ZusammenfassungDie Endophthalmitis ist eine der schwersten ophthalmologischen Notfälle. Bei den meisten Patienten kommt es nach dem Ereignis zu einer bleibenden Visusminderung, das Auge kann aber in den meisten Fällen erhalten werden. Wenn das Auge nach einer Endophthalmitis enukleiert wird, kann die ophthalmopathologische Aufarbeitung des Bulbus unter Berücksichtigung der klinischen Vorgeschichte zumindest wertvolle Informationen zum letztlich frustranen Krankheitsverlauf geben, die dann auch zukünftigen Patienten zugutekommen können. Oft ergeben sich auch relevante Aspekte im Hinblick auf das therapeutische Vorgehen. Beispielhaft soll die Notwendigkeit einer perforierenden Keratoplastik mit großem Transplantatdurchmesser und evtl. Entfernung von Linsenkapsel mit Kapselsack bei therapieresistenter Pilzendophthalmitis genannt werden. An 5 enukleierten Augen werden im Folgenden verschiedene Ausprägungsgrade und Entstehungswege der exogenen und selteneren endogenen Endophthalmitis klinisch und ophthalmopathologisch verständlich aufgearbeitet. Zusammenfassend erfordert eine Endophthalmitis schnelles Handeln, dennoch gilt es, verschiedene Differenzialdiagnosen auszuschließen. Die histopathologische Aufarbeitung enukleierter Augen ist hilfreich, um den Krankheitsverlauf nachzuvollziehen und kann auch forensische Bedeutung haben.

ZusammenfassungDen kornealen Ektasien werden der Keratokonus, der Keratoglobus, die pelluzide marginale Degeneration sowie iatrogene Keratektasien zugeordnet. Bei allen Formen der kornealen Ektasie kommt es zu einer Verdünnung der Hornhaut, die mit einer kornealen Aufsteilung sowie einem irregulären Astigmatismus einhergeht. Nachfolgend wird ein Überblick über histopathologische Veränderungen der verschiedenen kornealen Ektasien gegeben. Ebenso werden histologische Veränderungen nach operativen Verfahren, die im Zusammenhang mit Ektasien durchgeführt werden, wie das korneale Crosslinking und die perforierende Keratoplastik (pKPL) sowie nach refraktiven chirurgischen Verfahren, die potenziell zu einer Ektasie führen können, dargestellt. Neben einer Literaturrecherche erfolgte auch die Aufarbeitung und Untersuchung von histopathologischem Archivmaterial zur exemplarischen Darstellung der spezifischen histologischen Veränderungen.

Endophthalmitis is one of the most severe ophthalmic emergencies. Most patients experience a permanent decrease in visual acuity after the event, but the eye can be preserved in most cases. However, when the eye is enucleated after endophthalmitis, ophthalmopathologic investigation of the globe with respect to the clinical history can provide valuable information regarding the ultimately frustrating course of the disease that can be helpful for the treatment of future patients. Often, valuable aspects also emerge with regard to the therapeutic approach. For example, in therapy-resistant fungal endophthalmitis the necessity of penetrating keratoplasty with a large graft diameter and possibly even removal of the lens including the capsular bag should be stressed. In the following, five enucleated eyes with a different spectrum of endophthalmitis, as well as different potential pathways of exogenous and endogenous endophthalmitis, are illustrated clinically and ophthalmopathologically. In summary, endophthalmitis requires urgent intervention; however, various differential diagnoses must be excluded. Histopathologic examination of enucleated eyes is helpful for understanding the course of the disease and may also have forensic significance.Die Endophthalmitis ist eine der schwersten ophthalmologischen Notfälle. Ocular involvement in mucous membrane pemphigoid (MMP) is relatively rare, with a prevalence of 25 cases per million population, equating to approx. 2,100 patients throughout Germany. Diagnosis can be difficult - especially in cases of isolated ocular involvement - and treatment can be complex and lengthy. Immunosuppressants or immunomodulatory drugs are often used. Due to the complexity of diagnosis and treatment, MMP patients are usually referred to specialized centers. The aim of this project was to evaluate the current care situation of patients with ocular MMP in Germany.A paper-based survey was designed and sent to all university eye clinics and other specialized centers in Germany in April 2020. The survey asked about the existence of a specialized outpatient service, the total annual number of patients with MMP, the annual number of newly diagnosed patients, any interdisciplinary collaboration for diagnostic or therapeutic purposes, as well as the local and systemic therapy used.Of a total of 44 clinics, 28 (64%) responded, reporting a total average of 27 ± 42 (0 - 200) patients and 3.6 ± 2.2 (0 - 10) new cases per year. This corresponds to a total of 741 patients. Only nine (32%) of the responding clinics offer specialized MMP clinics. 93% of the centers collaborate with the local dermatology department. 79% perform serological and histological diagnostics in-house. About half of the centers (n = 16) apply a standardized treatment regime. Systemic glucocorticoids (66.7%) are most commonly used, followed by mycophenolate mofetil and dapsone (57.1%), rituximab (33.3%), azathioprine and cyclophosphamide (28.6%), as well as methotrexate (19.0%). The least frequently used treatment is intravenous immunoglobulin (14.3%).This survey of German ophthalmology departments obtained data from about one third of the estimated total cohort of all patients with MMP in Germany. These are presumed to be exclusively patients with at least one ocular involvement. The complex care of these patients is usually provided in collaboration with a dermatologist and with the use of systemic anti-inflammatory medication. Dabei handelt es sich vermutlich ausschließlich um Patienten mit mindestens einer okulären Beteiligung. Aktuell wird eine augenärztliche SHP-Registerstudie etabliert, um die Epidemiologie und Versorgungssituation besser zu erfassen und langfristig zu verbessern.

