the accumulation of leukocytes.
