Hybrid Emergency Room Systems (HERS) have emerged as a promising solution to address the challenges of timely and effective trauma management and acute care interventions. This review provides an in-depth exploration of HERS, its definition, historical development, advantages, disadvantages, successful implementations, economic considerations, and its potential role in the future of emergency care. HERS is designed to facilitate rapid assessment, diagnosis, and interventions by integrating various medical specialties, advanced imaging technologies, and interventional capabilities within the emergency care environment. Through comprehensive literature review and case studies, this article demonstrates how HERS has been utilized to improve patient outcomes in cases of trauma, cardiovascular emergencies, and other critical conditions. The advantages of HERS include enhanced coordination among medical specialties, streamlined communication, real-time imaging capabilities, and optimized resource utilization. However, challenges such as high costs, complexity, and accessibility limitations need to be considered. This review also discusses the economic implications of implementing HERS and presents a model-based cost-benefit analysis, underscores the importance of carefully planning and evaluating the implementation of HERS, considering its potential benefits and challenges. Objective: To evaluate the situation concerning oral and poster presentations given at Turkish and International Scientific Medical Congresses and adding to scientific knowledge before and after the change in associate professorship eligibility criteria that occurred in December 2016. Methods: Poster and oral presentations at four consecutive emergency medicine congresses held between 2015 and 2018 by the Emergency Medicine Physicians Association of Turkey were included in the study. As data were collected from presentation booklets accessed online, ethics committee approval was not obtained. Since the associate professorship eligibility criteria changed in December 2016, the last two congresses before that date and the following two after they were compared. Numbers of authors, domestic and international participation rates, the departments involved, and the presence or absence of oral or poster presentations were assessed. Results: Members of emergency medicine departments contributed to congresses with significantly more case reports than other participants (p<0.001). Presentations at congresses held before the change in associate professorship criteria in December 2016 involved larger numbers of authors, while those appearing after the change involved fewer names (p<0.001). Participation involved more poster presentations before the change in criteria, while the proportion of oral presentations increased after the change (p<0.001). Participation in congresses prior to the change in criteria more frequently involved case reports, while the proportion of original articles increased thereafter (p<0.001). Conclusion: Presentations at congresses held prior to the change in associate professorship criteria involved a large number of author names, while fewer author names were observed following the change in criteria. Multiple sclerosis (MS) is the most prevalent immune-mediated inflammatory demyelinating central nervous system disorder, with a diverse set of clinical signs and symptoms. This study aimed to investigate the diagnostic values of the monocyte/lymphocyte ratio (MLR), red cell distribution width/lymphocyte ratio (RLR), and systemic immune-inflammation index (SII) in detecting multiple sclerosis attacks in patients with Relapsing-remitting MS (RRMS) presenting to the emergency department (ED). This retrospective observational study was conducted among patients with RRMS presenting to the ED of a third-level hospital. The laboratory parameters of 165 patients were compared during the attack and non-attack periods. The paired t-test statistic was used to compare means of inflammatory biomarker measurements between attack and non-attack groups. The neutrophil/lymphocyte ratio (NLR), MLR, RLR, and SII mean of the patients in the MS attack periods were higher than those in the non-attack period. The mean difference of NLR, MLR, RLR, and SII between both groups was 5.40±7.25, 0.37±0.43, 7.77±11.61, 1469.19±1978.88, respectively (p<0.001). In ROC analysis, NLR, RLR, MLR, and SII had excellent diagnostic power in detecting MS relapse (AUC: 0.87, 0.81, 0.86, and 0.87, respectively).According to our findings, SII, MLR, NLR, and RLR may be beneficial in confirming the diagnosis of attack in patients with RRMS. OBJECTIVES: Limited clinical studies have investigated the effects of synthetic cannabinoids (SCs) on the cardiovascular system (CVS). The aim of this study was to evaluate the effects of SCs on the CVS. METHODS: The patient group of this single-center, prospective, case-control study consisted of adult patients presenting to the emergency department (ED) with symptoms of SC use. Vital signs and electrocardiogram (ECG) after use of SC of patients were followed. A control group with a similar number of patients and patient demographics were formed following the patient