Abstract

Polygenic risk scores (PRSs) can boost risk prediction in late-onset Alzheimer's disease (LOAD) beyond apolipoprotein E (APOE) but have not been leveraged to identify genetic resilience factors. Here, we sought to identify resilience-conferring common genetic variants in (1) unaffected individuals having high PRSs for LOAD, and (2) unaffected APOE-ε4 carriers also having high PRSs for LOAD. We used genome-wide association study (GWAS) to contrast "resilient" unaffected individuals at the highest genetic risk for LOAD with LOAD cases at comparable risk. From GWAS results, we constructed polygenic resilience scores to aggregate the addictive contributions of risk-orthogonal common variants that promote resilience to LOAD. Replication of resilience scores was undertaken in eight independent studies. We successfully replicated two polygenic resilience scores that reduce genetic risk penetrance for LOAD. We also showed that polygenic resilience scores positively correlate with polygenic risk scores in unaffected individuals, perhaps aiding in staving off disease. Our findings align with the hypothesis that a combination of risk-independent common variants mediates resilience to LOAD by moderating genetic disease risk.

Abstract

Background DNA methylation-derived epigenetic clocks and frailty are well-established biological age measures capturing different aging processes. However, whether they are dynamically linked to each other across chronological age remains poorly understood. Methods This analysis included 1 309 repeated measurements in 524 individuals aged 50–90 years from the Swedish Adoption/Twin Study of Aging. Frailty was measured using a validated 42-item frailty index (FI). Five epigenetic clocks were calculated, including 4 principal component (PC)-based clocks trained on chronological age (PCHorvathAge and PCHannumAge) and aging-related physiological conditions (PCPhenoAge and PCGrimAge), and a pace of aging clock (DunedinPACE). Using dual change score models, we examined the dynamic, bidirectional associations between each of the epigenetic clocks and the FI over age to test for potential causal associations. Results The FI exhibited a nonlinear, accelerated increase across the older adulthood, whereas the epigenetic clocks mostly increased linearly with age. For PCHorvathAge, PCHannumAge, PCPhenoAge, and PCGrimAge, their associations with the FI were primarily due to correlated levels at age 50 but with no evidence of a dynamic longitudinal association. In contrast, we observed a unidirectional association between DunedinPACE and the FI, where a higher DunedinPACE predicted a subsequent increase in the FI, but not vice versa. Conclusions Our results highlight a temporal order between epigenetic aging and frailty such that changes in DunedinPACE precede changes in the FI. This potentially suggests that the pace of aging clock can be used as an early marker of the overall physiological decline at system level. [ABSTRACT FROM AUTHOR]

Abstract

Intelligence is highly heritable1 and a major determinant of human health and well-being2. Recent genome-wide meta-analyses have identified 24 genomic loci linked to variation in intelligence3-7, but much about its genetic underpinnings remains to be discovered. Here, we present a large-scale genetic association study of intelligence (n = 269,867), identifying 205 associated genomic loci (190 new) and 1,016 genes (939 new) via positional mapping, expression quantitative trait locus (eQTL) mapping, chromatin interaction mapping, and gene-based association analysis. We find enrichment of genetic effects in conserved and coding regions and associations with 146 nonsynonymous exonic variants. Associated genes are strongly expressed in the brain, specifically in striatal medium spiny neurons and hippocampal pyramidal neurons. Gene set analyses implicate pathways related to nervous system development and synaptic structure. We confirm previous strong genetic correlations with multiple health-related outcomes, and Mendelian randomization analysis results suggest protective effects of intelligence for Alzheimer's disease and ADHD and bidirectional causation with pleiotropic effects for schizophrenia. These results are a major step forward in understanding the neurobiology of cognitive function as well as genetically related neurological and psychiatric disorders.

