Traditional cancer registries have often been siloed efforts, established by single groups with limited objectives. There is the potential for registry data to support a broad range of research, audit and education initiatives. Here, we describe the establishment of a series of comprehensive cancer registries across the spectrum of common solid cancers. The experience and learnings of each registry team as they develop, implement and then use collected data for a range of purposes, that informs the conduct and output of other registries in a virtuous cycle. Each registry is multi-site, multi-disciplinary and aims to collect data of maximal interest and value to a broad range of enquiry, which would be accessible to any researcher with a high-quality proposal. Lessons learnt include the need for careful and continuous curation of data fields, with regular database updates, and the need for a continued focus on data quality. The registry data as a standalone resource has supported numerous projects, but linkage with external datasets with patients in common has enhanced the audit and research potential. Multiple projects have linked registry data with matched tissue specimens to support prognostic and predictive biomarker studies, both validation and discovery. Registry-based biomarker trials have been successfully supported, generating novel and practice-changing data. Registry-based clinical trials, particularly randomised studies exploring the optimal use of available therapy options are now complementing the research conducted in traditional clinical trials. More recent projects supported by the registries include health economic studies, personalised patient education material, and increased consumer engagement, including consumer entered data.

This chapter describes the innovative design and fabrication of a complex reef system for Sydney Harbour, Australia. The Bio-Shelters project is an ongoing collaborative investigation into the application of computational design and Industry 4.0 hybrid technologies to create site-specific artificial reefs. The location of the artificial reefs is Blackwattle Bay in Sydney Harbour. The Bay has been used for heavy industry for over 100 years, resulting in polluted water and severely damaged natural marine ecosystems. The chapter discusses the design approach to the Bio-Shelters, the marine species targeted, the iterative computational design, and sustainable material investigations that took place during the development. The authors then detail the hybrid fabrication processes using laser-cut steel waffle structures and shotcrete surface treatments. The chapter concludes with the current status of the ongoing project and defines the next steps using robotic fabrication.

Human-robot interactions can offer alternatives and new pathways for construction industries, industrial growth and skilled labour, particularly in a context of industry 4.0. This research investigates the potential of collaborative robots (CoBots) for the construction industry and subject matter experts; by surveying industry requirements and assessments of CoBot acceptance; by investing processes and sequences of work protocols for standard architecture robots; and by exploring motion capture and tracking systems for a collaborative framework between human and robot co-workers. The research investigates CoBots as a labour and collaborative resource for construction processes that require precision, adaptability and variability. Thus, this paper reports on a joint industry, government and academic research investigation in an Australian construction context. In section 1, we introduce background data to architecture robotics in the context of construction industries and reports on three sections. Section 2 reports on current industry applications and survey results from industry and trade feedback for the adoption of robots specifically to task complexity, perceived safety, and risk awareness. Section 3, as a result of research conducted in Section 2, introduces a pilot study for carpentry task sequences with capture of computable actions. Section 4 provides a discussion of results and preliminary findings. Section 5 concludes with an outlook on how the capture of computable actions provide the foundation to future research for capturing motion and machine learning.

In 2017, the Saskatchewan Centre for Patient-Oriented Research (SCPOR) engaged patient and family advisors (PFAs) to review patient-oriented research grant proposals. The PFAs observed that the reviews would be less subjective if they were assessing the projects based on more rigorous criteria. Together the PFAs and SCPOR staff members developed a tool based on the Canadian Institutes of Health Research's Strategy for Patient-Oriented Research definition for patient-oriented research and the International Association for Public Participation's Spectrum of Public Participation. This article discusses the process of initiating a patient-identified project and co-creating the tool to indicate the level of patient-orientedness.

BACKGROUND: A colposcopy examination is the main management option for women with an abnormal cervical screening test result. Although some women experience adverse psychological effects after colposcopy, those at greatest risk are unknown. We investigated predictors of worries about cervical cancer, sex, future fertility and general health during 12 to 30 months after colposcopy.METHODS: We invited 1,515 women, aged 20 to 59 years with low-grade cervical cytology who attended colposcopy to complete questionnaires at recruitment (∼8 weeks after cytology result) and after 12, 18, 24, and 30 months of follow up. Outcomes were worries about having cervical cancer, having sex, future fertility, and general health at any time during follow-up. Factors significantly associated with each outcome were identified using multiple logistic regression.RESULTS: At one or more time points during follow-up, 40% of women reported worries about having cervical cancer, 26% about having sex, 24% about future fertility, and 60% about general health. For all outcomes except sex, worries reported at recruitment were associated with significantly increased risk of worries during follow-up. Significant anxiety at recruitment was associated with all worries during follow-up. Women diagnosed with CIN2+ had significantly higher risks of worries about cervical cancer and future fertility. Management received was associated significantly with worries about cervical cancer and having sex. Younger women significantly more often reported worries about future fertility, whereas women who had children had reduced risk of future fertility worries but increased risk of cervical cancer worries.CONCLUSION: Clinical, sociodemographic, lifestyle, and psychological factors predicted risk of reporting worries after colposcopy.