admission process. Pulse rate, arterial blood pressure (ABP), and ECG of patient and control groups were compared using Mann-Whitney U and Chi-squared tests. RESULTS: A total of 148 people were included in the study, 74 in the patient group and 74 in the control group. Systolic and diastolic ABPs of patient group were statistically significantly lower than those of the control group (P < 0.001). P-wave width and amplitude in the patient group were significantly higher compared to the control group (P 0.027 and P. 0.004, respectively). QRS width on patient group ECGs was significantly higher than in the control group, while T-wave amplitude was significantly lower (P 0.045 and P < 0.001, respectively). ST elevation was seen in 12 (16.2%) subjects in the patient group, while no ST elevation was seen in the control group (P < 0.001). CONCLUSION: SCs can reduce systemic tension and SCs may cause changes in ECG, especially P wave, ST segment, T wave, and QRS. Further large-scale studies are needed to show whether these changes are associated with fatal arrhythmias or myocardial infarction. [ABSTRACT FROM AUTHOR]

Objective: It is known that coronavirus disease-2019 (COVID-19) can manifest with neurologic findings in addition to respiratory symptoms. This study aimed to investigate the rate of patients with COVID-19 who presented with a first epileptic seizure as a neurologic finding. Methods: The study included patients who presented to our hospital's emergency department reporting a seizure between March 11 and May 30, 2020. Of 90 patients, 32 (35.5%) presented first seizures. The patients' demographic characteristics, medical history, complications, the presence of risk factors relating to their treatment and seizures, laboratory findings, clinical properties, and imaging findings were recorded. Results: Of the patients with first seizures, 19 were men, and 13 were women. Their mean age was 49.5 (minimum: 18, maximum: 89) years. The percentage of patients with COVID-19 who presented with a first seizure was 31.2% (n=10). A risk factor was identified in 8 (80%) patients who presented with a first seizure, and no risk factor was found in 2 (20%) patients. In all patients, COVID-19 was diagnosed following a seizure presentation. The intensive care requirement rate was 30% (n=3), and 2 (20%) patients died. Conclusion: The rate of COVID-19 among patients presenting with a first seizure is high. It would be appropriate to consider patients arriving with a seizure without any other respiratory or systemic issues as being related to COVID-19 and plan the necessary analyses and treatment. Background: SARS-CoV-2 has caused an outbreak all over the World. Age is the most important factor for mortality. However, it is not known exactly why SARS-CoV-2 infections are more severe and fatal in the elderly population. We examined the clinical course and the causes of increasing mortality in all hospitalized patients diagnosed with COVID-19 over 65 years of age. Methods: Hospitalized elderly patients diagnosed with COVID-19 were examined in this retrospective observational study. The blood results, length of stay, comorbid diseases, admission symptoms, clinical results and demographic data of the patients were recorded. It was examined whether there was a significant difference between surviving and non-surviving patients in terms of comorbid diseases and symptoms. The effects of these parameters on the 30-day mortality alone were investigated. Results: A total of 263 patients (125 males) were included in the study. Cough (53.2%) followed by dyspnea (35.7%) were the two most common symptoms. There was no statistically significant difference age or sex distribution between survivor and nonsurvivor patients. Patients with dyspnea had a significantly lower survival rate compared to patients who did not have dyspnea at presentation and patients who have chronic obstructive pulmonary disease and cerebrovascular disease were associated with a significantly increased risk of mortality Conclusions: It has been shown that there is a significant increase in the risk of mortality in COVID-19 patients with chronic obstructive pulmonary disease and cerebrovascular diseases. Objective: Ayahuasca tea contains N, N-dimethyltryptamine (DMT) found in the leaves of the Psychotria viridis vine and |3-carbolines found in Banisteriopsiscaapi. Case Report: A 59-year-old man was brought to the emergency department due to hallucinations, agitated behavior, aggression, nausea and vomiting developing 1.5 h after drinking approximately one tea glassful of Ayahuasca tea. Hypertension and mydriasis were determined, while blood, EKG and chest x-ray were normal. Symptomatic treatment was administered, and the patient was discharged following 24-h clinical observation in the emergency department. Discussion: It has been suggested that DMT exhibits anxiolytic effects in low doses, but that hallucinogenic effects emerge when taken in large doses. In order to be effective when taken orally, DMT must be used with a monoamine oxidase inhibitor. Our case exhibited hallucinations and the majority of the autonomic symptoms associated with DMT use described in the literature. Ayahuasca tea is