1. Modelling of Noise Pollution Due to Heterogeneous Highway Traffic in India

Abstract

Compared to homogeneous traffic flow, traffic speed variation is drastic with the involvement of heterogeneity. With an intent of studying the negative upshot of fluctuating speeds of heterogeneous traffic on the environment, the current paper is the outcome of the research done on various highways located in the states of Andhra Pradesh and Telangana in India, with an objective of developing a comprehensive noise prediction model by taking into account the traffic and roadway factors. Quantified noise levels [Leq (dBA) and L10 (dBA)] revealed that for the traffic speed variation of 10 to 95 kmph, the traffic noise levels were significantly affected by the variations in the proportion of the vehicle. On a specific note, the proposed model can be effectively used for the highway traffic noise prediction especially for the heterogeneous traffic, as the difference between the measured and predicted noise levels are within 1 to 10 dB (A).

Abstract

Whilst holding great promise for low noise, ease of operation and networking, useful photonic quantum computing has been precluded by the need for beyond-state-of-the-art components, manufactured by the millions. Here we introduce a manufacturable platform for quantum computing with photons. We benchmark a set of monolithically-integrated silicon photonics-based modules to generate, manipulate, network, and detect photonic qubits, demonstrating dual-rail photonic qubits with $99.98\% \pm 0.01\%$ state preparation and measurement fidelity, Hong-Ou-Mandel quantum interference between independent photon sources with $99.50\%\pm0.25\%$ visibility, two-qubit fusion with $99.22\%\pm0.12\%$ fidelity, and a chip-to-chip qubit interconnect with $99.72\%\pm0.04\%$ fidelity, not accounting for loss. In addition, we preview a selection of next generation technologies, demonstrating low-loss silicon nitride waveguides and components, fabrication-tolerant photon sources, high-efficiency photon-number-resolving detectors, low-loss chip-to-fiber coupling, and barium titanate electro-optic phase shifters., Comment: 8 pages, 5 figures

Abstract

Nitrogen-Vacancy (NV) centers in diamond have useful properties for detecting both AC and DC magnetic fields with high sensitivity at nano-scale resolution. Vector detection of AC magnetic fields can be achieved by using NV centers having three different orientations. Here, we propose a method to achieve this by using NV centers of single orientation. In this method, a static magnetic field is applied perpendicular to the NV axis, leading to strong mixing of the $m_{s}=-1$ and $1$ electron spin states. As a result, all three electron spin transitions of the triplet ground state have non-zero dipole moments, with each transition coupling to a single component of the magnetic field. This can be used to measure both strength and orientation of the applied AC field. To validate the technique, we perform a proof of principle experiment using a subset of ensemble NV centers in diamond, all having the same orientation., Comment: 7 pages, 5 figures

13. Effects of Non-Compulsory and Mandatory COVID-19 Interventions on Travel Distance and Time Away from Home: The Case of Norway in 2021

Abstract

Background: Due to the societal, economic, and health costs of COVID-19 non-pharmaceutical interventions (NPIs), it is important to assess their effects. Human mobility serves as a surrogate for human contacts and compliance to NPIs. In Nordic countries, NPIs have mostly been advised and sometimes made mandatory. It is unclear if making NPIs mandatory further reduced mobility. Aim: We investigated the effect of non-compulsory and follow-up mandatory measures in major cities and rural regions on human mobility in Norway. We identified NPI categories that most affected mobility. Methods: We used mobile phone mobility data from the largest Norwegian operator. We analysed non-compulsory and mandatory measures with before-after and synthetic difference-in-differences approaches. By regression, we investigated the impact of different NPIs on mobility. Results: Nationally and in less populated regions, follow-up mandatory measures further decreased time, but not distance travelled. In urban areas, however, follow-up mandates also decreased distance, and the effect exceeded that of initial non-compulsory measures. Stricter metre rules, gyms closing and reopening, restrictions on guests in homes, and face mask recommendations most impacted distance travelled. Time travelled was most affected by gyms closing and restaurants and shops reopening. Conclusion: Overall, non-compulsory measures appeared to decrease distance travelled from home, while mandates further decreased this metric in urban areas. Time travelled is reduced more by mandates than by non-compulsory measures for all regions and interventions. Abstract

