This article examines bone grafting of the alveolar process with simultaneous autologous tooth transplantation in patients congenital cleft of the lip, alveolar process and palate. Materials and methods. At the Department of maxillofacial surgery and hospital surgical Dentistry from October 2016 to present, 26 patients between the ages of 9 to 12 years with congenital cleft of the upper lip, alveolar process and palate were clinically examined and treated. Patients underwent bone grafting of the alveolar process of the upper jaw with simultaneous autologous tooth transplantation and the use of miniplates. During the year monitoring with x-ray control was performed to assess the degree of resorption of bone regenerate and the apical part of the roots of autotransplanted teeth. Results. The proposed method of bone grafting of the alveolar process with simultaneous autotransplantation allowed to stabilize the fragments of the upper jaw, restore the volume of the alveolar process in the cleft area, and to restore the dentition. Conclusion. The resulting clinical results and postoperative assessment of the bone structure after bone grafting of the alveolar process with simultaneous autologous tooth transplantation and fixation of fragments of the upper jaw using mini-plates provide a predictable result of restoring the volume of the alveolar process and restoring the dentition.

MLL is involved in fusion genes with more than 100 partner genes, approximately 80 of which have been characterized at the molecular level. MLL fusion genes are often found in infants (60-80% of acute lymphoblastic leukemia (ALL) cases and 40-50% of acute myeloblastic leukemia (AML) cases) and are appreciably rarer (8-10%) in children older than 1 year of age. MLL rearrangements are important markers in diagnosis and treatment choice. To identify the partner gene is of primary importance for prognosis and minimal residual disease monitoring. The structure of the fusion gene, including localization of the MLL breakpoints, is also informative. A method was developed to examine the fusion transcripts in order to identify the partner gene among the six most common ones and to establish the exon structure of the rearranged MLL. The method includes a multiplex reverse transcriptase-polymerase chain reaction (RT-PCR) to amplify and to fluorescently label a fusion transcript fragment and subsequent hybridization of the product on a biological microchip with immobilized oligonucleotides complementary to exons of MLL and its partner genes AFF1, MLLT1, MLLT3, MLLT4, MLLT10, and ELL. Hybridization results were verified by sequencing the RT-PCR products and, in some cases, performing long-distance inverse PCR (LDI-PCR). The study involved 38 bone marrow samples from ALL patients (including 33 children younger than 1 year of age) and 15 samples from AML patients (including 10 from children younger than 1 year of age). The main partner genes were AFF1 (49%), MLLT1 (27%), MLLT3 (12%), and MLLT10 (12%) in ALL and MLLT3 (80%), MLLT10 (10%), and MLLT4 (10%) in AML. Fusion gene transcripts most commonly included MLL exon 11 (58% of ALL cases and 50% of AML cases), suggesting a breakpoint in MLL intron 11.

The prognostic significance of genetic lesions in T-cell ALL still needs to be elucidated. Karyotyping and FISH were performed in samples from 120 patients with T-cell ALL registered in the trial Moscow-Berlin 2008. Most frequent rearrangements were TLX3 (N = 29; 24%) and TAL1 (N = 18; 15%), followed by KMT2A (N = 6; 5%), TLX1 (N = 5; 4.2%), and 11p13-15 (N = 5; 4.2%). In 16.7% of patients, the karyotype was normal, and in 30.8% 'other' aberrations were seen. Patients with a normal karyotype, TAL1, or KMT2A rearrangements had the most favorable outcome (probability of event free survival (pEFS): 82% ± 6%), while prognosis for patients with TLX3 and TLX1 rearrangements and 'other' aberrations was less favorable (pEFS: 62% ± 6%). Worst outcome was observed for five patients with 11p rearrangements (pEFS: 20% ± 18%). In summary, three subgroups of patients with T-cell ALL with significantly different outcomes could be defined by cytogenetic profiling.

Currently, many forms of leukemia are considered potentially curable, with prognosis and clinical outcome strongly dependent on the underlying molecular pathophysiology. A substantial number of leukemia patients harbor nonrandom karyotypic abnormalities that define subgroups with unique biological and clinical features. For detection of these types of gene rearrangements, a combination of multiplex RT-PCR with hybridization on oligonucleotide gel array was presented previously, which identified five chromosomal translocations with fusion variants. In the present study, additional clinically relevant translocations were included in our analysis using a second generation of microarrays. We also expanded significantly on the clinical correlation of our findings.An oligonucleotide microarray was designed for hybridization with products of a multiplex RT-PCR to identify the following translocations: t(9;22)p190, t(4;11), t(12;21), t(1;19), typical for acute lymphoblastic leukemia; t(9;22)p210 for chronic myeloid leukemia; and t(8;21), t(15;17), inv16, typical for acute myeloblastic leukemia.To demonstrate the potential clinical application of the method, 247 cases of childhood leukemia were screened, and the above-mentioned gene rearrangements were found in 30% of cases. The sensitivity and specificity of the assay is comparable with the RT-PCR technique, so that it can be used to follow minimal residual disease. The feasibility of an additional refinement of the method, on-chip-multiplex PCR, has been successfully demonstrated by identifying a common translocation, t(9;22), in chronic myeloid leukemia.Our data suggest that the microarray-based assay can be an effective and reliable tool in the clinical screening of leukemia patients for the presence of specific gene rearrangements with important diagnostic and prognostic implications. The method is amenable for automation and high-throughput analysis.

Histological and electron-microscopic studies of intact lung specimens were carried out in 30 patients with pulmonary tuberculosis developing in the presence of type I (insulin-dependent) diabetes mellitus. Material for investigation was obtained by intrapulmonary biopsy carried out during diagnostic bronchoscopy. Signs of adenopathy were detected in lung areas distant from tuberculous in foci all the cases. The diabetic origin of this condition was confirmed by expressed correlation with changes in the retinal and renal vessels and absence of correlation with the duration of tuberculous process. The degree of pulmonary adenopathy has a negative impact on the course of tuberculosis and efficacy of its therapy.

Progression of a tuberculosis process during antituberculous therapy is most commonly evoked by its inadequacy. The latter may be associated with undiagnosed drug resistance, patients' incompliance due to the lack of their consciousness or discipline. In these circumstances, the use of stimulating pathogenetic treatments can provoke an exacerbation. Decreased immune defense mechanisms after experienced intercurrent infections may also serve as one of the common causes of tuberculosis exacerbations. Only in rare cases, progression of clinical and X-ray symptoms is explained by the massive pathogen lysis syndrome (Jarisch-Herxheimer's syndrome) The management policy for these patients should be different due to the causes of exacerbations.