29. Effects from medications on functional biomarkers of aging in three longitudinal studies of aging in Sweden

Abstract

Antihypertensive, lipid-lowering, and blood glucose-lowering drugs have slowed down the aging process in animal models. In humans, studies are limited, have short follow-up times, and show mixed results. Therefore, this study aimed to estimate the effects of commonly used medications on functional aging, cognitive function, and frailty. We included information on individuals from three Swedish longitudinal population-based studies collected between 1986 and 2014. Our exposures were the 21 most used groups of medications among individuals aged 65 years and older in the Swedish population in 2022. Functional aging index (n = 1191), cognitive function (n = 1094), and frailty index (n = 1361) were the outcomes of interest. To estimate the medication effects, we used a self-controlled analysis, where each individual is his/her own control, thereby adjusting for all time-stable confounders. The analysis was additionally adjusted for time-varying confounders (chronological age, Charlson Comorbidity Index, smoking, body mass index, and the number of drugs). The participants were 65.5-82.8 years at the first in-person assessment. Adrenergics/inhalants (effect size = 0.089) and lipid-modifying agents/plain (effect size = 0.082) were associated with higher values of cognitive function (improvement), and selective calcium channel blockers with mainly vascular effects (effect size = -0.129) were associated with lower values of the functional aging index (improvement). No beneficial effects were found on the frailty index. Adrenergics/inhalants, lipid-modifying agents/plain, and selective calcium channel blockers with mainly vascular effects may benefit functional biomarkers of aging. More research is needed to investigate their clinical value in preventing adverse aging outcomes.

30. “Du ska väl amma?” : En kvalitativ intervjustudie om kvinnors val angående amning och sociala normer

Abstract

Ett nytt kapitel i livet ska helt plötsligt börja när det nyfödda barnet kommer till världen. När man som kvinna precis har fått barn kan det kännas överväldigande och man kan ha svårt att veta om de känslorna kommer från en själv eller andra, då många kommer med egna åsikter. Hur man väljer att ge sitt barn mat kan ske på olika sätt, och detta kan skapa blandade känslor hos kvinnan. I samhället kan man uppleva en minskning av amning då substitut, som exempelvis ersättning, finns lättillgängliga som är innehållsrika. Syftet med denna kvalitativa intervjustudie var att förstå kvinnors resonemang kring amning och hur detta påverkas av samhälleliga sociala faktorer. Med en socialpsykologisk ansats studerades detta genom teorier som genusteori och symbolisk interaktionism. I studien genomfördes tio stycken semistrukturerade intervjuer på kvinnor som fått barn de senaste tre åren och bott i Sverige i minst tio år. Resultatet visar att respondenternas sociala omgivning har stor påverkan på deras resonemang om amning via bröstet. Vi kan i resultatet även läsa hur respondenterna upplever tvetydiga åsikter och signaler från deras sociala omgivning. When you become a “mother”, it can feel overwhelming and it can be difficult to know whether those feelings come from yourself or others, as many may come with their own opinions. How you choose to feed your baby can happen indifferent ways, and this can create mixed feelings for the women. In society, you can experience a reduction in breastfeeding because substitutes, such as formula, are and rich in content. The purpose of this qualitative interview study was to understand the mothers' reasoning regarding breastfeeding and how this is affected by societal social factors. With a social psychological approach, this was studied through theories such as gender theory and symbolic interactionism. In the study, ten semi-structured interviews were conducted with women who had children in the last three years and lived in Sweden for at least ten years. The results show that the respondents' social environment has a great influence on their reasoning of breastfeeding via the breast. In the results, we can also read how the respondents experience ambiguous opinions and signals from their social environment. The part of the environment that is considered to have the greatest impact is considered to be the respondents' significant other.

Abstract

Dagens arbetsliv kännetecknas av kunskapsarbete inom organisationer där medlemmar sammanförs. Under 1980-talet innebar teknikens framsteg att nya former av styrning blev nödvändiga för att kontrollera ett svårövervakat arbete. Inslag av mjuk styrning genom värderingar och organisationskultur blev vanligare. Idag talas det om att arbetet blivit friare trots att både mjuka och hårda styrformer kombineras, möjligtvis är därför organisationers medarbetare mer styrda idag än någonsin. Med värderingsstyrning menas att ledare styr medarbetare utefter organisationens värderingar eller värdegrund. Värderingsstyrning har visat sig ha fördelar för organisationers framgång. Det har visat sig att organisationers värderingar blivit viktiga vid valet av arbetsgivare. Att organisationer framhåller sina värderingar i form av värdeord eller organisationskultur är snarare en regel än undantag. IT-konsultbranschen är beroende av medarbetares kompetens. Det råder hård konkurrens bland organisationer inom IT-konsultmarknaden. IT-konsulter är eftertraktade på grund av sin kompetens och har goda möjligheter att välja arbetsgivare. Intresset för studien är värderingarnas roll inom IT-konsultbranschen samt hur värderingar kontrolleras och upplevs av medarbetare. Studiens resultat analyseras med hjälp av nyinstitutionell teori som förklarar varför organisationer ser ut och agerar som de gör. Teorin bidrar till att analysera varför värderingsstyrning används och dess konsekvenser. Eftersom syftet för studien är att undersöka organisationers ställningstagande samt medarbetares upplevelser används konventionell innehållsanalys av organisationernas policydokument samt semistrukturerade intervjuer med organisationernas medlemmar. Semi-structured interviews were performed with coordinators (n = 6), employees (n = 13), and first-line managers (n = 8). Reflexive thematic analysis was used to analyse and interpret themes.ResultsA main theme was identified "the workplace and healthcare hold different organizational value logics" and four sub-themes: "the PSI-WPI challenged the organizational goals and values of the workplace and healthcare", "the PSI-WPI challenged organizational values on fairness", "the PSI-WPI challenged the professional roles of first-line managers and rehabilitation coordinators" and "the PSI-WPI introduced a need for the employee to juggle the employee and patient roles".ConclusionDifferent organizational value logics, values, and goals can introduce ethical challenges. We advise clarifying stakeholders' roles and preparing employees and managers for the return to work process by providing sufficient information. 33. A problem-solving intervention for employees on sickness absence due to common mental disorders : effects, ethics and process

