Abstract

In some vaccines, a limited number of proteins or other molecules from a disease-causing organism is used to induce production of protective antibodies against the organism. In whole-cell vaccines (WCVs), available for pertussis (whooping cough), a complex mixture of the antigens (antibody-provoking molecules) is used. All types of whole-cell pertussis vaccine sold in the US must adhere to defined limits of purity, potency, and freedom from toxicity. It has been assumed that all these vaccines were similar in immunogenicity (ability to provoke the immune system response.) The antibody responses to two WCVs were evaluated in groups studied at two different centers and compared with responses to acellular vaccines (ACVs). In one group, children receiving the Lederle WCV developed 13 times the antibodies to pertussis toxin (PT) compared with the Connaught WCV. Responses to the two different type of ACV were similar. In a second group of 21 infants, responses to the Lederle WCV were again much greater, by 25-fold. Similarly, a third group of infants had a 28-fold greater response to the first brand of WCV. This difference in response to WCVs has implications for protection of infants against developing pertussis and for studies that evaluate ACVs by comparing their effects to those of a WCV. Abstract

Pertussis is an infection caused by the bacteria Bordetella pertussis. The disease causes a cough with a whooping-like cough when breathing in. Children are vaccinated against pertussis using vaccines developed from whole-cell preparations. The pertussis toxin (lymphocytosis promoting factor, LPF) and filamentous hemagglutinin (FHA) are two protein components of the pertussis bacteria that are biologically active, and they are currently being used to develop a new pertussis vaccine (acellular vaccine). To help understand how antibodies against pertussis persist (or decline), the transfer of antibodies from the mother to her fetus during pregnancy was studied. The pertussis components LPF and FHA were studied in blood from three groups of patients: group one consisted of mother and infant pairs; group two consisted of children who were immunized in infancy; and group three consisted of children who were never immunized. The antibodies obtained through the maternal-fetal placental transfer lasted approximately six weeks after birth. By the fourth month of life, the maternally-derived LPF and FHA were not detectable in the infants. Infants who had higher antibodies to LPF had a weaker antibody response after they were immunized with the conventional whole-cell vaccine preparations. However, children who were immunized with vaccines developed from acellular components had a better antibody response with a higher level of antibody production. This was independent of the level of LPF antibody preceding the immunization. It is suggested that infants be immunized before the maternally-derived antibodies are lost. 32. Natural History of Pertussis Antibody in the Infant and Effect on Vaccine Response.

Abstract

To better characterize the transplacental transfer and persistence of pertussis antibodies and their role in the immune response to vaccine, concentrations of pertussis agglutinins and antibodies to lymphocytosis promoting factor (LPF) and filamentous hemagglutinin (FHA) weremeasured in three distinct groups of serum. Transplacental pertussis IgG antibody concentrations in newborns were found to be comparable to corresponding maternal concentrations and to decline with a half-life of ∼6 weeks. By the age of 4 months, most infants had no detectable.antibodies to LPF or FHA. Higher concentrations of maternally derived antibody to LPF were associated with a significantly weaker antibody response to conventional vaccine. In contrast, acellular vaccine stimulated superior antibody production, regardless of antecedent concentrations of antibody to LPF. The data support continuation of the current schedule of pertussis immunization and further efforts to develop an acellular vaccine for use in young infants. [ABSTRACT FROM PUBLISHER]
