Abstract

In intensive care units (ICUs), critically ill patients are monitored with electroencephalograms (EEGs) to prevent serious brain injury. The number of patients who can be monitored is constrained by the availability of trained physicians to read EEGs, and EEG interpretation can be subjective and prone to inter-observer variability. Automated deep learning systems for EEG could reduce human bias and accelerate the diagnostic process. However, black box deep learning models are untrustworthy, difficult to troubleshoot, and lack accountability in real-world applications, leading to a lack of trust and adoption by clinicians. To address these challenges, we propose a novel interpretable deep learning model that not only predicts the presence of harmful brainwave patterns but also provides high-quality case-based explanations of its decisions. Our model performs better than the corresponding black box model, despite being constrained to be interpretable. The learned 2D embedded space provides the first global overview of the structure of ictal-interictal-injury continuum brainwave patterns. The ability to understand how our model arrived at its decisions will not only help clinicians to diagnose and treat harmful brain activities more accurately but also increase their trust and adoption of machine learning models in clinical practice; this could be an integral component of the ICU neurologists' standard workflow., Comment: 24 pages including appendices, 9 figures, published at NEJM AI

Abstract

DICOM (Digital Imaging and Communications in Medicine – Tıpta Dijital Görüntüleme ve İletişim) tıbbi görüntülerin arşivlenmesi, daha sonradan kullanılması, farklı yerlere dağıtılması ve sunulmasını sağlayan bir dosya standartıdır. Bu standartta dosyalar, dosya başlığı ve görüntü piksellerinden oluşmaktadır. Tıbbi alanda veri güvenliğinin sağlanması için hem dosya başlığında hem de piksellerinde çeşitli güvenlik önlemleri alınması gerekmektedir. Yapılan çalışmada bu güvenlik önlemlerinin alınması için steganografi, kriptografi ve hash özet fonksiyonu bir arada kullanılmıştır. Dosya başlığında yapılan çalışmalar ile hastaya ait demografik bilgilerin güvenliği sağlanırken, pikseller üzerinde yapılan çalışmalarla ise doktor tanısı, hasta demografik bilgileri ve görüntü bütünlüğü sağlanmıştır. Bu çalışmada yapılan analizler PSNR, MSE, SSIM gibi karşılaştırma algoritmaları ile karşılaştırılmış ve geliştirilen yöntemin üstünlükleri belirtilmiştir. Önerilen yöntem ile birlikte hasta tanı ve tedavi süreci minimum düzeyde etkilenmiş ve hasta veri güvenliği sağlanmıştır.

39. Perioperative analgesic efficacy of bilateral superficial cervical plexus block in patients undergoing thyroidectomy: a randomized controlled trial

Abstract

Abstract Introduction: Bilateral superficial cervical plexus block (BSCPB) is a common method used for analgesia in thyroid surgery. We investigated the analgesic efficacy of bilateral superficial cervical plexus block in the intraoperative and postoperative periods. Materials and methods: Patients (n = 46) undergoing thyroidectomy were randomly separated into the following 2 groups: the general anesthesia group (GA; n = 23) and the general anesthesia plus BSCPB group (GS; n = 23). The intraoperative analgesic requirement (remifentanil) and visual analog scale (VAS) score at multiple time points during the postoperative period (after extubation, at 15 , 30 minutes and 1, 2, 6, 12, 24, and 48 hours post operation) were evaluated. Total tramadol and paracetamol consumption as well as the amount of ondansetron used was recorded. Results: The intraoperative remifentanil requirement was significantly lower in the GS Group than in the GA Group (p= 0.009). The postoperative pain scores were significantly lower in the GS Group than in the GA Group at 15 (p< 0.01), 30 (p< 0.01) minutes, and 1 (p< 0.01), 2 (p< 0.01), 6 (p< 0.01), 12 (p< 0.01) and 24 (p= 0.03) hours. The postoperative tramadol requirement was significantly lower in the GS Group than in the GA Group (p= 0.01). The number of patients that used ondansetron was significantly lower in the GS Group than in the GA Group (p= 0.004). Conclusion: We concluded that BSCPB with 0.25% bupivacaine reduces the postoperative pain intensity and opioid dependency in thyroid surgery patients.

INTRODUCTION[|]Patients with chronic obstructive pulmonary disease (COPD) admitted to the pulmonary rehabilitation (PR) program may lose their post-PR gains over time. In this study, we aimed to investigate whether the gains after PR continued in the first month following PR. [¤]METHODS[|]Patients with COPD who completed the PR program were evaluated retrospectively. The incremental shuttle walking test (ISWT) was used to measure the exercise capacity. In PR, patients were trained in strengthening exercises, cycling, walking band, and respiratory exercises in the presence of physiotherapist 2 days a week for a total of 8 weeks. After PR, patients were asked to record their exercise status each day into an exercise log. [¤]RESULTS[|]A total of 35 patients with COPD (mean age 64+-8 years) participated in the study. Abstract

Abstract Objective This clinical trial sought to evaluate the clinical effectiveness of concentrated growth factor (CGF) and compare it with connective tissue graft (CTG) with coronally advanced flap (CAF) in the treatment of Miller Class I gingival recessions (GR). Methodology This split-mouth study included 74 Miller Class I isolated (24 teeth) or multiple (50 teeth) GRs in 23 jaws of 19 patients. GRs were randomly treated using CGF (test group: 37 teeth; 12 teeth in isolated GRs, 25 teeth in multiple GRs) or CTG with CAF (control group: 37 teeth;12 teeth isolated GRs, 25 teeth in multiple GRs). Clinical variables, plaque index (PI), gingival index (GI), probing depth (PD), recession depth (RD), recession width (RW), clinical attachment level (CAL), keratinized tissue thickness (KTT), keratinized tissue width (KTW), and root coverage (RC) were assessed at the baseline as well as at three and six months post-surgery. Healing index (HI) were obtained in the second and third weeks post-surgery. Postoperative pain was assessed for the first seven days using a horizontal visual analog scale (VAS). Results No significant change was observed in PI, GI, or PD values in either the intergroup or the intragroup comparisons. A statistically significant decrease was observed in CAL, RD, and RW, and KTT increased in all groups at three and six months compared with the baseline. The control group had greater increases in KTW, KTT, and RC at three and six months. No significant difference was found in CAL or RD at the third and sixth months between the two groups. Healing was found to be similar for both groups in the second and third weeks post-surgery. The VAS values in the control group were higher than in the test group, especially at the second, fourth, fifth, and seventh days postoperatively. Conclusions CTG is superior to CGF with CAF for increasing KTT, KTW, and RC. CGF may be preferable due to decreased postoperative pain.
