Introdução/Objetivo: Descrever as características clínico-epidemiológicas de um grupo de mulheres com HIV/Aids no centro sul da Bahia, identificando fatores clínico-comportamentais associados a níveis de CD4 < 350 células no momento do diagnóstico. Materiais e métodos: Trata-se de um estudo retrospectivo, quantitativo e descritivo com caráter documental. A coleta de dados foi realizada por meio da análise das informações contidas em prontuários médicos do Serviço de Atenção Especializada de Guanambi-BA, referentes à admissão de pacientes do sexo feminino diagnosticadas com HIV/Aids no próprio CTA, durante a sua consulta de admissão. Não foram consideradas pacientes transferidas de outro serviço ou que tenham iniciado terapia antirretroviral antes da coleta de CD4 inicial. Foram avaliados 408 prontuários, sendo selecionados 24 que preencheram os critérios de inclusão. Este estudo foi aprovado pelo Comitê de Ética da UNIFG sob protocolo n° 5.600.715. Resultados: Ficou constatado que o perfil destas pacientes é composto, em sua maioria, por mulheres heterossexuais (100%), solteiras (37,5%), trabalhadoras do lar (55%) e com idade média de 44 anos. Registramos que 60,8% destas pacientes são procedentes do município de Guanambi e 58,3% delas já foram admitidas na classificação clínica de Aids, sendo a perda de peso (66%) o sintoma mais relatado como motivo de procura para realização do teste. Quando são associados os dados clínicos aos níveis de CD4, identificamos que o principal fator relacionado a níveis de CD4 abaixo de 350 células (Aids) foi a procura do serviço pela presença de sinais e sintomas da doença, neste grupo a média de CD4 foi de 44 células. Em contrapartida, mulheres que procuraram o serviço para a triagem de pré-natal e as que compareceram por procura espontânea pelo teste, ainda sem sintomas específicos da doença, foram fatores que se associaram a níveis de CD4 de 491 células e 430 células, respectivamente. Conclusão: Os dados coletados neste estudo ajudam a delinear algumas características da infecção pelo HIV/Aids na população feminina do Centro Sul baiano e identifica fatores fortemente associados à Aids e baixos níveis de CD4, como a procura tardia pelo serviço de testagem, após o aparecimento de sinais e sintomas da doença. Este estudo mostra a necessidade de se construir políticas públicas voltadas para ampliação de testagem para HIV na população feminina ainda assintomática, o que melhora o prognóstico destas pacientes.

Rio de Janeiro is endemic for tuberculosis (TB) and presents the second largest prevalence of the disease in Brazil. Here, we present the bacterial population structure of 218 isolates of Mycobacterium tuberculosis, derived from 186 patients that were diagnosed between January 2008 and December 2009. Genotypes were generated by means of spoligotyping, 24 MIRU-VNTR typing and presence of fbpC103, RDRio and RD174. The results confirmed earlier data that predominant genotypes in Rio de Janeiro are those of the Euro American Lineages (99%). However, we observed differences between the classification by spoligotyping when comparing to that of 24 MIRU-VNTR typing, being respectively 43.6% vs. 62.4% of LAM, 34.9% vs. 9.6% of T and 18.3% vs. 21.5% of Haarlem. Among isolates classified as LAM by MIRU typing, 28.0% did not present the characteristic spoligotype profile with absence of spacers 21 to 24 and 32 to 36 and we designated these conveniently as "LAM-like", 79.3% of these presenting the LAM-specific SNP fbpC103. The frequency of RDRio and RD174 in the LAM strains, as defined both by spoligotyping and 24 MIRU-VNTR loci, were respectively 11% and 15.4%, demonstrating that RD174 is not always a marker for LAM/RDRio strains. We conclude that, although spoligotyping alone is a tool for classification of strains of the Euro-American lineage, when combined with MIRU-VNTRs, SNPs and RD typing, it leads to a much better understanding of the bacterial population structure and phylogenetic relationships among strains of M. tuberculosis in regions with high incidence of TB.

BackgroundAn extended outbreak of mycobacterial surgical infections occurred in Brazil during 2004-2008. Most infections were caused by a single strain of Mycobacterium abscessus subsp. bolletii, which was characterized by a specific rpoB sequevar and two highly similar pulsed-field gel electrophoresis (PFGE) patterns differentiated by the presence of a ∼50 kb band. The nature of this band was investigated.Methodology/principal findingsGenomic sequencing of the prototype outbreak isolate INCQS 00594 using the SOLiD platform demonstrated the presence of a 56,267-bp [corrected] circular plasmid, designated pMAB01. Identity matrices, genetic distances and phylogeny analyses indicated that pMAB01 belongs to the broad-host-range plasmid subgroup IncP-1β and is highly related to BRA100, pJP4, pAKD33 and pB10. The presence of pMAB01-derived sequences in 41 M. abscessus subsp. bolletii isolates was evaluated using PCR, PFGE and Southern blot hybridization. Sixteen of the 41 isolates showed the presence of the plasmid. The plasmid was visualized as a ∼50-kb band using PFGE and Southern blot hybridization in 12 isolates. The remaining 25 isolates did not exhibit any evidence of this plasmid. The plasmid was successfully transferred to Escherichia coli by conjugation and transformation. Lateral transfer of pMAB01 to the high efficient plasmid transformation strain Mycobacterium smegmatis mc(2)155 could not be demonstrated.Conclusions/significanceThe occurrence of a broad-host-range IncP-1β plasmid in mycobacteria is reported for the first time. Thus, genetic exchange could result in the emergence of specific strains that might be better adapted to cause human disease.
