Amaç: Klinimizde, non-Hodgkin lenfoma tanısı konulan ve tedavi edilen çocuk hastalarımızın demografik ve klinik özelliklerini, tedavi yaklaşımlarını ve sonuçlarını değerlendirmektir.Gereç ve Yöntem: 2006-2002 yılları arasında non-Hodgkin lenfoma tanısı alan ve takip edilen hastaların onkolojik dosyaları geriye dönük olarak incelendi.Bulgular: Bu çalışmaya Hodgkin dışı lenfomalı seksen çocuk dahil edildi. Elli beş erkek (%68,8) ve 25 kız (%31,2) vardı. Hastaların yaşları 2 ile 18 yıl arasında değişmekteydi (ortanca: 11,1 yıl). Dokuz hastada (%11,3) primer immün yetmezlik vardı. Hastaların 63'ü evre III (%78,7) idi. Çoğunluk patolojik alt tip Burkitt lenfoma idi (n: 31, %38,8). Genel sağkalım ve olaysız sağkalım oranları sırasıyla %71,7 ve %71,5 idi. Primer immün yetmezliği olmayan ve olan hastaların genel sağkalım oranları sırasıyla %81,1 ve %11,1 idi. Bu iki grup arasında anlamlı bir fark vardı. Cox regresyon analizi, ileri evre ve eşlik eden primer immün yetmezliğin prognoz için risk faktörleri olduğunu göstermiştir.Sonuç: Yoğun tedavi yaklaşımları, Hodgkin olmayan lenfoma olan çocuklarda genel sağkalım oranlarını artırmıştır. Ancak primer immün yetmezliği olan non-Hodgkin lenfoma çocuklarında bu başarı oranı elde edilememektedir., Aim: To evaluate the demographic and clinical characteristics, treatment approaches and outcomes of our pediatric patients with non-Hodgkin lymphoma diagnosed and treated in our center.Material and Methods: Between 2006 and 2002, the oncologic charts of the patients diagnosed and followed up as non-Hodgkin lymphoma were reviewed retrospectively.Results: Eighty children with non-Hodgkin lymphoma were included in this study. There were 55 boys (68.8%) and 25 girls (31.2%). The patients’ ages ranged from 2 to 18 years (median: 11.1 years). Nine patients (11.3%) had primary immunodeficiency. Sixty-three of the patients were stage III (78.7%). The majority pathologic subtype was Burkitt lymphoma (n: 31, 38.8%). The overall survival and event-free survival rates were 71.7% and 71.5%, respectively. The patients’ overall survival rates without and with primary immunodeficiency was 81.1% and 11.1%, respectively. There was a significant difference between these two groups. Cox regression analysis showed that advanced stage and concomitant primary immunodeficiency have been risk factors for prognosis. Conclusion: Intensive treatment approaches have increased overall survival rates in children with non-Hodgkin lymphoma. However, this success rate cannot be achieved in non-Hodgkin lymphoma children with primary immunodeficiency.

