The envelope (env) protein of SARS-CoV-2, a pivotal component of the viral architecture, plays a multifaceted role in viral assembly, replication, pathogenesis, and ion channel activity. These features make it a significant target for understanding virus–host interactions and developing vaccines to combat COVID-19. Recent structural studies provide valuable insights into the conformational dynamics and membrane topology of the SARS-CoV-2 env protein, shedding light on its functional mechanisms. The strong homology and highly conserved structure of the SARS-CoV-2 env protein shape its immunogenicity and functional characteristics. This study examines the ability of the recombinant SARS-CoV-2 env protein to stimulate an immune response. In this study, recombinant envelope proteins were produced using the baculovirus expression system, and their potential efficacy was evaluated in both in vivo and in vitro models. Our results reveal that the env protein of SARS-CoV-2 stimulates humoral and cellular responses and highlight its potential as a promising vaccine candidate for combating the ongoing pandemic. Objective: Heart rate variability (HRV) is a non-invasive method used to assess the status of autonomic function of the heart. The aim of this prospective study was to assess HRV and cardiac function in children with epilepsy. In addition, the effects over HRV of epilepsy type, therapy regimens, refractory and well controlled epilepsy were investigated. Material and Method: Between September 2013 and March 2014, 61 epileptic children and 53 healthy children were included in the study. All subjects were evaluated by electrocardiography, echocardiography, and 24 hours rhythm holter. Results: In the children with epilepsy, it was determined a significant reduction in all the time domain (SDNN, SDNN index, SDANN, RMSSD, pNN50 p-values 0.011, 0.001, 0.021, 0.001, 0.001, respectively) and frequency domain (LF, HF, VLF p-values 0.001, 0.001, 0.001 respectively) parameters. There were not statistically significant differences in both time and frequency domain parameters between children with partial and generalized epilepsy. There was a significant reduction in SDNN, SDNN index, SDANN, RMSSD, LF, HF, VLF (p-values 0.009, 0.008, 0.015, 0.039, 0.004, 0.010, 0.005, respectively) values in the children who underwent polytherapy regimen. Conclusion: Epilepsy is associated with suppressed HRV values indicating decreased vagal activity. Having pathological EEG findings, and polytherapy regimen associated with refractory epilepsy are effective on suppressed HRV. In addition, cardiac systolic functions may be affected in patients with epilepsy under drug therapy. [ABSTRACT FROM AUTHOR]

The aim of this study is to characterise a bio-waste natural material and investigate its reuse for other purposes. Menengic (Pistacia terebinthus) coffee bio-waste (MCW) was characterised and used to remove malachite green oxalate (MGO) and methylene blue (MB) from aqueous solutions. Characterisation of adsorbent was performed with Brunauer-Emmett-Teller method, Fourier-transform infrared spectroscopy, scanning electron microscopy, and X-ray diffraction techniques. Adsorption parameters such as pH, amount of adsorbent, contact time, mixing speed, initial dye concentration, and temperature were investigated by classical batch adsorption process. Maximum dye adsorption (99.59 ± 0.05% for MGO and 96.54 ± 0.21% for MB) onto MCW were observed at pH values of 8 and 6, and 150 mg and 200 mg of adsorbent in 200 mg/L dye solution at 25°C for 45 min with a mixing speed of 400 rpm, respectively. In this context, Langmuir, Freundlich and Temkin isotherms were tested and it was found that the most suitable isotherm for the experimental data was the Langmuir isotherm with an R2 value of 0.9915 for MB and 0.9852 for MGO. In addition, the maximum adsorption capacity for MGO and MB was determined as 59.17 mg/g and 32.68 mg/g, respectively. The adsorption characteristics of MGO and MB indicate that they coincided with pseudo-second-order kinetics. According to this kinetic model, the mechanism is compatible with chemisorption as the rate determination step and the thermodynamic data suggested the spontaneous and exothermic process. © 2021 Paediatrics and Child Health Division (The Royal Australasian College of Physicians)Aim: To evaluate the incidence and clinical features of acute rheumatic fever (ARF) in Turkey, following the revised Jones criteria in 2015. Methods: This multicentre study was designed by the Acquired Heart Diseases Working Group of the Turkish Pediatric Cardiology and Pediatric Cardiac Surgery Association in 2016. The data during the first attack of 1103 ARF patients were collected from the paediatric cardiologists between 1 January 2016 and 31 December 2016. Results: Turkey National Institute of Statistics records of 2016 were used for the determination of ARF incidence with regard to various cities and regions separately. The estimated incidence rate of ARF was 8.84/100 000 in Turkey. The ARF incidence varied considerably among different regions. The highest incidence was found in the Eastern Anatolia Region as 14.4/100 000, and the lowest incidence was found in the Black Sea Region as 3.3/100 000 (P < 0.05). Clinical carditis was the most common finding. The incidence of clinical carditis, subclinical carditis, polyarthritis, aseptic monoarthritis, polyarthralgia and Sydenham's Chorea was 53.5%, 29.1%, 52.8%, 10.3%, 18.6% and 7.9%, respectively. The incidences of clinical carditis, subclinical carditis, polyarthritis and polyarthralgia were found to be significantly different among different regions (P < 0.05). Conclusion: The findings of this nationwide screening of ARF suggest that Turkey should be included in the moderate-risk group.

