3. Genetic overlap between type 2 diabetes and depression

Abstract

Background: An association between type 2 diabetes (T2DM) and depression has been reported in epidemiological studies. The mechanisms underlying the T2DM-depression link remain unclear. One possible question is whether the co-occurrence of T2DM and depression is due to common genetic and/or common environmental vulnerabilities. A genetic overlap between T2DM and depression will provide evidence supporting a common biological pathway to both disorders. Method: This thesis applied three methodological approaches: i) structural equation modelling of twin data, ii) polygenic score analysis and iii) linkage disequilibrium score regression using genome-wide association studies (GWAS) data. For the first approach, the primary dataset was the Swedish Twin Registry with replication in the Danish Twin Registry and Colombo Twin and Singleton Study (COTASS-2). For the second and third approach, the population cohorts deCODE and UK Biobank, and the Psychiatric Genomics Consortium Major Depressive Disorder (PGC-MDD-29) dataset were used. Result: In the twin studies, a genetic overlap between T2DM and depression was observed in the Swedish Twin Registry and the finding was replicated in the Danish Twin Registry and COTASS-2. In the GWAS datasets, T2DM-polygenic scores were not a major contribution to depression nor depression-polygenic scores for T2DM in both deCODE and UK Biobank. A small, negative but statistically significant association was observed between feelings of guilt/worthlessness and T2DM-polygenic scores in the PGC-MDD-29 dataset. Conclusion: Twin studies have suggested a genetic overlap between T2DM and depression. There are many reasons to explain the discrepancies in findings between twin studies and GWAS. Clarifying the shared heritability between these two complex traits is an important next step while gene-environment interaction is an area that needs to be explored, given genotypes can affect an individual’s responses to the environment and environment can differentially affect genotypes expression.

Following publication of this article, the authors noticed an error in Supplementary Table 1. In the original Supplementary Table 1, one of the criteria for control participants was incorrectly given as ‘Report extensive recent symptoms of depression: less than 14 on summed response (where “not at all” = 1 and “nearly every day” = 4) to recent’. This has now been corrected to: ‘Report extensive recent symptoms of depression: less than 5 on summed response (where “not at all” = 1 and “nearly every day” = 4) to recent’.

Abstract

Depression is more frequent among individuals exposed to traumatic events. Both trauma exposure and depression are heritable. However, the relationship between these traits, including the role of genetic risk factors, is complex and poorly understood. When modelling trauma exposure as an environmental influence on depression, both gene-environment correlations and gene-environment interactions have been observed. The UK Biobank concurrently assessed Major Depressive Disorder (MDD) and self-reported lifetime exposure to traumatic events in 126,522 genotyped individuals of European ancestry. We contrasted genetic influences on MDD stratified by reported trauma exposure (final sample size range: 24,094–92,957). The SNP-based heritability of MDD with reported trauma exposure (24%) was greater than MDD without reported trauma exposure (12%). Simulations showed that this is not confounded by the strong, positive genetic correlation observed between MDD and reported trauma exposure. We also observed that the genetic correlation between MDD and waist circumference was only significant in individuals reporting trauma exposure (rg = 0.24, p = 1.8 × 10−7 versus rg = −0.05, p = 0.39 in individuals not reporting trauma exposure, difference p = 2.3 × 10−4). Our results suggest that the genetic contribution to MDD is greater when reported trauma is present, and that a complex relationship exists between reported trauma exposure, body composition, and MDD.

