- Abstract
Background: In 2020, ACIE Appy study showed that COVID-19 pandemic heavily affected the management of patients with acute appendicitis (AA) worldwide, with an increased rate of non-operative management (NOM) strategies and a trend toward open surgery due to concern of virus transmission by laparoscopy and controversial recommendations on this issue. The aim of this study was to survey again the same group of surgeons to assess if any difference in management attitudes of AA had occurred in the later stages of the outbreak. Methods: From August 15 to September 30, 2021, an online questionnaire was sent to all 709 participants of the ACIE Appy study. The questionnaire included questions on personal protective equipment (PPE), local policies and screening for SARS-CoV-2 infection, NOM, surgical approach and disease presentations in 2021. The results were compared with the results from the previous study. Results: A total of 476 answers were collected (response rate 67.1%). Screening policies were significatively improved with most patients screened regardless of symptoms (89.5% vs. 37.4%) with PCR and antigenic test as the preferred test (74.1% vs. 26.3%). More patients tested positive before surgery and commercial systems were the preferred ones to filter smoke plumes during laparoscopy. Laparoscopic appendicectomy was the first option in the treatment of AA, with a declined use of NOM. Conclusion: Management of AA has improved in the last waves of pandemic. Increased evidence regarding SARS-COV-2 infection along with a timely healthcare systems response has been translated into tailored attitudes and a better care for patients with AA worldwide.

8. A Study of Prevalence of Childhood Obesity among School Children in Karimnagar Town

Background: The problem of obesity is confined not only in adults but also among the children and adolescents. The most significant long term consequence of childhood obesity is its persistence into adulthood along with its attendant risks. The surveys on childhood obesity in under surveyed areas help to identify risk factors. Objectives: 1.To study the prevalence of obesity among school children of Karimnagar town. 2. To study the factors associated with obesity. Methods: A cross sectional study was conducted in schools of Karimnagar town. A total of 892 students were randomly selected belonging to 6th to 10th standard in randomly selected schools. Data was collected in a predesigned questionnaire. The cut off points for age and gender specific BMI recommended by Agarwal et al were used. Statistical analysis: The data was analyzed using chi square test. P value of less than 0.05 was considered as significant. Results: The mean age of study subjects was 13.03+1.52 years. The overall prevalence of overweight and obesity was 11.99% and 2.69% respectively and 14.68% combined together. Out of the factors studied, children belonging to open category (caste), upper social class were found to be significantly associated with obesity (p < 0.05). Other factors were not associated with obesity. Conclusion: The overall prevalence of obesity in the present study is high it is significantly associated with caste and upper social class. There is a need to increase awareness on physical fitness, regular exercise and appropriate dietary habits among children through schools.

Acute appendicitis remains a common reason for hospital admission. Reports have suggested a reduction in patients attending emergency departments during the acute phase of the SARSCoV- 2 pandemic. Moreover, a global surge in conservative management of acute appendicitis has recently been registered by the Appy Study of the Association of Italian Surgeons in Europe (ACIE)1. This is a treatment option that has been known for some years, although quite seldom used before the pandemic2–4. As most countries are experiencing new waves of the pandemic, the attitude of surgeons towards SARS-CoV-2 screening policies and personal protective equipment (PPE) used during the management of patients with acute appendicitis need to be established. According to a subanalysis of the ACIE Appy Study, half of surgeons globally were testing patients for SARS-CoV-2 only when symptomatic or there was suspicion of infection; approximately 12 per cent did not test patients at all (Fig. 1 and Table S1). There were regional differences. In Europe, respondents tested all patients (50.8 per cent) or those with suspected infection (43.9 per cent), with only 5.3 per cent not being tested at all. In the USA, the majority of participants only tested patients with a suspected infection (65.6 per cent). A similar picture of testing only those with a suspected infection was also reported from Latin America (57.2 per cent), Asia/Middle East (76.8 per cent), and Africa (41.7 per cent). Even more worrisome, 58.3 per cent of respondents from Africa and 27.6 per cent from Latin America were not testing patients at all before appendicectomy. Concerning the screening modality, most respondents used PCR alone or in combination with chest imaging. Serology was rarely used overall and never in Africa (Fig. 1 and Table S2 ). It is now accepted that chest imaging is not routinely required and that PCR is an accurate screening modality. Serology might, however, be useful to shed light on the disease course and previous exposure to the virus, but respondents from some countries still have restricted access to this test. In terms of PPE during appendicectomy, most African respondents did not use different PPE compared with the prepandemic period in patients who tested negative for COVID-19. More concerning is that 58.3 per cent did not use different PPE in untested patients. This differed from other regions where the rate of those not considering a change of PPE in untested patients did not exceed 22 per cent. One in 10 respondents from Latin America also reported that they were not using different PPE compared with the prepandemic phase in patients who tested positive for COVID-19. These data, and taking into account the high prevalence of acute appendicitis, leads to the conclusion that omission of routine patient screening may have contributed to local clusters among patients and threatened the safety of healthcare workers5. In this respect, it is likely that limited access to PPE explains the attitude of surgeons towards patients with unknown SARSCoV- 2 status or those infected, raising ethical concerns about the safety of surgical staff. It is of outmost importance that, even during challenging times and stress on economic stability, industrialized countries make efforts to sustain low-income countries and those with limited resources. This would ensure equal working conditions, safer treatment for patients with acute appendicitis, and better control of the pandemic

