OBJECTIVE: Video-assisted thoracic surgery (VATS) is usually performed under general anaesthesia. However, some pleural and pulmonary interventions may be performed under local anaesthesia with sedation. In this study, patients who underwent VATS without general anaesthesia were evaluated to determine the benefits and safety of the procedure. MATERIAL AND METHODS: The medical records of all patients who underwent VATS with local anaesthesia and sedation between January 2007 and December 2011 were reviewed. All procedures were performed with the patient in a semi-recumbent position in an operating room. Prilocaine-HCI (20 mg/rnL; 10 cc) was used for local anaesthesia. Sedation was obtained with intravenous administration of propofol (0.5 mg/kg), fentanyl (0.5-1 μg/kg) and midazolam (1-2 mg in total). RESULTS: There were 67 male and 34 female patients, ranging from 17 to 88 years of age. Most of the awake VATS procedures were performed to evaluate pleural disease (93 patients; 92.0%). Pleural biopsy and/or drainage was performed in 84 patients (83.2%), pleural biopsy/drainage with talc pleurodesis in 10 patients (9.9%), treatment of empyaema in fourpatients (4.0%), evacuation of haemothorax in two patients (1.9%) and diaphragmatic pore closure in one patient (1.0%). Of the 101 patients, 90 (89.1%) had uniport VATS and 11 (10.9%) had two-port VATS. Conversion to general anaesthesia was necessary in 4.0% of cases due to severe coughing in three patients and pleural adhesions in one patient. CONCLUSION: VATS utilising local anaesthesia and sedation is well tolerated and safe for diagnosis and treatment for pleural pathologies. [ABSTRACT FROM AUTHOR]

Aim: In this case, we aimed to present the F-18 FDG PET/CT findings of a patient with primary pericardial malignant mesothelioma which is rarely reported in the literature. Case: A 56-year-old male patient was admitted with the complaint of shortness of breath. In thorax CT examination, pleural effusion was observed in the left hemithorax with a thickness of 4 cm and a thickness of 1.5 cm on the right side. Pericardial effusion which reached 5 cm in thickness and pericardial thickening was also observed in pericardium. Lymph nodes which to reach ~ 1.5 cm in size were observed in the mediastinum. The cytologic examination from pericardial effusion was reported as malignant cytology, which is thought to be primarily compatible with malignant mesothelioma. F-18-FDG PET/CT examination performed for staging of the patient and it showed that the heart was observed to be bigger than normal, pericardial effusion measured as ~ 5 cm in pericardium and F-18 FDG uptake in the whole pericardium (SUVmax: 12.2). In addition to the large pleural effusion observed in the left hemithorax, increased F-18 FDG uptake was observed in the atelectatic area related with pericardium. F-18 FDG uptake in multiple lymph nodes with continuity of the lesion identified in the pericardium which has conglomerate-like appearance was observed in the prevascular, upper-lower paratracheal, subaortic, aortopulmonary window and bilateral hilar lymphatic stations in mediastinum and these lymph nodes was invading the pulmonary trunk and the pulmonary arteries (highest SUVmax: 10.8). F-F-18DG uptake in conglomerated lymph nodes was observed in the right neck region starting from level 3 and extending to subclavicular and upper mediastinal area and F-18 FDG uptake in lymph nodes was observed in right pectoral and axillary lymphatic stations (Figure 1 and 2). The patient died as a result of sudden respiratory failure and cardiac arrest after completion of PET/CT. He did not respond to resuscitation. Conclusion: Although the most common malignancy of pericardium is mesothelioma, primary pericardial malignant mesothelioma is a very rare fatal tumor with a incidence of 0.0022%. Since the clinical findings are nonspecific, the diagnosis is extremely difficult. F-18 FDG PET/CT is important in the diagnosis and staging of pericardial malignancies. [ABSTRACT FROM AUTHOR]

Objectives: The aim of this study was to investigate the correlation between procalcitonin and other acute phase reactants, and also analyze their relationship with clinical situation in chronic obstructive pulmonary disease (COPD) acute exacerbations. Materials and methods: The study was made with 122 acute COPD exacerbated patients, who were admitted to emergency service. Patients with below 0.25 ng/ml PCT value included Group 1, and the patients with PCT values ⩾ 0.25 ng/ml Group 2. Serum procalcitonin levels, erythrocyte sedimentation rate (ESR), C-reactive protein (CRP) values and white blood cell (WBC) counts were measured. Also, patients hospitalization time and mortality rates were recorded and compared with PCT. Results: Patients were divided in 3 groups according to their clinical diagnosis; Pneumonia (n=27), Mycoplasma-Chlamydia pneumonia (n=11) and the patients with only COPD exacerbation(n=84). Mean PCT values according to the groups were 9.47 ± 8.1 ng/ml, 0.41 ± 0.2 ng/ml, and 0.21 ± 0.05 ng/ml respectively. The relationship between PCT with CRP and white blood cell has been found between significiant (p=0.001, p=0.005 respectively), whereas the relationship between PCT and ESR was nonsignificant (p=0.55). Procalcitonin and CRP had a positive correlation with the hospitalization time (p=0.034, p=0.022 respectively). The mean ± standard error of PCT for the patients who died was 28.3 ± 27.5 ng/ml, and the difference between patients who died or were discharged was statistically significant (p= 0.012). Conclusion: PCT can be a useful indicator for morbidity and prognosis in COPD patients. [ABSTRACT FROM AUTHOR]

Malignant pleural effusion can be described as the presence of exudate in the pleural space, accompanied by the detection of cancer cells in pleural fluid or tissue, which is the indicator of the disease in its advance stages, and survival time of these patients ranges between 3 and 12 months. Background: This study aims to share our experiences in complications developing in patients who underwent Nuss procedure and the management of these complications., Methods: In the study, files of 59 patients (50 males, 9 females; mean age 17.6±5.1 years; range, 2.5 to 33 years) who were applied Nuss procedure for pectus excavatum in our clinic between July 2007 and May 2016 were retrospectively assessed. Patients" age, gender, surgical method-complications and hospitalization durations were recorded. Fisher"s chisquare test and logistic regression analysis were used for data evaluation., Results: Nuss procedure was performed in all patients without severe complications such as death, organ injury or massive hemorrhage. The most frequently observed postoperative earlyperiod complication was minimal pneumothorax (n=16, 27.1%), while bar dislocation was most frequently observed in the lateperiod (n=5, 8.3%)., Conclusion: Being male and/or over 23 years of age were determined as risk factors for complication development after Nuss procedure. Background: In the present study, we aimed to present our experience about retrieval of foreign bodies over a 28-year period., Methods: We retrospectively analyzed the files of 22 patients (18 males, 4 females; mean age 34.9 years; range, 9 months to 80 years) who required removal of a foreign body from the tracheobronchial tree between April 1987 and December 2015., Results: A total of 72.7% of the study group were 10 years old or older. There was no history of aspiration in 37% of cases, most often in older patients. Of the 22 unusual foreign bodies, seven (31.8%) were aspirated through permanent tracheostomy. The strangest foreign bodies were grass inflorescences, an acacia thorn, and construction nail. The foreign bodies were removed by rigid bronchoscopy in 18 patients, while thoracotomy was performed in two patients, and pericardiotomy in one patient. No intervention was required in one patient., Conclusion: The elderly and patients with tracheostomies may aspirate unusual foreign bodies.