In this study, the synthesis of an easily synthesized, non-toxic and inexpensive material and its use in dye removal have been demonstrated. The synthesis of novel material, tin(IV) chelate compound, was carried out using simple and inexpensive Schiff base ligands. This novel material was used for the first time as an adsorbent to remove murexide dye from a model aqueous solution. 1H-NMR, 13C-NMR, Fourier-transform infrared spectroscopy, MS, and elemental analysis techniques were performed for the characterization of the new tin-based adsorbent and the open structure of the compound was proposed. Murexide dye removal from a model aqueous solution was performed by examining five important factors of adsorption. Batch adsorption is the easiest way to remove dyes at a low cost. The optimum parameter values to maximize the adsorption efficiency were obtained at pH 3, 100 mg/L dye concentration, 20 min of contact time, 40 mg adsorbent dose, and 25°C. The highest adsorption recovery value and the adsorption capacity found at optimum parameters were 99.00%, and 248.8 mg/g, respectively. Three adsorption isotherms were calculated at 25°C. The highest R2 value was found at 0.99 for the Langmuir isotherm model, the adsorption kinetic obeyed the pseudo-second-order kinetics, and the thermodynamic data suggested a spontaneous and exothermic process. Bu çalışmanın temel amacı, kamu kurumu alanlarından biri olan yerel yönetimlerde çalışanların örgütsel güven duygusunun var olup olmadığını ve örgütsel özdeşleşme kavramını nasıl algıladıklarını, iş tatmini boyutunun ne kadar hissedildiğini, bu kavramların birbirini nasıl etkilediklerini, birbirleriyle nerede kesiştiklerini, ne anlamda ve hangi boyutta ilişkilerinin olduğunu ölçmektir.Araştırma Elazığ Belediyesi'nde çalışanlar evreninde gerçekleştirilmiştir. Örneklem ise evrenden rastgele yöntemle seçilen dörtyüziki kişiden oluşmaktadır. Çalışmada elde edilen bulgular değerlendirilirken, istatistiksel analizler için SPSS 21,0 İstatistik paket programı kullanılmıştır. Çalışma verileri değerlendirilirken tanımlayıcı istatistiksel metotları (Frekans, Yüzde, Ortalama, Standart sapma), tek yönlü varyans analizi, t–testi, Regresyon ve korelasyan analizi yöntemlerinden faydalanılacaktır.Çalışmada; `örgütsel güven ile örgütsel özdeşleşme arasında anlamlı pozitif bir etki` `örgütsel güven ile iş tatmini arasında anlamlı pozitif bir etki` ve `örgütsel özdeşleşme ile İş tatmini arasında anlamlı pozitif bir etki` araştırılmıştır. Sonuç olarak, Araştırma neticesinde gerek örgütsel güven örgütsel özdeşleşme ve İştatminine ilişkin görüşlerin, çalışanların sosyo-demografik özelliklerine göre farklı düzeyde olduğu görülmüştür. Erkek katılımcıların kuruma olan örgütsel özdeşleşmesinin daha yüksek olduğu görülmüştür. Yaş değişkeni açından 60+yaş üstü katılımcıların örgütsel özdeşleşmelerin daha yüksek olduğu, yaş ilerledikçe örgütsel bağlılığının ve örgütsel özdeşleşmenin artığı sonucuna varılmıştır. Evli katılımcıların kurumla daha fazla özdeşleştiği, daha fazla örgüte güvendiği, sonucuna varılmıştır. Çalışma yılının artması iş tatminin azalttığı, örgütsel özdeşleşmeyi ve örgüte güveni artırdığı, fakat yöneticiye güveni azalttığı görülmüştür. Regresyon analizi sonuçlarına göre; örgütsel güvenin örgütsel özdeşleşme üzerindeki etkisi % 13,9,yöneticiye güvenin özdeşleşemeye etkisi%0,9, örgüte güvenin örgütsel özdeşleşmeye etkisi %12,8, örgütsel güvenin iş tatminine etkisi %25,8, yöneticiye güvenin iş tatminine etkisi %26,7, örgüte güvenin iş tatminine etkisi %0,089,örgütsel özdeşleşmenin iş tatminini %0,09 oranında pozitif yönle etkilediği görülmüştür. The main purpose of this study is to determine whether there is an organizational trust feeling in the local administrations, which is one of the public institutions, and how they perceive the concept of organizational identification; aims to measure.The research was carried out in Elazığ Municipality employees. The sample consisted of 402 people randomly selected from the universe. SPSS 21.0 Statistics package program will be used for statistical analysis. Descriptive statistical methods (frequency, percentage, mean, standard deviation), one-way analysis of variance, t-test, regression and correlation analysis methods will be used.Study; A significant positive relationship between organizational trust and organizational identification de arasında Significant positive relationship