Introduction: Seasonal keratoconjunctivitis of military personals in Chattogram area causes loss of many working hours with prolonged sufferings. Objective: To explore the outcome of using Ganciclovir in seasonal viral keratoconjunctivitis in Combined Military Hospital, Chattogram. Methods: This prospective study was conducted in department of ophthalmology of Combined Military Hospital (CMH), Chattogram, from January 2013 to September 2018. Diagnosis was made by clinical signs, symptoms and laboratory study of conjunctival smear to exclude bacterial or fungal cause. Initially they were treated with artificial tear and antibiotic to prevent secondary bacterial infection. After three days of complete rest, the cases presented with static or further deteriorated condition were treated with Ganciclovir eye gel five times a day for two weeks. After subsidence of acute redness, topical dexamethasone was also applied for 4 times and then 3 times at one week interval. Follow up periods were 3 days, 1 week, 2 weeks, 4 weeks and 3 months subsequently. Results: A total of 2678 cases were included in our study. About 78% of the patients were male and 22% were female. 95% patients were from age group of young and serving personnel. Ganciclovir was used in 750 (28%) cases. With conventional treatment, recovery of sign symptoms took 3 weeks (SD-3±1 weeks). Recovery was observed within 10 (9±2 day) days in 450 (60%) cases of using Ganciclovir. Short term complications such as pseudomembrane formation were found in 24% cases and corneal ulcer in 1 case. Superficial punctate keratitis (SPK) was found in 22% cases even after 3 months. Conclusion: Treating viral conjunctivitis without long term vision impairing sequel is a challenging job due to nonspecific presentation. Without specific features of bacterial or fungal conjunctivitis, patients of seasonal keratoconjunctivitis treated with Ganciclovir provide prompt resolution which save working hours of serving personnel. Introduction: Dry eye is a multifactorial disease of the tears and ocular surface, is highly prevalent and has a significant impact on quality of life. Long term use of Cyclosporine has demonstrated that this drug halt progression of chronic dry eye disease and is associated with a cure of signs and symptoms of Dry Eye disease. The aim of our study is to evaluate the efficacy of topical Cyclosporine A (0˙05%) for the treatment of mild, moderate and severe dry eye disease. Methods: This prospective study was conducted in the ophthalmology department of Combined Military Hospital, Jashore from January 2019 to December 2019 among selected 20 patients aged 20-26 years with a confirmed diagnosis of dry eye syndrome refractory to conventional management. All the patients were treated with Cyclosporine A twice daily and were evaluated at month 1,2,3,4,5 and 6 for changes from base line in tear film break up time, schirmer test, fluorescein staining, tear meniscus height, symptoms of ocular discomfort and visual acuity. Result: Mean age of 20 cases was 48.7 years among them 14(70%) female and 6(30%) male. Mean TBUT improved from 4.4 second to 8.7 second (p=0.001) after 6 months of treatment. SCHIRMERʼS paper test was performed before the beginning of the treatment and showed improvement of wetting from mean 3.5mm to 8.2mm (p=0.001) after 6 month. Mean lower tear meniscus height improved from 0.2 mm to 0.63 mm (p=0.002) after 6 months of treatment. Fluorescein staining was significantly lowered from mean 2.8 to 1.8 (p=0.001) with significant reduction of ocular symptoms and improvement of visual acuity after 6 months of treatment. Conclusion: Topical Cyclosporine A (0˙05%) has been demonstrated to be effective in all categories of dry eye disease. It reduces symptoms and signs of dry eye disease with the greatest improvement of signs in patients with severe dry eye disease. CBMJ 2021 January: vol. 10 no. 02 P: 98-104

