1. Continuation of Treatment in Children with ADHD: A Multicenter Turkish Sample Study

Abstract

Objectives: The aim of this study was to investigate the variables that may affect treatment continuation in children aged 6 to 12 years who were newly diagnosed with ADHD. Methods: A total of 132 children diagnosed with ADHD and their parents participated in the study. Sociodemographic and clinical risk factors affecting continuation of treatment were examined using logistic regression analysis. Results: Multiple model examination revealed that greater age increased the risk of treatment discontinuation 1.824 times (p = 0.003) while a lower total length of paternal education increased the risk of discontinuation (1/0.835) 1.198 times (p = 0.022). Other variables emerging as significant in the univariate model lost that significance in the multiple model. Conclusions: Understanding the variables associated with medication discontinuation in ADHD in different populations and taking these variables into account in the development of health policies, will be useful in improving the long-term devastating effects of the disorder.

2. Evaluation of Carotid Intima Media Thickness as an Early Marker of Atherosclerosis in Children and Adolescents with Autism Spectrum Disorder: A Case-Control Study

Abstract

There are studies reporting different results on whether there is an increased incidence of cardiovascular problems in autism spectrum disorder (ASD). The aim of this study was to examine carotid intima-media thickness (cIMT), an early marker of atherosclerosis in children and adolescents with ASD. This single-center case-control study was conducted in a tertiary care hospital. Eighty-five children (36 children with autism and 49 healthy controls) aged 6-18 were enrolled. After a clinical interview, Children's Global Assessment Scale (CGAS) and Autism Behavior Checklist (ABC) were filled out and laboratory measures were collected for ASD group. Anthropometric measurements (height, weight, waist and hip) of each participant were assessed. The cIMT was measured with an ultrasound probe on both sides of the neck. Age, gender, body mass index percentile and waist/hip ratio adjusted right/left common carotid arteries IMT and right/left bulb IMT were statistically significantly higher in the autism group than controls (0.45 ± 0.01 mm and 0.55 ± 0.01 mm, respectively, p < 0.001). We performed multiple linear regression analysis to determine statistically significant related factors of the cIMT in ASD. Early onset of autism symptoms were related with high carotid IMT ([beta] = -0.496, p = 0.01). These findings suggest that atherosclerosis may begin earlier in children with ASD, which is one of the most common neurodevelopmental disorders in childhood. The cIMT measurement, which is an easy and noninvasive method in the cardiovascular assessment and follow-up can be recommended for clinical use for individuals with ASD.

7. The effects of a maternal personality, children’s behavioral characteristics, and parenting styles on the dental anxiety of 3- to 6-year-old children

Abstract

Background: Many individual and environmental factors cause dental anxiety in children. The aim of this study was to examine the effects of a maternal personality, children’s behavioral characteristics, and parenting styles on the dental anxiety of three- to six-year-old children. Methods: The parents of 230 children who applied to the Department of Pediatric Dentistry, Faculty of Ondokuz, Mayıs University were involved in this study. The “Temperament and Character Inventory”, “Child Behavior List”, and “Parenting Attitude Scale” were applied to the volunteer parents. All data were statistically analyzed (p < 0.05). Results: From the results of determining the maternal personality, it was found that the score of “novelty seeking” and those of all the character dimensions were statistically significantly higher in the dental anxiety group than in the control group, while the scores of “reward dependence” and persistence were statistically significantly lower in the dental anxiety group (p < 0.001). From the aspect of children’s behavior characteristics, the scores of “inhibitory control” (p = 0.029) and permissive attitude among the parenting styles were statistically significantly higher in the dental anxiety group (p = 0.027). Conclusion: In this study, it was determined that maternal characteristics, children’s behavioral characteristics, and parenting attitudes affect the dental anxiety of three- to six-year-old children.