Abstract

This study investigated the genetic and environmental contributions to emotional overeating (EOE) and depressive symptoms, and their covariation, in a Sri-Lankan population, using genetic model-fitting analysis. In total, 3957 twins and singletons in the Colombo Twin and Singleton Study-Phase 2 rated their EOE behaviour and depressive symptoms, which were significantly associated (men: r = 0.11, 95% confidence interval (CI) 0.06–0.16, women: r = 0.12, 95% CI 0.07–0.16). Non-shared environmental factors explained the majority of variance in men (EOE e2 = 87%, 95% CI 78–95%; depressive symptoms e2 = 72%, 95% CI 61–83%) and women (EOE e2 = 76%, 95% CI 68–83%; depressive symptoms e2 = 64%, 95% CI 55–74%). Genetic factors were more important for EOE in women (h2 = 21%, 95% CI 4–32%) than men (h2 = 9%, 95% CI 0–20%). Shared-environmental factors were more important for depressive symptoms in men (c2 = 25%, 95% CI 10–36%) than women (c2 = 9%, 95% CI 0–35%). Non-shared environmental factors explained the overlap between depressive symptoms and EOE in women but not in men. Results differed from high-income populations, highlighting the need for behavioural genetic research in global populations.

Abstract

Approximately 1.25 million people in the UK suffer from an eating disorder, yet the treatment options show limited efficacy, warranting the need for novel approaches. This study aimed to investigate the perspectives of people with eating disorders on the use of complementary therapies and psychedelic research and treatment. Two hundred participants with eating disorders took part in this web survey study. The majority of participants (70%) had used a complementary treatment to manage their eating disorder. Participants believed that psychedelic research was worthwhile in the context of a moderate level of concern. The most popular solutions to meet these concerns included providing education around psychedelics and their effects and use in psychiatry and experiencing endorsement from professionals in the area. Moreover, participant responses emphasized the need for a safe, monitored environment and the patient-therapist rapport in the context of psychedelic treatment. The findings are explored concerning future trials of psychedelics as a treatment for eating disorders. [ABSTRACT FROM AUTHOR]

All rights reserved.IMPORTANCE The association between major depressive disorder (MDD) and obesitymay stem from shared immunometabolic mechanisms particularly evident in MDD with atypical features, characterized by increased appetite and/or weight (A/W) during an active episode. OBJECTIVE To determine whether subgroups of patients with MDD stratified according to the A/W criterion had a different degree of genetic overlap with obesity-related traits (body mass index [BMI] and levels of C-reactive protein [CRP] and leptin). DESIGN, SETTING, AND PATIENTS This multicenter study assembled genome-wide genotypic and phenotypic measures from 14 data sets of the Psychiatric Genomics Consortium. Data sets were drawn from case-control, cohort, and population-based studies, including 26 628 participants with established psychiatric diagnoses and genome-wide genotype data. Data on BMI were available for 15 237 participants. Data were retrieved and analyzed from September 28, 2015, through May 20, 2017. MAIN OUTCOMES AND MEASURES Lifetime DSM-IV MDDwas diagnosed using structured diagnostic instruments. Patients with MDD were stratified into subgroups according to change in the DSM-IV A/W symptoms as decreased or increased. RESULTS Data included 11 837 participants with MDD and 14 791 control individuals, for a total of 26 628 participants (59.1% female and 40.9%male). Among participants with MDD, 5347 (45.2%) were classified in the decreased A/W and 1871 (15.8%) in the increased A/W subgroups. Common genetic variants explained approximately 10% of the heritability in the 2 subgroups. The increased A/W subgroup showed a strong and positive genetic correlation (SE) with BMI (0.53 [0.15]; P = 6.3 × 10-4), whereas the decreased A/W subgroup showed an inverse correlation (-0.28 [0.14]; P = .06). Furthermore, the decreased A/W subgroup had a higher polygenic risk for increased BMI (odds ratio [OR], 1.18; 95%CI, 1.12-1.25; P = 1.6 × 10-10) and levels of CRP (OR, 1.08; 95%CI, 1.02-1.13; P = 7.3 × 10-3) and leptin (OR, 1.09; 95%CI, 1.06-1.12; P = 1.7 × 10-3). CONCLUSIONS AND RELEVANCE The phenotypic associations between atypical depressive symptoms and obesity-related traits may arise from shared pathophysiologic mechanisms in patients with MDD. Development of treatments effectively targeting immunometabolic dysregulations may benefit patients with depression and obesity, both syndromes with important disability.