Abstract Background Efficiently identifying patients with human immunodeficiency virus (HIV) using administrative health care data (e.g., claims) can facilitate research on their quality of care and health outcomes. No prior study has validated the use of only ICD-10-CM HIV diagnosis codes to identify patients with HIV. Methods We validated HIV diagnosis codes among women enrolled in a large U.S. integrated health care system during 2010–2020. We examined HIV diagnosis code-based algorithms that varied by type, frequency, and timing of the codes in patients’ claims data. We calculated the positive predictive values (PPVs) and 95% confidence intervals (CIs) of the algorithms using a medical record-confirmed diagnosis of HIV as the gold standard. Results A total of 272 women with ≥ 1 HIV diagnosis code in the administrative claims data were identified and medical records were reviewed for all 272 women. The PPV of an algorithm classifying women as having HIV as of the first HIV diagnosis code during the observation period was 80.5% (95% CI: 75.4–84.8%), and it was 93.9% (95% CI: 90.0-96.3%) as of the second. Little additional increase in PPV was observed when a third code was required. The PPV of an algorithm based on ICD-10-CM-era codes was similar to one based on ICD-9-CM-era codes. Conclusion If the accuracy measure of greatest interest is PPV, our findings suggest that use of ≥ 2 HIV diagnosis codes to identify patients with HIV may perform well. However, health care coding practices may vary across settings, which may impact generalizability of our results.

Abstract

A novel control scheme applicable to in-road inductive wireless charging pads in a dynamic system is presented. Primary inverter current sensing allows the primary pad to detect an approaching secondary pad. Zero-crossing detection of the same current allows the primary to synchronize to the secondary, functioning as a synchronous inverter with dual-active switching. Such primary synchronous inverters allow for independent primary pads. Necessary measures to ensure stable operation of the design are discussed. The control scheme only adds an analog filter to the bare-bones open-loop design. The design is validated through simulation and hardware experimentation, with a strong demonstration of robustness.

Abstract

This paper proposes a novel reflexive tuning dynamic inductive power transfer (DIPT) system with a single inverter connected to multiple transmitter coils, which makes DIPT systems simpler and more cost-effective. Traditional DIPT systems have an individual inverter per transmitter coil, leading to considerable costs when transmitter coils span long distances on roads. The proposed reflexive tuning DIPT system leverages a reflected reactance from a receiver coil to allow a single inverter to drive multiple transmitter coils. Using reflexive tuning, the approach naturally achieves high currents solely on the transmitter coil coupled with a receiver coil when a vehicle moves along a DIPT roadway. Simultaneously, the other uncoupled transmitter coils connected to the shared inverter operate at notably reduced currents. Theoretical analysis, design, simulation, and experimental results are provided for the proposed reflexive tuning circuits. Simulation results indicate that the proposed reflexive tuning circuit can reduce the uncoupled transmitter current by 37$\%$ compared to the conventional double-sided LCC tuning circuit while maintaining the output power at the same level. Experimental results with a 50$\;$kW prototype demonstrate operation on both automated rail and vehicle systems. The results demonstrate dc-dc efficiency of 90$\%$ per a single transmitter coil configuration with 223$\;$mm air gap and capability of power delivery at least up to 60 km/h speed.

Abstract

Power Density and cost are the key design criteria for the modern power electronics unit in Electric Vehicles (EVs). EVs widely adopt inductive wireless charging technology, which contains the receiving coil and the AC-DC converter on the vehicle side. However, the vertical space on the chassis for charging electronics is very limited. Moreover, with a non-compact package comes a high damaging probability due to unpredictable road conditions, higher costs, and vehicle integration issues. This research presents the concept of distributed planar magnetics with the integrated rectifier hardware design for the vehicle side charging electronics by combining the concept of planar-distributed magnetics with the design space optimization research. A planar-distributed inductor module consisting of 16 EI cores with PCB-embedded windings was designed, modeled (using a novel high-fidelity model), and fabricated using the design space optimization technique, which resulted in a 66% increase in the power density. This research treated and solved the power density problem as an optimization problem considering every constraint involved and presented a thorough investigation from the design space utilization, power loss, loss and frequency relation along with the magnetostatic and electrostatic analysis perspective. It also provides a comparative study of various materials for the inductor fabrication keeping in view the automotive application environment. Moreover, a hybrid approach (metalcore and FR4) PCB was used instead of a regular FR4 PCB after an in-depth study and performance comparison.

