Abstract Background Although the “obesity paradox” is comprehensively elucidated in heart failure (HF) with reduced ejection fraction (HFrEF) and HF with preserved ejection fraction (HFpEF), the role of body composition in left ventricular (LV) remodeling, LV reverse remodeling (LVRR), and clinical outcomes is still unclear for HF with mildly reduced ejection fraction (HFmrEF). Methods Our study is a single-centre, prospective, and echocardiography-based study. Consecutive HFmrEF patients, defined as HF patients with a left ventricular ejection fraction (LVEF) between 40 and 49%, between January 2016 to December 2021 were included. Echocardiography was re-examined at 3-, 6-, and 12-month follow-up to assess the LVRR dynamically. Body mass index (BMI), fat mass, fat-free mass, percent body fat (PBF), CUN-BAE index, and lean mass index (LMI) were adopted as anthropometric parameters in our study to assess body composition. ObjectivesTo develop a novel automatic delineation model, the Multi-Scale Channel Attention U-Net (MCAU-Net) model, for gallbladder segmentation on CT images of patients with liver cancer.MethodsWe retrospectively collected the CT images from 120 patients with liver cancer, based on which ground truth was manually delineated by physicians. The images and ground truth constitute a dataset, which was proportionally divided into a training set (54%), a validation set (6%), and a test set (40%). Data augmentation was performed on the training set. Our proposed MCAU-Net model was employed for gallbladder segmentation and its performance was evaluated using Dice Similarity Coefficient (DSC), Jaccard Similarity Coefficient (JSC), Positive Predictive Value (PPV), Sensitivity (SE), Hausdorff Distance (HD), Relative Volume Difference (RVD), and Volumetric Overlap Error (VOE) metrics.ResultsOn the test set, MCAU-Net achieved DSC, JSC, PPV, SE, HD, RVD, and VOE values of 0.85 ± 0.22, 0.79 ± 0.23, 0.92 ± 0.14, 0.84 ± 0.23, 2.75 ± 0.98, 0.18 ± 0.48, and 0.22 ± 0.42, respectively. Compared to the control models, U-Net, SEU-Net and TransUNet, the MCAU-Net improved DSC 0.06, 0.04 and 0.06, JSC by 0.09, 0.06 and 0.09, PPV by 0.08, 0.08 and 0.05, SE by 0.05,0.05 and 0.07, and reduced HD by 0.45, 0.28 and 0.41, RVD by 0.07, 0.03 and 0.07, VOE by 0.04, 0.02 and 0.08 respectively. Qualitative results revealed that MCAU-Net produced smoother and more accurate boundaries, closer to the expert delineation, with less over-segmentation and under-segmentation and improved robustness.ConclusionsThe MCAU-Net model significantly improves gallbladder segmentation on CT images. It satisfies clinical requirements and enhances the efficiency of physicians, particularly in segmenting complex anatomical structures.

Multi-quantum well (MQW) diodes exhibit simultaneous emission and detection, allowing them to serve as multifunctional devices, including light emitters, receivers, energy transmitters, and information transmitters. Leveraging this capability, we designed a Multifunctional Energy Transfer Information System (METIS) that integrates contactless control, energy harvesting, and information transfer. At the core of this system, the multifunctional energy communication chip operates effectively across a broad range of extreme temperatures and in various solution environments. As the ambient temperature varies from −60 to 120 °C, the peak emission wavelength shifts from 465 to 476 nm, and even with further temperature changes from −70 to 150 °C, the communication function remains stable. Encapsulated for durability, METIS functions reliably in extreme conditions such as ice, water, salt solutions, and other light-transmitting fluids without needing external circuitry. Additionally, we demonstrate passive control of analog switches via MQW diodes. The MQW diodes also enable contactless energy and optical information transfer, ensuring stable and controllable information reconstruction at the receiving end. This approach offers an innovative solution for energy and information transmission in extreme environments.

