Abstract

Background: The pins and rubber traction system (PRTS) has proven effective in managing intra-articular fractures of the proximal interphalangeal joint. However, there is scant evidence in the literature regarding its efficacy in treating distal interphalangeal joint (DIPJ). This study aims to investigate the outcomes of PRTS in the treatment of comminuted intra-articular fractures of the DIPJ. Methods: We conducted a retrospective review of patients with comminuted intra-articular fractures of the DIPJ treated with PRTS between 2017 and 2021. At the final follow-up, we measured and compared the active range of motion (ROM) in both affected and non-injured contralateral fingers. The subjective evaluation utilised the Quick Disabilities of the Arm, Shoulder and Hand (Quick-DASH) questionnaire and the Visual Analogue Scale (VAS). Results: Ten patients with a mean follow-up of 13.2 months (range: 12–17) were included in the study. Fracture locations included the base of the distal phalanx in two patients, the condyle of the middle phalanx in seven and both in one patient. At the final follow-up, the average VAS score was 0.5 (range: 0–2). The average active motion of the DIPJ was 61° (range: 50°–70°) for the injured side and 76° (range: 75°–80°) for the opposite side. The mean range of DIPJ movement was 80% (range: 68%–87%) of the non-injured side. Extension deficits were observed in five patients, with a median deficit value of 10° (range: 5°–10°). The average Quick-DASH score was 2.9 (range: 0–11.3). Conclusions: The PRTS can be considered as an effective surgical technique in managing comminuted intra-articular fractures of the DIPJ. Abstract

An {\em antimagic labeling} of a graph with $m$ edges and $n$ vertices is a bijection from the set of edges to the integers $1,...,m$ such that all $n$ vertex sums are pairwise distinct, where a vertex sum is the sum of labels of all edges incident with the same vertex. A graph is called {\em antimagic} if it has an antimagic labeling. A conjecture of Ringel (see \cite{HaRi}) states that every connected graph, but $K_2$, is antimagic. Our main result validates this conjecture for graphs having minimum degree $\Omega(\log n)$. The proof combines probabilistic arguments with simple tools from analytic number theory and combinatorial techniques. Abstract

The possibility of construction disputes can be reduced, but they cannot be avoided due to the uncertain and risky nature of the building industry. Conflicts between construction parties often have very unfavourable effects, such as cost increases, poor construction quality and time extension in the schedule. Lots of studies have been carried out in order to try and avoid these disagreements. However, there are no common resolution tools or techniques due to the improving conditions and scope of contracted works. Advanced methods and dispute reasons should be fully monitored and updated for the applicable solutions. This paper discusses the current major constructional dispute reasons in Turkey. The questionnaire method was applied within the scope of this study. The questionnaire documents were randomly distributed to 80 contractors to analyse major dispute reasons in Turkey. Analysis of the questionnaire results indicates that the major current dispute causes are poor quality of performed works, delays in progress payments, inefficient site management, poorly written contracts and design mistakes.

However, there may also be negative effects of social environments on the diabetic patient. This study developed and validated a new diabetic unsupportive social interaction scale (USIS), including two subscales: interference and insensitivity.Methods: A list of 22 items depicting unsupportive interactions associated with management of diabetes was developed. A telephone survey was administered to 764 Israelis with diabetes. The questionnaire included the USIS and questions about social networks, social support, health behaviors, and health. The characteristics, validity, and reliability of the scale were tested.Results: A principal component analysis was performed for extraction of two factors describing unsupportive social interaction concepts: interference and insensitivity. Cronbach’s alpha for the full 15-item scale was 0.84, indicating internal consistency. The two subscales were calculated to have Cronbach’s alphas of 0.85 and 0.73, respectively. The USIS showed construct validity as it was associated with social support, some measures of social networks, subjective measures of health, and health behaviors. Arabs, older respondents, those defining themselves as more religious, and the less educated reported higher rates of unsupportive interactions.Conclusion: This study suggests a new concept of unsupportive interactions including interference and insensitivity. These unsupportive interactions may adversely affect patients’ ability to adhere to treatment and may undermine their health in various ways. Abstract

Environmental and lifestyle factors play an important role in the natural history of Crohn's disease and ulcerative colitis. A group of international experts from the International Organization for the Study of Inflammatory Bowel Diseases voted on a series of consensus statements to inform the management of inflammatory bowel disease (IBD). The recommendations include avoiding traditional cigarette smoking in patients with Crohn's disease or ulcerative colitis, screening for symptoms of depression, anxiety, and psychosocial stressors at diagnosis and during flares (with referral to mental health professionals when appropriate), and encouraging regular physical activity as tolerated. Patients using dietary approaches for treatment of their IBD should be encouraged to adopt diets that are best supported by evidence and involve monitoring for the objective resolution of inflammation. We recommend formal assessment for obesity and nutritional deficiencies, and patients should be encouraged to maintain a normal body-mass index. A shared decision-making approach to contraception should include the consideration of IBD-related factors, and risk factors for venous thromboembolism. Long-term or frequent use of high-dose non-steroidal anti-inflammatory drugs should be avoided. For primary prevention of disease in the offspring of patients with IBD, we recommend avoiding passive exposure to tobacco, using antibiotics judiciously, and considering breastfeeding when able.

Even with treatment, the majority of patients relapse within 5 years, and suicide may occur in up to 10% of patients. Poor adherence to oral antipsychotics is the most common cause of relapse. The discontinuation rate for oral antipsychotics in schizophrenia ranges from 26% to 44%, and as many as two-thirds of patients are at least partially nonadherent, resulting in increased risk of hospitalization. A very helpful approach to improve adherence in schizophrenia is the use of long-acting injectable (LAI) antipsychotics, although only a minority of patients receive these. Reasons for underutilization may include negative attitudes, perceptions, and beliefs of both patients and health care professionals. Research shows, however, significant improvements in adherence with LAIs compared with oral drugs, and this is accompanied by lower rates of discontinuation, relapse, and hospitalization. In addition, LAIs are associated with better functioning, quality of life, and patient satisfaction. A need exists to encourage broader LAI use, especially among patients with a history of nonadherence with oral antipsychotics. This paper reviews the impact of nonadherence with antipsychotic drug therapy overall, as well as specific outcomes of the schizophrenia patient, and highlights the potential benefits of LAIs.