Activated carbon has a huge demand for its broad applications in batteries, supercapacitors, wastewater treatment, etc. Activated carbon possesses a porous morphology, which facilitates its enhanced thermal stability, and surface area. Researchers employing agro-waste have focused on meeting the high demand of low-cost activated carbon production. This paper reports coconut-shell-derived activated carbon prepared by the thermal and chemical activation processes. The prepared samples are investigated for their structural, morphological, elemental composition, electrochemical properties, and functional groups. The prepared samples evidenced the amorphous nature, and some graphitic disordered, as confirmed by the x-ray diffraction (XRD) and Raman investigations. The morphological analysis demonstrated the formation of a nanoflakes-like structure; however, another sample prepared with chemical activation endorsed the porous carbon nanostructures. - Abstract
Background: Surgical strategies are being adapted to face the COVID-19 pandemic. Recommendations on the management of acute appendicitis have been based on expert opinion, but very little evidence is available. This study addressed that dearth with a snapshot of worldwide approaches to appendicitis. Methods: The Association of Italian Surgeons in Europe designed an online survey to assess the current attitude of surgeons globally regarding the management of patients with acute appendicitis during the pandemic. Questions were divided into baseline information, hospital organization and screening, personal protective equipment, management and surgical approach, and patient presentation before versus during the pandemic. Results: Of 744 answers, 709 (from 66 countries) were complete and were included in the analysis. Most hospitals were treating both patients with and those without COVID. There was variation in screening indications and modality used, with chest X-ray plus molecular testing (PCR) being the commonest (19·8 per cent). Conservative management of complicated and uncomplicated appendicitis was used by 6·6 and 2·4 per cent respectively before, but 23·7 and 5·3 per cent, during the pandemic (both P < 0·001). One-third changed their approach from laparoscopic to open surgery owing to the popular (but evidence-lacking) advice from expert groups during the initial phase of the pandemic. No agreement on how to filter surgical smoke plume during laparoscopy was identified. There was an overall reduction in the number of patients admitted with appendicitis and one-third felt that patients who did present had more severe appendicitis than they usually observe. Conclusion: Conservative management of mild appendicitis has been possible during the pandemic. The fact that some surgeons switched to open appendicectomy may reflect the poor guidelines that emanated in the early phase of SARS-CoV-2.

Dependence of flame suppression concentration on the equivalence ratio was analyzed using modeling premixed propane/air flame inhibited by 2-BTP, Novec 1230, CF
3 Br, H2 O, CO2 , and N2 . It was obtained that the suppression concentration of additives with fuel component is increased in the fuel-lean mixtures as a result of the increase of adiabatic flame temperature with their addition. Calculations of adiabatic explosion pressure (combustion temperature) under constant volume conditions demonstrate that the addition of inhibitors with fuel component to the lean mixtures increases the combustion pressure even for CF3 Br additive. Suppression concentrations of inert additives (N2 , CO2 , H2 O) were at the maximum for slightly rich mixtures where the maximum adiabatic flame temperature is observed. Calculations for the blend (inert and effective chemical inhibitor; CO2 /2-BTP) demonstrate that their use might be effective in the lean mixtures close to the flammability limits. The suppression concentration of 2-BTP in the blend (2-BTP/CO2 = 50/50) was less than for pure inhibitor (2-BTP), thus leading to some decrease of the combustion pressure for inhibited flames near the flammability limits in comparison with pure 2-BTP. Diffusion flames are commonly used for industrial burners in furnaces and flares. Oxygen/fuel burners are usually diffusion burners, primarily for safety reasons, to prevent flashback and explosion in a potentially dangerous system. Furthermore, in most fires, condensed materials pyrolyze, vaporize, and burn in air as diffusion flames. As a result of the interaction of a diffusion flame with burner or condensed-fuel surfaces, a quenched space is formed, thus leaving a diffusion flame edge, which plays an important role in flame holding in combustion systems and fire spread through condensed fuels. Despite a long history of jet diffusion flame studies, lifting/blowoff mechanisms have not yet been fully understood, compared to those of premixed flames. In this study, the structure and stability of diffusion flames of gaseous hydrocarbon fuels in coflowing air at normal earth gravity have been investigated experimentally and computationally. Measurements of the critical mean jet velocity (U(sub jc)) of methane, ethane, or propane at lifting or blowoff were made as a function of the coflowing air velocity (U(sub a)) using a tube burner (i.d.: 2.87 mm). By using a computational fluid dynamics code with 33 species and 112 elementary reaction steps, the internal chemical-kinetic structures of the stabilizing region of methane and propane flames were investigated. A peak reactivity spot, i.e., reaction kernel, is formed in the flame stabilizing region due to back-diffusion of heat and radical species against an oxygen-rich incoming flow, thus holding the trailing diffusion flame. The simulated flame base moved downstream under flow conditions close to the measured stability limit.