The flame retardancy and smoke suppression performance of silicone rubber foam (SiFs) greatly affected its promotion and application in the field of high performance materials. Improving the flame retardancy and smoke suppression performance of SiFs had became a hot and difficult topic in recent years. The current research status of flame retardancy and smoke suppression of SiFs in flame retardant systems such as metal hydroxide flame retardants, boron- based flame retardants, phosphorus-and nitrogen-based flame retardants, platinum catalysts, microcapsule flame retardants, and ceramic-based flame retardants had been summarized with 67 references. Besides, the corresponding flame retardancy mechanisms had been expounded. Finally, the future research direction of flame retardancy and smoke suppression of SiFs was presented.

Background: Non-alcoholic fatty liver disease (NAFLD) has become the most prevalent chronic liver condition globally, with its severe form, cirrhosis, leading to liver failure, hepatocellular carcinoma (HCC), and mortality. The causal relationships among them remain unclear. Methods: This study employed a two-sample Mendelian randomization (MR) approach to systematically investigate the causal relationships among inflammatory proteins, non-alcoholic steatohepatitis (NASH), and HCC. We utilized NASH data and HCC data from the FinnGen biobank, and genomic data for 91 circulating inflammatory factors from GWAS database. Bidirectional MR analyses were conducted using the Inverse-Variance Weighted method as the primary analysis, supplemented by MR-Egger and weighted median methods for sensitivity analyses. Additionally, we performed heterogeneity, pleiotropy, and MR-PRESSO tests to assess the robustness of our findings. Results: Forward MR analysis identified five inflammatory factors significantly associated with NASH risk: CCL25 (OR = 0.8703, p = .0159), S100A12 (OR = 0.7582, p = .0204), IL18 (OR = 0.7412, p = .0026), and LIFR (OR = 0.6804, p < .001) showed negative correlations, while CXCL11 (OR = 1.2625, p = .0354) demonstrated a positive correlation. Reverse analysis revealed that NASH significantly influenced the levels of 10 circulating inflammatory factors, including Beta-nerve growth factor (OR = 1.0286, p = .0245), CCL20 (OR = 1.0382, p = .0012), CD5 (OR = 1.0268, p = .0466), CUBD1 (OR = 1.0371, p = .0078), CSF1 (OR = 1.0231, p = .0458), IL-12β (OR = 1.0261, p = .0381), IL-22Rα1 (OR = 0.9722, p = .0311), CCL3 (OR = 1.0372, p = .0013), OPG (OR = 1.0436, p = .0103), and uPA (OR = 1.0320, p = .0054). Further analysis revealed that IL-1α levels were positively associated with HCC risk (OR = 1.5335, p = .0377), while SULT1A1 levels showed a negative correlation (OR = 0.7190, p = .0200). Pleiotropy analyses showed no significant heterogeneity or horizontal pleiotropy for most identified associations, except for OPG (heterogeneity p = .001, MR-PRESSO p < .001) and CUBD1 (MR-PRESSO p < .05) in the reverse analysis of NASH, which warrant cautious interpretation. Conclusion: This study unveils complex bidirectional causal relationships among inflammatory factors, NASH, and HCC. Intervention targeting IL-1α in patients with liver cirrhosis will significantly impact the progression of HCC.

BackgroundPeripartum cardiomyopathy (PPCM) is a form of heart failure that severely affects women during the late stages of pregnancy and in the postpartum period. Currently, the diagnosis of PPCM is not fully understood and is likely multifactorial. Abnormal lipid metabolism plays an important role in the onset of cardiovascular diseases, especially in patients with heart failure. Moreover, animal experiments have confirmed a possible association between abnormal lipid metabolism and PPCM onset. However, clinical evidence is currently lacking, and reliable and effective indicators for predicting the onset of PPCM are insufficient. The ratio of triglyceride to high-density lipoprotein cholesterol (TG/HDL-C) is a novel metabolic marker that is associated with the risk of coronary heart disease. However, the relationship between the TG/HDL-C ratio and the risk of PPCM in parturients remains unknown. Therefore, this study aimed to explore the role of the TG/HDL-C ratio in predicting the risk of PPCM.MethodsThis study included 600 parturients hospitalized at Qilu Hospital of Shandong University between January 2010 and August 2023 (150 patients with PPCM and 450 healthy parturients). The TG/HDL-C ratio was calculated as triglyceride levels (mmol/L)/high-density lipoprotein cholesterol levels (mmol/L). The relationship between the TG/HDL-C ratio and PPCM was analyzed using logistic regression analysis and receiver operating characteristic curve analysis.ResultsSignificant differences were observed in the TG/HDL-C ratio between patients with PPCM and healthy parturients. The TG/HDL-C ratio was higher in patients with PPCM than in healthy parturients (p < 0.001). Logistic regression analysis revealed that the TG/HDL-C ratio increased the risk of PPCM and had predictive accuracy for the onset of PPCM (odds ratio, 1.249; 95% confidence interval, 1.004–1.553; p = 0.046). Moreover, the TG/HDL-C ratio was positively correlated with the NT-proBNP levels at the onset of PPCM (R2 = 0.081, p = 0.008).ConclusionsA higher TG/HDL-C ratio was significantly associated with the risk of PPCM.