Abstract

Background: COVID-19 is primarily known as a respiratory illness; however, many patients present to hospital without respiratory symptoms. The association between non-respiratory presentations of COVID-19 and outcomes remains unclear. We investigated risk factors and clinical outcomes in patients with no respiratory symptoms (NRS) and respiratory symptoms (RS) at hospital admission. Methods: This study describes clinical features, physiological parameters, and outcomes of hospitalised COVID-19 patients, stratified by the presence or absence of respiratory symptoms at hospital admission. RS patients had one or more of: cough, shortness of breath, sore throat, runny nose or wheezing; while NRS patients did not. Results: Of 178,640 patients in the study, 86.4 % presented with RS, while 13.6 % had NRS. NRS patients were older (median age: NRS: 74 vs RS: 65) and less likely to be admitted to the ICU (NRS: 36.7 % vs RS: 37.5 %). NRS patients had a higher crude in-hospital case-fatality ratio (NRS 41.1 % vs. RS 32.0 %), but a lower risk of death after adjusting for confounders (HR 0.88 [0.83–0.93]). Conclusion: Approximately one in seven COVID-19 patients presented at hospital admission without respiratory symptoms. These patients were older, had lower ICU admission rates, and had a lower risk of in-hospital mortality after adjusting for confounders.

Abstract

IntroductionFrontotemporal dementia (FTD) encompasses a clinically and pathologically diverse group of neurodegenerative disorders, yet little work has quantified the unique phenotypic clinical presentations of FTD among post-9/11 era veterans. To identify phenotypes of FTD using natural language processing (NLP) aided medical chart reviews of post-9/11 era U.S. military Veterans diagnosed with FTD in Veterans Health Administration care.MethodsA medical record chart review of clinician/provider notes was conducted using a Natural Language Processing (NLP) tool, which extracted features related to cognitive dysfunction. NLP features were further organized into seven Research Domain Criteria Initiative (RDoC) domains, which were clustered to identify distinct phenotypes.ResultsVeterans with FTD were more likely to have notes that reflected the RDoC domains, with cognitive and positive valence domains showing the greatest difference across groups. Clustering of domains identified three symptom phenotypes agnostic to time of an individual having FTD, categorized as Low (16.4%), Moderate (69.2%), and High (14.5%) distress. Comparison across distress groups showed significant differences in physical and psychological characteristics, particularly prior history of head injury, insomnia, cardiac issues, anxiety, and alcohol misuse. The clustering result within the FTD group demonstrated a phenotype variant that exhibited a combination of language and behavioral symptoms. This phenotype presented with manifestations indicative of both language-related impairments and behavioral changes, showcasing the coexistence of features from both domains within the same individual.DiscussionThis study suggests FTD also presents across a continuum of severity and symptom distress, both within and across variants. The intensity of distress evident in clinical notes tends to cluster with more co-occurring conditions. This examination of phenotypic heterogeneity in clinical notes indicates that sensitivity to FTD diagnosis may be correlated to overall symptom distress, and future work incorporating NLP and phenotyping may help promote strategies for early detection of FTD.

Abstract

During the late 1980s, techniques were evolving to prevent acetabular component loosening. Inadequate acetabular bone stock further complicated this concern, which was traditionally addressed with cementation and bone grafting during this time period. However, one evolving tactic to address acetabular component loosening in the setting of inadequate acetabular bone stock was to augment bone graft with an acetabular reinforcement ring. In 1963, a 26-year-old, active male sustained a right-sided femoral neck fracture following a skiing accident. He ultimately developed a collapsed femoral head and varus deformity of the femoral neck requiring a total hip arthroplasty with a cemented monoblock femoral component and a polyethylene acetabular component cemented into a reinforcement ring. The initial procedure was performed in 1988, and this prosthesis is still functioning 35 years later and represents one of the longest follow-ups of a patient with a primary total hip arthroplasty with a reinforcement ring.

