Background: Glaucoma is one of the most common causes of blindness worldwide. The only evidence-based treatment to slow down the progression of glaucoma is the reduction of intraocular pressure (IOP) using local medication or through surgery. During the last years, a large number of microinvasive glaucoma surgery techniques (MIGS) has been developed, in order to reduce the IOP in glaucoma patients safely and effectively. Until now, efficacy of MIGS has been assessed mainly according to the postoperative IOP and the number of medications used. Results from long-term studies are rare or not available in the majority of the cases. In order to better evaluate the functionality of MIGS, a new examination method has been developed with the help of a new oculopressor device. In this study the efficacy of different MIGS techniques will be examined using the new oculopressor., Material/methods: At first, glaucoma patients that had previously received a MIGS surgery (iStent inject, XEN Stent, ELT) were examined with the new oculopression test. Their results were compared with those of non-operated patients and healthy individuals. Overall, 38 healthy subjects (group 1), 10 non-operated patients (group 2), 19 patients after iStent inject implantation (group 3), 14 patients after XEN Stent implantation (group 4) and 5 patients after ELT (group 5) were examined. The new examination measures the IOP-reduction that occurs after oculopression and can be seen as an indirect measurement of the outflow facility of the eye., Results: The IOP-reduction after oculopression differed among the study groups. Non-operated patients showed a significantly lower IOP-reduction compared to healthy individuals. Patients after iStent inject and XEN stent implantation showed a larger reduction of IOP after oculopression in relation to non-operated patients and their results approximated those of healthy individuals. These patients needed fewer medications postoperatively in relation to non-operated patients. Patients after ELT showed postoperatively a smaller reduction of IOP after oculopression compared to iStent inject and XEN stent patients., Conclusion: MIGS can increase the outflow facility of the eye in patients with glaucoma. Though ELT had the lowest impact on the aqueous outflow among the studied procedures in this study. The percentage of elderly people with head and neck cancers (HNC) is rising due to increasing average lifespan. As with younger patients, elderly patients require a multidisciplinary approach in order to optimise treatment results. The biological, not the chronological, age should be defined individually based on co-morbidities and performance status. A comprehensive geriatric assessment represents the first and essential step for selecting further treatment options. Major improvements have been accomplished in surgical techniques and radiotherapy delivery. Several chemotherapeutic agents and targeted therapies with different toxicity profile are also available. However, the randomised studies that defined the nature of these improvements included only a small proportion of patients older than 65 years. In deciding which treatment strategy would be suitable for an individual elderly patient, we review the literature regarding surgery, radiotherapy, and chemotherapy or their various combinations.

Study Design: Prospective controlled cohort study of 27 adult osteoporotic patients who underwent kyphoplasty for fresh osteoporotic spinal fractures., Objectives: To define the evolution of vertebral bone mineral density (BMD) at kyphoplasty and adjacent levels along with sagittal spinal alignment to contribute to the etiology of adjacent vertebral fractures after augmentation., Summary of Background Data: Osteoporotic compression fractures can be effectively treated with methylmethacrylate vertebral augmentation. However, to the authors' knowledge the effect of vertebral augmentation on the vertebral endplate BMD of the augmented and adjacent nonaugmented levels has not as yet been described., Methods: Twenty-seven consecutive selected patients (9 men, 18 women), with an average age of 72+/-9 years underwent 1, 2, or 3-level percutaneous kyphoplasty for painful fresh osteoporotic vertebral fractures at the thoracolumbar spine. All patients were radiologically examined with plain roentgenograms, computed tomography, and magnetic resonance imaging. Lateral dual energy x-ray absorptiometry in the augmented and on the adjacent vertebrae (1 level above and below kyphoplasty) was used to measure BMD preoperatively to the last postoperative observation in the subchondral bone of the vertebral endplates. Anthropometric data, sagittal global balance (plumbline), and segmental spine reconstruction (vertebral body height, Gardner kyphotic angle) were recorded and analyzed. The patients were followed for at least 2 years., Results: Kyphoplasty was performed between T12 and L5. A total of 48 vertebral bodies were augmented. Thirteen patients received 1 level and the remaining 14 received 2 or 3-level kyphoplasty. No significant changes in the sagittal spinal balance were shown postoperatively. Gardner kyphotic angle and posterior vertebral body height improved postoperatively, however, insignificantly. Significant [analysis of variance (ANOVA), P=0.008] increase of anterior vertebral body height in the fractured vertebra was achieved postoperatively without subsequent loss of correction. BMD increased significantly in the lower endplate of the augmented vertebra (ANOVA, P=0.05). In 1-level augmentation, no BMD changes were shown at the adjacent vertebrae above and below kyphoplasty. On the contrary, in the multilevel augmentation, a statistically significant (ANOVA, P=0.05) decrease of the BMD was shown in the upper endplate of the adjacent level above kyphoplasty. During the 2-year follow-up, there were 5 (18%) new fractures at the T11-T12 area above the augmented vertebra. All of the fractures occurred in patients who received 2 and 3-level kyphoplasty., Conclusions: The observed 2-year evolution of vertebral endplate BMD, after kyphoplasty under stable global sagittal spinal balance, might contribute to the pathogenesis of new fractures in adjacent vertebra. However, other studies with control series and longer follow-up are necessary to show if these BMD changes are the result of vertebral augmentation or are merely natural history.