Amaç: Bu çalışmanın amacı hemanjiyom tanısı alan çocuklarda demografik ve klinik özellikleri ile tedavileri ve tedavi sonuçlarının incelenmesidir.Hastalar ve Yöntemler: Ekim 2017- Eylül 2019 tarihleri arasında hemanjiyom tanısı alan 100 çocuk hastanın dosyası geriye dönük olarak incelendi. Hastaların demografik ve klinik özellikleri, tedavi yaklaşımları ve sonuçları not edildi.Bulgular: Çalışmaya dâhil edilen 100 hastanın 66’sı (%66) kız, 34’ü (%34) erkekti. Hastaların yaş gruplarına göre, 0-3 ay, 3-6 ay, 6-12 ay, 12-24 ay ve >24 ay yaş gruplarında sırasıyla 35, 28, 23, 8 ve 6 hasta vardı. Atmış bir hastada (%61), hemanjiyom baş-boyun bölgesinde yerleşimli iken, baş-boyun dışı yerleşim 39 (%39) hastada saptandı. Tedavi yaklaşımları 71 hastada oral propranolol, 21 hastada topikal timolol damla ve 8 hastada oral propranolol + oral metilprednisolon idi. Oral propranolol ile tedavi edilen 37 hastada (%52), topikal timolol damla ile tedavi edilen 7 hastada (%33,3) ve oral propranolol + oral metilprednisolon ile tedavi edilen üç hastada (37,5) yanıt alındı. Stabil hastalık, oral propranolol alan hastaların 13'ünde (%45,1), topikal timolol damla uygulanan hastaların 13'ünde (%61,9) ve oral propranolol + oral kortikosteroid alan hastaların dördünde (%50) saptandı.Sonuç: çocuklarda, hemanjiyom tedavisinde özellikle oral propranolol tedavisinin etkili ve güvenilir bir tedavi yöntemi olduğu gözlemlenmiştir., Childhood hemangiomas and outcome Objective: The aim of this study is to examine the demographic and clinical characteristics, treatments and outcomes in children diagnosed with hemangioma.Patients and Methods: Between October 2017 and September 2019, the files of 100 pediatric patients diagnosed with hemangioma were evaluated retrospectively. Demographic and clinical characteristics of the patients, treatment approaches and results were noted.Findings: Of the 100 patients included in the study, 66 (66%) were female and 34 (34%) were male. According the age of patients, there were 35, 28, 23, 8, and 6 patients in age groups 0-3 months, 3-6 months, 6-12 months, 12-24 months, and >24 months, respectively. Hemangioma was located in the head and neck region in 68 patients (61%), while extra-head and neck location was detected in 39 (39%) patients. Treatment approaches were oral proporanolol in 71 patients, topical timolol in 21 patients, and oral proporanolol + oral metilprednisolon in 8 patients. Response was obtained in 37 patients (52%) treated with oral propranolol, 7 patients (33%) treated with timolol drops, and three patients (37.5) treated with oral propranolol + oral metilprednisolon. Stable disease was found in 13 (45.1%) of patients receiving oral propranolol, 13 (61.9%) of patients performing topical timolol drop, and four (50%) patients receiving oral propranolol + oral corticosteroid.Result: Oral propranolol has been observed as an effective and safe treatment method for treatment hemangioma.

In recent years, genetic factors have attracted attention in NPC. A very limited data have been reported about clustering within families. Case. Herein, the familial clustering of nasopharyngeal carcinoma in the family of an adolescent with nasopharyngeal carcinoma is presented. Conclusions. There is familial clustering in nasopharyngeal carcinoma (NPC), but our knowledge on this subject is limited, especially in children or adolescent populations. Therefore, we should be more careful in NPC in childhood, especially in first-degree relatives.