BackgroundPancreatic ductal adenocarcinoma (PDAC) is a highly lethal malignancy with poor prognosis. Epigenetic dysregulation plays a crucial role in PDAC progression, but its comprehensive landscape and clinical implications remain unclear.MethodsWe integrated single-cell RNA sequencing, bulk RNA sequencing, and clinical data from multiple public databases. Single-cell analysis was performed using Seurat and hdWGCNA packages to reveal cell heterogeneity and epigenetic features. Weighted gene co-expression network analysis (WGCNA) identified key epigenetic modules. A machine learning-based prognostic model was constructed using multiple algorithms, including Lasso and Random Survival Forest. We further analyzed mutations, immune microenvironment, and drug sensitivity associated with the epigenetic risk score.ResultsSingle-cell analysis revealed distinct epigenetic patterns across different cell types in PDAC. WGCNA identified key modules associated with histone modifications and DNA methylation. Our machine learning model, based on 17 epigenetic genes, showed robust prognostic value (AUC >0.7 for 1-, 3-, and 5-year survival) and outperformed existing models. High-risk patients exhibited distinct mutation patterns, including higher frequencies of KRAS and TP53 mutations. Low-risk patients showed higher immune and stromal scores, with increased infiltration of CD8+ T cells and M2 macrophages. Drug sensitivity analysis revealed differential responses to various therapeutic agents between high- and low-risk groups, with low-risk patients showing higher sensitivity to EGFR and MEK inhibitors.ConclusionOur study provides a comprehensive landscape of epigenetic regulation in PDAC at single-cell resolution and establishes a robust epigenetics-based prognostic model. The integration of epigenetic features with mutation profiles, immune microenvironment, and drug sensitivity offers new insights into PDAC heterogeneity and potential therapeutic strategies. These findings pave the way for personalized medicine in PDAC management and highlight the importance of epigenetic regulation in cancer research.

The properties of DNA polymerases isolated from thermophilic and mesophilic microorganisms, such as the thermophilic Geobacillus stearothermophilus (Bst) and mesophilic Bacillus subtilis phage (Phi29), have been widely researched. However, DNA polymerases in psychrophilic microorganisms remain poorly understood. In this study, we present for the first time the expression and functional characterization of DNA polymerases PWT-WT and FWT-WT from Psychrobacillus sp. BL-248-WT-3 and FJAT-21963. Enzymatic activity assays revealed that FWT-WT possessed strand displacement but lacked exonuclease activity and high ionic strength tolerance, whereas PWT-WT lacked all these properties. Further protein engineering and biochemical analysis identified D423 and S490 as critical mutation sites for improving strand displacement and tolerance to high ionic strength, specifically in the presence of 0–0.3 M potassium chloride (KCl), sodium chloride (NaCl), and potassium acetate (KAc). Three-dimensional structural analysis demonstrated that the size and the electric charge of the single-stranded DNA (ssDNA) encapsulation entrance were pivotal factors in the binding of the ssDNA template.