Abstract

OBJECTIVE Studies show partial improvements in some core symptoms of Autism Spectrum Disorders (ASD) in time. However, the predictive factors (e.g. pretreatment IQ, comorbid psychiatric disorders, adaptive, and language skills, etc.) for a better the outcome was not studied with machine learning methods. We aimed to examine the predictors of outcome with machine learning methods, which are novel computational methods including statistical estimation, information theories and mathematical learning automatically discovering useful patterns in large amounts of data. METHOD The study the group comprised 433 children (mean age: 72.3 ± 45.9 months) with ASD diagnosis. The ASD symptoms were assessed by the Autism Behavior Checklist, Aberrant Behavior Checklist, Clinical Global Impression scales at baseline (T0) and 12th (T1), 24th (T2), and 36th (T3) months. We tested the performance of for machine learning algorithms (Naive Bayes, Generalized Linear Model, Logistic Regression, Decision Tree) on our data, including the 254 items in the baseline forms. Patients with ≤2 CGI points in ASD symptoms at in 36 months were accepted as the group who has “better outcome” as the prediction class. RESULTS The significant proportion of the cases showed significant improvement in ASD symptoms (39.7% in T1, 60.7% in T2; 77.8% in T3). Our machine learning model in T3 showed that diagnosis group affected the prognosis. In the autism group, older father and mother age; in PDD-NOS group, MR comorbidity, less birth weight and older age at diagnosis have a worse outcome. In Asperger’s Disorder age at diagnosis, age at first evaluation and developmental cornerstones has affected prognosis. CONCLUSION In accordance with other studies we found early age diagnosis, early start rehabilitation, the severity of ASD symptoms at baseline assessment predicted outcome. Also, we found comorbid psychiatric diagnoses are affecting the outcome of ASD symptoms in clinical observation. The machine learning models reveal several others are more significant (e.g. parental age, birth weight, sociodemographic variables, etc.) in terms of prognostic information and also planning treatment of children with ASD.

Abstract

Theory of mind is a social cognition skills demonstrated its importance in the last forty years with psychiatric clinical trials. Theory of mind is seen as an effective and necessary skill in the social function-ing of human who is a social creature as the ability to recognize the mental states and emotions of others. In the first six years of life, theory of mind has been associated with many fields. Findings related to many neurobiological bases, such as limbic-paralimbic structures, prefrontal cortex, which start with mirror neurons, help this sense of meaning. Areas associated with theory of mind development provide better understanding of theory of mind skills and deficits, the first psychopathology studies have been carried out in children with autism, and the studies about theory of mind skills in the diagnosis of neurodevelopmental disorders are becoming more and more interesting. In this review, theory of mind development, neurobiological basis and related areas will be explained and the relation of theory of mind with psychopathology will be examined.

Amaç: Bu çalışmanın amacı, tanısı konmuş nörolojik ve otolojik hastalığı olmayan, 3 ay ve daha uzun süredir başdönmesi şikayeti olan bireyler için Brandt Daroff ve bakış stabilizasyon egzersizlerinden oluşturulan vestibüler rehabilitasyon ev programının etkinliğinin değerlendirilmesidir. Yöntem: Çalışmaya baş dönmesi şikayeti olan, otolojik ve nörolojik hastalığı olmayan, 25–45 yaş arası, 13 birey dahil edildi. Baş dönmesinin katılımcıların günlük yaşam aktiviteleri üzerindeki olumsuz etkisi Aktiviteye Özgü Denge Güven Ölçeği (AÖDGÖ) ile değerlendirildi. Kişiye özel vestibüler rehabilitasyon programı, günlük yaşam aktiviteleri sırasında baş dönmesinin artış gösterdiği baş hareketlerinin yönüne göre belirlendi. Brandt-Daroff Egzersizleri ve bakış stabilizasyon egzersizleri ev programı şeklinde verildi. Hastalar 3 hafta sonra ve 3 ay sonra AÖDGÖ ile tekrar değerlendirildi. Sonuçlar karşılaştırıldı. Abstract