Introduction: Torsional ultrasound energy and burst mode interrupted energy delivery system are recent advancementsin the technique of phacoemulsification surgery. It has been reported that both these advancementshelpto reduce corneal wound burn and thermal induced endothelial cell loss and thereby helps to achieve excellent early postoperative visual outcome Objectives: To evaluate the efficacy of burst mode torsional phacoemulsification surgery in age-related cataract. Materials and Methods: Prospective observational study of 120 cases that underwent phacoemulsificationcataract surgery in which torsional energy was used in burst mode. Phacoemulsification was performed by “vertical –chop” technique. Intraoperatively corneal wound burn, Descemet’s membrane detachment, posterior capsule ruptureetc.were evaluated. Postoperatively corneal oedema, best corrected visual acuity, cystoid macular oedema were evaluated. Postoperatively eyes were examined at day 1, 1 week, 6 weeks and 3 months. Results: Mean age of the patients was 57.28± 8.20 years. Most of the patients 81(67.5%) had nuclear sclerosis grade-II cataract.Intraoperatively, moderate wound burn occurred in 02(1.6%) eyes, localized Descemet’s membrane detachmentin 03(2.5%) eyes and posterior capsule rupture occurred in 03(2.5%) cases. Postoperatively, moderate and severe corneal edema was found in 06(5.0%) and 04(3.3%) eyes respectively at day 1. At the end of 3 months follow-up, 114(95%) eyes maintained a best corrected visual acuity of ≥ 6/18, of which 109(90.8%) eyes achieved ≥ 6/9. Conclusion: Burst mode torsional phacoemulsfication is a very effective and advanced technique of cataract surgery. Intraoperative complications are very less and visual outcome is excellent. Chicken egg yolk immunoglobulin (IgY) is a functional substitute for mammalian IgG for antigen detection. Traditional IgY purification methods involve multi-step procedures resulting in low purity and recovery of IgY. In this study, we developed a simple IgY purification system using IgY-specific peptides identified by T7 phage display technology. From disulfide-constrained random peptide libraries constructed on a T7 phage, we identified three specific binding clones (Y4-4, Y5-14, and Y5-55) through repeated biopanning. The synthetic peptides showed high binding specificity to IgY-Fc and moderate affinity for IgY-Fc (Kd : Y4-4 = 7.3 ± 0.2 μM and Y5-55 = 4.4 ± 0.1 μM) by surface plasmon resonance analysis. To evaluate the ability to purify IgY, we performed immunoprecipitation and affinity high-performance liquid chromatography using IgY-binding peptides; the result indicated that these peptides can be used as affinity ligands for IgY purification. We then used a peptide-conjugated column to purify IgY from egg yolks pre-treated using an optimized delipidation technique. Here, we report the construction of a cost-effective, one-step IgY purification system, with high purity and recovery. © 2017 The Authors. To design an affinity ligand for purification of antigen-binding fragment (Fab) antibody, variable domain of heavy chain antibody (VHH) phage libraries were constructed from Fab-immunized Alpaca and subjected to biopanning against Fabs. To find the specific binders, we directly applied high-throughput sequencing (HTS) analysis of the VHH sequences in the panned phages on next-generation sequencer. The efficiently enriched sequences were aligned for construction of the phylogenetic tree to be categorized into five groups. VHHs from three major groups were first selected to analyze their properties as an affinity ligand. However, those VHHs were not suitable as an affinity ligand because of lack of resistance against alkaline pH and/or difficulty in acidic elution from the affinity column. So, we further searched the candidates from minor group sequences. Among five, one VHH showed the binding ability but with low affinity against Fabs. Therefore, we improved its affinity-by-affinity maturation through error-prone PCR library techniques. The final designed VHH showed highly alkaline pH resistance and easy acidic elution together with high affinity to Fabs. These results indicate that HTS techniques combined with biopanning and followed by error-prone PCR library techniques is powerful in designing specific binders with desired properties.