Introduction: For ophthalmologic research, the systematic correlation of clinical data with data obtained from postmortem tissue donation is of great benefit. In this respect, the establishment of an eye donation registry represents a prerequisite for the acquisition of such data. Methods: A total of 300 patients were interviewed at a tertiary referral center in Germany by means of a standardized questionnaire. Binary questions were evaluated by percentage; Likert-scaled questions (1 = does apply; 5 = does not apply) were analyzed by the median and 25th (Q25) and 75th (Q75) percentiles. Results: The majority of patients (77.0%) would agree to donate their eyes for research purposes. When asked about reasons against an eye donation, 60.9% of all patients only stated reasons in the category “addressable” (e.g., not enough awareness of the topic). The vast majority of patients considered it appropriate for an ophthalmologist to approach them on the issue of postmortem eye donation (median 1, Q25 1, Q75 1). Conclusion: Overall, patients had a positive attitude towards postmortem eye donation for research purposes. Importantly, reasons given against postmortem eye donation were often related to misconceptions and were potentially addressable. These results underline the fundamental willingness of ophthalmological patients in Germany to donate their eyes postmortem for research purposes.

Aus der Untersuchung enukleierter Augen lasst sich eine Vielzahl von Befunden erheben, die dem Ophthalmologen Erkenntnisse hinsichtlich der Effektivitat, der Verbesserungsmoglichkeit und naturlich auch der moglichen Komplikationen chirurgischer Eingriffe liefern. Stellvertretend fur viele bedeutsame Befunde werden anhand dreier enukleierter Bulbi die klinisch und ophthalmopathologisch relevanten Befunde dargestellt und diskutiert. Es erfolgte die klinisch-histopathologische Korrelation dreier enukleierter Augen. Ein Auge, das enukleiert wird, weist in den allermeisten Fallen fortgeschrittene Veranderungen der verschiedenen Gewebe auf, sodass nicht immer direkte Ruckschlusse auf die ursprunglichen chirurgischen Masnahmen moglich sind. Dennoch lassen sich viele Erkenntnisse zur Pathophysiologie verschiedener Erkrankungen, zur Wundheilung und auch zum chirurgischen Vorgehen ableiten, wie hier anhand einer Epithelinvasion, historischer Netzhautchirurgie und einer endogenen Pilzendophthalmitis gezeigt wird. Jedes enukleierte Auge sollte histopathologisch und unter Berucksichtigung der klinischen Vorgeschichte untersucht werden, um den Krankheitsverlauf nachvollziehen und das gewonnene Wissen fur die Behandlung zukunftiger Patienten einsetzen zu konnen.

Von der kontinuierlichen Weiterentwicklung technischer und molekularbiologischer Methoden profitiert auch dieser spezielle Bereich der allgemeinen Pathologie. Als Basis grundlagenwissenschaftlicher Forschung, aber auch im Bezug auf die Vermittlung pathophysiologischer Zusammenhänge und in der Evaluation innovativer Operationsmethoden ist die Ophthalmopathologie auch heutzutage nicht wegzudenken. Anhand verschiedener Fallbeispiele werden die 4 Säulen der Ophthalmopathologie – die Diagnostik, die klinisch-pathologische Korrelation, die Evaluation neuer Therapiemethoden und die Forschung – illustriert. Die Ophthalmopathologie ist nicht nur ein Fach der Vergangenheit, sondern vor allem ein Fach der Zukunft. Sie entwickelt sich mit dem medizinischen und ophthalmologischen Fortschritt weiter und dient auch dazu, diesen kritisch zu evaluieren. Die klinisch-pathologische Korrelation ist ein unschätzbares Gut für die nachhaltige Qualität der Augenheilkunde, die wir uns nicht durch das „Sparen an falscher Stelle“ (d. h. der Schließung ophthalmopathologischer Labore) leichtfertig nehmen lassen sollten. Die Ophthalmopathologie war, ist und wird weiterhin der Goldstandard bei vielen ophthalmologischen Fragestellungen bleiben.