Brain tumors have a poor prognosis. Early, accurate diagnosis and treatment are crucial. Although brain surgical biopsy can provide an accurate diagnosis, it is highly invasive and risky and is not suitable for follow-up examination. Blood-based liquid biopsies have a low detection rate of tumor biomarkers and limited evaluation ability due to the existence of the blood-brain barrier (BBB). The BBB is composed of brain capillary endothelial cells through tight junctions, which prevents the release of brain tumor markers to the human peripheral circulation, making it more difficult to diagnose, predict prognosis, and evaluate therapeutic response through brain tumor markers than other tumors. Focused ultrasound (FUS)-enabled liquid biopsy (sonobiopsy) is an emerging technique using FUS to promote the release of tumor markers into the circulatory system and cerebrospinal fluid, thus facilitating tumor detection. The feasibility and safety data from both animal models and clinical trials support sonobiopsy as a great potential in the diagnosis of brain diseases.

Abstract Background Serum uric acid (SUA) is an important pathogenetic and prognostic factor for heart failure (HF). Gender differences are apparent in HF. Furthermore, gender differences also exist in the association between SUA and prognosis in various cardiovascular diseases. However, the gender difference for SUA in the prediction of long-term prognosis in HF is still ambiguous. Methods A total of 1593 HF patients (897 men, 696 women) from the National Health and Nutrition Examination Survey (NHANES) 1999–2018 cycle were enrolled in our final analysis. Participants were categorized according to gender-specific SUA tertile. We assessed the association between SUA and long-term prognosis of HF patients, defined as all-cause mortality and cardiovascular mortality, in different genders via Kaplan–Meier curve analysis, Cox proportional hazard model, and Fine-Gray competing risk model. The restricted cubic spline (RCS) was performed to investigate the dose-response relationship between SUA and outcomes. Results Gender differences exist in demographic characteristics, clinical parameters, laboratory tests, and medication of HF patients. After a median follow-up of 127 months (95% CI 120–134 months), there were 853 all-cause deaths (493 events in men, 360 events in women) and 361 cardiovascular deaths (206 events in men, 155 events in women). Unveiling the potential application of psychrophilic polymerases as candidates for polymerase-nanopore long-read sequencing presents a departure from conventional choices such as thermophilic Bacillus stearothermophilus (Bst) renowned for its limitation in temperature and mesophilic Bacillus subtilis phage (phi29) polymerases for limitations in strong exonuclease activity and weak salt tolerance. Exploiting the PB-Bst fusion DNA polymerases from Psychrobacillus (PB) and Bacillus stearothermophilus (Bst), our structural and biochemical analysis reveal a remarkable enhancement in salt tolerance and a concurrent reduction in exonuclease activity, achieved through targeted substitution of a pivotal functional domain. The sulfolobus 7-kDa protein (Sso7d) emerges as a standout fusion domain, imparting significant improvements in PB-Bst processivity. Notably, this study elucidates additional functional sites regulating exonuclease activity (Asp43 and Glu45) and processivity using artificial nucleotides (Glu266, Gln283, Leu334, Glu335, Ser426, and Asp430). By disclosing the intricate dynamics in exonuclease activity, strand displacement, and artificial nucleotide-based processivity at specific functional sites, our findings not only advance the fundamental understanding of psychrophilic polymerases but also provide novel insights into polymerase engineering.

Owing to emission-detection spectral overlap, a quantum well (QW) diode can detect shorter-wavelength photons emitted from another diode sharing the same QW active region. Therefore, a wireless light communication system can be established by using identical QW diodes that function separately as transmitters and receivers. Here, we investigate the irreversibility between light emission and detection of the QW diode and present a time-division multiplexing (TDM) visible light communication (VLC) network using five identical blue QW diodes that are defined by software as transmitters or receivers to achieve real-time TDM-integrated interconnection via the same optical paths. These results indicate the great potential of realizing an advanced TDM VLC network for diverse applications.

