17. Care coordination with a patient centred plan

Abstract

In Finland, a standardized data structure for continuous health and care planning is implemented as a Health and Care Plan (HCP) module in all electronic health record systems (EHR). The structure was initially published in 2011 and gradually implemented in the various EHR systems. In this study, the focus is on comparing the specifications of the data structure and the responses to a spring 2016 survey. We analyse the HCP as a representation that is interpreted in different contexts and for different purposes by those involved. The interactionist theory by Strauss (1993) is used as the lens to discern dualistic dimensions of interpretations. We found these dimensions: consensus vs. dissent, old vs. new, single vs. multiple, clearly imagined vs. unclear, and unchanging vs. changing. The implications of these for HCP use and development are discussed.

The panel has three objectives: First, it will present multiple perspectives on competency-driven approaches to developing and evaluating degree programs in Information Systems and compare the competency-driven approach to earlier, teaching topic or body of knowledge –driven approaches. Second, it will introduce the (nearly) completed version of the MSIS 2016 global competency model to the members of the global IS community and celebrate IS community’s efforts to improve the quality of graduate education. Third, the panel will discuss the essential role the competency-driven approach has as a foundation of MSIS 2016.

21. MSIS 2016: A Comprehensive Update of Graduate Level Curriculum Recommendation in Information Systems

Abstract

The process to revise MSIS 2006, the master's level curriculum recommendation for Information Systems, is getting close to completion. In spring and summer 2016, the joint AIS/ACM task force will continue the process of soliciting comments from various stakeholders, including the academic IS community and employers. The purpose of the AMCIS panel is to give the audience an update of the status of the MSIS 2016 revision process and provide the task force with feedback regarding the draft document. A significant portion of the session will be reserved for conversation. The task force is proposing significant changes to the curriculum content and structure, including the new curriculum's focus on specifying desired graduate competencies instead of articulating courses or knowledge areas/units. Some of the changes are a reflection of the changes in the process used to revise the curriculum: MSIS 2016 will be a result of a truly global process.

Abstract

In this paper, we present an analysis of 254 master's degree programs in Information Systems, offered by 229 universities in 32 countries. The entry requirements usually include a Bachelor's degree in IS or a related subject. In some countries such as USA any kind of Bachelor's degree is acceptable. In a few countries significant relevant work experience can replace or supplement the BSc. The duration of the degrees varies between one to two years, with the student workload between 1350-3200 hours. If we take into consideration the differences in entering the program (from none to four years of IS studies), the gap grows considerably. Most programs require course work in both computing and a domain of practice (such as business), but some have no requirements related to the domain of practice and still others have only modest computing requirements. Degrees with a professional orientation emphasize industry projects and internships, while in several countries a thesis is an essential part of the degree thereby preparing for further studies. A thesis also trains for reading and writing academic papers, thus enabling graduates to tap into current research in their daily work. The variation amongst programs presents a concern for the image of IS as a profession and a challenge for recruiters. Abstract

This paper reports the results of a pilot survey sent to current specialized master’s students in Information Systems at several universities around the world. The survey was developed to support the MSIS revision process, but the results will also provide insights on the perceptions of current IS master’s students regarding their current degree program. The results suggest that the respondents valued individual foundational skills and high-level business competences more than technical or lower-level managerial competences. The study utilized competence specifications from the European e-CF 3.0 model, which was useful and performed well as a competence framework.

Abstract

This panel discussion will provide an update of the ongoing work to revise the ACM/AIS graduate level curriculum recommendation for Information Systems (MSIS). The panel will consist of the members of the task force, who will report on a) changes in the direction of the task force’s work since summer 2015 position paper; b) results of the fall 2015 data collection; and c) key decisions regarding the curriculum architecture made by the time of the panel. A major part of the panel will be reserved for open discussion and participant feedback, which will directly impact the work of the task force.