This study aimed to investigate the effect of universal primers on resin composite repair with and without silica-coating while assessing adhesion durability under various aging methods. Specimens were fabricated using multiple increments of composite resin (Clearfil Majesty Esthetic) placed into cylindrical forms (diameter: 3.2 mm, height: 4 mm) and photopolymerized. Specimens (N = 300, n = 15 per group) were randomly assigned to three groups for aging (control: fresh-dry, 6 months water storage and thermocycling ×5000 cycles, 5–55 °C). These three main groups were further randomly assigned to two groups according to the surface conditioning procedures; (a) Control: No air-abrasion, and (b) Air abrasion with silica-coating (Co-Jet, 30 µm, 5 s, 2.5 bar). All subgroups were then randomly divided into five subgroups to be treated with adhesion promoters (a) All Bond Universal: AB (Bisco), (b) Clearfil Universal Bond Quick: SKB (Kuraray), (c) Monobond Plus: MP (Ivoclar Vivadent), (d) G-Premio Bond: GP (GC), (e) Clearfil Universal Bond: CU (Kuraray). After bonding according to the manufacturer’s instructions to new composite blocks, shear bond strength tests were conducted using a Universal testing machine (1 mm/min). Obtained Data (MPa) were analyzed by 3-way ANOVA and Tukey`s post-hoc tests (alpha = 0.05). Bond strength results were significantly affected by the adhesive type (p < 0.05), silica-coating (p < 0.05), and aging (p < 0.001) but the interaction terms were not significant (p > 0.05). In non-aged groups (24 h), no significant effect on bond strength was observed using silica-coating (min; 8.41; max:12.98) but after 6 months, silica-coating increased the results for AB, GP, and CU ranging between 9.56 and 11.25 (p < 0.05) and even more after thermocycling for SKB, MP, GP, CU (10.23–13.95) (p < 0.05) compared to non-conditioned groups (6.83–10.37 for 6 months, 8.96–11.18 for thermocycling, respectively). Abstract

Today, obesity is a public health problem with significant negative effects on mortality and morbidity rates in developing countries, and impact on all levels of the society. In recent years cognitive behavioral therapy approach has been considered as an important part of the obesity treatment. Behavioral therapy for obesity includes sections like self-monitoring, stimulus control, food control, consolidation and reinforcement, cognitive restructuring, proper nutrition education, increase in physical activity, and behavior contracts. As part of the obesity treatment, combining cognitive-behavioral treatments with lifestyle changes such as increase in physical activity increases effectiveness of the treatment and ensures durability of the achieved weight loss. 23. Effect of alternative modes of application on microleakage of one-step self-etch adhesives

Abstract

Background/purpose: Contemporary one-step self-etch adhesives are certainly the most user-friendly adhesives; however, typically could not compete with the more traditional multi-step adhesives. This in vitro study evaluated the efficacy of two modes of application (double application or placement of a hydrophobic resin coating) for the reduction of microleakage, using three different one-step self-etch adhesives (AdheSE One VivaPen, Ivoclar Vivadent, Schaan, Liechtenstein; Xeno V, Dentsply De Trey, Konstanz, Germany; OptiBond All-In-One, Kerr Italia S.r.l, Scafati, Italy). Materials and methods: We prepared Class V cavities with the occlusal margin in enamel and gingival margin in dentin on both buccal and lingual surfaces of 108 human molar teeth. Cavities were divided into nine groups according to the combination of adhesives and application modes used (n = 24). For applying adhesives, we followed three types of procedures: (1) according to the manufacturers’ directions; (2) using a double-application method; and (3) with the additional placement of a hydrophobic resin layer after following the manufacturers’ directions. We restored the cavities with a composite resin. After thermocycling (500 cycles at 5°C/55°C) and immersion in 0.5% basic fuchsin, we sectioned the teeth and evaluated them for microleakage using a stereo microscope (20×). Enamel and dentin margins were scored separately, and we analyzed the data using Kruskal–Wallis, Bonferroni-corrected Mann–Whitney U, and Wilcoxon signed-rank tests. Results: Application of a hydrophobic resin over the cured Xeno V adhesive decreased the amount of microleakage at the enamel margins, compared with the application according to the manufacturer’s instructions (P = 0.005). 25. Validity of the Aberrant Behavior Checklist in a Clinical Sample of Toddlers

Abstract

We investigated the congruent and criterion validity of the Aberrant Behavior Checklist (ABC) in a clinical sample of toddlers seen over 1 year in Turkey. All consecutive patients (N = 93), 14-43 months old (mean, 30.6 mos.), in a child psychiatry outpatient clinic were included. The ABC, Autism Behavior Checklist (AuBC), and Child Behavior Checklist 2/3 (CBCL) were completed by the children's parents. Internal consistency for ABC subscales was moderate to high. The total ABC score, which is interdependent with subscales (e.g., Irritability, Social Withdrawal) of the ABC, was significantly correlated with the CBCL-total (r = 0.73) and AuBC-total (r = 0.71) scores. Subscales of the ABC revealed significant differences between diagnostic groups. ABC Total, and the Irritability and Hyperactivity subscale scores, were significantly higher in children with externalizing disorders; the Lethargy/Social Withdrawal and Stereotypic Behavior subscale scores were significantly higher in toddlers with autism. The ABC appears to be capable of discriminating several syndromes, such as disruptive behavior disorders and autism in early childhood.

