We study a differentiated product market in which an investor initially owns a controlling stake in one of two competing firms and may acquire a non-controlling or a controlling stake in a competitor, either directly using her own assets, or indirectly via the controlled firm. While industry profits are maximized within a symmetric two product monopoly, the investor attains this only in exceptional cases. Instead, she sometimes acquires a noncontrolling stake. Or she invests asymmetrically rather than pursuing a full takeover if she acquires a controlling one. Generally, she invests indirectly if she only wants to affect the product market outcome, and directly if acquiring shares is profitable per se.

Medical education is presently unveiling trends, which also dominate other fields of higher education. Within the framework of internationalisation and globalisation, and driven by developments in information and communication technology and the pronounced migration of medical doctors, one has observed consequences such as commercialisation and privatisation. This, in turn, has engendered a quality assurance reaction with focus on standardisation and accreditation.The Bologna Declaration and Process can be seen as part of this new quality concern, but also raises problems for medical education regarding the two-cycle (bachelor and master degrees) system. Standard setting in medical education in Europe took its start with the EU Medical Directive, which has essentially not been changed in 30 years. The increasing internationalisation of medical education, which shows a number of innovative trends, but which also unveils many problems and challenges, emphasises the role of the WFME Global Standards Programme launched in 1997. The Trilogy of Global Standards, covering the continuum of medical education, has been adopted in all parts of the world. The Standards for Basic Medical Education are used in reform programmes in many medical schools and are now also being incorporated in definition of national accreditation standards. This development has recently been strengthened in the WHO/WFME Strategic Partnership to Improve Medical Education. One practical result of this collaboration was the development of WHO/WFME Guidelines for Accreditation of Basic Medical Education.A Task Force under the new EU Thematic Network MEDINE is now working with definition of European accreditation standards using the WFME Global Standards as a template. The immediate perspective will be establishment of national accreditation systems in the European region and there is probably no need at the moment for a common accreditation system in Europe. Die Medizinische Ausbildung zeigt Entwicklungen, die auch in anderen Feldern der universitären Ausbildung vorherrschen. Im Rahmen der Internationalisierung und Globalisierung in Verbindung mit Innovationen der Informations- und Kommunikationstechnologien sowie der ausgeprägten Migration von Ärzten wurden Folgen wie Kommerzialisierung und Privatisierung beobachtet. Dies wiederum hat eine Qualitätssicherungsbewegung hervorgebracht, die eine Standardisierung und Akkreditierung in den Mittelpunkt stellte. Bologna-Deklaration und -Prozess können als Teil dieser neuen Besorgnis um Ausbildungsqualität gesehen werden; gleichzeitig entstehen durch sie aber Probleme für die medizinische Ausbildung, insbesondere durch die zwei Ausbildungssequenzen (Bachelor- und Masters-Diplome).Die Entwicklung von Standards in der medizinischen Ausbildung Europas begann mit der EU Richtlinie Medizin, die sich im Kern seit 30 Jahren nicht geändert hat. Ein praktisches Ergebnis war die Entwicklung von WHO/WFME-Leitlinien für die Akkreditierung der Medizinischen Ausbildung.Eine Arbeitsgruppe in dem neuen EU Thematischen Netzwerk MEDINE wurde nun mit der Definition europäischer Akkreditierungsstandards beauftragt und nutzt dabei die Globalen Standards der WFME als Schablone. Die unmittelbare Folge wird die Entstehung nationaler Akkreditierungssysteme in der europäischen Region sein, so dass es zum jetzigen Zeitpunkt wohl keinen Bedarf für ein zentrales Akkreditierungssystem der EU gibt.

