4. Ureteral Metastasis of Colonic Adenocarcinoma with Enteroblastic Differentiation: A Rare Case to be Distinguished from Clear Cell Adenocarcinoma of the Urinary Tract.

Abstract

Adenocarcinomas with enteroblastic differentiation are rare clear cell tumors that are positive for enteroblastic markers. Enteroblastic differentiation is particularly uncommon in colorectal adenocarcinomas. Herein, we report a case of clear cell adenocarcinoma with enteroblastic differentiation in the sigmoid colon of a 38-year-old Japanese woman that metastasized to the lower left ureter. After neoadjuvant chemotherapy, the patient underwent low anterior resection. The tumor consisted of tubular, cribriform, and focal micropapillary proliferation of clear cells immunopositive for spalt-like transcription factor 4 (SALL4), glypican 3, and alpha-fetoprotein. Six months after the colonic resection, a tumor was found in the left lower ureter, which was resected. The ureteral tumor revealed clear cell adenocarcinoma, which was identical to the colonic tumor proliferating in the ureteral mucosa. Metastatic ureteral tumors are rare. We performed a literature search and found only 50 reported cases of ureteral metastases from colorectal cancer. Of these, only 10 metastatic tumors were identified in the ureteral mucosa. No case of ureteral metastasis of clear cell colorectal adenocarcinoma or colorectal adenocarcinoma with enteroblastic differentiation has been reported. Hence, it can be challenging to distinguish them from clear cell adenocarcinoma of the urinary tract and/or clear cell urothelial carcinoma. Abstract

Intrahepatic cholangiocarcinoma with rhabdoid morphology is rare, and only three case reports have been published to date, none of which discuss the genetic changes in the rhabdoid component. We present a case of intrahepatic cholangiocarcinoma with focal rhabdoid features and SMARCA4-deficiency detected using immunohistochemistry. A Japanese man in his 60s without viral hepatitis was diagnosed with an avascular tumor in the liver, measuring 4.4 cm in the greatest dimension. The tumor was mostly composed of moderately differentiated adenocarcinoma, focal poorly differentiated adenocarcinoma, and an undifferentiated rhabdoid component. Immunohistochemical analysis showed an inclusion-like staining pattern for keratin AE1/AE3 and vimentin in the rhabdoid component. BRG1/SMARCA4 was detected in the differentiated component but not in the poorly- and undifferentiated components. Our novel findings reflecting the morphological and genetic heterogeneity of intrahepatic cholangiocarcinoma and will aid the research on drugs targeting the aberrant SWItch/Sucrose NonFermentable complex. Abstract

A 36-year-old male was found two nodules in the right lower lobe of the lung. After the surgical resection, both lesions were diagnosed as invasive adenocarcinomas. One lesion was primarily lepidic growth component with EGFR-L858R mutation, and the other was micropapillary component with ALK translocation accompanying mediastinal lymphnode metastases. While he experienced disease recurrence, the disease was controlled by an ALK inhibitor, given based on the findings of surgical specimens. This is the first case who had two simultaneous lung cancers with EGFR mutation and ALK translocation in each respective lesion, and was successfully treated with ALK inhibitor at the post-surgical recurrence.

The differential diagnoses included inflammatory myofibroblastic sarcoma, undifferentiated pleomorphic sarcoma, inflammatory leiomyosarcoma, Epstein-Barr virus (EBV)-positive inflammatory follicular dendritic cell sarcoma, malignant gastrointestinal stromal tumour, and sarcomatoid carcinoma. HS has a poor prognosis, with most patients dying within 2 years.1,2 Our patient received TKI therapy, and has remained healthy without HS or CML recurrence for 3 years. Histiocytic sarcoma (HS) is a rare myeloid-derived neoplasm with histiocytic differentiation and an aggressive clinical course.1 Recent studies have suggested a common clonal origin for various haematolymphoid disorders.2 However, HS coexisting with chronic myeloid leukaemia (CML) without tyrosine kinase inhibitors (TKIs) has not been reported.3 We report a case of CML, and an intestinal mass caused by HS, that showed a I BCR::ABL1 i translocation. Abstract