Materials and methods: The sample included 6–18 year old children and adolescents from Turkey (N=108; 83 boys, 25 girls) diagnosed with ADHD. All comorbid diagnoses were determined based on the Kiddie Schedule for Affective Disorders and Schizophrenia for School-Age Children-Present and Lifetime Version assessment. Results: 96.3% of the cases were found to have at least one psychiatric comorbid diagnosis. The most frequent psychiatric comorbid disorder was oppositional defiant disorder (69.4%) followed by anxiety disorders (49%) and elimination disorders (27.8%). Disruptive behavior disorders were more common in ADHD-combined type. Depression and anxiety disorders were more common in girls. Separation anxiety disorder and elimination disorder were more common in children, whereas depression, bipolar disorder, obsessive–compulsive disorder, and social phobia were more common in the adolescents. Conclusion: According to our results, when a diagnostic tool was used to assess the presence of comorbid psychiatric disorders in children and adolescents diagnosed with ADHD, almost all cases had at least one comorbid diagnosis. Therefore, especially in the clinical sample, ADHD cases should not be solely interpreted with ADHD symptom domains, instead they should be investigated properly in terms of accompanying psychiatric disorders. Abstract

Tourette syndrome (TS) is a childhood-onset neurodevelopmental disorder characterized by several motor and phonic tics.In this study, we aimed to compare activated regulatory T cell (Treg) values between patients with TS/chronic tic disorder (CTD) and age- and sex-matched healthy controls (HCs).Patients with TS/CTD and age- and sex-matched HCs were included in the study. The severity of the disease was evaluated using the Yale Global Tic Severity Scale. CD4The study included 48 patients diagnosed with TS/CTD (36 males and 12 females, mean age: 11.58 ± 2.61) and 24 HCs (18 males and 6 females, mean age: 11.63 ± 2.60). The TS/CTD group had significantly higher activated regulatory T percentile with respect to the T helper value compared to the HCs (Our results suggest that Tregs may have a role in the pathogenesis of TS/CTD.

28. The effects of a maternal personality, children’s behavioral characteristics, and parenting styles on the dental anxiety of 3- to 6-year-old children

Background: Many individual and environmental factors cause dental anxiety in children. The aim of this study was to examine the effects of a maternal personality, children’s behavioral characteristics, and parenting styles on the dental anxiety of three- to six-year-old children. Methods: The parents of 230 children who applied to the Department of Pediatric Dentistry, Faculty of Ondokuz, Mayıs University were involved in this study. The “Temperament and Character Inventory”, “Child Behavior List”, and “Parenting Attitude Scale” were applied to the volunteer parents. All data were statistically analyzed (p < 0.05). Results: From the results of determining the maternal personality, it was found that the score of “novelty seeking” and those of all the character dimensions were statistically significantly higher in the dental anxiety group than in the control group, while the scores of “reward dependence” and persistence were statistically significantly lower in the dental anxiety group (p < 0.001). From the aspect of children’s behavior characteristics, the scores of “inhibitory control” (p = 0.029) and permissive attitude among the parenting styles were statistically significantly higher in the dental anxiety group (p = 0.027). Conclusion: In this study, it was determined that maternal characteristics, children’s behavioral characteristics, and parenting attitudes affect the dental anxiety of three- to six-year-old children.

