Abstract

Idiopathic granulomatous mastitis (IGM) is an uncommon inflammatory disease of the breast. Differential diagnosis and exclusion of breast cancer is the most challenging issue for patients and physicians. IGM may be differentiated from cancer and other diseases with histopathological evaluation; however, the response to empirically initiated steroid therapy may sometimes be useful to distinguish it from other diseases. In this context, whether erythema nodosum may assist in the IGM diagnosis is an unasked question. Here we present the case of a 30-year-old woman presenting with tenderness, redness and discharge in her left breast accompanied by a mass, together with pain in her ankles, the findings of IGM and erythema nodosum (EN). We aim to review the medical literature regarding the association between IGM and EN. IGM should be primarily considered when a patient presents with signs of inflammatory breast disease and EN. İdiyopatik granülomatöz mastit (İGM), memenin etyolojisi çok iyi bilinmeyen ve nadir görülen bir inflamatuar hastalığıdır. Meme kanserini taklit edebilmesiyle önem kazanan bu durum, ayırıcı tanı ve meme kanserlerinin dışlanması aşamasında hasta ve klinisyen açısından zorlayıcı olabilmektedir. İGM tablosu kanser ve diğer hastalıklardan histopatolojik inceleme ile ayırt edilse de, ampirik başlanılan steroid tedavisinden alınan olumlu yanıt da İGM’nin diğer hastalıklardan ayırt edilmesinde klinisyene yarar sağlayabilir ve hekim açısından yol gösterici olabilir. Bu bağlamda, eritema nodozumun (EN) da İGM’nin tanısında destekleyici bir bulgu olup olamadığı hususu daha önce sorgulanmamış bir konudur. Burada, sol memede kitlenin de eşlik ettiği ağrı, kızarıklık, hassasiyet ve akıntı yakınmaları olan, beraberinde her iki ayak bileğinde ağrının var olduğu eritema nodozum ve idiyopatik granülomatöz mastit bulguları ile presente olan 30 yaşındaki bir kadın hasta sunulmuştur. Bu yazıda aynı zamanda medikal literatürdeki İGM ve EN arasındaki birliktelik ve ilişkiyi de tartışmayı amaçladık. İnflamatuar meme hastalığı ve EN bulguları ile prezante olan olgularda İGM ayırıcı tanılar arasında ilk planda düşünülmelidir.

11. Clinical Significance of Neutrophil to Lymphocyte Ratio and Mean Platelet Volume for Predicting the Severity of Hepatic Fibrosis in Patients with Chronic Hepatitis B