Abstract

Frequently changing cervical cancer screening guidelines over the past two decades have been inconsistently adopted in the United States. Current guidelines set the recommended screening interval to three years for average-risk women aged 21–29 years. Few studies have evaluated how patient and provider factors are associated with implementation of cervical cancer screening intervals among younger women. This study evaluated multilevel factors associated with screening interval length among 69,939 women aged 21–29 years with an initial negative Pap screen between 2010 and 2015 across three large health systems in the U.S. Shorter-interval screening was defined as a second screening Pap within 2.5 years of an initial negative Pap. Mixed-effects logistic regression was performed for each site to identify provider and patient characteristics associated with shorter-interval screening. The odds of shorter-interval screening decreased over the study period across all sites, though the proportion of patients screened within 2.5 years remained between 7.5% and 20.7% across sites in 2014–2015. Patient factors including insurance, race/ethnicity, and pregnancy were associated with shorter-interval screening, though the patterns differed across sites. Abstract

Abstract Background Potential care gaps in the cervical cancer screening process among women diagnosed with cervical cancer in an era with increased human papillomavirus (HPV) testing have not been extensively evaluated. Methods Women diagnosed with cervical cancer between ages 21 and 65 at four study sites between 2010 and 2014 were included. Screening histories were ascertained from 0.5 to 4 years prior to cervical cancer diagnosis. We identified potential care gaps in the screening history for each woman and classified them into one of three mutually exclusive types: lack of a screening test, screening test failure, and diagnostic/treatment care gap. Distributions of care gaps were tabulated by stage, histology, and study site. Multivariable nominal logistic regression was used to examine the associations between demographic and cancer characteristics and type of care gap. Results Of 499 women evaluated, 46% lacked a screening test in the time window examined, 31% experienced a screening test failure, and 22% experienced a diagnostic/treatment care gap. More than half of the women with advanced cancer and squamous cell carcinoma lacked a screening test compared to 31% and 24% of women with localized cancer and adenocarcinoma, respectively. Women aged 21–29 at diagnosis were more likely to experience screening test failure and diagnostic/treatment care gap, while those aged 50–65 were more likely to lack a screening test, compared to women aged 30–39. Conclusions Our findings demonstrate a continuing need to develop interventions targeting unscreened and under‐screened women and improve detection and diagnosis of adenocarcinoma in women undergoing cervical cancer screening and diagnostic follow‐up.

Abstract

WHO estimates show that 296 million people were living with chronic hepatitis B infection in 2019 with 1.5 million new infections occurring every year and approximately 290 000 people died from hepatitis C, mostly from cirrhosis and hepatocellular carcinoma. The prevalence and trends of Hepatitis B and Hepatitis C infections were affected during the pandemic, hence this study aimed to compare the difference in prevalence rates, trends, demographic data, and outcomes of Hepatitis B and Hepatitis C cases in pre-pandemic and pandemic era. The study was carried out in a 1060 bedded tertiary care teaching hospital located 90 kilometers away from Hyderabad, Telangana catering to a majorly rural population from around 200 villages. This study was a retrospective observational study where data of 4 years (March 2018 to Feb 2022) of patients whose samples were sent to Microbiology laboratory and were found to be positive for Hepatitis B surface antigen (HBsAg) or antibodies to Hepatitis C (Anti-HCV) were included. The medical records of Hepatitis B and Hepatitis C positive cases were analysed for demographic data like age, sex, address, requesting department, and present status retrieved from the hospital information system. The prevalence rates of Hepatitis B and Hepatitis C infections and trends every year were calculated and compared. Out of the total 39,578 samples tested for Hepatitis B surface antigen, 413 were positive with a seroprevalence of 1.04%. Among the 20,394 samples tested for anti-Hepatitis C antibodies, 53 samples were found to be positive showing a seroprevalence of 0.25%. There was a 23.63% decrease in the number of samples received during the pandemic period demonstrating the impact of COVID-19 on various laboratory testing. Male predominance was observed for both Hepatitis B (65.37%) and Hepatitis C (56.60%) positivity in this study. Hepatitis B was highest in the 61-80 years age group before the pandemic but during the pandemic, Hepatitis B positivity was equally distributed in the 41 to 60 years and 61-80 years age groups. Hepatitis C positive cases were equally distributed in the 41 to 60 years and 61-80 years age groups before the pandemic whereas during the pandemic Hepatitis C positivity was highest among the 41 to 60 years age group. Among the 413 positive cases of Hepatitis B, 315 (76.27%) cases belonged to the rural population and among the 53 Hepatitis C positive cases, 37 (69.81%) cases were from rural areas. The seroprevalence for Hepatitis B surface antigen displayed a decreasing trend in the pandemic era when compared to the pre-pandemic era. Seroprevalence for anti-HCV antibodies showed a small increase in the pandemic era when compared to the pre-pandemic era. Male predominance was observed for both Hepatitis B and Hepatitis C positivity in this study. Hepatitis B was highest in the 61-80 years age group before the pandemic but during the pandemic, Hepatitis B positivity was equally distributed in the 41 to 60 years and 61-80 years age groups. Abstract