ABSTRACT Accurate crop classification, crucial for a macro-level understanding of food production, formulating relevant agricultural policies, and predicting comprehensive agricultural productivity, enables precise crop distribution. In remote sensing image classification, feature selection and representation play a pivotal role in accuracy. An augmented U-Net algorithm, named ASPP-SAM-UNet, integrating spatial attention mechanisms and multi-scale features is proposed for the enhancement of typical crop classification accuracy in remote sensing. The ASPP-SAM-UNet design integrates features over multiple scales, boosts the representational capacity of shallow features, and expands the neural network’s receptive field by incorporating Atrous Spatial Pyramid Pooling (ASPP) into the convolutional components of the standard U-Net encoder via residual connections. The integration of the residual module allows for a profound fusion of deep and shallow features, thereby enhancing their utility. The spatial attention mechanism amalgamates spatial and semantic information, empowering the decoder to reclaim more spatial information. This study focused on Bayan County, Harbin City, Heilongjiang Province, China, employing GF-6 WFV remote sensing images for crop classification. Empirical outcomes showed a significant improvement in classification accuracy with the advanced algorithm, boosting the overall accuracy (OA) from 89.49 to 92.80%. Specifically, the segmentation accuracy for maize, rice, and soybean increased from 89.90, 89.96, and 87.37% to 93.47, 94.82, and 89.35%, respectively. The suggested algorithm offers a pioneering performance standard for crop classification leveraging GF-6 WFV remote sensing imagery.

Summary: Orexin is a neuropeptide released from hypothalamus regulating feeding, sleeping, arousal, and cardiovascular activity. Past research has demonstrated that orexin receptor 2 (OX2R) agonist infusion in the brain results in sympathoexcitatory responses. Here, we found that epicardial administration of OX2R agonism leads to opposite responses. We proved that OX2R is expressed mainly in DRG neurons and transported to sensory nerve endings innervating the heart. In a capsaicin-induced cardiac sympathetic afferent reflex (CSAR) model, we recorded the calcium influx in DRG neurons, measured heart rate variability, and examined the PVN c-Fos activity to prove that epicardial OX2R agonism administration could attenuate capsaicin-induced CSAR. We further showed that OX2R agonism could partially rescue acute myocardial infarction by reducing sympathetic overactivation. Our data indicate that epicardial application of OX2R agonist exerts a cardioprotective effect by attenuating CSAR. This OX2R-mediated heart-brain axis may provide therapeutic targets for acute cardiovascular diseases.

Abstract Aims The study aims to explore the real‐world titration patterns of sacubitril/valsartan in a chronic heart failure (HF) follow‐up management system and the effect on the recovery of ventricular remodelling and cardiac function in China. Methods and results This is a single‐centre, observational study of 153 adult outpatients with HF and reduced ejection fraction who were managed in the chronic HF follow‐up management system and prescribed with sacubitril/valsartan from August 2017 to August 2021 in China. All patients tried to titrated sacubitril/valsartan to the tolerant dose during follow‐up. The primary outcome was the proportion of patients who reached and maintained the target dose of sacubitril/valsartan. The main secondary outcomes were the changes in left atrium diameter, left ventricular end‐diastolic diameter (LVEDD), and left ventricular ejection fraction (LVEF) from baseline to 12 months. Among the patients, 69.3% were male, with a median age of 49 years. The baseline systolic blood pressure (SBP) was 117.6 ± 18.3 mmHg before starting the treatment of sacubitril/valsartan. Benefiting from the management system, 117 (76.5%) patients achieved the target dose of sacubitril/valsartan, and the median time to reach the target dose was 3 (IQR 1–5) months. Advanced age and lower SBP may be predictors of failure to reach the target dose. Compared with baseline, standard treatment resulted in a pronounced improvement in left ventricular geometry and cardiac function. The patients showed a significant increase in LVEF [28 (IQR 21–34) % vs. 42 (IQR 37.0–54.3) %, P 40%, and 81.1% experienced an increase in LVEF of ≥10%. After 12 month follow‐up, the proportion of patients with New York Heart Association classification I or II increased from 41.8% to 96.4%. Abstract Aims Peripartum cardiomyopathy (PPCM) are more vulnerable to intracardiac thrombus than other types of cardiomyopathies, although explicit anticoagulant strategy is not sure. Too aggressive anticoagulation therapy can lead to severe bleeding events. Hence, we want to construct a risk stratification model for intracardiac thrombus in PPCM patients. Methods and results A total of 159 suspected PPCM cases were initially screened, whereas 123 confirmed cases were enrolled in the final analysis. The study population was randomly assigned as derivation group (N = 83) and validation group (N = 40). The derivation cohort was utilized to develop the model, and the validation cohort was used to internal validate the discriminatory ability of the model. Formation of intracardiac thrombus was detected in 22 patients. After adjusted by multivariable logistic regression analysis, left ventricle ejection fraction (LVEF, OR 0.772, 95% CI 0.665–0.897, P = 0.001), haemoglobin levels (OR 1.050, 95% CI 1.003–1.099, P = 0.038), and thrombocyte counts (OR 1.018, 95% CI 1.006–1.029, P = 0.003) were identified as risk factors independently associated with intracardiac thrombus and were finally included in the tentative risk stratification model with a C‐indexes of 0.916 (95% CI: 0.850–0.982, P