Abstract

Bu çalışmada; kronik hepatit B hastalarında nötrofil/lenfosit oranı (NLO), ortalama trombosit hacmi (OTH) ve diğer biyokimyasal parametrelerin karaciğer fibrozisinin ciddiyetini belirlemedeki klinik önemini değerlendirmeyi amaçladık. Karaciğer biyopsisi uygulanmış toplam 95 naif KHB hastası çalışmaya dahil edildi. Lökosit, trombosit, hemoglobin sayımı, nötrofil, lenfosit, OTH ve diğer rutin biyokimyasal parametreler test edildi. Ishak skorlama sistemine göre karaciğer biyopsi örnekleri incelendi. Fibrozis derecesi 0, 1 ve 2 olanlar Grup 1, fibrozis düzeyi 3 ve üzeri olanlar Grup 2 olarak belirlendi. Mevcut verilerimizin analizleri SPSS 15 programı ile gerçekleştirildi. ‘p’ değerinin < 0,05 olduğu sonuçlar istatistiksel olarak anlamlı kabul edildi. 95 hastadan, 76’sı erkek (%80) ve 19’u kadındı (%20). Hastaların ortalama yaşı 39.91±18.6 yıldı, 30 vakanın (%31,5) HBeAg pozitifliği mevcuttu. 36 hastanın (%37,8) fibrozis skoru 3 ve üzerinde idi, buna karşılık 59 hastanın (%62,2) fibrozis skoru 3’ün altındaydı. İki grup arasında NLO (Group 1: 2,38± 0,96, Group 2: 1,64 ± 0,60, p < 0,001) ve OTH (Group 1: 8,13± 1,20, Group 2: 8,77 ± 1,47, p < 0,05) açısından anlamlı farklılık mevcuttu. KHB hastalarında, fibrozis ciddiyetini ortaya koyabilecek tanısal performanslarını belirlemek amaçlı AST, ALT, trombosit, NLO, OTH ve albümin için ROC (Receiver Operating Characteristic) eğrileri çizildi ve kıyaslamalar yapıldı, eğri altında kalan alanlar sırasıyla 0,70, 0,49, 0,27, 0,20, 0,67 ve 0,13’tü. İleri düzeyde karaciğer fibrozisi olan KHB hastalarında, NLO düzeyleri anlamlı düzeyde düşük ve OTH düzeyleri anlamlı düzeyde yüksek olduğundan bu iki parametre hepatik fibrozisi belirleme açısından bağımsız noninvazif belirteçler olarak tanımlanabilirler. Kronik hepatit B hastalarında, fibrozis ciddiyeti ile NLO ve OTH arasındaki ilişkiyi irdeleyecek ileri çalışmalara ihtiyaç duyulmaktadır, In this study, we aimed to determine the clinical significance of the neutrophil to lymphocyte (NLR) ratio, mean platelet volume (MPV) and other biochemical parameters for predicting the degree of liver fibrosis in patients with chronic hepatitis B (CHB) infection. A total of 95 biopsy-proven naïve CHB cases patients were included in the study. The complete blood count variables including white blood cell, hemoglobin, hematocrit, platelet count, neutrophil, lymphocyte, MPV, and other routine biochemical parameters were tested. Liver biopsy samples were examined using the Ishak scoring system. Individuals with a fibrosis score of 0, 1 and 2 were included in Group 1 and 3,4,5 and 6 were included in Group 2. Data analyses were carried out using SPSS 15 software. Statistical significance was set at a p-value of less than 0.05.Of the 95 cases, 76 (80%) were men and 19 (20%) were women. The mean age of the patients was 39.91±18.6 years, and 30 cases (31.5%) had HBeAg positivity. Fibrosis scores of 36 cases (37.8%) were greater than or equal to 3, whereas 59 cases (62.2%) had fibrosis scores less than 3. There was a significant difference between these two groups for NLR (Group 1; 2.38± 0.96, Group 2; 1.64 ± 0.60, p < 0.001) and MPV (Group 1; 8.13± 1.20, Group 2; 8.77 ± 1.47, p < 0.05). We have compared the receiver operating characteristic curves for the diagnostic performance of aspartate aminotransferase, alanine aminotransferase, platelet count, NLR, MPV, and albumin identifying fibrosis in CHB and area under the curve values for these variables were 0.70, 0.49, 0.27, 0.20, 0.67, and 0.13, respectively.NLR values were significantly lower and MPV values were significantly higher in hepatitis B virus-infected patients with advanced liver fibrosis and can be defined as independent noninvasive predicting factors for hepatic fibrosis. Further studies are required to determine the associations between NLR, MPV and the severity of fibrosis in chronic hepatitis B patients

Abstract

Adult-onset Still's disease (AOSD) is a rare multisystemic immune-mediated disease of unknown etiology with quotidian spiking fever, evanescent rash, arthralgia, and multiple organ involvement. The few AOSD cases that have been reported developed Purtscher's-like retinopathy associated with thrombotic microangiopathy (TMA). Here, we report Purtscher's-like retinopathy without TMA in a patient with AOSD. A 29-year-old-man who presented for evaluation of blurred vision was diagnosed with AOSD based on Yamaguchi criteria. He had Purtscher's-like retinopathy in his right eye. Lesions improved after steroid treatment. Although almost all reported AOSD cases with Purtscher's-like retinopathy are associated with TMA, in this case such a complication was not encountered. [ABSTRACT FROM AUTHOR]
