1. A Phase I Trial of Pox PSA vaccines (PROSTVAC®-VF) with B7-1, ICAM-1, and LFA-3 co-stimulatory molecules (TRICOM™) in Patients with Prostate Cancer

Abstract

Abstract Purpose Based on previous studies that demonstrated the safety profile and preliminary clinical activity of prostate specific antigen (PSA) targeted therapeutic vaccines, as well as recent laboratory data supporting the value of the addition of co-stimulatory molecules B7-1, ICAM-1, and LFA-3 (designated TRICOM™) to these vaccines, we conducted a Phase I study to evaluate the safety and immunogenicity of a novel vaccinia and fowlpox vaccine incorporating the PSA gene sequence and TRICOM. Methods In this study, ten patients with androgen independent prostate cancer with or without metastatic disease were enrolled. Patients were treated with 2 × l08 pfu of a recombinant vaccinia virus vaccine (PROSTVAC-V) followed by 1 × 109 pfu of the booster recombinant fowlpox virus (PROSTVAC-F) both with gene sequences for PSA and TRICOM. The mean age of patients enrolled in the study was 70 (range 63 to 79). The mean PSA at baseline was 434 (range 9 – 1424). Results There were no deaths, and no Grade 3 or 4 adverse events. The most commonly reported adverse events, regardless of causality, were injection site reactions and fatigue. One serious adverse event (SAE) occurred that was unrelated to vaccine; this patient developed progressive disease with a new sphenoid metastasis. PSA was measured at week 4 and week 8. Four patients had stable disease (with less than 25% increase in PSA) through the week 8 study period. Anti-PSA antibodies were not induced with therapy: however, anti-vaccinia titers increased in all patients. Conclusion This study demonstrated that vaccination with PROSTVAC-V and PROSTVAC-F combined with TRICOM is well-tolerated and generated an immune response to vaccinia. Therefore, PROSTVAC-VF/TRICOM represents a feasible therapeutic approach for further phase II and III study in patients with prostate cancer.

Abstract

A new controller for the generator excitation system is described that uses a combination of feedback linearization and the observation decoupled state space. This creates a controller that can be realistically implemented using only local measurements, and whose performance is consistent with respect to changes in network configuration, loading and power transfer conditions. The control differs in this respect from linear constant-gain controllers such as power system stabilizers, whose characteristics can vary significantly with changes in operating conditions. The design is well-suited to a multimachine setting, in that it is not based on an infinite-bus approximation. Simulations are performed on a 38-bus reduced model of the Northeast Power Coordinating Council system and benchmarked against simulations in which automatic voltage regulators with power system stabilizers are substituted in place of the nonlinear controls.

Abstract

The book is primarily concerned with understanding the causes of and prescribing cures for the obsolescence of knowledge and skills among professionals. The focus is on the role of the organization in creating, as well as overcoming, obsolescence. Since management bears much of the onus for allowing obsolescence to occur, it is only just that it bear the primary responsibility for providing the conditions that will facilitate professional development. This is in management's own interest, but it can do no more than create a climate which encourages and rewards the continuous maintenance of competence. The rest is up to the individual, upon whom rests the ultimate responsibility for life-long learning and professional career development. There are a large number of sound organizational policies and programs which, if introduced and pursued, can contain obsolescence. Chapters devoted to personnel practices, the work environment and the job design, and organizational climate offer such policies. Another chapter closely examines the continuing-education activities of organizational professionals to determine the degree to which they contribute to updating, and how the effectiveness of such activities can be improved. A concise summary of suggestions for future organizational directions concludes the book. (Author/AJ)
