Abstract

In this review, we aimed to outline the clinical and pathological conditions for which multidetector computed tomography coronary angiography (MDCT-CA) should be the preferred method because of its advantages over conventional coronary angiography (CCA). A consistent body of literature suggests that MDCT-CA is more than just complementary to CCA and that it provides more valuable diagnostic information in certain clinical situations, such as complex coronary artery variations, aorto-ostial lesions, follow-up of bypass grafts, myocardial bridging, coronary artery fistulas, aortic and coronary artery dissections, and cases in which the coronary ostia cannot be cannulated by a catheter because of massive atherosclerosis or extremely tortuous vascular structures.

26. Diagnostic Accuracy of Dual-Source 64-Slice Multidetector Computed Tomography in Evaluation of Coronary Artery Bypass Grafts

Abstract

Background The aim of this study was to compare the diagnostic accuracy of 64-slice multidetector computed tomography (MDCT) with conventional coronary angiography to detect graft patency and stenosis.Methods In this retrospective analysis, we included a total of 284 subjects (210 men, 73.9%; mean ± SD age, 62.6 ± 9.9) and evaluated 684 bypass grafts using a dual-source 64-slice MDCT scanner The mean ± SD time interval between coronary artery bypass grafting operation and MDCT was 30.8 ± 6.2 months. The mean ± SD interval between MDCT angiography and conventional coronary angiography was 14.2 ± 3.6 days. Significant stenosis was defined as lesions causing 50% or greater luminal narrowing. All atherosclerotic lesion components were assessed on per-segment basis.Results All of the 684 grafts (420 venous and 264 arterial grafts) were evaluable and included in the analysis. For the detection of 50% or greater graft stenosis, the sensitivity, specificity, positive predictive value and negative predictive value of MDCT was 98.3%, 99.3%, 98.3%, and 99.3% for venous grafts and 100%, 99.5%, 98.0%, and 100% for arterial grafts. In detection of graft patency, the sensitivity, specificity, positive predictive value, and negative predictive value of MDCT was 99.6%, 97.2%, 99.0%, and 99.0% for venous grafts and 99.5%, 97.5%, 99.5%, and 97.5% for arterial grafts. Diagnostic accuracy for the detection of graft patency was 99% (416/420) and 99.2% (262/264) for venous and arterial grafts, respectively.Conclusion The diagnostic accuracy of dual-source 64-slice MDCT angiography for evaluating coronary artery bypass grafts patency and stenosis was high. Dual-source 64-slice MDCT can be used for the evaluation of patients after coronary artery bypass grafting.

Abstract

We present CT findings of a young woman who has bilateral pulmonary nodules mimicking metastases. Clinical presentation with active multiple pulmonary macronodules without cavitation responsive to treatment is an atypical manifestation of pulmonary tuberculosis. We reviewed the causes of multiple pulmonary nodules, role of radiological findings in differential diagnosis and parenchymal manifestations of pulmonary tuberculosis in this report. Abstract

Objectives: In this prospective study, the effect of calcitriol therapy on bone mineral density and osteopenia in patients with severe secondary hyperparathyroidism has been investigated., Materials and Methods: The study was carried out on 24 chronic dialysis patients consisting of 13 boys and 11 girls, aged between 8-18 years. Patients were divided into 3 groups according to the severity of hyperparathyroidism and therapy regimens. Group I consisted of 5 patients with normal parathormon levels who did not receive calcitriol therapy. In group II and III, there were patients with secondary hyperparathyroidism. Group II consisted of 10 patients receiving oral calcitriol therapy. Group III consisted of 9 patients receiving intravenous (i.v.) calcitriol. Bone mineral density was measured by dual energy x-ray absorptiometry. Osteopenia was defined as a Z-score worse than -2. Bone mineral density was assessed as baseline and at the end of one year., Results: A significant improvement was observed in Z-score in the group III whereas the mean value of Z-score tended to be worse in group I and it was not significantly different in group II from the initial values. The better Z-score in group III was associated with more effective stabilization of alkaline phosphatase level and bone specific alkaline phosphatases (BAP) concentrations., Conclusion: Significant improvement of Z-score in group III suggested the beneficial role in i.v. administration of calcitriol in chronic dialysis patients.