Bulk metallic glass (BMG) samples are amalgamated by high-energy ball milling using the general formula (Ni75Cr15Si10)100−xTix (labelled as NCSTx, where x = 0,4, 8 and 12 at.%). The crystallinity and phase attribution properties of nickel (Ni), chromium (Cr), silicon (Si) and titanium (Ti) are scrutinized using the XRD analysis. The trend related to glass-forming ability (ΔT) and the GFA parameter (γ) is NCST8 > NCST12 > NCST4 > NCST0, which confirms that NCST8 sample shows superior glass-forming ability. The reduced glass transition temperature (Trg) for all the present BMG systems is greater than 0.5, which offers its good thermal stability and glass-forming ability (GFA). Corrosion studies suggested that a sample, having dynamic nature towards corrosion resistance and potential (Ec) (NCST8, (Ni75Cr15Si10)92Ti8), exhibits relatively lower values of both ic (4.7 × 10−4 A/cm2) and ip (0.0079 A/cm2), as recommended in potentio-dynamic findings. A fire, whether in a spacecraft or in occupied spaces on extraterrestrial bases, can lead to mission termination or loss of life. While the fire-safety record of US space missions has been excellent, the advent of longer duration missions to Mars, the moon, or aboard the International Space Station (ISS) increases the likelihood of fire events, with more limited mission termination options. The fire safety program of NASA's manned space flight program is based largely upon the principles of controlling the flammability of on-board materials and greatly eliminating sources of ignition. As a result, very little research has been conducted on fire suppression in the microgravity or reduced-gravity environment. The objectives of this study are: to obtain fundamental knowledge of physical and chemical processes of fire suppression, using gravity and oxygen concentration as independent variables to simulate various extraterrestrial environments, including spacecraft and surface bases in Mars and moon missions; to provide rigorous testing of analytical models, which include comprehensive descriptions of combustion and suppression chemistry; and to provide basic research results useful for technological advances in fire safety, including the development of new fire-extinguishing agents and approaches, in the microgravity environment associated with ISS and in the partial-gravity Martian and lunar environments.

Diffusion flame stabilization in normal earth gravity (1 g) has long been a fundamental research subject in combustion. Local flame-flow phenomena, including heat and species transport and chemical reactions, around the flame base in the vicinity of condensed surfaces control flame stabilization and fire spreading processes. Therefore, gravity plays an important role in the subject topic because buoyancy induces flow in the flame zone, thus increasing the convective (and diffusive) oxygen transport into the flame zone and, in turn, reaction rates. Recent computations show that a peak reactivity (heat-release or oxygen-consumption rate) spot, or reaction kernel, is formed in the flame base by back-diffusion and reactions of radical species in the incoming oxygen-abundant flow at relatively low temperatures (about 1550 K). Quasi-linear correlations were found between the peak heat-release or oxygen-consumption rate and the velocity at the reaction kernel for cases including both jet and flat-plate diffusion flames in airflow. The reaction kernel provides a stationary ignition source to incoming reactants, sustains combustion, and thus stabilizes the trailing diffusion flame. In a quiescent microgravity environment, no buoyancy-induced flow exits and thus purely diffusive transport controls the reaction rates. Flame stabilization mechanisms in such purely diffusion-controlled regime remain largely unstudied. Therefore, it will be a rigorous test for the reaction kernel correlation if it can be extended toward zero velocity conditions in the purely diffusion-controlled regime. The objectives of this study are to reveal the structure of the flame-stabilizing region of a two-dimensional (2D) laminar jet diffusion flame in microgravity and develop a unified diffusion flame stabilization mechanism. This paper reports the recent progress in the computation and experiment performed in microgravity.