A variety of findings can be obtained from enucleated eyes, which provide the ophthalmologist with information with respect to the effectiveness, improvement options and of course also the possible complications of surgical interventions. Representative for many important findings, relevant clinical and ophthalmopathological findings are presented and discussed on the basis of three enucleated eyeballs.Clinical histopathological correlation of three enucleated eyeballs.Enucleated eyeballs typically exhibit advanced changes in different tissues making it sometimes difficult to draw direct conclusions about the original surgical interventions. Nevertheless, many findings on the pathophysiology of different diseases, on wound healing and also on the surgical procedures can be deduced, as shown here in three different cases of epithelial invasion, historical retinal surgery and an endogenous fungal endophthalmitis.Each enucleated eye should be histopathologically examined and under consideration of the clinical history in order to understand the course of the disease and to use the knowledge gained for the treatment of future patients.

Purpose We sought to compare the sensitivity, specificity, accuracy, and interobserver agreement of the two most commonly used classification systems for conjunctival melanocytic intraepithelial lesions with the new World Health Organization (WHO) classification. Design Retrospective case series and evaluation of classification systems. Methods We reviewed the pathology and medical records of all patients who underwent a primary biopsy procedure for conjunctival primary acquired melanosis (PAM) at Wills Eye Hospital between 1974 and 2002 who had ≥36 months of follow-up. Data collected included age, sex, clinical findings, recurrence, and progression to melanoma. Twelve ophthalmic pathologists analyzed scanned hematoxylin and eosin–stained virtual microscopic slides using 3 classification systems: PAM, conjunctival melanocytic intraepithelial neoplasia, and the WHO 4th edition classification of conjunctival melanocytic intraepithelial lesions. Observer agreement, sensitivity, specificity, and diagnostic accuracy of each classification system were assessed. Results There were 64 patients who underwent 83 primary excisions with cryotherapy for conjunctival PAM who had adequate tissue for histopathologic evaluation. The interobserver agreement in distinction between the low- and high-grade lesions was 76% for PAM, 67% for conjunctival melanocytic intraepithelial neoplasia, and 81% for WHO classification system. Low-grade lesions provided the greatest interpretative challenge with all 3 classification systems. The 3 classification systems had comparable accuracy of 81%-83% in their ability to identify lesions with potential for recurrence. Conclusions This study highlights the comparable strengths and limitations of the 3 classification systems for conjunctival melanocytic intraepithelial lesions and suggests that the simplified WHO classification scheme is appropriate for evaluation of these lesions.

Ophthalmic pathology has a long tradition in Germany. And, like in general pathology, there is continuous progress due to new technologies and the improvement of molecular biology techniques. Ophthalmic pathology cannot be disregarded, particularly in the context of basic research but also as a medium for understanding pathophysiologic interrelationships and evaluating innovative surgical techniques. By means of various examples, the "four columns" of ophthalmic pathology shall be illustrated: diagnostics, clinicopathologic correlation, evaluation of new surgical and medical techniques and finally research. Ophthalmic pathology is not a discipline of the past but is rather one of the future. It develops and improves together with medical and ophthalmological progress and serves, at the same time, as a critical evaluation tool. Clinicopathologic correlations are of paramount importance for a lasting quality in ophthalmology, and we should not risk depriving ourselves of this instrument by carelessly saving at the wrong end and closing our laboratories. Zusammenfassung Hintergrund Lidtumoren insgesamt weisen ein sehr heterogenes Erscheinungsbild auf. Auch Tumoren, die einige Malignitätskriterien aufweisen, können sich histologisch als entzündliche Läsion ohne Nachweis einer Neoplasie darstellen. Umgekehrt können maligne Tumoren zu einer klinisch im Vordergrund stehenden, zumeist therapieresistenten Entzündungsreaktion führen, die entsprechend diagnostiziert und therapiert werden muss. Material und Methoden/Ergebnisse Anhand ausgewählter Beispiele von Lidtumoren werden potenzielle Problemfälle sowie klinisch unklare Fälle dargestellt und mit dem histopathologischen Befund korreliert. Schlussfolgerung Lidtumoren können von verschiedenen Strukturen ausgehen und weisen ein breites Spektrum an klinischen Erscheinungsbildern auf. In unklaren Fällen bzw. bei V. a. einen malignen Tumor ist eine Biopsie/Exzision zur histologischen Diagnosestellung erforderlich. Inadäquat behandelte benigne Läsionen wie z. B. inkomplett exzidierte Nävi oder ein Molluscum contagiosum können zu klinisch relevanten Problemen führen.

ZusammenfassungDer Keratokonus ist eine relativ häufige (1 : 2000), meist bilateral auftretende Erkrankung, die zu einer Verdünnung, Veränderung der biochemischen und biomechanischen Struktur sowie Vorwölbung der Hornhaut führt. Seit mehr als 10 Jahren steht das Crosslinking als therapeutisches Verfahren für den progredienten Keratokonus zur Verfügung. Dabei wird mittels Riboflavin und UVA-Bestrahlung eine Verfestigung der Quervernetzung der Kollagenfasern in den vorderen 300 µm des Hornhautstromas erreicht. Es ist als wirksam und komplikationsarm beschrieben worden, sofern entsprechende Protokolle und Indikationen eingehalten werden. Diese Übersicht beinhaltet die physiologischen und pathologischen morphologischen Veränderungen der Hornhaut vor und nach Crosslinking bei Keratokonus. Die verfügbare Literatur wird aufgearbeitet und um eigene Beobachtungen und Erfahrungen, basierend auf histopathologischen Untersuchungen, ergänzt.