Cell-associated and serum β2-microglobulin was estimated in seven patients with chronic lymphocytic leukaemia. The amount of cell-associated β2-microglobulin was significantly reduced (P<0.01), due to a decrease in the fraction of β2-microglobulin that passes unretarded through a concanavalin A affinity column (presumably non-HLA-associated β2-microglobulin). Serum concentrations of β2-microglobulin were increased, but no correlation was found between the decrease in cell-associated β2-microglobulin and the increase in serum β2-microglobulin. All of the β2-microglobulin from leukaemic serum was eluted corresponding to a molecular weight of 11,800 and none of it was retarded on a concanavalin A affinity column. The decrease in cell-associated β2-microglobulin might reflect a change in the qualitative or quantitative expression of β2-microglobulin-associated membrane structures on the leukaemic cells, perhaps conferring resistance to the cells against hypothetical immunological host defence mechanisms, [ABSTRACT FROM AUTHOR]

The changes in intraneutrophilic and plasma concentrations of the three antibacterial proteins lysozyme, lactoferrin, and myeloperoxidase were studied sequentially during acute bacterial infection in nine patients. Intraneutrophilic concentrations of the three proteins were decreased by more than 50% during the 1st week of infection, followed by a slow increase over the following 2 weeks. Nadir values coincided with maximal toxic granulation of the neutrophils. The data suggest that neutrophilic granulocytes are deficient during early bacterial infection, possibly because of deficient synthesis of antibacterial proteins in the bone marrow, and that neutrophil toxic granulation is the visual counterpart of this defect. The plasma concentrations of the three proteins showed considerable differences: whereas plasma lysozyme did not show any sequential changes, plasma myeloperoxidase was high at the start of infection and quickly decreased towards normal values, and plasma lactoferrin, high in the first samples, showed a secondary peak 1 week after onset of disease, before normalization was seen. These differences may result from differences in the signals governing the release of the proteins from neutrophilic granulocytes, and it is suggested that these signals are specific for the individual antibacterial protein and not for the different types of neutrophil granules. Lymphocyte transformation responses in vitro were studied in eight patients with acute bacterial meningitis (in five due to Neisseria meningitidis). Sequential studies were done from 24-48 hr after the first symptoms of infection to complete recovery. In all cases lymphocyte transformation was depressed during the acute phases of illness. The responses to microbial antigens were more affected than the responses to mitogens. The course of the lymphocyte responses to the causative micro-organism showed no difference from the responses to other microbial species. A moderate shift towards increased sensitivity of the lymphocytes to lower doses of the causative micro-organism was observed during the course of illness in three cases. In N., meningitidis infection, a rapid rise was seen in the serum titres of complement-fixing antibodies and in the number of precipitating antibodies, whereas the rise in immunoglobulin concentrations was more prolonged. Characteristic patterns of elevation and return towards normal were found in the serum concentrations of the acute-phase reactants α
1 -antitrypsin, haptoglobin, and orosomucoid. It is concluded that the lymphocyte transformation responses in vitro during severe bacterial infection are largely governed by non-specific factors, and that studies of lymphocyte responses to micro-organisms should always include other microbial species as controls. Therapy with DNA topoisomerase II inhibitors has been shown to result in an increased risk of acute myeloid leukemia (AML), often presenting balanced translocations to chromosome bands 11q23 and 21q22. Also other balanced aberrations, more rarely observed in therapy-related AML (t-AML), such as t(15;17) and inv(16) have been associated with these drugs. Recently we observed a case of chronic myeloid leukemia (CML) with t(9;22) after therapy of a germ cell tumor with etoposide, cisplatin and bleomycin. Based on this case and a review of chemotherapy-related leukemias with t(9;22) from the literature, we suggest a causal relationship between therapy with DNA topoisomerase II inhibitors and development of various types of leukemia carrying the Philadelphia chromosome. - Abstract
Nine patients with myelodysplastic syndrome (MDS) were examined with magnetic resonance imaging and in vivo T1 relaxation time measurements of the vertebral bone marrow in a 1.5 tesla whole body scanner. Two patients underwent transformation to acute myeloid leukemia and were evaluated at follow-up examinations. At the time of diagnosis the T1 relaxation times of the vertebral bone marrow were significantly prolonged compared with normal values. The T1 relaxation times of the vertebral bone marrow in patients with MDS showed significantly lower values compared with patients with acute leukemia and did not differ from patients with polycythemia vera.