between organizational trust and job satisfaction güven and ş Significant positive relationship between identification and job satisfaction güven was investigated.As a result, the results of the study showed that the opinions on organizational trust identification and business process were different according to the socio-demographic characteristics of the employees. Male participants were more likely to identify with the institution. In terms of age variable, it is concluded that the identification of the participants over 60+ years is higher and the organizational commitment and identification as the age increases. It was concluded that married participants were more associated with the institution and trusted more organizations. It is seen that increasing the working year decreases job satisfaction, identifies and increases trust in colleagues and decreases trust in managers. According to the results of the regression analysis, identification can explain 13,9% of the variables of organizational trust. Confidence of the manager to the identification of the impact on the identification of trust is 12,8%, organizational trust is the effect of job satisfaction is 25,8%, trust to the manager 26,7% of the satisfaction of the job satisfaction, organization 0,089%, identification of job satisfaction was observed to affect 0.09% of all hypotheses in the model was accepted. 133

Ülkelerde teknolojik ilerlemeler, nüfusun artması ve sosyo-ekonomik kalkınmanın gerçekleştirilmesinin istenmesi nedeniyle yeni altyapı yatırımları ve buna bağlı planlamaların hayata geçirilmesi gündeme gelmektedir. Ancak öz kaynakların yetersizliği ve ihtiyaç nedeniyle artan altyapı projeleri yatırımlarının finanse edilmesinin gerekliliği ülkeleri yeni finansman modelleri arayışına sürüklemiştir. Yap-İşlet-Devret Modeli (YİD), ülkelerin kısıtlı bütçeleriyle gerçekleştiremedikleri altyapı yatırım ve hizmetlerini, özel sektörün katkılarıyla daha hızlı, etkin ve verimli bir şekilde gerçekleştirilmesine olanak sağlamaktadır. Söz konusu ülkelerde YİD modeli ile tedarik edilen altyapı projeleri ülkelerde giderek tercih edilen bir seçenek haline geldi. Bu projeler, siyasi ve ekonomik istikrarsızlığın, sosyal, teknolojik ve diğer mali olmayan faktörlerin yatırımın uzun vadede projenin mali açıdan uygulanabilirliğini önemli ölçüde etkileyebileceği doğal olarak yüksek riskli yatırımlardır. Dolayısıyla, bu projelere yatırım yapma kararı, projeyi çevreleyen finansal ve mali olmayan faktörlerin birleştirilmiş bir değerlendirmesine dayanmalıdır. Bu çalışmada, Yap-İşlet-Devret (YİD) modeli çerçevesinde olan projeler, ülkeler açısından değerlendirilerek ülkelere özgü mali olmayan (risk) faktörlerin ele alınamamış olduğunu vurgulayan, yatırımcılara hâlihazırda mevcut olan karar destek sistemlerinin bir özetini ve sunduğu fırsatları ortaya koymaktadır. Bu doğrultuda literatür taraması ve arşiv araştırması yöntemi kullanılmıştır. Due to the technological advances in the countries, the increase of the population and the socio-economic development, the new infrastructure investments and the related plans have come to be alive. However, the necessity of financing investments in infrastructural projects due to the inadequacy of the equity resources and the need has led countries to look for new financing models. The Build-Operate-Transfer Model (BOT) allows infrastructure investments and services that countries can not realize with limited budgets to be realized faster, more efficiently and efficiently with the contributions of the private sector. Infrastructure projects supplied with the BOT model in these countries have become an increasingly preferred option in countries. These projects are naturally high-risk investments where political and economic instability, social, technological and other non-financial factors can significantly affect the financial viability of the project in the long run. Therefore, the decision to invest in these projects should be based on a combined assessment of the financial and non-financial factors surrounding the project. In this study, the projects within the framework of the Build-Operate-Transfer (BOT) model provide a summary of the decision support systems currently available to investors and opportunities presented by the investors, emphasizing that the country-specific non-financial (risk) factors have not been addressed. - Abstract