BackgroundLarge language model (LLM)–based artificial intelligence chatbots direct the power of large training data sets toward successive, related tasks as opposed to single-ask tasks, for which artificial intelligence already achieves impressive performance. The capacity of LLMs to assist in the full scope of iterative clinical reasoning via successive prompting, in effect acting as artificial physicians, has not yet been evaluated. ObjectiveThis study aimed to evaluate ChatGPT’s capacity for ongoing clinical decision support via its performance on standardized clinical vignettes. MethodsWe inputted all 36 published clinical vignettes from the Merck Sharpe & Dohme (MSD) Clinical Manual into ChatGPT and compared its accuracy on differential diagnoses, diagnostic testing, final diagnosis, and management based on patient age, gender, and case acuity. Accuracy was measured by the proportion of correct responses to the questions posed within the clinical vignettes tested, as calculated by human scorers. We further conducted linear regression to assess the contributing factors toward ChatGPT’s performance on clinical tasks. ResultsChatGPT achieved an overall accuracy of 71.7% (95% CI 69.3%-74.1%) across all 36 clinical vignettes. The LLM demonstrated the highest performance in making a final diagnosis with an accuracy of 76.9% (95% CI 67.8%-86.1%) and the lowest performance in generating an initial differential diagnosis with an accuracy of 60.3% (95% CI 54.2%-66.6%). 45. Allograft reconstruction of olecranon after traumatic bone loss: a case report

Abstract

Despite 2.2 million bone allografts conducted annually, their complication rate remains high, with recipients incurring infection, fracture, instability, and failure to incorporate. Nonunion rates in massive bone allografts—a bone segment ≥5 cm in length that also contains the total circumference of replaced bone—have been documented as high as 50%. However, if early complication can be avoided, a 75% success rate at 20 years postoperatively has been reported. Nonmassive allografts may yield decreased nonunion rates, as massive bone allografts must overcome a greater metaphyseal to diaphyseal incorporation rate and osteoconduction may not ensue beyond the bone periphery. The patient in this case is a 23-year-old male demonstrating absent bone in the right olecranon process of the ulna without attachment of the triceps brachii after a motorbike accident. The patient underwent olecranon allograft reconstruction with triceps brachii tendon reattachment. Four and a half years after allograft reconstruction of the right olecranon, the patient presents with minimal symptoms. However, he reports occasional aching at the site of injury. His current active arc of sagittal motion was 20°-130°, and pronation-supination was 70°-80°. His triceps strength was 4/5 Medical Research Council grade. Radiographic evaluation revealed a well-incorporated graft with a recontoured olecranon tip. Overall, this report demonstrates that operations involving a nonmassive allograft about the olecranon process may display minimal side effects in comparison to massive allografts, specifically regarding nonunion. Furthermore, this operation allows for improved range of motion after bone loss, allowing the patient to partake in activities of daily living.