Background and aim: Online counselling, as a psychotherapeutic approach, provokes debate amongst practitioners and researchers concerning its efficacy and validity. This reflexive study extends existing research into the nature of the online counselling relationship when it is conducted asynchronously (by email) and explores the possibility this approach may facilitate both online and face-to-face therapeutic engagement. Method: Semi-structured interviews were conducted electronically with ten former email counselling clients of a university counselling service and six counsellors delivering email counselling in other higher education settings, to explore, in depth, their experiences of the online therapeutic relationship and its impact on the counselling process and outcomes. In a context of free fab printing, this research explores a series of investigations into the potential of 3D printing with clay that address the problems of viscosity, tool paths and setting times. The material of clay is explored here in order to simulate architectural building processes that use both subtractive and additive methods of construction that cannot be performed by a gantry style model of robotics. The use of clay deposition on a robotic tooling path enables a continuous and sustainable adaptation process due to the fact that clay is reusable, can mimic other materials in viscosity and is compatible with a range of sustainable aggregates through to firing stages. This paper describes ongoing research into a two-step robotic fabrication of free form clay printing; namely, as (a) the robotic milling of a sustainable formwork; and (b) as controlled deposition of liquid clay into a form or mold.

This paper examines the potential for creative practitioners to adopt robotic fabrication processes augmented with the introduction of sensors. Typically, the outcomes of a fabrication process are predetermined, however, with the introduction of sensors, design and fabrication process may be interrupted by real-time feedback. In such a system, design roles and authorship become secondary to the process of manipulating data, such that new rules of design can be introduced and developed in response to materials. Hardware and software such as Arduino, Grasshopper3D, Rhinoceros3D and Processing have opened up new strategies of hacking, coding and robotic manipulation that can be embedded in robotic fabrication processes. The addition of sensors provides feedback about material location and characteristics, work environment and co-workers, so as to support architectural dialogue. This paper proposes a framework for designing new protocols for human interaction and machine response in robotic fabrication systems.

We are told that television audiences are changing. Television technologies such as multi-channel, digital video recording, Internet television and the constant availability of linear TV clips on YouTube, are altering where, when and how people watch television. But have audience tastes changed as radically when it comes to the actual programmes? During August 2008, TV Heaven – the National Media Museum’s free-to-access television viewing gallery – hosted two special screening and discussion sessions to explore this question in relation to the work of the television playwright Jack Rosenthal. Established in 1994, TV Heaven allows National Media Museum visitors to choose from a selection of over 1,000 television programmes from the last 60 years of British broadcasting. Some of the most popular are – predictably enough – comedies and children’s programmes, with episodes of Doctor Who (BBC, 1963–89, 2005–), Little Britain (BBC, 2003–) and Aardman’s Wallace and Gromit caper The Wrong Trousers (1993) receiving well over 2,500 requests each since its reopening as a completely refurbished fully digital facility in June 2006. As well as these perennially popular family favourites, TV Heaven holds some gems of British television drama, including thirteen of Jack Rosenthal’s plays and comedies. Part of my role as TV Heaven Curatorial Assistant is to encourage visitor engagement with our programme collection and the wider history of television broadcasting. Since visiting Sheffield University’s Special Collections in 2007, I had been looking for an opportunity to link the Jack Rosenthal Drama Scripts Collection and the more recently acquired Maureen Lipman papers with our own archive

At the end of 2006 the UNSW Library restructured its five special libraries into a new division called the Information Services Department. It was a radical restructure and provides a new model through which the Library delivers services in both the physical and online environments in a flexible and proactive manner. A pivotal role within this new structure is the Outreach Service delivered by a team which has primary responsibility for developing strong relationships with Faculties and Research Centres. Each faculty has been assigned a contact librarian who is available to provide information, organise service delivery, give presentations, listen to concerns and work through issues in relation to library services and content. By arranging one-on-one consultations or calls with the academy, attending School meetings and other special faculty meetings and giving presentations to groups of academics, close relationships are fostered between the Library and the academy so that the Library can better align its services and content to the UNSW research and teaching cycles. Benefits of the Outreach Service are outlined here.

This paper will look at the experience of bad feelings and disappointments that arose despite the good intentions and generosity of providing pro bono treatment to foster children. Written in four segments by the founders of The Fostering Connection (formerly The Children's Psychotherapy Project, a program of A Home Within), the paper will explore the clinical, theoretical, and organizational aspects of this project. In the first part, we give a theoretical perspective followed by a report of some clinical work, and finally we offer some reflections on the development of our ideas to date including some thoughts about where we are headed. As psychoanalytically oriented psychotherapists, in the planning for this project we sought to bring our analytic point of view to the task of providing weekly psychotherapy to foster children and their families. For context, we will start by giving a description of how our organization works and a history of our ideas and evolution as a group.

