29. Cardiopulmonary exercise testing in children: an individualized protocol for workload increase.

Abstract

Objectives: To investigate the feasibility of individualized workload increments, as used in adults, for exercise testing in children; and to investigate whether this individualized protocol makes it possible to satisfy the usual criteria for maximal exercise (clinical exhaustion, predicted maximum heart rate [HRmax], oxygen uptake [O(2)] plateau, maximal respiratory exchange ratio > 1.1).Design: Prospective clinical study.Setting: Pediatric exercise testing laboratory.Subjects: Ninety-two children aged 5 to 17 years with various cardiac and respiratory diseases (33 with asthma, 11 with bronchopulmonary dysplasia, 6 with cystic fibrosis, 10 with congenital heart disease, and 32 miscellaneous).Interventions: Individualized maximal incremental exercise testing. The increase in workload was adapted to the individual and was calculated from predicted maximal oxygen uptake (O(2)max) for each child. The test lasted 10 to 12 min.Results: The exercise test was well tolerated by all children and was maximal in all but seven patients. A total of 65.7% of children reached the predicted O(2)max and 68.4% satisfied the criteria for a O(2) plateau at peak exercise. The predicted HRmax was achieved in all but two children. The mean maximal respiratory exchange ratio was 1.06.Conclusion: The individualized protocol for increasing workload, based on O(2) rather than power, was well tolerated by children. In our view, the best two criteria for assessing the maximality of the tests were clinical exhaustion and HRmax, especially if the O(2) plateau was not reached. These results suggest that individualized protocols could be used instead of standardized tests for exercise testing in children. [ABSTRACT FROM AUTHOR]

Abstract

Abstract: Allergic hypersensitivity reactions occurring during anaesthesia are a major cause of concern, since these reactions remain unpredictable and may be life-threatening for the patient. Neuromuscular blocking agents, latex and antibiotics are the three agents most frequently involved in these reactions. A systematic allergoanaesthetic screening after every peri-anaesthetic incident is clearly warranted, both to avoid the patient''s having a reaction during any subsequent anaesthesia and to obtain additional epidemiological data that can be used to improve the future management of all anaesthesia procedures. This allergy screening includes standardized laboratory and skin test procedures, with verification of the concordance between the clinical history and the test results. Atopic infants and those having had multiple surgical interventions are at risk for allergic reactions to latex. In the case of children with spina-bifida, strict latex avoidance measures must be followed systematically, beginning with the first intervention. [Copyright &y& Elsevier]

Abstract

Neuromuscular blocking agents are the agents most frequently involved in peri-operative anaphylaxis, being more important than other drugs or latex; the incidence of these reactions is reported to be 1/6500 operations under anaesthesia. Suxamethonium and rocuronium are muscle relaxants with the highest risk of anaphylaxis. Their divalent ammonium quaternary ionic molecular structure of curare favours allergic sensitisation. The symptoms of anaphylaxis due to muscle relaxants are usually severe. Risk factors are a previous sensitisation and the female sex. The diagnosis of an allergic reaction is made with blood tests (plasma tryptase and histamine levels and specific IgE antibody assays) and skin prick and intradermal tests. The results of these tests must always be considered along with the clinical history, even if a coherent clinical history is not obtained. The recommendations for a subsequent anaesthesia in a child allergic to a curare relaxant should be based on the results of the skin tests, including those obtained with cross-reacting agents.