Das Internet gewinnt zunehmend an Bedeutung als Quelle fur gesundheitsbezogene Informationen. Fur die meisten Patienten gelten die Internetseiten von Krankenhausern und Universitaten als besonders zuverlassig. Die Lesbarkeit der verfugbaren Informationen gilt allerdings haufig als masig. Instrument zur Untersuchung der Lesbarkeit sind sog. Lesbarkeitsindizes, welche mittels objektiver Kriterien die Lesbarkeit von Texten ermitteln. Ziel war es, eine breite Datenlage uber die Lesbarkeit von Patienteninformation der deutschen Universitatskliniken in der Augenheilkunde zu erhalten. Hierfur wahlten wir 3 Augenerkrankungen mit potenziell irreversibler Schadigung aus. Die Internetseiten von 32 Abteilungen fur Augenheilkunde der jeweiligen Universitatskliniken wurden untersucht. Texte zu „Katarakt“, „Glaukom“ und „Amotio retinae“ wurden ermittelt und analysiert. Die Texte wurden einer systematischen Lesbarkeitsprufung mittels 5 anerkannter Lesbarkeitsindizes unterzogen (Hohenheimer Verstandlichkeitsindex [HVI], Amstad-Index [Amstad], Simple Measure of Gobbledygook [G‑SMOG-Index], Wiener Sachtextformel [W-STX] und LIX-Lesbarkeitsindex [LIX]) und mit dem Analyseprogramm „Text-Lab“ ( http://www.text-lab.de ) analysiert und ausgewertet. In insgesamt 59 Fallen (61,46 %) konnten Texte auf den Internetseiten der Kliniken ermittelt und analysiert werden. Es zeigte sich im Mittel bei allen Indizes eine schlechte Lesbarkeit (HVI: 7,91 ± 3,94; Amstad: 35,45 ± 11,85; Wiener Sachtextformel: 11,19 ± 1,93; G‑SMOG: 9,77 ± 1,42; LIX: 54,53 ± 6,67). In den meisten Fallen ist der Schwierigkeitsgrad der analysierten Texte mit dem wissenschaftlicher Publikationen vergleichbar. Es ist davon auszugehen, dass die Texte fur einen grosen Teil der Leser nur schwer lesbar sind. Eine Reevaluation der Texte und eine Bearbeitung hinsichtlich der Lesbarkeit waren wunschenswert.

A 52-year-old male caucasian patient presented with a limbal subconjunctival pigmentation of unknown origin with progressive enlargement over the past years. Differential diagnoses included a malignant melanocytic lesion; therefore, an excisional biopsy was performed. Prior investigations showed no ciliary body or anterior chamber angle involvement. Histological and immunohistochemical analysis revealed the rare diagnosis of a scleral nevus. The clinical, histological and immunohistochemical findings as well as the relevant differential diagnoses are discussed.

Bei einer langwierigen therapieresistenten Keratokonjunktivitis sollte man, insbesondere bei Kindern und Jugendlichen, auch nach Dellwarzen in Augennahe suchen. Auch wenn es sich um eine selbstlimitierende Erkrankung handelt, sollten sie im Augenbereich operativ entfernt werden.

Fungal keratitis shows a variable clinical picture and is therefore often diagnosed only late. Fungi in general are quite resistant organisms, and in addition treatment is difficult due to limited and specially manufactured medications.We describe the unfavourable outcome in a young female patient with keratitis due to fusarium that ended - despite extensive medical and surgical treatment - in enucleation. Histologic examnination of the enucleated globe revealed massive (post-)inflammatory changes throughout the anterior segment but only few persistent fungal elements that were found in the lens capsule (after phacoemulsification) and the peripheral Descemet's membrane.Fungus as the pathogenic agent in therapy-resistent keratitis should be included in the differential diagnosis as early as possible. Should surgical interventions become necessary, a keratoplasty as large as possible and removal of the lens capsule should be considered in order to eliminate potential reservoirs for the causative organism.Die Pilz-Keratitis zeigt ein klinisch sehr variables Erscheinungsbild und wird nicht zuletzt auch deshalb oft erst spät diagnostiziert. Pilze sind generell sehr resistente Keime, und die Therapie gestaltet sich aufgrund der wenigen und speziell anzufertigenden Medikamente zusätzlich schwierig.Es wird der besonders ungünstige Verlauf bei einer jungen Patientin mit Fusarien-Keratitis dargestellt, bei der das Auge zuletzt trotz maximaler medikamentöser und auch chirurgischer Therapie entfernt werden musste. Die histologische Aufarbeitung zeigte massive (post-)entzündliche Veränderungen im gesamten vorderen Augenabschnitt, persistierende Pilzelemente aber lediglich in der nach Katarakt-Operation verblieben Linsenkapsel und der peripheren Descemet-Membran.Pilze als Verursacher einer therapierefraktären Keratitis sollten möglichst rasch in die Differentialdiagnose miteinbezogen werden. Falls chirurgische Maßnahmen notwendig werden, sollte die Keratoplastik so groß wie möglich gewählt und bei einer Katarkat-Operation die Kapsel mitentfernt werden, um potentielle Keimreservoire zu beseitigen.