Abstract

The aim of this study is to demonstrate the validity and reliability of the Categorical and Dimensional Psychometric Properties of the Structured Clinical Interview for DSM-5 Personality Disorders (SCID-5-PD) after its translation to the Turkish.The study was carried out with 102 volunteers from two university hospitals. The SCID interview was conducted by two experienced psychiatrists who interchanged positions as interviewer and observer; and completed the research forms without discussing the patient. The diagnostic agreement between the interviewers and the Kappa coefficient were calculated. Divergent and convergent validity analyses were carried out for diagnostic validity and the scores obtained from the self-report form as well as the dimensional evaluation scores were used in the statistical analyses.The group mean age for volunteers was 39.6±11.6 years and 66.7% consisted of females. The Kappa values for personality categories were 0.79 for avoidant personality structure, 0.64 for dependent personality structure, 0.81 for obsessive-compulsive personality structure, 0.76 for paranoid personality structure, 0.49 for schizotypal personality structure, 0.90 for histrionic personality structure, 0.66 for narcissistic personality structure, 0.89 for borderline personality structure and 0.71 for antisocial personality structure. Dimensional evaluation showed significant correlation with the diagnostic agreement between the interviewers and also with the scores of the self-report forms completed by the participants.The results demosntrated that the Turkish version of the Structured Clinical Interview for DSM-5 Personality Disorders (SCID- 5-PD-CV-TR) is valid and reliable.

Theory of mind is a social cognition skills demonstrated its importance in the last forty years with psychiatric clinical trials. Theory of mind is seen as an effective and necessary skill in the social function-ing of human who is a social creature as the ability to recognize the mental states and emotions of others. In the first six years of life, theory of mind has been associated with many fields. Findings related to many neurobiological bases, such as limbic-paralimbic structures, prefrontal cortex, which start with mirror neurons, help this sense of meaning. Areas associated with theory of mind development provide better understanding of theory of mind skills and deficits, the first psychopathology studies have been carried out in children with autism, and the studies about theory of mind skills in the diagnosis of neurodevelopmental disorders are becoming more and more interesting. In this review, theory of mind development, neurobiological basis and related areas will be explained and the relation of theory of mind with psychopathology will be examined., Zihin kuramı son kırk yılda yapılan klinikçalışmalarla psikiyatrideki önemi ortaya konulan bir sosyal biliş becerisidir.Zihin kuramı becerisi başkalarının zihinsel durumlarını ve duygularını farkedebilme yeteneği olarak sosyal bir canlı olan insanın sosyal işlevselliğindeetkili ve gerekli bir beceri olarak görülmektedir. BEYAZYUZ, MURAT/0000-0002-5770-344X; Potas, Nihan/0000-0002-0393-3135 WOS: 000445634300023 PubMed: 30053573 Attention Deficit and Hyperactivity Disorder (ADHD) is one of the most common psychiatric disorders in childhood and causes significant functional impairments in children. Behavioral genetic and molecular genetic studies have provided significant evidence in terms of highlighting the etiology of ADHD. Folate deficiency during pregnancy is an established risk factor for ADHD. Polymorphisms in the Methyltetrahydrofolate Reductase (MTHFR) encoding gene, such as A1298C and C667T, are associated with the decreased bioavailability of folate, and this condition can act like folate deficiency. In the literature, no study has investigated MTHFR polymorphisms in mothers of children with ADHD. Sixty-four children diagnosed with ADHD and their mothers as well as 40 healthy children and their mothers participated in this study. MTHFR polymorphisms were investigated in all participants. Comparison of the C677C and A1298C MTHFR polymorphisms in children with and without ADHD revealed no significant differences. We found that the maternal C677C_CT genotype counts, both observed and expected values, were significantly different from those based on Hardy-Weinberg Principle Analysis in the ADHD group. The most important result of this study was that maternal C677C MTHFR gene polymorphisms are significant risk factors in for ADHD, and we argue that children with ADHD are exposed to folate deficiency, even if their mothers received a sufficient amount of folate during pregnancy. This result also highlights one of the genetic factors of ADHD. Further studies should be performed to confirm this finding. Abstract