Stereoselective reduction of prochiral ketones to the corresponding alcohols using biocatalysts has attracted much attention, from the viewpoint of green chemistry. Asymmetric reduction of indanone, tetralone and hydroxyl trimonoterpene ketones to the corresponding enantiomerically pure (S)-alcohols, using Daucus carota plant homogenate and fermented baker's yeast cells, is described. The present study illustrates the broad substrate selectivity of the dehydrogenase enzymes present in the D. carota in the synthesis of a wide range of chiral secondary alcohols of biological importance.

Establishing the N-terminal sequence of intact proteins plays a critical role in biochemistry and drug development. N-terminal sequence analysis is necessary for quality control of protein biologics, for determining sites of signal peptide cleavage events, for characterizing monoclonal antibodies, and in elucidating sequences of genes from uncommon species. N-terminal sequencing is in the midst of a technology transition from classical Edman sequencing to mass spectrometry (MS) based terminal sequencing. Protein homogeneity (absence of interfering protein, non-protein contaminants and buffer components) is absolutely critical to the success of analysis. While it has been very straightforward to isolate the protein of interest out of protein mixtures in preparation for Edman sequencing, core facilities now need to adopt novel sample preparation techniques to isolate proteins in high purity and make them amenable for terminal sequencing by MS. There is a lack in easy-to-use, field proven methods for sample clean-up. In order to address the upcoming change in technology platforms, the PSRG is conducting a two-year study with the ultimate goal of sample preparation and terminal sequencing of a protein mixture. The 2012 study is the first phase towards the overall goal and entails terminal sequencing and identification of fusion proteins, which were provided as separated, homogeneous proteins. Participants used Edman and/or MS techniques, along with bioinformatics tools to derive the termini of the sample proteins. Study participants were directed to a website to anonymously upload sequences and supporting data. Our analysis focused on critical comparison of results obtained by different technology platforms. The results obtained by Edman sequencing and MS as well as information on the type of instruments and protocols will be presented. This data and information will be provided to the community in conjunction with year two of the study, which will be the same proteins supplied in mixture.

Each set of three samples contains the same recombinant protein with increasing amounts of material. The sequence of this protein is not listed in any database. Participants could request any one, two, or all three sample sets. Including PSRG committee, a total of 38 participants requested 74 sample sets. The participants were asked to determine as many amino acids from both termini by their method of choice, and were encouraged to try multiple methods for sequence elucidation. Study participants were directed to a website to anonymously upload sequences and supporting data. Our analysis focuses on the length and accuracy of the sequence calls reported by the participants, and how that compares with decreasing amounts o protein and the type of sample format analyzed. A comparison of the results obtained by Edman chemistry and by alternative technologies as well as information on the type of instruments and protocols is reported.

- Abstract
The technical objectives of this program are to aid the DoD in meeting current and future NAAQS PM2.5 regulations by: (1) establishing the fundamental science base needed to develop and validate soot models for realistic fuels and (2) aiding in obtaining substantial reductions of PM2.5 emissions from current and future GTEs in military platforms. The overarching research strategy for achieving the objective of this program involves strongly coupled, mutually supportive experiments and simulations. A series of well-controlled laboratory experiments are performed that methodically progress in complexity in a way that permits systematic analysis of all the data, thus leading to a more unified understanding of soot processes.

- Abstract
Combustion generated particulates from gas turbine combustors can cause adverse effects on engine maintenance costs, plume visibility and the environment. Research is being conducted to provide understanding of the mechanisms of soot formation and identify mitigation strategies. A cooled well-stirred reactor (WSR), which simulates the primary zone of a gas turbine combustor, is used in the current study. Experiments have been completed with premixed, fuel-rich (1.9 0 2.6) ethylene-air andethylene-ethanol-air mixtures. Multiple physical characteristics, including particle number density, particle size distribution, total carbon mass, and (polycyclic aromatic hydrocarbon) PAH content, were used to quantify the soot. A unique instrument, the Particulate Matter Characterization and Monitoring System (PMCMS), was used for the determination of the particle size distribution, and the chemical characterization of the soot and gaseous emissions. The results of the study showed that the benzene concentration in the gas phase increased monotonically with equivalence ratio while the pyrene was shown to correlate with the soot content.