toxic. Uncontrolled use may even result in mortality. Conclusion: Emergency physicians should consider Ayahuasca tea ingestion in patients presenting to the emergency department with agitation, hypertension, mydriasis and, particularly, hallucination. Introduction: Patients presenting at the emergency department with a range of complaints are likely to have a diagnosis of more than one disease. These patients with more than one system involvement, who will receive a diagnosis of concomitant diseases, cause various difficulties for emergency department physicians. Case Report: The case is presented here of a 28-year old male who presented at the emergency department with clinical findings of Guillan-Barre Syndrome, meningitis and spinal astrocytoma following an upper respiratory tract infection. Conclusion: This paper aimed to discuss the diagnoses, follow-up and treatment of this rare disease patient who has presented with clinical findings of GBS and who was then diagnosed of concomitant GBS, meningitis and spinal astrocytoma. Introduction: Leukaemias are a group of heterogenous neoplastic diseases resulting from malignant transformation of hematopoietic cells. Acute leukaemias are the group of diseases characterised by the infiltration of immature myeloid or lymphoid series cells into the blood, bone marrow or other tissues. In acute leukaemias, joint pain is one of the rare causes of presentation at the emergency services. In this report, we addressed the importance of haemogram evaluation by the emergency department physicians for patients with joint pain. Case Report: An 18-year old male presented at the emergency department with complaints of pain in the left wrist which had been ongoing for one week. The patient had no history of trauma and direct graphics was normal. Blood test results were determined as leukocytes 41900/mm³. Conclusion: It is important to look at haemogram results of patients presenting at the emergency department with complaints of simple joint pain. As the basis for evaluation of leukaemia and similar haematological diseases, a haemogram is a simple test which can be applied in every emergency department. [ABSTRACT FROM AUTHOR]

Ectodermal dysplasia is a rare disease that present with: hypotrichosis, hypodontia, and typically absence of eccrine sweating. It consists of two basic forms; anhidrotic and hidrotic. In anhidrotic ectodermal dysplasia, the body's thermoregulatory mechanism is impaired due to lack of sweat glands. These patients may therefore present to the emergency department with hyperthermia. This report describes a patient with anhidrotic ectodermal dysplasia brought to the emergency department in an unconscious state and with a fever of 41°C. This rare disease should be kept in mind in patients presented to ED with an unconscious state due to hyperthermia. Typical clinical appearance is a useful clue for the diagnosis. Introduction: The nutraceutical value and ingredients of honey make it attractive for complementary and alternative medicine. Herein, we report a case of mad honey intoxication in an infant following the ingestion of honey for an intractable cough. Case Report: A 5-month-old male infant was given two teaspoons of honey by his mother with his breakfast for coughing for the past 2 days. Four hours later following ingestion, he started to vomit and became drowsy. On arrival at the emergency department, he was drowsy and his Glasgow Coma Scale score was 10. With respect to systemic examination, abdominal sonography, performed to exclude any intraabdominal and renal pathology, revealed no pathology. Urine examination revealed proteins and hemoglobin. He was not given any other treatment except fluid replacement. At the fifth hour of arrival, his symptoms and physical examination findings all resolved. He was thought to have honey poisoning and was admitted to the pediatric clinic for follow-up. Conclusion: Mad honey is a natural product, but because of its ability to cause allergic reactions in susceptible people, honey should not be given to infants younger than 1 year. Intoxication should be considered in emergency department patients in the eastern Black Sea region of Turkey. Although not clearly established, there might be nephrotoxic effects in human beings. Osgood-Schlatter disease is the traction apophysitis of tuberositas tibia and the most common cause of knee pain in adolescents. Despite the frequent discomfort, sequelae are rarely observed. In this paper, sequelae of Osgood-Schlatter disease, presenting with knee pain in the emergency room were submitted. Rarely encountered in the emergency department in adult patients presenting with knee pain with sequelae of Osgood-Schlatter disease, the need to be aware of the diagnosis, clinical and radiological features and differential diagnosis are discussed. The article presents a case study of a 24-year-old man who was suffering from left wrist pain and weakness of grip. Data reveals that swelling and tenderness over the dorsal region of the wrist was found on physical examination but the neurovascular examination was normal. After the radiography, trans-scaphoid perilunate fracture-dislocations (TSPLFD), a wrist injury, was diagnosed.