Lid tumors show a heterogenous clinical spectrum. Tumors which display some criteria of malignancy may histologically be diagnosed as inflammatory lesions without any neoplastic component. In contrast, malignant tumors can induce changes on the lid margin mimicking inflammatory changes.Certain examples of lid tumors are shown to illustrate potential pitfalls as well as clinical unequivocal cases. The clinical appearance of the lesions is correlated with the histologic findings.Lid tumors can develop from different structures of the eyelid and, therefore, show a wide spectrum of clinical findings. If a malignant process is suspected or the clinical diagnosis cannot be unequivocally determined, a biopsy (incisional vs. excisional) is necessary followed by histologic evaluation. Furthermore, inadequately treated benign lesions, such as an incomplete excised nevus or a molluscum contagiosum, can lead to serious problems.Lidtumoren insgesamt weisen ein sehr heterogenes Erscheinungsbild auf. Auch Tumoren, die einige Malignitätskriterien aufweisen, können sich histologisch als entzündliche Läsion ohne Nachweis einer Neoplasie darstellen. Umgekehrt können maligne Tumoren zu einer klinisch im Vordergrund stehenden, zumeist therapieresistenten Entzündungsreaktion führen, die entsprechend diagnostiziert und therapiert werden muss.Anhand ausgewählter Beispiele von Lidtumoren werden potenzielle Problemfälle sowie klinisch unklare Fälle dargestellt und mit dem histopathologischen Befund korreliert.Lidtumoren können von verschiedenen Strukturen ausgehen und weisen ein breites Spektrum an klinischen Erscheinungsbildern auf. In unklaren Fällen bzw. bei V. a. einen malignen Tumor ist eine Biopsie/Exzision zur histologischen Diagnosestellung erforderlich. Inadäquat behandelte benigne Läsionen wie z. B. inkomplett exzidierte Nävi oder ein Molluscum contagiosum können zu klinisch relevanten Problemen führen.

Ein 65-jahriger Patient stellte sich mit einer progredienten Visusminderung am rechten Auge vor. Bei unauffalligem Fundusbefund und relativem afferentem Pupillendefizit sowie einer Gesichtsfeldeinschrankung wurde zunachst die Verdachtsdiagnose einer Durchblutungsstorung im Bereich des N. opticus gestellt. Die weitere Bildgebung fuhrte jedoch zur Diagnose einer orbitalen Metastase mit Optikuskompression, ausgehend von einem ausgedehnten, nekrotisch zerfallenden Bronchialkarzinom im Bereich des linken Oberlappens. Die Befunde werden zusammen mit dem moglichen Ursprung orbitaler Metastasen diskutiert.

Die Untersuchung fetaler Augen gibt zum einen Aufschluss uber die Entwicklung des Auges, zum anderen tragt eine ophthalmopathologische Untersuchung zur Diagnostik systemischer und okularer Syndrome bei. Der folgende Ubersichtsartikel behandelt die Untersuchung fetaler Augen, die Zuordnung der Augen zu verschiedenen Entwicklungsstadien, Herausforderungen bei der Beurteilung fetaler Augen, die klinische Relevanz anhand von Fallbeispielen sowie akademische Fragestellungen. Die morphologische Entwicklung des Auges wurde bereits seit dem 19. Jahrhundert untersucht und ist nicht Gegenstand dieser Arbeit. Bei der Untersuchung fetaler Augen, die im Rahmen einer kinderpathologischen Obduktion entnommen werden, stehen der Diagnostik oft Artefakte im Wege, die durch Autolyse, Fixation oder mechanische Manipulation entstanden sind. Diese gilt es, von Krankheitsbildern zu unterscheiden. Neben der Untersuchung auf z. B. Kolobome und Katarakte ist auch die morphologische Einordnung des Auges zu einem Entwicklungsstadium wichtig. Anhand des anterior–posterioren Durchmessers des Auges kann z. B. ein Mikrophthalmus diagnostiziert werden. Die Fallbeispiele umfassen u. a. das Goldenhar-Syndrom, das MIDAS-Syndrom und Kolobome. Zusammenfassend ist die Beurteilung fetaler Augen eine verantwortungsvolle Aufgabe, da die okularen Befunde in die kinderpathologische Diagnose mit einfliesen, auf die sich die Beratung der Eltern stutzt.

