Striae distensae are common innocuous skin lesions. We report a patient of nephrotic syndrome with corticosteroid-induced large striae, which suddenly developed bulging and fluid accumulation. There were no associated cutaneous or systemic symptoms. Analysis of aspirated fluid showed it was a transudate and culture was sterile. It was concluded that edema fluid, as part of anasarca, had preferentially accumulated within the striae, probably due to their altered dermal mechanical properties. Recognizing this entity is important to avoid unnecessary alarm or irrational use of antibiotics.

Papuloerythroderma of Ofuji is an enigmatic entity. It is commonly idiopathic; at times, it may occur secondary to atopy, infections , malignancies or drugs. W e report a patient who developed the characteristic generalized itchy eruption of confluent cobblestone-lik e papules, sparing the flexures, a few days after he was put on terbinafine, empirically , for suspected dermatophytosis. Other associated features included relative lymphopenia and dermatopathic lymphadenopathy. The rash responded to withdrawal of terbinafine and treatment with systemic corticoster oids. Terbinafine has a wide range of cutaneous adverse effects, but this is the first report of terbinafine-associated papuloerythroderma. Hundred children (upto 12 years of age) with vitiligo seen over a 1-year period were evaluated for their clinical pattern and therapeutic response to various modalities. Maximum number (29 percent) of patients were in the age group 6 to 9 years. Most of the patients (94 percent) had upto 5 percent skin involvement and the commonest (61 percent) clinical variant was vitiligo vulgaris followed by focal vitiligo (23 percent). None of the patients had any associated systemic diseases. Most satisfactory response was seen with PUVA-Sol and systemic corticosteroid combination. Overall good response with topical corticosteroids was seen in 44 (55 percent) patients. Lesions over the face showed best response. There was no correlation of response to treatment with age, sex, duration of disease and family history of vitiligo.

An upper GI endoscopy was carried out in 21 patients with lichen planus. Thirteen (61.9 percent) patients had oral mucosal lesions characteristic of lichen planus. Mean duration of skin lesions was 1.9 years and that of oral mucosal lesions was 1.2 years. Commnest site of oral mucosal involvement was mucous membrane over the cheek. Upper GI endoscopy revealed mucosal abnormalities in 16 (76.2 percent) patients. The lesions were suggestive of lichen planus in 14 (66.6 percent). Associated changes were oesophagitis and gastritis. None of the 21 patients had oesophageal stricture. Musocal biopsy from the lesions in the upper GI tract in 11 patients was suggestive of lichen planus.

Menkes′-Kinky disease, also known as trichopoliodystrophy, is an X-linked recessive disorder first described in 1962 by Menkes et al 1. The basic defect of copper metabolism incriminated in this disease was discovered 10 years later by Danks et al 2. In this disorder affected infants appear normal at birth but gradually develop all the features over a few months to years. The condition is incompatible with life and death usually occurs by 4 year of age 3. The incidence of premature birth is 50 percent 4. We herein describe this rare condition in a patient recently seen by us.

A 19 year old male presented with multiple atrophic patches with ′Cliff-drop′ borders on the trunk of 3 years duration. Clinically and histopathologically, the lesions were consistent with a diagnosis of idiopathic atrophoderma of Pasini and Pierini. It is a distinct entity and is not related to morphoea .

Oral mucosa plays an imortant role in the natural history of pemphigus vulgaris. In approximately 50-70 percent of patients, the disease starts in the oral cavity, 1-4 and the lesions in oral mucosae preceed the cutaneous lesions by several months. This time period is variable; On an average a generalized bullous phase in most patients occurs some 5 or more months after the onset of the oral lesions. 1-3 very rarely lesions remain restricted to oral mucosa (e) for prolonged period. We report a family where 7 members in three successive generations were affected with lichen amyloidosis. Histopathology of the lesion with special stain for amyloid and polaroscopic examinations confirmed the diagnosis. Primary localized cutaneous amyloidosis is not an uncommon entity. Familial forms are however, very rare 1, and there have been only few case reports in the literature where more than one member of the family was affected 2-4.

A patient with creeping eruption and dermographism is reported. Both the disorders responded satisfactorily to a single dose of albendazole.

A case of Bowen′s disease of the periareolar skin causing destruction of the nipple is reported. There was no history of chronic arsenicalism and no evidence of underlying visceral malignancy.

Grinspan′s syndrome as originally described is a triad of conditions, namely essential vascular hypertension, diabetes mellitus and lichen planus of the oral mucosa 1,2. Skin lesions of lichen planus, however, may or may not be associated and the syndrome may be a drug related phenomenon 3. A case recently seen by us is being reported.