Amaç: Bu çalışmada dikkat eksikliği hiperaktivite bozukluğu (DEHB) ve/ özgül öğrenme bozukluğu (ÖÖB) tanılı çocuklarda, Wechsler Çocuklar İçin Zekâ Ölçeği-Geliştirilmiş Formu (WISC-R) ve Özgül Öğrenme Bozukluğu bataryası kullanılarak bilişsel ve akademik farkların araştırılması amaçlanmıştır.Gereç ve Yöntem: Çalışmaya DEHB ve/veya ÖÖB tanılı 72-131 ay aralığında olan 81 çocuk (%77.8 erkek) dahil edildi. Teste alınan çocukların bir kısmında aktif semptomlar remisyondaydı (Klinik Global İzlenim Ölçeği: 3’den daha az). Değerlendirme süreci sırasında olgular (n=81) dört gruba ayrılmıştır: (1) DEHB tanılı çocuklar (n=11), (2) ÖÖB tanılı çocuklar (n=33), (3) DEHB+ÖÖB tanılı çocuklar (n=15) ve (4) tedavi sürecinde aktif olarak ÖÖB ve DEHB tanısı almayacak düzeyde olan (Klinik Global İzlenim Ölçeği: 3’den daha az) çocuklar (n=22). Ölçüm aracı olarak WISC-R ve ÖÖB bataryası kullanıldı.Bulgular: ÖÖB tanılı çocuklar ÖÖB bataryası okuma süresi dışındaki tüm testlerde ve WISC-R benzerlikler alt testi dışındaki tüm alt testlerde ÖÖB tanısı olmayanlara göre daha düşük puan aldı. DEHB tanılı çocuklar DEHB tanısı olmayanlar ile karşılaştırıldığında sadece ÖÖB bataryası “saat çizme testi” ve WISC-R “şifre” alt testinden daha düşük puan aldılar. Sonuç: WISC-R ve ÖÖB bataryası, birbiri ile ilişkili bozukluklarda (ÖÖB, DEHB ve ÖÖB+DEHB) farkların ayırt edilmesinde geçerli ve güvenilir araçlardır.

Sahin, Berkan/0000-0003-4699-3418 WOS: 000453813200007 PubMed: 30518170 Objective One of the areas of social cognition is Theory of Mind (ToM) is defined as the capacity to interpret, infer and explain mental states underlying the behavior of others. When social cognition studies on neurodevelopmental disorders are examined, it can he seen that this skill has not been studied sufficiently in children with Specific Learning Disorder (SLD). Methods In this study, social cognition skills in children diagnosed with attention deficit hyperactivity disorder (ADHD), SLD or Autism Spectrum Disorder (ASD) evaluated before puberty and compared with controls. To evaluate the ToM skills, the first and second-order false belief tasks, the Hinting Task, the Faux Pas Test and the Reading the Mind in the Eyes Task were used. Results We found that children with neurodevelopmental disorders as ADHD, ASD, and SLD had ToM deficits independent of intelligence and language development. There was a significant correlation between social cognition deficits and problems experienced in many areas such as social communication and interaction, attention, behavior, and learning. Conclusion Social cognition is an important area of impairment in SLD and there is a strong relationship between clinical symptoms and impaired functionality.

WOS: 000368752800003, PubMed ID: 26773986, Background: Dimensional Yale-Brown Obsessive-Compulsive Scale (DY-BOCS) is a promising scale for assessing frequency and severity of symptom dimensions. The main objective of the study was to assess the psychometric properties of the DY-BOCS in a large sample of children and adolescents from Turkey. Methods: We studied 143 children and adolescents, 7-18 years, with well characterized DSM-IV-R OCD, ascertained from seven collaborating university or state hospital sites. We compared the DY-BOCS scores with the Children's Yale-Brown Obsessive-Compulsive Scale (CY-BOCS), the Children's Depression Inventory (CDI), the Yale Global Tic Severity Scale (YGTSS) and the Child Behavior Checklist 6-18 years (CBCL 6-18). Results: The internal consistency of the DY-BOCS symptom dimensions and inter-rater agreement of component scores were excellent. The agreement between global DY-BOCS score and the total CY-BOCS score was highly significant (Pearson's r = 0.55, p < 0.0001). Severity scores for individual symptom dimensions were independent of one another, only modestly correlating with the global ratings, and were also differentially related to ratings of depression, anxiety and tic severity. Conclusion: The DY-BOCS is a reliable and valid instrument for assessing multiple aspects of OCD symptom severity in children and adolescents from Turkey. (C) 2015 Elsevier Inc. Abstract