Erkrankungen des hinteren Augenabschnitts und der Orbita sind durch histologische Charakteristika gekennzeichnet, die sich oft auch klinisch nachvollziehen lassen wie z. B. die ultrasonografisch nachweisbaren Verkalkungsherde beim Retinoblastom. In dem folgenden Beitrag werden histologische Befunde von Tumoren und anderen Erkrankungen des hinteren Augenabschnitts und der Orbita vorgestellt und mit dem klinischen Erscheinungsbild sowie bildgebenden Verfahren korreliert: Aderhaut-/Ziliarkorpermelanom, Aderhautnavus, Aderhautmetastasen, Hamangiom der Aderhaut, Retinoblastom, Morbus Coats, sympathische Ophthalmie, pleomorphes Adenom der Tranendruse, Dakryoadenitis, Lymphom, Rhabdomyosarkom, Langerhans-Zellhistiozytose, orbitale Metastasen und Phthisis bulbi. Die Histopathologie ist nach wie vor der Goldstandard fur die definitive Diagnose vieler Erkrankungen. Insbesondere fur Augenarzte in der Weiterbildung ist es wichtig, fruhzeitig mit klinisch-pathologischen Korrelationen in Beruhrung zu kommen. Vom Verstandnis pathophysiologischer Zusammenhange, das dabei sehr bildlich vermittelt werden kann, profitiert jeder klinisch tatige Augenarzt. Operativ tatige Augenarzte konnen die postoperativen Wundheilungsvorgange am histologischen Praparat studieren. Eine enge Zusammenarbeit zwischen Klinikern und Ophthalmopathologen fuhrt zudem nicht nur zu einer optimierten Materialaufbereitung und Diagnostik, sondern auch zu einer verbesserten Patientenvor- und -nachsorge. Diese augenarztliche Kompetenz sollte auch weiterhin gefordert und unterstutzt werden, um die Qualitat der augenarztlichen Versorgung auf einem hohen Niveau zu halten.

BACKGROUND: Pilomatrixoma (calcifying epithelioma of Malherbe) is a rare benign tumour originating from the matrix of the hair root. In this study, we evaluated the clinical and histopathological characteristics of all the respective tumour specimens submitted to our laboratory since 1997. PATIENTS/MATERIALS AND METHODS: We reviewed the clinical and histologic findings of 13 pilomatrixomas localised to the ocular adnexae. Paraffin sections were stained with H&E and PAS and, in addition, immunohistochemistry was performed using markers for macrophages (CD68), for proliferative activity (Ki67), and for proliferation/apoptosis (CAS/CSE1L). RESULTS: All 13 pilomatrixomas were of reddish-livid colour and localised on the upper eyelid and/or near the eyebrow. The patients ages ranged from 4 to 84 years without preference for either gender. In 3 cases the correct diagnosis was made intraoperatively, while no pilomatrixoma was diagnosed prior to surgery. The most frequent false diagnoses were retention cyst (n = 3), atheroma (n = 2), and abscess (n = 2). There was no tumour recurrence in any of the cases within the time period presented. All 13 pilomatrixomas demonstrated the characteristic histology with areas of basophilic islands, shadow cells, calcification and a chronic granulomatous inflammation with foreign body giant cells in varying amounts. Immunhistochemical staining with anti-Ki67 and anti-CAS/CSE1L indicated a regular cellular maturation of the basophilic cells. No correlation was found between any of the histological or immunohistochemical markers and clinical parameters such as size, age and gender of the patient and duration of tumour presence prior to surgery. CONCLUSIONS: This case series represents one of the largest published so far, and underscores that pilomatrixoma can appear at any age. While it is usually not recognised clinically, it should be considered in the differential diagnosis especially for tumours of the upper eyelid. Our immunohistochemical findings support the basically benign nature of a pilomatrixoma but also point to the chronic inflammatory stimulus that can lead to complications after incomplete removal.

Ein heute 44-jahriger Patient bemerkte 1983 erstmals eine schmerzlose, hellrote Schwellung am linken Auge. Die fruhe Indozyaningrunangiographie, MRT und Ultraschall ergaben die Diagnose eines razemosen Hamangioms der Bindehaut. Obwohl ein razemoses Hamangiom bereits bei der Geburt besteht, bleibt die Erkrankung klinisch lange stumm, und Symptome treten oft erst im fruhen Erwachsenenalter auf. Bei dem Patienten wurden auswarts chirurgische Exzisionen wegen erheblicher Blutungen abgebrochen. Eine symptomatische Behandlung im Sinne einer Oberflachenpflege steht im Vordergrund.

In 1939 F. Wegener published a report concerning a peculiar febrile syndrome with necrotizing inflammation of the upper respiratory tract, focal glomerulonephritis and systemic angiitis (Wegener 1939). Clinically Wegener's granulomatosis can be divided into a limited and a generalized form. We describe a patient presenting with unilateral ocular symptoms suspicious of a neoplasm, one episode of angina pectoris and general malaise without any clinical signs of upper respiratory tract involvement. Before Wegener's granulomatosis was diagnosed by a positive titre of antineutrophil cytoplasmic antibodies and open lung biopsy, immunoscintigraphy for malignant melanoma showed a false positive result. Systemic treatment with corticosteroids and cyclophosphamide resulted in an improvement of the ocular symptoms within weeks.