Giriş: Febrilkonvülziyon (FK)'nın patogonezi tam olarak bilinmemekle birlikte, ateş ile birliktelik gösteren multifaktöryel nedensellik üzerinde durulmaktadır. Bunlar arasında, son yıllarda yapılan çalışmalarda, genetik yatkınlık ön plana çıkmaktadır.GABA–A reseptörünün Ɣ 2 subünitindeki R43Q (rs211037) mutasyonunun FK gelişiminde etkili olduğu bulunmuştur.Ayrıca; sitokrom P450 (CYP450) sistemi, ilaç metabolizmasında önemli bir yeri olan enzim sistemidir ve antiepileptik ilaçların metabolizmasından sorumludur. CYP2C9 ve CYP2C19 polimorfizmiantiepileptik ilaç tedavisine yanıtı belirlemede önemli rol oynamaktadır; dolaysıyla nöbet türünün davranışına ait bilgiler de sunabilmektedir. FK genetiğinde çok önemli role sahip bu iki antite, bizim çalışmamızın konusu oldu.Gereç ve Yöntem:Ağustos-2015 / Ocak -2016 tarihleri arasında çocuk nöroloji, çocuk acil ve çocuk sağlığı hastalıkları kliniğine başvuran çocuk nöroloğu tarafından FK tanısı alan tedavi ve takip altında olan hastalar retrospektif ve prospektif olarak çalışmaya dahil edildi. Hasta grubu belirlenen kriterlere göre 102 kişiden oluşturuldu. Kontrol grubu belirlenen kriterlere göre 96 kişiden oluşturuldu. Hastaların genetik analizlerinin yanında EEG ve ilaç kullanma durumları da değerlendirildi.Bulgular:Çalışma sonuçlarımızda öne çıkan anlamlı bulgular arasında: (i). Hasta grubunda aile öyküsünde FK bulunması kontrol grubuna göre anlamlı bulundu (p=0,003). (ii). FK'lı hastalarımızın çoğunluğu 1-3 yaş arasında (%58) iken; 1 yaş altı ve 3 yaş üstü daha az sıklıkla belirlendi (sırasıyla %30; %12) (iii). Hastalarımızda erkek cinsiyet baskındı (1,56/1). (iv). Hastalarımızda rekürrens oranı yüksek bulundu (%62; ortalama nöbet sayısı 2,26) (v). Hastalarımız büyük çoğunluğu basit FK (%73) iken; daha az oranını komplike FK oluşturmaktaydı (%27). (vi). Hastalarımızın büyük çoğunluğuna EEG çekimi yapıldı (%92; 92/100) ve bunların yarıdan fazlasında anormal EEG sonuçları elde edildi (%56). (vii). EEG anormalliği olan hastalarımızda ilaç kullanma oranıda yüksekti (37/56, %66,7,p=0,001). (viii).Hasta ve kontrol gruplarımızda R43Q mutasyon dağılımı benzerdi (sırasıyla; hasta grubu: 58 wild tip, %36 heterozigot, %6 homozigot mutasyon ve kontrol grubu: %56,3 wild tip, %35 heterozigot, %7,3 homozigot) (p=0,927). (ix). Hasta grubunda R43Q mutasyonun kliniğe etkisine bakıldığında; EEG anormalliği ve ilaç kullanımını anlamlı bulduk (sırasıyla p=0,032; p=0,021). (x).R43Q homozigot mutasyonu olan hastaların hepsinin EEG anormalliği mevcuttu ve ilaç kullanma oranı da ( 5/6, %83,3) yüksekti. (xi). Hasta grubunda CYP2C19'a ait metabolizör sıklığı: hızlı metabolizör %77, orta metabolizör %22, yavaş metabolizör %1 oranında bulundu. Bu oranların kontrol grubuyla benzeştiği belirlendi (hızlı metabolizör %80,2, orta metabolizör %16,7, yavaş metabolizör %3) (p=0,393) (xii). CYP2C19 polimorfizmine ait hasta grubunda hiçbir anlamlılık bulamadık (cinsiyet, ilk FK yaşı, nöbet sayısı, nöbet tipi, aile öyküsü, akrabalık, EEG ve ilaç); (p>0,05).Sonuç:FK'lı hastalarda R43Q mutasyonu ile CYP2C19 polimorfizmin kontrol grubuna göre benzer dağılım göstermekteydi. Ancak hasta grubunda R43Q mutasyonuna sahip olma EEG anormalliği ve ilaç kullanım gerekliliği için olumsuz katkı sağlamaktaydı. Introduction: Althoughthe pathogenesis for febrile convulsions (FC) is not known for certain, much emphasis is placed on multifactorial casuality accompanied by high temperature. Recent studies have taken the possibility of genetic susceptibility to the foreground, among others. It has been shown that the mutation R43Q (rs211037) in the Ɣ 2 subunit of the receptor GABA–A leads to the development of FC. In addition, the system cytochrome P450 (CYP450) is an important enzyme system for drug metabolism, and is responsible for the metabolism of antiepileptic drugs. CYP2C9 and CYP2C19 polymorphism is definitive of response to antiepileptic drug treatment, and therefore may provide information on the behaviour of the seizure type. These two entities, both of which play an important part in the genetics of FC, constitute the subject of this study.Materials and Methods:Patients who applied to the clinics of paediatric neurology, paediatric emergency department, and paediatrics and were diagnosed with FC by the paediatric neurologist, whose treatment process or follow up continued between the dates of August 2015 and January 2016 were retrospectively and prospectively included in the study. The