BackgroundThe symptoms of obsessive−compulsive disorder (OCD) are highly heterogeneous and it is unclear what is the optimal way to conceptualize this heterogeneity. This study aimed to establish a comprehensive symptom structure model of OCD across the lifespan using factor and network analytic techniques.MethodsA large multinational cohort of well-characterized children, adolescents, and adults diagnosed with OCD (N = 1366) participated in the study. All completed the Dimensional Yale-Brown Obsessive−Compulsive Scale, which contains an expanded checklist of 87 distinct OCD symptoms. Exploratory and confirmatory factor analysis were used to outline empirically supported symptom dimensions, and interconnections among the resulting dimensions were established using network analysis. Associations between dimensions and sociodemographic and clinical variables were explored using structural equation modeling (SEM).ResultsThirteen first-order symptom dimensions emerged that could be parsimoniously reduced to eight broad dimensions, which were valid across the lifespan: Disturbing Thoughts, Incompleteness, Contamination, Hoarding, Transformation, Body Focus, Superstition, and Loss/Separation. A general OCD factor could be included in the final factor model without a significant decline in model fit according to most fit indices. Network analysis showed that Incompleteness and Disturbing Thoughts were most central (i.e. had most unique interconnections with other dimensions). SEM showed that the eight broad dimensions were differentially related to sociodemographic and clinical variables.ConclusionsFuture research will need to establish if this expanded hierarchical and multidimensional model can help improve our understanding of the etiology, neurobiology and treatment of OCD.

Abstract

Health-care workers exposed to coronavirus19 disease could be psychologically stressed. The objective of this study is to assess the anxiety, depression levels, and psychological resilience of physicians working during the Covid-19 outbreak and to evaluate the related factors that are associated with their psychological resilience.The sample of this descriptive study was composed of medical doctors and dentists. The data were obtained online between April 13-23, 2020 through a survey prepared by the researchers. In addition, a questionnaire about the participants' sociodemographic characteristics, the Psychological Resilience Scale and Hospital Anxiety Depression Scale (HAD-A/HAD-D) was given.The average age of the 671 participants was 44.0 ± 9.0 years. Psychological resilience scores were significantly higher in those who had children, who had worked for 15 years or more, and who had received training about COVID-19 (p 0.05). Depression scores were higher among women and in those who reported having a chronic disease, whose workload increased after the outbreak, and who had physical contact with COVID-positive patients. The anxiety scores were also higher among women and in those whose workload had increased and who had contact with COVID-positive patients (p 0.05). The physicians with scores below the cutoff point on the HAD-D/HAD-A had significantly higher scores on the Psychological Resilience Scale (p 0.05).Depression and anxiety levels were found to be significantly lower in physicians with greater psychological resilience. Psychological and social support of all health-care workers, especially physicians, is important in the struggle with the pandemic. It is thought that determining the variables related to psychological resilience in health-care workers will be a guide for psychosocial services.

41. Sexual abuse of children in Turkey: psychiatric evaluation of 1785 victims

Abstract

INTRODUCTION: This study aims to investigate the characteristic features of sexual abuse of children with a focus on psychiatric consequences and other associated elements. METHODS: We evaluated 1785 child sexual abuse cases who were referred to the Ondokuz Mayis University Faculty of Medicine Forensic Medicine Department for examination and forensic reports during the 14-year period between 2006 - 2019. The study involved the examination of the patient files/records of victims of child sexual abuse. RESULTS: The girls represented 81.8% of victims of sexual abuse, while all perpetrators were male. The mean ages at the time of abuse were 12.7±3.1 (2-17) years in girls and 10.7±3.2 (2-17) years in boys. In vast majority of the cases, perpetrators were found to be acquaintances of the victims (87.2%). 43% of child sexual abuse cases involved penetration. Psychological examination revealed that psychiatric pathology developed in 64.6% of the children who were exposed to sexual abuse. Being female, the presence of penetration and the presence of incest significantly increased the development of any psychiatric pathologies. CONCLUSION: Sexual abuse of children is a multidimensional public health problem. Additional education services to raise awareness of the children and their relatives, and the establishment of social and legal support schemes for victims and the families for sexual abuse and its consequences are deemed necessary.