In Phi29-α–hemolysin (α-HL) nanopore sequencing systems, a strong electrochemical signal is dependent on a high concentration of salt. However, high salt concentrations adversely affect polymerase activity. Sequencing by synthesis (SBS) requires the use of phi29 polymerase without exonuclease activity to prevent the degradation of modified nucleotide tags; however, the lack of exonuclease activity also affects polymerase processivity. This study aimed to optimize phi29 polymerase for improved salt tolerance and processivity while maintaining its lack of exonuclease activity to meet the requirements of nanopore sequencing. Using salt tolerance compartmentalized self-replication (stCSR) and a microfluidic platform, we obtained 11 mutant sites with enhanced salt tolerance attributes. Sequencing and biochemical analyses revealed that the substitution of conserved amino acids such as G197D, Y369E, T372N, and I378R plays a critical role in maintaining the processivity of exonuclease-deficient phi29 polymerase under high salt conditions. Furthermore, Y369E and T372N have been identified as important determinants of DNA polymerase binding affinity. This study provides insights into optimizing polymerase processability under high-salt conditions for real-time polymerase nanopore sequencing, paving the way for improved performance and applications in nanopore sequencing technologies.

Intensive aquaculture combatting the decline of large yellow croaker populations can trigger bacterial outbreaks, resulting in extensive antibiotic use. In this study, we screened 5 aquaculture sites in the coastal areas of Zhejiang and identified 17 antibiotics in large yellow croakers using UPLC-MS/MS. The distribution and occurrence of antibiotic pollutants were different in the different tissues of large yellow croakers, being primarily dominated by quinolones. Relatively higher average residue levels of enrofloxacin and ciprofloxacin were detected in the inedible parts, specifically the gills (37.29 μg/kg). Meanwhile, relatively high average residue levels of enrofloxacin and ciprofloxacin were also found in the edible parts, particularly in the muscle (23.18 μg/kg). We observed that the residue levels detected in the swim bladder exceeded the prescribed limit for fish muscle, but there is currently no specific regulatory limit established for this particular tissue. Despite the HI values of enrofloxacin and ciprofloxacin being below 0.01, the health risks should not be disregarded. The findings of this research provide significant practical implications for assessing antibiotic contamination and enhancing the risk management of coastal regions.

When a quantum well (QW) diode is biased with a forward voltage and illuminated with an external shorter-wavelength light, the device simultaneously emits and detects light, with the injected current and the induced current mixed inside the wells. Separating these superimposed and dynamic electrical signals is useful for the development of multifunctional displays that can simultaneously transmit and receive light signals. By utilizing the unique overlap between the electroluminescence and detection spectra, we establish a wireless optical communication system using two AlGaInP diodes that have identical QW structures. The communication distance is 25 m, with one diode functioning as the transmitter and the other as the receiver. In particular, at the receiver end, the QW diode demonstrates simultaneous light emission and reception ability, and the mixed signals can be efficiently extracted, suggesting great potential for applications from light communication to advanced displays.

Phase Shifting Transformer (PST) can help improve the power flow distribution of the transmission section, which can increase the wind power consumption of the grid. In order to adapt the PST allocation to the grid evolution, this paper presents a dynamic programming method to allocate PST in each planning stage of the grid optimally. The optimal allocation model of PST under a single grid seeks to maximize the wind power consumption and the Total Transfer Capacity (TTC) between areas. A calculation method for TTC of grids containing PST and wind power is proposed. The Non-Dominated Sorting Genetic Algorithm II (NSGA2) is used to solve the Pareto sets under each planning stage of the grid. Then, the optimal planning path of PST is derived based on dynamic programming. The superiority of the proposed method is demonstrated by comparing the IEEE-118 system results of dynamic and static programming.