study group comprised of 102 patients, and the control group included 96 children who met the above mentioned criteria. Hirschprung Hastalığı enterik sinir sisteminin gelişimsel bir bozukluğu olup distal kolondaki myenterik ve submukozal pleksuslarda ganglion hücrelerinin yokluğu ile karakteriz edir. Bu durum etkilenen barsak kısmında peristaltizmin yokluğuna ve fonksiyonel intestinal tıkanıklığa yol açar. Yaklaşık 5000 canlı doğumda bir görülmekte ve genellikle mekonyum çıkışında gecikme, karın distansiyonu, beslenme intoleransı ve safralı kusma ile karşımıza çıkmaktadır. Burada yenidoğan döneminde Hirschprung Hastalığı tanısı konulmuş erkek bebeğin kliniği ve radyolojik tetkikleriyle değerlendirilmesi sunulmuştur., Hirschsprung disease is a developmental disorder of the enteric nervous system and is characterized by the absence of ganglion cells in the myenteric and submucosal plexuses of the distal in testine. This condition results in absence of peristalsis in the affected bowel and the development of a functional intestinal obstruction. It occurs in approximately 1 in 5000 live born infants and usually presented with delay in the meconium passage, dis tended abdomen, feeding intolerance and bilious vomiting. Here in, clinical and radiological evaluation of a male infant who diagnosed with Hirschsprung's disease in the neonatal period was presented.

Bu çalışmada, tehlike sinyalleyen uyarıcılara karşı tercihli olarak harekete geçen bilişsel mekanizmalardaki hemisferik yanallaşma incelenmiştir. Tehlikeli uyarıcıların erken ve hızlı saptanması, insan zihninin evrimsel süreçte kazandığı düşünülen savunma mekanizmasıyla yakından ilişkilidir. Söz konusu mekanizmayı incelenmek için ekolojik açıdan geçerli uyarıcılar olarak, yüzlerinde duygu ifadesi bulunan kalabalık insan grupları, bir görsel tarama görevinde kullanılmıştır. Davranışsal tepkiler ve göz izleme verileri, kalabalık gruplar içinde sunulan tehlikeli yüz ifadesine sahip (öfkeli) bireylerin dikkati daha erken çektiğini, tehlikeli ifadeli kalabalık gruların ise dikkati daha uzun sure tuttuğunu göstermiştir. Kabalık gruplar içindeki duygu ifadelerinin saptanmasında görsel alan yanlılığının varlığına ilişkin birinde görsel yarı alan tekniği, diğerinde değişimi saptamada yanıp sönme paradigması kullanılan iki çalışma daha yürütülmüştür. Sonuçlar, yüzdeki ifadelerin işlemlenmesine yönelik olarak öne çıkan iki hipotez kapsamında değerlendirilmiştir. Kalabalık yüz grupları içindeki duygu ifadelerinin saptanmasında gözlemlenen sol görsel alan yanlılığının yüz ifadelerinin taranması ve işlemlenmesiyle ilgili olarak çalışan bilişsel mekanizmaların anlaşılmasında yeni bakış açıları kazandıracağı düşünülmektedir. The present thesis examined hemispheric lateralization in preferential activation of cognitive mechanisms in response to threatening stimuli. The early and faster detection of threatening stimuli is closely associated with a defense mechanism, which is likely to be acquired through the evolutionary process of the human mind. In this respect, crowd of human faces with emotional content were used to provide ecologically valid stimuli, and a visual search task was adopted to investigate the mechanism. Behavioral and eye tracking data indicated early attentional capture of faces with a threatening expression (i.e. anger), which were presented among distractor faces, and longer attentional allocation of faces with a threatening expression, which were used as distractors. A visual field bias for detecting emotional faces in crowds was investigated in two follow up experiments by adopting divided visual field methodology and flicker paradigm of change detection. The results were evaluated on the basis of two prominent hypotheses on processing of facial emotions. Evidence for left visual field bias in the detection of emotional faces among crowds would provide new perspectives to understand the cognitive mechanisms for processing and search of facial stimuli. 172