Background Malignant melanoma of the conjunctiva is a rare tumour. Early disease stages may be difficult to distinguish from benign lesions such as pigmented nevi or primary acquired melanosis. We describe the therapeutic procedure and histological findings in two patients and review the epidemiology and pathogenesis of malignant conjunctival melanoma. Two female patients (84 and 85 years old, respectively) presented with a pigmented tumor close to the limbus with surrounding conjunctival pigmentation and involvement of the cornea. Results Following complete excision of the tumour, conjunctival malignant melanoma arising from primary acquired melanosis was diagnosed histologically. Subsequent treatment with mitomycin C eye drops was initiated. There was no recurrence of the tumor within the follow-up period (24 and 6 months). Discussion Patients with primary acquired melanosis need to be reviewed on a regular basis to detect malignant transformation at an early stage.

Background Clinically, corneal endotheliitis is characterized by endothelial cell edema associated with immune precipitates. It is supposed to be triggered by a humoral or cellular immunologic reaction against antigens of the corneal endothelium. As yet, only few histopathologic studies of this disease have been performed. Patient A 42-years-old patient developed a complete corneal nebula with neovascularizations due to relapsing herpetic keratitis. He underwent perforating keratoplasty. The inflammatory infiltrate lining Descemet's membrane and the corneal endothelium consisted mostly of CD 45 R0-positive T-lymphocytes with only few neutrophils, macrophages and CD 20-positive B-lymphocytes. We could not demonstrate any viral particles in the removed corneal tissue by either electron microscopy or using antibodies against Herpes simplex type I or II. Conclusions Histologically, herpetic endotheliitis appears to correspond to corneal endotheliitis due to graft rejection. Complete virus particles are not necessarily required in case of long-standing inflammation and corneal neovascularizations.

We present the clinical and histopathological findings in the unusual case of a recurrent corneal tumour in a child. Light and ultrastructural observations as well as immunohistochemistry indicate a benign reactive process. However, the prognosis regarding visual function is poor due to the infiltrative nature of the lesion and its tendency to recur. A possible diagnosis of nodular fasciitis is discussed.

Background In recent years TPA (tissue-plasminogen activator) has been increasingly and successfully used for the treatment of severe, postoperative fibrin reaction in the anterior chamber. So far no serious side effects of this treatment have been reported. Patients and methods Altogether, 32 patients received 0.2 ml solution with 20 micrograms TPA intracamerally. In 2 cases a dense corneal opacity was observed 12-24 hours after the injection of TPA which was resistant to treatment with local dexamethasone and lubricants. Therefore it was removed by superficial keratectomy. In one case the keratectomy specimen could be examined by light- and electronmicroscopy. Results In the keratectomy specimen a selective, fine-granular calcification of Bowman's membrane could be demonstrated. Conclusions The intracameral TPA treatment for postoperative fibrin reaction can cause a rapid band keratopathy. Therefore the application of TPA should be restricted to severe therapy-resistant cases of intracameral fibrin reaction. In cases with the development of a band keratopathy EDTA-treatment is recommended.

BACKGROUND Large uncovered defects of lid-skin which extend beyond the margin of the orbital rim frequently cause wound contracture and malposition of the lid. PATIENTS AND METHODS Between April 1989 and October 1992 we have successfully used synthetic skin in severe cases of traumatic skin ablation (n = 3) and in complicated tumor surgery of the lids and of the medial canthal area (n = 14). Polyurethan foam material was used for temporary skin replacement (Epigard). RESULTS The synthetic skin stimulated wound granulation and vascularization of the host area before final closure of the defect was achieved. No wound contracture occurred. CONCLUSION Temporary bandage with synthetic skin is recommended if large skin defects can not be closed within 48 hours.

Purpose To explore the role of integrin alpha5beta1 in proliferative vitreoretinopathy (PVR) pathogenesis by evaluating the expression alpha5beta1 on ARPE-19 cells and patient proliferative membranes, quantifying the inhibitory effects of JSM6427 (a small molecule alpha5beta1 inhibitor) on ARPE-19 cell adhesion and migration, and assessing the therapeutic potential of JSM6427 in a rabbit retinal detachment model. Methods Expression of alpha5beta1 was evaluated on activated ARPE-19 cells by flow cytometry and on PVR membranes by immunohistochemistry. ARPE-19 cells were used in fibronectin-dependent adhesion and migration assays with various concentrations of JSM6427; IC(50) was calculated. In the rabbit model, eyes were intravitreally injected with vehicle or JSM6427 on day 0 or 1 after retinal detachment; BrdU was administered intravitreally on day 3, and retinal tissues were harvested on day 3 (4 hours later) or 7. Retinal scarring, cellular proliferation, and inflammatory responses were quantified, and retinal morphology was analyzed in retinal sections. Results Activated ARPE-19 cells and PVR membranes expressed high levels of alpha5beta1; expression was low in control eyes. JSM6427 provided a dose-dependent blockade of ARPE-19 cell adhesion to fibronectin (IC(50), 7.1 +/- 2.5 microM) and inhibition of migration (IC(50), 6.0 +/- 4.5 microM). In the rabbit model, intravitreal injection of JSM6427 provided significant inhibition of proliferation of retinal cells (Muller cells, microglia, and macrophages) on days 3 and 7 after detachment and inhibition of inflammatory response and retinal scarring on day 7 after detachment. Conclusions JSM6427 is a promising treatment for PVR, with data suggesting that inhibition of alpha5beta1-fibronectin interactions addresses multiple pathways involving retinal pigment epithelial, glial, and inflammatory cells.

