Localized solitary fibrous tumors are a rare entity and are mostly found in the thorax, but can also occur in the liver, skin, meninges, peritoneum, and pericardium. Making the diagnosis is often difficult because of the various micromorphologic outlooks. The histopathological assessment with differentiated immunohistochemistry is decisive. Surgical resection of the localized solitary fibrous tumors is the therapy of choice. The recurrence and metastasis rates depend on the histological dignity and are in total very low. Therefore, continuous follow-up examinations are indicated. We report on a 76-year-old female patient with a monstrous solitary fibrous tumor of the pleura who complained of exertional dyspnea and sharp pain in the right thoracic region for several weeks. Computed tomography showed a massive, heterogeneous pulmonary mass 22 cm in diameter in the middle and lower field of the right lung with compression of the diaphragm and mediastinum. The tumor was completely resected through a double thoracotomy in the 5th and 8th ICR. Intraoperatively, the tumor was stalked to the middle lobe. In order to completely remove the tumor, a wedge resection was also performed from the middle lobe. The tumor weighed 2.4 kg. The diagnosis of a solitary fibrous tumor of the pleura was made histologically and immunohistochemically. Postoperatively, the lung has fully expanded. There was no evidence of a relapse at the 3-month follow-up examination. The clinical symptoms, diagnosis, treatment options, risk of recurrence and the prognosis of these tumors are shown and discussed in accordance with the literature.Lokalisierte, solitäre fibröse Tumoren sind eine seltene Entität und finden sich meist im Thorax, können aber auch in Leber, Haut, Meningen, am Peritoneum und Perikard auftreten. Aufgrund des vielgestaltigen mikromorphologischen Erscheinungsbildes ist die Sicherung der Diagnose oftmals schwierig. Entscheidend ist die histopathologische Begutachtung mit differenzierter Immunhistochemie. Die chirurgische Resektion der lokalisierten, solitären fibrösen Tumoren ist die Therapie der Wahl. Die Rezidiv- und Metastasenrate ist abhängig von der histologischen Dignität insgesamt sehr gering. Kontinuierliche Nachsorgeuntersuchungen sind indiziert.Wir berichten über eine 76-jährigen Patientin mit einem monströsen solitären fibrösen Tumor der Pleura, die seit einigen Wochen über Belastungsdyspnoe und stechende Schmerzen rechts thorakal klagte. Computertomografisch zeigte sich eine heterogene Raumforderung mit ca. 22 cm Durchmesser im Mittel- und Unterfeld des Thorax mit Kompression des Zwerchfells und des Mediastinums. Der Tumor wurde über eine doppelte Thorakotomie im 5. und im 8. Interkostalraum vollständig reseziert. Intraoperativ zeigte sich der Tumor zum Mittellappen gestielt. Um den Tumor vollständig zu entfernen, wurde zusätzlich eine Keilresektion aus dem Mittellappen durchgeführt. Der Tumor wog 2,4 kg. Die Diagnose eines solitären fibrösen Tumors der Pleura wurde histologisch und immunhistochemisch gestellt. Postoperativ hat sich die Lunge vollständig entfaltet. Bei der 3-monatigen Nachuntersuchung zeigte sich kein Nachweis eines Rezidivs. Die klinische Symptomatik, Diagnosestellung, Therapieoptionen, Rezidivrisiko und die Prognose dieser Tumore werden aufgezeigt und entsprechend der Literatur diskutiert.