Olgu, Cystic fibrosis and alpha 1 antitrypsin deficiency are two important autosomal recessive multi organ diseases They are complex and severe diseases that involve various organs including pancreas Pancreatic disease in cystic fibrosis varies from complete loss of exocrine and endocrine functions to nearly normal pancreatic function A strong correlation between genotype and phenotype is recognized Pancreatic involvement is characterized by malabsorption of fat and protein and growth failure Fat malabsorption leads to special problems with the fat soluble vitamins Enzyme and fat soluble vitamin replacements are the milestones of the malabsorption treatment Gene therapy offers the best hope for a cure of cystic fibrosis Although an association between alpha1 antitrypsin deficiency and chronic pancreatitis has been reported in several case reports in prospective studies it has been shown that pancreatitis prevalence in patients with alpha1 antitrypsin deficiency was not different from that of healthy controls Thus it was concluded that alpha 1 antitrypsin deficiency was not related to the pathogenesis of idiopathic or hereditary chronic pancreatitis However protective role of alpha 1 antitrypsin against pancreatic auto digestion suggests that the deficiency of that protein may worsen the pancreatic disease rather than causing it Early detection and management of pancreatic insufficiency is essential to optimize health and outcomes in cystic fibrosis and alpha 1 antitrypsin deficiency patients Turk Arch Ped 2009; 44: 7 11 Key words: Alfa 1 antitripsin deficinecy cystic fibrosis pancreas

Kalp hızı değişkenliği (KHD), kalbin otonomik fonksiyon durumunu değerlendirmede kullanılan noninvaziv bir yöntemdir. Epilepsi hastalarında azalmış KHD, epilepsi ile ilişkili ölümlerin en yaygın nedeni olan ani beklenmeyen ölümün (SUDEP) patofizyolojisinde majör rol oynayabilir. Bu prospektif çalışmanın amacı, epilepsili çocuklarda kalp hızı değişkenliğini değerlendirmekdi.Yaş ortalamaları 9.15 ± 4.60 yıl olan 61 epileptik çocuk ve yaş ortalamaları 9.78 ± 4.05 yıl olan 53 sağlıklı çocuk, çalışmaya dahil edildi. Çalışmadaki tüm olgular, KHD için 24 saatlik Holter görüntüleme, elektrokardiyografi (EKG) ve ekokardiyografi (EKO) ile değerlendirildi. Kontrol grubu ile karşılaştırıldığında epilepsi hasta grubunda, EKG ve EKO parametrelerinde kısalma fraksiyonu dışında anlamlı farklılık saptanmadı. Hasta grubunda, zaman bağımlı (SDNN, SDNN index, SDANN, RMSSD, and pNN50) ve frekans bağımlı (LF, HF, VLF) parametrelerin tamamında anlamlı bir azalma saptandı. Patolojik EEG'li hastalarda, SDNN, SDNN index, SDANN index, LF ve VLF değerlerinde normal EEG'li hastalar ile karşılaştırıldığında anlamlı bir baskılanma tespit edildi. Monoterapi rejimi uygulanan hastalar ile karşılaştırıldığında politerapi rejim uygulanan hastalarda, SDNN, SDNN index, SDANN index, RMSSD, LF, HF ve VLF değerlerinde anlamlı bir azalma vardı. Refrakter epilepsili hastalarda SDNN ve SDANN index, iyi kontrollü epilepsili hastaların değerleri ile karşılaştırıldığında anlamlı olarak baskılanmıştı. Bu çalışmanın verileri, epilepsi hastalarında baskılanmış KHD'nin politerapi, refrakter epilepsi ve patolojik EEG ile ilişkili olduğunu göstermiştir. Azalmış parasempatik tonüs ile ilişkili olan bu artmış sempatik aktivite, epilepsili çocuklarda ventriküler taşiaritmilerin gelişiminde anahtar bir rol oynayabilir ve epilepsili çocuklarda SUDEP'in altında yatan mekanizmayı açıklayabilir. Noninvaziv bir yöntem olan KHD, politerapi rejimi uygulanan refrakter epilepsili hastalarda olası fatal kardiyak aritmi riskini değerlendirmek için kullanılabilir. Heart rate variability (HRV) is a noninvasive method used to assess the status of autonomic function of heart. Reduced HRV in epilepsy patients may play a major role in the pathophysiology of sudden unexpected death (SUDEP) that is the commonest cause of death related to epilepsy. The goal of this prospective study was to evaluate heart rate variability in children with epilepsy.61 epileptic children with average age = 9.15 ± 4.60 years and 53 healthy children with average age = 9.78 ± 4.05 years were included in the study. All subjects in the study were assessed by 24h Holter monitoring for HRV, electrocardiography (ECG) and echocardiography (ECO). In the epilepsy patient group compared to controls was not determined a significant difference in parameters of ECG and ECO except for shortening fraction. In