Abstract

Purpose: Employees with common mental disorders (CMDs) which include depression, anxiety-, and adjustment disorder, are more often on sickness absence and have longer sickness absence episodes than employees with other diagnoses. To support these employees in their return-to-work process, problemsolving in combination with a work-directed intervention have previously been evaluated with positive results on earlier return-to-work and reduced sickness absence when provided in an occupational health service context. However, this combination has not been tested in primary health care in Sweden. A problemsolving intervention with workplace involvement (PSI-WPI) on top of care as usual (CAU) was provided in primary health care to employees on sickness absence due to common mental disorders with the aim of decreasing sickness absence. The overall aim of this thesis was to evaluate the effectiveness of the PSI-WPI on top of CAU for employees on sickness absence due to CMDs when compared to CAU alone, and to examine related contextual factors, ethical aspects, and process outcomes. Methods: A cluster-randomised controlled trial was conducted including 19 coordinators (PSI-WPI=9 and CAU=10) working in the primary health care delivering the intervention and 197 employees (PSI-WPI=85 and CAU=112) receiving the intervention. Data were collected from the Swedish Social Insurance Agency register on sickness absence, semi-structured interviews with coordinators, employees, and managers, and questionnaires to coordinators and employees. To evaluate the effectiveness of the intervention, generalized estimating equations were used to estimate the difference in mean registered net sickness absence days per month between PSI-WPI and CAU during an 18-month follow-up. Abstract

The COVID-19 pandemic highlighted the need for a rapid, convenient, and scalable diagnostic method for detecting a novel pathogen amidst a global pandemic. While command-line interface tools offer automation for SARS-CoV-2 Oxford Nanopore Technology sequencing data analysis, they are inapplicable to users with limited programming skills. A solution is to establish such automated workflows within a graphical user interface software. We developed two workflows in the software Geneious Prime 2022.1.1, adapted for data obtained from the Midnight and Artic's nCoV-2019 sequencing protocols. Both workflows perform trimming, read mapping, consensus generation, and annotation on SARS-CoV-2 Nanopore sequencing data. Additionally, one workflow includes phylogenetic assignment using the bioinformatic tools pangolin and Nextclade as plugins. The basic workflow was validated in 2020, adhering to the requirements of the European Centre for Disease Prevention and Control for SARS-CoV-2 sequencing and analysis. The enhanced workflow, providing phylogenetic assignment, underwent validation at Uppsala University Hospital by analysing 96 clinical samples. It provided accurate diagnoses matching the original results of the basic workflow while also reducing manual clicks and analysis time. These bioinformatic workflows streamline SARS-CoV-2 Nanopore data analysis in Geneious Prime, saving time and manual work for operators lacking programming knowledge. [ABSTRACT FROM AUTHOR]

50. Dementia and mortality in older adults: A twin study.

Abstract

INTRODUCTION: Dementia predicts increased mortality. We used case‐control and co‐twin control models to investigate genetic and shared environmental influences on this association. METHODS: Case‐control design, including 987 twins with dementia and 2938 age‐ and sex‐matched controls in the Swedish Twin Registry. Co‐twin control design, including 90 monozygotic (MZ) and 288 dizygotic (DZ) twin pairs discordant for dementia. To test for genetic and environmental confounding, differences were examined in mortality risk between twins with dementia and their matched or co‐twin controls. RESULTS: Twins with dementia showed greater mortality risk than age‐ and sex‐matched controls (HR = 2.02 [1.86, 2.18]). Mortality risk is significantly elevated but attenuated substantially in discordant twin pairs, for example, comparing MZ twins with dementia to their co‐twin controls (HR = 1.48 [1.08, 2.04]).