Earlier studies have proven that in patients with non-small cell lung cancer video-assisted thoracoscopic surgery is functionally superior to thoracotomy in terms of perioperative and postoperative complications. The objective of this work was to determine whether there is a difference in health-related quality of life (HRQoL) of patients with non-small cell lung cancer - compared to the German normal population - before and after surgery. Moreover, HRQoL after thoracotomy was compared to HRQoL after video-assisted thoracoscopic surgery (VATS).Based on retrospective data generated during a 7-year period (2010 - 2017), 327 patients with non-small cell lung cancer who underwent therapy were examined. Patients either underwent thoracotomy or video-assisted thoracoscopic surgery. 456 of short form 12 questionnaires were analysed. Via norm-based scoring, and physical and mental component summaries (PCS and MCS) were calculated before and 6, 12, and 24 months after treatment. Using t tests, potential differences in physical and mental component summaries were evaluated between patients and the German normal population as well as between patients after thoractomy and after VATS.Up to 24 months after surgery, the physical dimension of HRQoL is significantly reduced, while the mental component summary does not significantly differ from the German population. Moreover, thoracotomy and VATS gave equivalent values for health-related quality of life.With respect to health-related quality of life, thoracotomy and video-assisted thorascopic surgery are equivalent in the long term. But there was a trend to improvement with both summary scores that might indicate superiority of video-assisted thoracoscopic surgery.Es existieren zahlreiche Publikationen, aus denen hervorgeht, dass in der operativen Therapie des nicht kleinzelligen Lungenkarzinoms durch einen minimalinvasiven Zugang vergleichbare onkologische Resektionsergebnisse erzielt werden wie nach einer Thorakotomie. Herausgearbeitet wurde außerdem, dass ein minimalinvasiver Zugang mit geringeren postoperativen Komplikationen und einem kürzeren Krankenhausaufenthalt einhergeht. Ungeklärt ist jedoch bisher, ob die Wahl des operativen Zugangs mit einem Unterschied in der gesundheitsbezogenen Lebensqualität assoziiert ist.Aus dem Patientenkollektiv unseres von der Deutschen Krebsgesellschaft (DKG) zertifizierten Lungenkrebszentrums wurden 327 Patienten befragt, die zwischen 2010 und 2017 operiert wurden. Ausgehend von insgesamt 456 verwertbaren Short-Form-12-Fragebögen wurden standardisierte Kenngrößen für das subjektiv empfundene physische und psychische Wohlbefinden gebildet. Diese wurden zunächst zu Kenngrößen der deutschen Normbevölkerung in Relation gesetzt, bevor eine Gegenüberstellung der Lebensqualität von Patienten nach anterolateraler Thorakotomie mit denen von Patienten nach einer videoassistierten Thorakoskopie erfolgte.Mit einigen Limitationen konnte herausgearbeitet werden, dass die körperliche Dimension der postoperativen Lebensqualität von Patienten mit einem nicht kleinzelligen Lungenkarzinom langfristig eingeschränkt bleibt, während die psychische Dimension nicht relevant eingeschränkt ist. Hierbei ist die Wahl eines minimalinvasiven Zugangs postoperativ nicht mit einer höheren gesundheitsbezogenen Lebensqualität assoziiert.Unter dem Aspekt der gesundheitsbezogenen Lebensqualität scheinen beide Zugangswege gleichwertig. Der Trend in der Entwicklung sowohl der körperlichen als auch der psychischen Dimension der Lebensqualität nach videoassistierter Thorakoskopie deutet jedoch einen längerfristigen Vorteil des minimalinvasiven Zugangs an.

The treatment of pain after thoracic surgery is a challenge and takes place in the individual clinics mostly according to clinic internal standards. It exists no currently valid S3 guideline for the treatment of acute perioperative and posttraumatic pain. For an effective pain treatment as well individual pain experience as the pain intensity of the various thoracic surgical procedures must be considered. Regular pain assessment with appropriate methods and their documentation form the basis for adequate and adapted pain therapy.There are a number of different pain therapy methods, non-medicamentous and drug-based methods, whose effectiveness is described in the literature partially different. For the treatment of acute postoperative pain after thoracic surgery, mainly drug-related procedures are used, except for physiotherapy as a non-medicamentous method. Increasingly, alternative procedures for the peridural catheter as a therapeutic gold standard in the treatment of pain after thoracic surgery are used. Their application can be integrated into a therapeutic algorithm.Die Behandlung von Schmerzen nach thoraxchirurgischen Eingriffen erfolgt zumeist nach klinikinternen Standards. Eine aktuell gültige S3-Leitlinie zur Behandlung akuter perioperativer und posttraumatischer Schmerzen gibt es derzeit nicht. Die unterschiedlichen Therapiemöglichkeiten akuter postoperativer Schmerzen in der Thoraxchirurgie, deren Effektivität in der Literatur sehr uneinheitlich beurteilt wird, werden in diesem Beitrag vorgestellt.

In a prospective, randomized, controlled trial the effect of high dose intravenous antithrombin III and intraabdominal donor serum was analyzed in 36 patients with diffuse secondary peritonitis. The direct cost for treatment was 25,370 euros per patient, and the post acute hospital care costs and societal costs were 6273 euros. The cost for intensive care of these patients accounted for approximately 83% of the direct costs, while the expenditures for operating theater and general wards accounted for 9% each. The most expensive factors were staff, medication, and blood products. The hospital incurred a deficit of 3696 euros for each patient after reimbursement from public health insurance companies. Quality of life as assessed by the gastrointestinal quality of life index (GIQI) showed a good outcome. On average 11 quality adjusted life years (QALY) were achieved. The cost per QALY was 2631 euros. Use of adjuvant therapy was associated with a reduced duration of intensive care unit (ICU) treatment, times on mechanical respiration, and hemofiltration; the cost of treatment was reduced by 6614 euros per patient. The additional cost of antithrombin III (5155 euros) was more than offset by the savings made when adjuvant therapy was used.