Introduction: Cervical cancer is the second most prevalent cancer among women worldwide. High-Risk Human Papilloma virus (hr-HPV) is an established cause of cervical cancer and precancerous lesion. Studies have shown the relationship between the HPV-DNA test with cervical precancerous and cancerous lesion in diagnosing cervical cancer. This study aimed to correlate HPV-DNA test with both cytology and histology. Objective: To find out the correlation of HPV-DNA test with cytology and histology for the diagnosis of cervical precancerous and cancerous lesion. Materials and Methods: Cervical smears and DNA samples were collected from the selected patients attending the colposcopy clinic of Bangabandhu Sheikh Mujib Medical University (BSMMU) from July 2011 to April 2013. Biopsy was done on colposcopically positive cases and histopathology reports were obtained. Thus 99 histologically proven patients of cervical cancer and precancerous lesion were selected. Pap smear was carried out on these 99 patients. HPV-DNA test (Hybrid Capture-2 assay) was carried out on the same samples and viral loads were estimated. Results: Among the 99 cases, 28(28.28%) cases were positive with Hybrid Capture-2(HC-2) assay. Out of 60 cases of CIN-I (Cervical Intraepithelial Neoplasia-1), 7(11.7%) cases were positive with HC-2. Among others, 3(15.0%) cases of CIN-II were positive with HC-2. The viral load was very high in invasive SCC (squamous cell carcinoma) cases in contrast to other categories of histological and cytological diagnosis. Introduction: Manual small incision cataract surgery (MSICS) is a cost-effective alternative to phacoemulsification cataract surgery for developing countries. This prospective study was carried out in Combined Military Hospital (CMH), Chittagong from October 2009 to March 2011 on 75 cataract patients who were operated by MSICS technique. Objectives: Aim of this study was to assess the visual outcome and complications of MSICS in a peripheral CMH. Methods: Seventy five cataract patients were operated by MSICS technique. All surgical procedures were performed by the principal author. Major per-operative and postoperative complications were documented. Visual outcome was assessed by Snellen’s visual acuity test 06 weeks after operation. Results: Uncorrected visual acuity (UCVA) was 6/6 – 6/18 in 57 (76.0%) patients, < 6/18 – 6/60 in 15 (20%) and < 6/60 in 03 (4.0%) patients. Best corrected visual acuity (BCVA) was 6/6-6/18 in 65 (86.7%) patients, < 6/18-6/60 in 07 (9.3%) and < 6/60 in 03(4.0%) patients. Visual outcome was good in 86.7% of patients according to World Health Orgnization (WHO) criteria and was not far away from the WHO expected outcome. Posterior capsule rupture was the most significant per-operative complication which was found in 7(9.3%) cases and surgically induced astigmatism was main postoperative complication that affected visual outcome. A general method of constructing layout with single factorial effect confounded in pn factorial experiments is proposed. Helicobacter pylori (H. pylori) infection is a common and significant public health problem. About 70-90% of the population in developing countries and 25-50% in developed countries were infected with H. pylori. Many studies have been undertaken concerning the role of H. pylori in the etiopathogenesis of chronic gastritis and peptic ulcer, as well as its role in the development of stomach malignancies. The objective of the present study was to find out the correlation between helicobacter pylori and gastric diseases among the Saudi population. This retrospective study was conducted at King Fahad Hospital at Al-Baha of Saudi Arabia between 1st January and 31st December 2004. Patients presented with gastrointestinal symptoms attending at hospital were included in the study. Gastric biopsy specimens were obtained from 187 patients with different type of gastroduodenal diseases. Histopathological study of gastric biopsy was carried out in the laboratory of the same hospital. Grades of H. pylori density in specimens in the present study was classified according to Sydney classification. Total 187 patients were included in the present study. There age range was 06-98 years and 108 were male and 79 were female. Maximum 30.5% patients were belonged to above 60 years age group. Out of all respondents maximum 86.6% patients had gastritis followed by 6.4% had normal histopathological finding, 3.7% had non specific findings and 3.2% had malignancy. Positive correlation was observed between severity of gastritis and grading of H. pylori load. (r=+0.382; p value0.001). The findings of the present study reveal that H. Pylori is responsible for many gastric diseases. It was also concluded from the study that severity of gastritis associated with H. Pylori increases with the heavy load of infection. To confirm the association prospective study with large sample size is recommended.

Leptin (meaning thin) is attracting the attention of many scientists of the world recently. It is an adipocyte-derived protein hormone discovered in 1994. Human leptin gene is located on chromosome 7. It is mainly expressed in adipose tissue but also in skeletal muscle, stomach, placenta and mammary gland. Leptin play key role in food intake, energy balance, and adiposity as well as in immune and endocrine system. It acts as feedback loop to maintain the constant store of body fat. Leptin acts as an antiobesity hormone raising the potential of its use as antiobesity drugs. In future, leptin or its analogue may offer noble therapeutic approach for obesity or other leptin related disorders. This review focuses on current knowledge of leptin biology and the role of leptin in various physiological and pathophysiological states.