BackgroundThe function of coronary microcirculation is an important factor in predicting the prognosis of patients with acute coronary syndrome (ACS) who receive percutaneous coronary intervention (PCI) therapy. Ticagrelor, a type of oral P2Y12 inhibitor, is widely prescribed to ACS patients and can improve prognosis compared to clopidogrel. However, the efficacy of ticagrelor on coronary microcirculation, compared to clopidogrel, remains unclear. The objective of this meta-analysis was to determine the efficacy of ticagrelor on coronary microcirculation.MethodsThe PubMed, Cochrane Central Register of Controlled Trials (CENTRAL), and ClinicalTrials.gov databases were comprehensively searched to identify studies until November 2022. Data was pooled using the fixed effects model or random effects model based on the level of heterogeneity. Sensitivity analyses were performed to measure the effects of potential confounders.ResultsAfter screening, 16 trials with a total of 3676 participants were ultimately included in the analysis. The meta-analysis revealed that compared to clopidogrel, patients receiving ticagrelor exhibited a more significant reduction in the IMR (WMD: -6.23, 95% CI: -8.41 to -4.04), a reduction in the cTFC (WMD: -1.88; 95% CI: -3.32 to -0.45), and greater increases in CFR (WMD: 0.38; 95% CI: 0.18 to 0.57), MBG (RR 1.29, 95% CI 1.12 to 1.48), and TIMI (RR 1.03, 95% CI 1.00 to 1.06).ConclusionOur findings suggest that, compared to clopidogrel, ticagrelor has a significant effect in reducing coronary microcirculatory resistance, enhancing coronary blood flow reserve, and improving myocardial perfusion.

Diabetic foot ulcers (DFUs) are pathological states of tissue destruction of the foot or lower extremity in diabetic patients and are one of the serious chronic complications of diabetes mellitus. Matrix metalloproteinases (MMPs) serve crucial roles in both pathogenesis and wound healing. The primary functions of MMPs are degradation, which involves removing the disrupted extracellular matrix (ECM) during the inflammatory phase, facilitating angiogenesis and cell migration during the proliferation phase, and contracting and rebuilding the tissue during the remodeling phase. Overexpression of MMPs is a feature of DFUs. The upregulated MMPs in DFUs can cause excessive tissue degradation and impaired wound healing. Regulation of MMP levels in wounds could promote wound healing in DFUs. In this review, we talk about the roles of MMPs in DFUs and list potential methods to prevent MMPs from behaving in a manner detrimental to wound healing in DFUs.

Type-1 cardiorenal syndrome refers to acute kidney injury induced by acute worsening cardiac function. Worsening renal function is a strong and independent predictive factor for poor prognosis. Currently, several problems of the type-1 cardiorenal syndrome have not been fully elucidated. The pathogenesis mechanism of renal dysfunction is unclear. Besides, the diagnostic efficiency, sensitivity, and specificity of the existing biomarkers are doubtful. Furthermore, the renal safety of the therapeutic strategies for acute heart failure (AHF) is still ambiguous. Based on these issues, we systematically summarized and depicted the research actualities and predicaments of the pathogenesis, diagnostic markers, and therapeutic strategies of worsening renal function in type-1 cardiorenal syndrome.

Serotonin is a well-known secondary metabolite that plays an important role in many growth and developmental processes, as well as biotic and abiotic stress responses of plants. Yet, whether serotonin biosynthesis regulates the resistance of rice to the brown planthopper (BPH) Nilaparvata lugens and its underling mechanisms have not been entirely investigated. In this study, we found that expression levels of four serotonin biosynthesis gene, OsTDC1, OsTDC2, OsTDC3, and OsT5H, and the serotonin content were significantly induced by BPH nymph infestation. The rice seedlings pretreated with serotonin were more attractive to BPH nymphs and promoted their feeding behavior as indicated by increased honeydew excretion. In addition, serotonin application to artificial diets increased the survival rates of BPH nymphs in a dose-dependent manner compared to the controls. Moreover, the exogenous serotonin application to rice seedlings increased the average injury scale and functional plant loss indices caused by BPH nymph infestation. Moreover, supplemented serotonin enhanced soluble sugar and free amino acid contents, as well as the activities of SOD, POD, and PPO, but repressed the production of flavonoids. Our findings illustrated that serotonin plays a negative role in the regulation of rice resistance to BPH nymphs probably by modulating contents of soluble sugars, free amino acids, and flavonoids.