Aminoglycoside antibiotics (mmol.l-1) gentamicin (0.74), streptomycin (1.02), netilmicin (1.24), amikacin (2.23), sisomicin (2.74), dactimicin (2.75), kanamycin (3.43), kanendomycin (3.45), tobramycin (3.53) and dibekacin (4.35) produce a complete neuromuscular blockade at the isolated phrenic nerve-hemidiaphragm preparation of the rat, which is only reversed by calcium chloride. On the other hand, verapamil (2.04 mmol.l-1), a calcium channel blocker, also produces a complete neuromuscular blockade at the above preparation which is reversed by calcium chloride. Aminoglycoside antibiotics are potentially capable of interacting with verapamil and produce a complete neuromuscular blockade at concentrations significantly reduced. The neuromuscular blockade which is produced by the concurrent administration of the aminoglycoside antibiotics and verapamil is obtained with the usual therapeutic blood concentrations of the individual agents. Furthermore, the neuromuscular blockade which is produced during verapamil-aminoglycoside antibiotics interactions is completely reversed after calcium chloride administration. The mechanism by which aminoglycoside antibiotics and verapamil produce neuromuscular blockade must be the same. Both classes of drugs interfere with calcium ions movements through the calcium channels of the membrane of the motor nerve-endings inhibiting acetylcholine release at the synaptic cleft. The interaction of aminoglycoside antibiotics and calcium channel blockers is of clinical significance because when these agents are given concurrently during the perioperative period they may lead to respiratory depression or prolonged apnoea. These respiratory disturbances can be managed by slow intravenous infusion of 50 to 200 mg of calcium gluconate.

Trimethaphan, a ganglionic blocking agent which is administered by intravenous drip to produce controlled hypotension during surgery, produces a complete neuromuscular blockade at the isolated phrenic nerve-hemidiaphragm preparation of the rat at a concentration of 0.3 mmol X l-1. This blockade is not reversed by neostigmine, a cholinesterase inhibitor, nor by calcium chloride, and this action is attributed to the local anaesthetic activity of the drug. Trimethaphan (1.5 X 10(-2) mmol X l-1) interacts with the following aminoglycoside antibiotics: gentamicin (0.04), streptomycin (0.05), netilmicin (0.06), amikacin (0.11), sisomicin (0.14), kanamycin (0.17), tobramycin (0.18) and dibekacin (0.21 mmol X l-1) to produce a complete neuromuscular blockade. These pharmacodynamic interactions of trimethaphan and aminoglycoside antibiotics occur at significantly reduced concentrations of the interacting drugs which are very close to the ones obtained after administration of therapeutic doses. When trimethaphan or aminoglycoside antibiotics are used alone at the above reduced concentrations they do not exert any neuromuscular blocking activity. The neuromuscular blockade which is obtained after the interaction of trimethaphan with aminoglycoside antibiotics is not reversed by either neostigmine or calcium chloride, although the neuromuscular blockade which is produced by aminoglycoside antibiotics alone is reversed by calcium chloride. It is concluded that the local anaesthetic effect of trimethaphan is the predominant factor of the mechanism of the above interactions. These interactions may produce severe respiratory disturbances (respiratory depression or apnoea) to the patients, during the perioperative period, which can be reversed only with artificial ventilation.