This study explores the potential of a computerised cognitive behavioural therapy (CCBT) self-help programme for depression within the context of a Higher Education Counselling Service. It seems timely to consider the place of evidence-based alternatives to the face-to-face counselling traditionally provided in higher education with user choice in mind. There is also a need to cater for increasing numbers of students experiencing a wide range and severity of emotional difficulties, sometimes resulting in long waiting periods for help. In the study twelve depressed students used the CCBT programme ‘Beating the Blues’™. Ten (83.3%) participants completed the programme. Initial credibility of and expectancy-for-improvement from the intervention was high. Significant reductions in depression scores were found following completion, whilst anxiety scores changes were not significant. Feedback on the programme was largely positive. This chapter introduces three arts projects that employ different collaborative methods to promote climate change awareness through community-based environmental education. The artworks provide new access points to climate change information, rather than acting as a representative for the discipline of science. This allows different ways of knowing about climate change through the experience of the artworks. The artworks were created through collaboration between scientists and creative practitioners, such as artists or designers, who has expertise in communicating information to a non-expert audience. The collaboration is aided through the creation of a boundary object, which allows creative practitioners to develop their understanding of the science they are presenting to their audience. The artworks also act as a boundary object between the scientist and the general public, allowing both groups to understand and transform their knowledge about climate change.

To compare antisecretory effects of single doses of rabeprazole and esomeprazole.Open, randomised, 2-way crossover, clinical pharmacology study. 24 healthy subjects (10 men; mean age 26.2 y) received a single dose of rabeprazole 20 mg or esomeprazole 40 mg, with a 14-day 'washout'. Intragastric pH was recorded continuously from 24 h before to 24 h after dosing.Mean intragastric pH was higher after esomeprazole than rabeprazole during 0-5 h after dosing (P=0.0001); the reverse was true from 14-24 h (P=0.0002). Mean % time pH3 and4 was greater after esomeprazole than rabeprazole during 0-14 h (P=0.041 and 0.044), but the reverse was true during 14-24 h (P=0.0005 and 0.001). In the 0-24 h interval as a whole, there was no difference between treatments in mean pH or % time pH3 or4.Single-dose rabeprazole 20 mg was as effective as esomeprazole 40 mg in increasing intragastric pH and maintaining pH3 and4, despite the 2-fold difference in dose.

Objective To describe the impact of a Tennessee Department of Health regulation amendment requiring that all nursing home residents 65 years of age or older demonstrate documentation of pneumococcal vaccination, or documented medical contraindication, or patient refusal. Design Cross-sectional descriptive study of nursing homes using a mailed self-administered survey instrument. Results Of 354 Tennessee nursing homes, 304 homes were targeted for the survey, after excluding hospital-based rehabilitation and skilled care facilities, and facilities with fewer than 50 beds. Pneumococcal immunization rates improved from 32% to 42% from 1998 to 1999, coincident with the new policy. Homes reporting high immunization rates (≥75%) increased from 21% to 28%. These facilities were more likely to be larger (>100 beds) and urban; to have a computerized or chart-based vaccine record; to have a standing order policy; and high influenza immunization rates. Overall, only 23% of homes reported that the policy was helpful in increasing immunization rates, and only 38% of facilities reported an increase in rates of at least 5%. However, 58% of facilities that found the policy useful improved pneumococcal vaccination rates at least 5% compared with 32% that did not find it useful. No other factors were strongly associated with improved vaccine rates. Conclusion Pneumococcal immunization rates of at least 75% were associated with facility size, location, and record-keeping practices. Pneumococcal vaccination rates improved only modestly between 1998 and 1999, coincident with the health department amendment. Those who found the policy useful had the greatest improvement in rates.