42. Does the psychopathology of the parents predict the developmental-emotional problems of the child in early childhood? A machine-learning study

Abstract

Introduction Parental psychopathology has been defined in respect of psychopathological development in early childhood. This study aimed to investigate the effects of parental psychopathologies on social and emotional problems in the age range of 1-3 years and to determine children at risk. Methods The study data were obtained from the 2009 Early Childhood Mental Health Profile taking population distribution into consideration with the properties representing Turkey. The primary caregiver of the child completed the Psychiatric Evaluation Form for 1-3 years, the Brief Infant-Toddler Social Emotional Assessment (BITSEA), the Ages and Stages Questionnaire (ASQ), and the Brief Symptom Inventory (BSI) for themselves. Machine learning models used for prediction. The performance of prediction models was evaluated with the ten-fold cross-validation method. Area Under Curve (AUC) values were calculated with Receiver Operating Characteristic (ROC) curves to evaluate the performance of each model. Results The evaluation was made of the data of 2775 children, comprising 1507 (54.3%) males and 1268 (45.7%) females with a mean age of 26.19±9.11 months (range, 10-48 months). A total of 106 children were identified as at risk, as they were above the clinical cut-off point (1.5 standard deviations) of the BITSEA points and below the cut-off points of any one of the developmental areas of the ASQ. Modeling was applied to the data of these 106 children. The Support Vector Machines (SVM) model was selected for prediction with the automatically optimized highest AUC value. Weighting for the SVM algorithm showed mothers' BSI scores, fathers' education and health problems, duration of breastfeeding, unplanned pregnancy are significant for predicting BITSEA-problem scores in the model. Conclusion To be able to understand the complex relationship with parental psychopathology and behavioral problems, machine learning methods were used successfully in this study. Further studies with more massive data sets, more extended follow-up periods, and stronger algorithms will be able to identify risk groups earlier and allow early interventions to be implemented.

Abstract

Objective: We aimed to determine the predictive factors for psychiatric problem severity and/or clinical approval 10 years after first assessment between ages 1-4 years. Method: The children that were assessed with the CBCL and the BITSEA in their 1-4 years of age were included. The follow-up assessments were made by telephone interviews 10 years later by child psychiatry residents. The primary caregivers responded the follow-up questionnaire items, such as social/academic/behavioral functioning and self-esteem and the items regarding the psychiatric approval, diagnosis, and treatment history. Results: The CBCL-Internalizing scores in toddlerhood were found to be significantly inversely correlated with social and behavioral functioning level and self-esteem scores. The paternal and maternal BITSEA-Competence scores were found to be positively correlated with social and academic functioning. The CBCL Withdrawn/Depressed points at first evaulation were determined to be statistically significantly predictive for psychiatric diagnosis. Conclusion: The study revealed that some measures including CBCL and BITSEA completed in toddlerhood significantly indicate the children under risk of psychiatric morbidity in adolescence. Therefore, we conclude that the widespread use of this assessment measures in toddlers and the referral of the group under risk to the psychiatry units for further investigations could be useful preventive interventions.

Abstract

WOS: 000441774100008 PubMed: 29349589 Temperament refers to the totality of individual characteristics present from birth that determine a child's unique style of behavior. Maternal personality and attitudes, one of the factors affecting temperament traits in children, is a frequently investigated subject. However, paternal variables have remained insufficiently studied. The purpose of this study is to investigate the associations between the fathers' temperament, character, attitudes, psychopathology and temperament of the 3-6 years-old children. The parents of 36-60 months-old children in the preschool settings in Samsun were included in the study (n:200). Their mothers completed "Maternal Sociodemographic Form" prepared by the researcher, and the temperament of children "Children Behaviour Questionnare" were scored by the mothers. Their fathers completed "Paternal Sociodemographic Form", and to assess father psychopathology "Brief Symptom Inventory", to determine father temperament and character "Temperament and Character Inventory" and to determine attitudes "Parenting Attitudes Scale" were scored by the fathers. In this study, we found several significant associations between children's temperament and fathers temperament and character, attitudes styles and psychopathology. The scores of paternal harm avoidance increase and self directedness decrease were found to be significantly positivily correlated with negative temperamental charecteristics of the children. The democratic attitudes of fathers were significantly correlated with positive temperamental scores of the children. All domains of paternal psychopathology were found to be in significant association with negative temperamental characteristics of the children. Our findings showed the complex interplay between determinants of parenting. Specifically, this study is one of the first to investigate paternal personality, psychopathology and attitudes, alone and in interaction with preschool child temperament.