The histological and ultrastructural pathology of two corneas that were removed from an infant with congenital glaucoma prior to keratoplasty is described. Mitomycin gehort zur Gruppe der alkylierend wirkenden Antibiotika und hemmt selektiv die DNS-Synthese. Nach intrazellularer Aufnahme erfolgt durch Reduktion des eigentlichen Molekuls eine Aktivierung zu einem bi- oder trifunktionellen Alkylans. Mitomycin wirkt Zellzyklus-unspezifisch, allerdings mit einer besonderen Sensitivitat in der spa,ten S-Phase, der G2- und M-Phase. Die G0- und G1-Phase werden am wenigsten beeinflust. Die Zellzykluswirkung scheint dosisabhangig zu sein. Mitomycin wird im Gastrointestinaltrakt absorbiert und bis zu einem gewissen Grad von allen Schleimhauten aufgenommen (nach oraler Gabe erreicht man eine Serumkonzentration von etwa 5% der Maximalkonzentration nach i.v.-Applikation der gleichen Menge). Mitomycin ist in allen Geweben nachweisbar, insbesondere jedoch in Niere, Muskeln, Herz und Lunge; in Gehirn, Leber, Milz und Haut reichert es sich deutlich weniger an (Meerschweinchen). Der Abbau erfolgt dann hauptsachlich durch Metabolisierung in der Leber. Insgesamt ist die Pharmakokinetik von Mitomycin nicht dosisabhangig, zeigt jedoch eine grose individuelle Schwankungsbreite. Die Dosierung bei systemischer Monotherapie betragt je nach Korpergewicht etwa 50 mg alle 6 Wochen, bei einer Kombinationstherapie nur etwa die Halfte. Nebenwirkungen nach systemischer Gabe sind vor allem Hamatotoxizitat, Alveolitis und Lungenfibrose sowie Nierenschadigungen in Form von hamolytisch-uramischem Syndrom und mikroangiopathischer Anamie. Bei direkter Exposition von Haut oder paravenoser Injektion kann es zu Hautnekrosen kommen.

Recombinant tissue plasminogen activator (rTPA) is commonly used in patients with myocardial infarction. Recently, it has also been applied intraocularly to dissolve postoperative fibrin with no serious complications being reported so far. In this study we describe our own experience with rTPA in 25 patients with persisting fibrinous membranes in the anterior segment. rTPA (Actilyse, Dr. Karl Thomae GmbH) was given in a single dose of 25 micrograms and injected into the anterior chamber via a paracentesis. We did not encounter any complications during the injection of rTPA. In 21 eyes fibrin could be reduced significantly, albeit sometimes only slowly. In 13 patients, the membrane had dissolved almost completely by the following day. In contrast, no success was observed after glaucoma surgery (2 eyes) and in chronic iritis (1 eye), or when fibrin mixed with blood was treated (1 eye). There were two (controllable) post-operative hemorrhages (rTPA after vitrectomy, and for fibrin/blood after cataract surgery). In addition, we noted 2 cases of irreversible superficial corneal clouding (rTPA after cataract surgery). We conclude that injection of rTPA can be a useful addition to steroid treatment in selective cases of persisting fibrin in the anterior segment. Long-standing membranes, however, are unlikely to be dissolved. Care should also be taken and rTPA be avoided when there is evidence of recent bleeding. Most worrying to us were the corneal complications that we cannot explain to date. With regard to the definite time correlation we feel that rTPA or one of the solution components might be the cause of this unusual feature.

Objective To evaluate the pharmacologic activity and tolerability of JSM6427, a potent and first selective small-molecule inhibitor of integrin α5β1, in monkey and rabbit models of choroidal neovascularization (CNV). Methods JSM6427 selectivity for α5β1 was evaluated by in vitro binding assays while the ability of JSM6427 to inhibit CNV was investigated in a laser-induced monkey model and a growth factor–induced rabbit model. Intravitreal injections of JSM6427 (100, 300, or 1000 μg) or vehicle were administered immediately after the CNV induction procedure and at weekly intervals for 4 weeks. Fluorescein angiography was performed weekly. Ocular tolerability was evaluated ophthalmoscopically and histologically in both models; additional assessments in monkeys included electroretinography, biomicroscopy, pathological examination, and analysis of JSM6427 pharmacokinetics. Results JSM6427 was highly selective for the α5β1-fibronectin interaction. Weekly intravitreal injections of JSM6427 resulted in a statistically significant dose-dependent inhibition of CNV in laser-induced and growth factor–induced models without any ocular JSM6427-related adverse effects. JSM6427 was cleared through the systemic circulation with no evidence of systemic accumulation. Conclusions Intravitreal JSM6427 provided dose-dependent inhibition of CNV in monkey and rabbit experimental models. Clinical Relevance JSM6427 may provide a new approach for the treatment of ocular neovascular diseases such as age-related macular degeneration in humans.