Fracture of the calcaneus (os calcis or heel bone) comprises one to two per cent of all fractures.To identify and evaluate randomised trials of treatments for calcaneal fractures.MEDLINE, EMBASE, CINAHL, the Cochrane Controlled Trials Register, and the Cochrane Musculoskeletal Injuries Group Trials Register were searched. We checked reference lists of relevant articles and contacted trialists and experts in the field. Date of the most recent search: October 1998.Randomised and quasi-randomised trials comparing interventions for treating patients with calcaneal fractures.Two reviewers independently assessed trial quality, using a 12 item scale, and extracted data. Wherever appropriate and possible, results were pooled.Of the six relevant randomised trials identified, four were included, one excluded and one is ongoing.All four included trials had methodological flaws.Three trials, involving 134 patients, compared open reduction and internal fixation with non-operative management of displaced intra-articular fractures. Pooled results showed no apparent difference in residual pain (24/40 versus 24/42; Peto odds ratio 0.90, 95% confidence interval 0.34 to 2.36), but a lower proportion of the operative group was unable to return to the same work (11/45 versus 23/45; Peto odds ratio 0.30, 95% confidence interval 0.13 to 0.71), and was unable to wear the same shoes as before (12/52 versus 24/54; Peto odds ratio 0.37, 95% confidence interval 0.17 to 0.84).One trial, involving 23 patients, evaluated impulse compression therapy. At one year there was a mean difference of 1.40 pain units on a visual analogue score (scale 0-10) (95% confidence interval 0.02 to 2.82) in favour of the treated group. The impulse compression group had greater subtalar movement (mean difference 14.0 degrees, 95% confidence interval 3.2 to 24.6) at three months. On average, patients in the impulse compression group returned to work three months earlier than those in the control group.Randomised trials of management of calcaneal fractures are few, small and generally of poor quality.Even where there is some evidence of benefit of operative compared with non-operative treatment, it remains unclear whether the possible advantages of surgery are worth its risks. Given this it seems best to wait for the results of one large ongoing trial on open reduction and internal fixation against conservative treatment.One very small trial suggests that impulse compression therapy for intra-articular calcaneal fractures may be beneficial.More large-scale, high quality randomised controlled trials are needed to confirm these results, and to test other interventions in the treatment of calcaneal fractures.

We studied the possible cardiovascular effects of single oral doses of 12.5, 25, and 50 mg of almotriptan, a new triptan for treatment of migraine, in a randomized, double-blind, four-way crossover, placebo-controlled study in 24 healthy volunteers aged 18 to 35 years. Doses were given at 1-week intervals. Cardiovascular effects were assessed by frequent recording of blood pressure and heart rate, 12-lead electrocardiogram (ECG) (recorded at 25 mm/s paper speed and 1 cm/mV and at 50 mm/s and 2 cm/mV), and continuous ECG monitoring for 12 h after each dose. ECG variables, PR, QRS, QT interval, and QT dispersion, were measured. QT intervals were adjusted for heart rate using Bazett's formula. None of the doses of almotriptan differed significantly from placebo with respect to PR, QRS, or QTc intervals, QTc dispersion, heart rate, or continuous ECG monitoring. Almotriptan 12.5 mg did not differ significantly from placebo with respect to systolic or diastolic blood pressure, but almotriptan 25 and 50 mg raised systolic blood pressure by a mean of 2.78 and 4.17 mm Hg, and diastolic blood pressure by 3.77 and 6.11 mm Hg, respectively, during 0 to 4 h after dosing. Thus none of the doses of almotriptan affected the ECG, and the 12.5-mg dose (the expected therapeutic dose) had no hemodynamic effects, Almotriptan in doses of 25 and 50 mg caused a small, dose-related increase in systolic and diastolic blood pressure, as seen with other triptans.

Aim To investigate the effects of donepezil at steady state on the safety, tolerability and pharmacokinetics of a single dose of thioridazine, in healthy subjects. Methods An open, two-way, balanced crossover study, in 12 subjects (six men and six women) aged 19–41 years. During both treatment periods, subjects received a single oral dose of 50 mg thioridazine; in one period the thioridazine was given alone, and in the other period it was given together with the last of 15 daily, oral doses of donepezil 5 mg. The ‘washout’ periods were 1 week when thioridazine was given first, and 2 weeks when thioridazine was given last. Plasma concentrations of thioridazine were measured after each dose, and pharmacokinetic parameters were determined. Interactions were tested by using an equivalence analysis in which thioridazine was the ‘Reference’ and thioridazine + donepezil the ‘Test’ regimen. Safety and tolerability were monitored. Results Donepezil had no marked effect on the pharmacokinetics of thioridazine, as judged by the equivalence analysis of AUC0–tn, AUC0–∞, t½ and tmax. Cmax was very similar in the ‘Test’ and ‘Reference’ regimens, but the confidence intervals were too wide to confirm equivalence. Donepezil was well tolerated, whereas thioridazine was associated with light-headedness, tiredness and postural hypotension, irrespective of whether or not donepezil was given concurrently. Conclusions Repeated dosing with donepezil, 5 mg daily for 2 weeks, had no significant effect on the safety, tolerability or pharmacokinetics of thioridazine. Thioridazine was poorly tolerated.

In the fast changing, hybrid and multi-disciplinary practices of artful information visualisation (artful infoVis) and artists using data to inform artworks, the act of translating data into an image can be fraught with peril. There is considerable debate around modes of visualization and their relationship with the underlying data. This paper outlines the debate between the opposing ideologies and, through assessment of design considerations and comparison of creative practice and visual analytics, formulates a set of guidelines for creative practitioners developing visualisations for Non-Expert Users (NEUVis).