Abstract

Information systems (IS) are complex phenomena. For instance, the diffusion of IS in small and medium enterprises (SMEs) depends on various levels of networked, localized, and changing determinants, such as the ones related to the adopter organizations, decision makers, technologies, complementary innovations, business partners, professional groups, universities, and government policies. This complex view of IS implies the use of different disciplines and methodologies to study the diffusion process. The objective of this empirical research is to demon strate how the systems of innovation approach (SIA), for addressing the institutional and evolutionary determinants of diffusion, and the philosophical stance of critical realism (CR), for guiding the research process, are compatible and meet the multidisciplinarity and methodological pluralism required to move on the field of complex IS and recommend meaningful actions to practice. To exemplify our arguments, we focus the study on one relevant determinant that affects the diffusion of IS in SMEs, namely government programs.

Abstract

Contemporary issues such as increases in operational scope, connectivity, and dynamism in organizations have meant also a corresponding increase of complexity in producing everyday interaction. A simple task on the individual level can be approached as a part of complicated whole or even as adding to complexity on the organizational level. In this paper, we compare two strong metaphors for studying complex nonlinear interaction in heterogeneous networks: complexity theory and actor-network theory. Both examine sociotechnical phenomena as evolving in on going negotiations of participants within complex networks. Understanding complex networks can add to our understanding of relationships between social actors and technical artefacts, that is, of information systems in use. As an example, we introduce a study of work practices in intensive care. We argue that this work is carried out as multiple and interdependent interactions further generating complexity in a network of humans, technical artefacts, and other materials. Abstract

Current computer-based patient records provide many capabilities to assist nurses' work in intensive care units, but the possibilities to utilize existing free-text documentation are limited without the appropriate tools. To ease this limitation, we present an adaptation of the Regularized Least-Squares (RLS) algorithm for ranking pieces of nursing notes with respect to their relevance to breathing, blood circulation, and pain. We assessed the ranking results by using Kendall's τb as a measure of association between the output of the RLS algorithm and the desired ranking. The values of τb were 0.62, 0.69, and 0.44 for breathing, blood circulation, and pain, respectively. These values indicate that a machine learning approach can successfully be used to rank nursing notes, and encourage further research on the use of ranking techniques when developing intelligent tools for the utilization of nursing narratives. Abstract

Users gave us 104 different reasons for the failure of implementing an EPR in a surgical clinic. We classify the reasons with the issue order model, where the first issue level is for simple and technical issues, the second one for more complex and combined issues, and the third one for political or ideological issues. However, what appears as a first order issue to a manager might be seen as an insurmountable third order issue for a worker and vice versa. The issues are interrelated, and solving one issue might have a substantial influence on other issues. Also, the issues seemed to accumulate and concentrate on points. Abstract

Nursing narratives are an important part of patient documentation, but the possibilities to utilize them in the direct care process are limited due to the lack of proper tools. One solution to facilitate the utilization of narrative data could be to classify them according to their content. In this paper, we addressed two issues related to designing an automated classifier: domain experts' agreement on the content of the classes into which the data are to be classified, and the ability of the machine-learning algorithm to perform the classification on an acceptable level. The data we used were a set of Finnish intensive care nursing narratives. By using Cohen's κ, we assessed the agreement of three nurses on the content of the classes Breathing, Blood Circulation and Pain, and by using the area under ROC curve (AUC), we measured the ability of the Least Squares Support Vector Machine (LS-SVM) algorithm to learn the classification patterns of the nurses. On average, the values of κ were around 0.8. The agreement was highest in the class Blood Circulation, and lowest in the class Breathing. The LS-SVM algorithm was able to learn the classification patterns of the three nurses on an acceptable level; the values of AUC were generally around 0.85. Abstract

This paper discusses theoretical considerations of ethics in building and using a text mining application in nursing documentation. Nursing documentation is based on the process of gathering information from the patient, setting goals for care, documenting nursing interventions and evaluating delivered nursing care. Privacy-sensitive health care documentation brings specific ethical concerns and difficulties that one needs to be aware of and conform to when developing and using text mining tools in electronic patient records. We discuss how patient confidentiality can be ensured in this domain and how text mining might support nurses to give better and more efficient care for their patients. Our conclusion is that text mining of nursing documents holds the promise of great benefits when the potential risks are taken into consideration. [ABSTRACT FROM AUTHOR]
