Objective: To measure the effect of doxycycline on inflammatory marker [IL-6, TNF-α, ferritin and C reactive protein (CRP)] levels in patients with dengue infection. Methods: A single-centre, open-label, parallel-group randomized controlled trial was done in PGIMER Chandigarh from June 2021 to October 2022. Patients were randomized using a simple randomization process into two groups: the doxycycline treatment group (n=35) and the control group (n=34). Patients in the treatment group were given oral doxycycline 100 mg twice daily for five days along with standard treatment, whereas patients in the control group received only standard treatment. The objective was to measure the effect of doxycycline on inflammatory markers in dengue infection. Results: On comparative analysis at day 5, there was a statistically significant reduction in the median values of ferritin and CRP in cases compared to the control group (ferritin: P=0.006 and CRP: P=0.006). No significant reduction was noted in the levels of IL-6 and TNF-α. Conclusions: Doxycycline treatment led to a reduction of inflammatory markers in dengue infection. [ABSTRACT FROM AUTHOR]

Introduction: Vitamin D deficiency and anemia frequently coexist. Moreover, vitamin D deficiency is found to play a role in chronic kidney disease (CKD)-associated anemia. We investigated the effect of cholecalciferol on serum hepcidin levels in vitamin D--deficient, non-diabetic individuals with CKD in a randomized, double-blind, placebo-controlled trial. Methods: This study was performed on stored samples of our previously published randomized, double-blind, placebo-controlled trial of cholecalciferol supplementation in non-diabetic patients with stage III-IV CKD and vitamin D deficiency. Stable patients of either sex, aged 18-70 years, with non-diabetic stage III-IV CKD (estimated glomerular filtration rate between 15 and 60 ml/min/1.73 m²), and having serum 25-hydroxyvitamin D3 [25(OH) D] levels ≤20 ng/ml were included. Participants received either two directly observed oral doses of cholecalciferol (300,000 IU) or matching placebo at baseline and at eight weeks. Follow-up was done at 16 weeks. Serum hepcidin levels were analyzed at baseline and at 16 weeks. Results: A total of 120 CKD patients were enrolled. Serum 25(OH) D levels were similar in the placebo and cholecalciferol groups at baseline (13.21 ± 4.78 ng/ml and 13.40 ± 4.42 ng/ml; P = 0.88). After 16 weeks, the serum 25(OH) D levels were found to be increased in the cholecalciferol group but not in the placebo group (between-group difference in mean change 23.40 ng/ml; 95% CI: 19.76 to 27.06; P < 0.001). Serum hepcidin levels were similar at baseline (median [IQR]: 33.6 [8.6-77.8] ng/ml vs. 24.6 [9.3-70.7] ng/ml, P = 0.903) and did not vary between groups at 16 weeks (median [IQR]: 41.5 [10.9-75.0] ng/ml vs. 34.8 [12.3-63.75] ng/ml, P = 0.703). Conclusion: Our study provides preliminary data based on which a larger adequately powered clinical trial can be conducted to conclusively assess the impact of vitamin D supplementation on hepcidin levels and anemia in patients with CKD and vitamin D deficiency. Background Chronic kidney disease (CKD) is an important cause of morbidity and mortality worldwide. There is a lack of information on epidemiology and progression of CKD in low–middle income countries. The Indian Chronic Kidney Disease (ICKD) study aims to identify factors that associate with CKD progression, and development of kidney failure and cardiovascular disease (CVD) in Indian patients with CKD. Methods ICKD study is prospective, multicentric cohort study enrolling patients with estimated glomerular filtration rate (eGFR) 15–60 mL/min/1.73 m2, or >60 mL/min/1.73 m2 with proteinuria. Clinical details and biological samples are collected at annual visits. We analysed the baseline characteristics including socio-demographic details, risk factors, disease characteristics and laboratory measurements. In addition, we compared characteristics between urban and rural participants. Results A total of 4056 patients have been enrolled up to 31 March 2020. The mean ± SD age was 50.3 ± 11.8 years, 67.2% were males, two-thirds of patients lived in rural areas and the median eGFR was 40 mL/min/1.73 m2. About 87% were hypertensive, 37% had diabetes, 22% had CVD, 6.7% had past history of acute kidney injury and 23% reported prior use of alternative drugs. Diabetic kidney disease, chronic interstitial nephritis (CIN) and CKD-cause unknown (CKDu) were the leading causes. Rural participants had more occupational exposure and tobacco use but lower educational status and income. CIN and unknown categories were leading causes in rural participants. Conclusions The ICKD study is the only large cohort study of patients with mild-to-moderate CKD in a lower middle income country. eDNA for birds identification is a non-invasive technique that inflicts no damage to the species or habitats under study. It is relatively new data collection approach and opens new ways by which free ranging animals can be studied. The present study aims at the use of non-invasive sampling techniques based on collection of molted or dropped feathers as a tool for identification of birds. For this purpose DNA was isolated and quantified from molted feathers. Since, 12S rRNA has been identified as a reliable molecular tool for genetic species identification, therefore, partial 12S rRNA sequences were amplified from the eDNA of feathers using universal primers. The PCR amplified DNA samples were sent for Sanger sequencing. Phylogenetic trees were constructed on the basis of bioinformatic analysis. This study, therefore, endorses the use of eDNA isolated from feathers as a useful non-invasive tool for identification of birds which can play a pivotal role in avian research and conservation.