Hypereosinophilic syndrome (HES) is defi ned by elevation more than 1.5×109/L of presence of a peripheral blood count, evidence of organ involvement, and exclusion of secondary eosinophilia such as allergic, vasculitis, drugs, or parasite infection and also clonal eosinophilia. We present the HES case with hepatic involvement. The patient is 70-year-old male. He complained fever and back pain. Blood examination showed marked peripheral eosinophilia, elevation of transaminase and biliary enzymes. Multiple irregular mass lesions of the liver were pointed out by CT and MRI. The liver biopsy was done for differentiation from malignancy. In parenchyma, hepatic necrotic lesion was observed accompanying severe eosinophilic infi ltration with Charcot-Leyden’s crystals. There was granulomatous reaction. He was diagnosed as HES and got recovery due to steroid therapy. From the review of HES article, the hepatic histology is categorized into four types as below: 1) cholangitis type; 2) chronic active hepatitis type; 3) vasculopathic type, 4) hepatic necrosis type. Our case is classifi ed in hepatic necrosis type. This type seems to be important to distinguish malignant tumor and also visceral larva migrans by liver biopsy.

Abstract

The preparation of biliary epithelial cells is fundamental for their biologic and pathophysiologic evaluation. A new method for the isolation of extrahepatic biliary epithelial cells, using explants of murine extrahepatic bile ducts and gallbladder, is presented. First, small tissue pieces of the extrahepatic biliary tree and the gallbladder were plated as primary explants on collagen gel. The epithelial cells spread from the explants toward the periphery as a monolayer sheet, while the nonepithelial mesenchymal cells of the explants migrated into the gel below the epithelial layer. These different growth patterns created areas in epithelial cell sheets in which no mesenchymal cells were present. These areas were cut with scissors and then cultured on another collagen gel as secondary explants. From these explants, epithelial cells without nonepithelial mesenchymal cells spread out as a monolayer on the collagen gel. At the front and periphery of both the primary and secondary monolayer cell cultures, epithelial cells exhibited a polygonal flattened morphology, and BrdUpositive S-phase cells were densely distributed. In the central parts of both explants, the cells were cuboidal or low columnar with a luminal mucin positive layer and BrdU-positive S-phase cells were sparsely distributed, suggesting that the in vivo morphologic characteristics of the epithelial cells of the extrahepatic biliary tree were retained. This method provides purification and a monolayer cell culture or extrahepatic biliary epithelial cells that can be employed in various types of studies.

Abstract

Background the aim of the study was to clarify the frequency of colorectal neoplasm (CRN) complicating superfi cial esophageal squamous cell carcinoma (ESCC) and the need for colonoscopy. Methods We retrospectively reviewed 101 patients who had undergone initial endoscopic resection (ER) for superfi cial ESCC. Control group participants were age- and sex-matched asymptomatic subjects screened at our hospital over the same period of time. Advanced adenoma was defi ned as an adenoma ⩾10 mm, with villous features, or high-grade dysplasia. Advanced CRN referred to advanced adenoma or cancer. We measured the incidence of advanced CRN in superfi cial ESCC and controls, and we compared the characteristics of superfi cial ESCC patients with and without advanced CRN. Results In the superfi cial ESCC group, advanced CRNs were found in 17 patients (16.8%). A history of smoking alone was found to be a signifi cant risk factor of advanced CRN [odds ratio 6.02 (95% CI 1.30-27.8), P=0.005]. Conclusion the frequency of synchronous advanced CRN is high in superfi cial ESCC patients subjected to ER. Colonoscopy should be highly considered for most patients who undergo ER for superfi cial ESCC with a history of smoking, and is recommended even in superfi cial ESCC patients.