Underwater wireless optical communication (UWOC) has been widely considered a supplement to traditional underwater acoustic communication. A real-time UWOC video delivery system was developed in a laboratory water tank based on a field-programmable gate array (FPGA) with binary frequency shift keying (2FSK) modulation. The system achieved full-duplex communication by using the transmission control protocol (TCP) and forward error correction (FEC). A high-power 445 nm light-emitting diode (LED) array was adopted to enhance the transmitted optical power and increase the transmission link distance. We present an underwater optical channel model that considers the effects of both geometry and channel loss, especially considering the impact of the refractive index of the optical medium and the non-line-of-sight (NLOS) links formed by water surface reflection. MATLAB was used to simulate this channel model and predict the received optical power distribution on the receiving plane. Additionally, we propose improved calculation methods for the consumed electrical power and transmitted optical power of the LED array. We also investigate the relationship between the optimum avalanche gain of an avalanche photodiode (APD) and the signal-to-noise ratio (SNR). This full-duplex system achieved a 1 Mbps data transmission rate at an SNR of 10.1 dB and a distance of 10 m for an underwater link. In addition, when the optical power of the LED array is enhanced, the link range is predicted to be 14.5 m with an attenuation coefficient of 0.056 /m.

Monitoring and predicting the deformation of surrounding rocks in the rock tunnel construction process is of great significance. This study implemented a wireless sensor network (WSN), including gateway transmission, relay point, and sensor nodes, to obtain high granularity deformation data during construction. A transformer model is proposed, which considers the construction sequence into the positional embedding and has an attention module to deeply learn the high dimensionality correlation between the nearby deformation data and the tunnel face deformation. The attention-enhanced LSTM model and the LSTM model are also constructed to compare them with the performance of the transformer model. A site study conducted on a shallow buried tunnel section suggested an excellent performance of the proposed WSN system. The transformer model shows the best performance in terms of the model prediction results, which can extract more information from the time sequence data than the attention-enhanced LSTM and LSTM models. The proposed system has great value as guidance and reference for the construction of rock tunnel projects in complex and unfavourable geological conditions.

For count data, though a zero-inflated model can work perfectly well with an excess of zeroes and the generalized Poisson model can tackle over- or under-dispersion, most models cannot simultaneously deal with both zero-inflated or zero-deflated data and over- or under-dispersion. Ear diseases are important in healthcare, and falls into this kind of count data. This paper introduces a generalized Poisson Hurdle model that work with count data of both too many/few zeroes and a sample variance not equal to the mean. To estimate parameters, we use the generalized method of moments. In addition, the asymptotic normality and efficiency of these estimators are established. Moreover, this model is applied to ear disease using data gained from the New South Wales Health Research Council in 1990. This model performs better than both the generalized Poisson model and the Hurdle model.

It also introduced composition, working principle and characteristics of the electromagnetic radiometer in details. The test of electromagnetic radiation in Xinzhouyao Coal Mine proved the electromagnetic radiometer not only can be used to forecast rock burst, but also to check effect of preventing measures of rock burst, which has characteristics such as non-contact, non-disturbance, simple operation practicality, safety and accuracy, and is a reasonable selection of forecasting rock burst.

Sepsis is one of the most frequent causes of death in intensive care unit (ICU) patients worldwide. According to recent reports, the incidence of sepsis has been rising at a rate of 1.5%-8% per year.1 About 9% of the sepsis develops to serious sepsis and 3% to septic shock. Although great progress has been made in the treatment of sepsis, themortality of patients with severe sepsis is still as high as 30%- 70%.2-4 Moreover, the high expenditure of sepsis management has resulted in a heavy financial burden to the government and patients.1 Therefore, howto estimatethe severity of sepsis early and apply targeted therapies timely is very important in the treatment of sepsis.