the patient group was determined a significant reduction in all of the time domain (SDNN, SDNN index, SDANN, RMSSD, and pNN50) and frequency domain (LF, HF, VLF) parameters. In the patients with pathological, EEG, SDNN, SDNN index, SDANN index, LF and VLF values were detected a significant suppression when compared with patients with normal EEG. There was a significant reduction in SDNN, SDNN index, SDANN index, RMSSD, LF, HF and VLF values in the patients underwent polytherapy regimen when compared to patients underwent monotherapy regimen. In patients with refractory epilepsy, SDNN and SDANN index were significantly suppressed when compared with the values of patients with well-controlled epilepsy.The data of this study showed that suppressed HRV in the patients with epilepsy was assosiated with polytherapy, refractory epilepsy and pathological EEG. This increased sympathetic activity in association with a decreased parasympathetic tone may play a key role in the development of ventricular tachyarrhythmias in children with epilepsy and may explain the mechanism underlying SUDEP in children with epilepsy. HRV which is a noninvasive method could used to evaluate the risk of the possible fatal cardiac arrhythmia in patients with refractory epilepsy underwent polytherapy regimen. 65

25. Akut Romatizmal Ateş Tanılı Çocuklarda MPV ve PCT Parametrelerinin Değerlendirilmesi.

- Abstract
Objective: The aim of this study was to show whether mean platelet volume (MPV) and platelet-crit (PCT) were useful tools for determining and following up inflammation during the acute phase and follow-up after treatment in patients with acute rheumatic fever (ARF) compared to the healthy group. Material and Method: 47 children with ARF and 40 healthy children were included in the study. Platelet indices and inflammatory parameters were measured for all children. Platelet indices of acute and remission phase of the disease were compared with the values of healthy children. Results: It was not detected statistically significant differences between children with ARF and healthy children with respect to age, gender and body mass index. MPV value in ARF group was lower than the healthy group. However, the detected this decrease was not statistically significant (p=0.177). PCT value was significantly higher in children with ARF (p<0.001). By the children with ARF it was determined that white blood cells count and platelet count (PLT) values have shown a significant increase, while hemoglobine values determined also significant decrease (p<0.001 for all parameters). It was not detected a significant difference between MPV values of acute and remission phase in the patients with ARF. PCT and PLT values in the both acute and remission phase by the patients with ARF were significantly higher compared the healthy group (p-0.021, p=0.015: p=0.006, p=0.029, respectively). Conclusion: We consider that PCT as well as PLT may be used as valuable parameters in follow-up of patients diagnosed with ARF. [ABSTRACT FROM AUTHOR]

- Abstract
Objective: Heart rate variability (HRV) is a noninvasive method used to assess the status of autonomic function of the heart. The aim of this study was to evaluate HRV in healthy children. Material and Method: 51 healthy children were included in the study. HRV of all subjects were evaluated using the data in 24 hours Bolter monitoring. IIRV was measured by calculating five time-domain (SDNN, SDNN-indeks, SDANN, RMSSD. pNN50), and four frequency-domain (LF. HF. VLF. LF/HF ratio) indices. Results: The mean age of 51 healthy children included in the study was 10.78 ± 3.80 years (range 4-17 years). There was no statistically significant difference in the heart rate parameters detected between females and males. SDNN, SDNN-index, SDANN, LF. VLF, total power parameters of the heart rate variability showed a significant positive correlation with age (r:0.30l. p:0.032; r:0.278 p:0.048; r:0.335 p:0.016: r:0.334 p:O.OI7; r:0.361 p:0.009; r: 0.321 p:0.021; respectivelyj. There was no correlation detected between body mass index and the parameters of HRV. Conclusion: HRV parameters denoting vagal activity did not show significantly difference between female and male children The majority of HRV parameters increased with increasing age. F1RV which is a noninvasive method could be used to evaluate the risk of the possible cardiac arrhythmia. [ABSTRACT FROM AUTHOR]
