Amaç: Bu çalışmanın amacı spor tırmanış yarışmalarında, yarışma performansı ile ilişkili olabilecek parametreleri belirlemektir. Gereç ve yöntem: Araştırmanın evreni, Türkiye Dağcılık Federayonunun (TDF) düzenlediği spor tırmanış yarışmasına katılan sporculardan, örneklem grubu ise bu yarışmaya katılan rastgele seçilen 31 erkek ve 21 kadın toplam 52 gönüllü katılımcıdan oluşmaktadır. Spor tırmanış yarışması “Lider Tırmanış” disiplininde yapılmış olup puanlama sistemi en fazla tutamak tutana göre belirlenmiştir. Tırmanıştan önce katılımcılardan üst ekstremite kuvvetini ölçmek için parmak (palmar, parmak ucu ve anahtar tutuş), el kavrama, sırt kuvveti ve sağlık topu atma, anaerobik gücü ölçmek için ise dikey sıçrama testi (DST) ölçümleri alınmıştır. Tırmanış faaliyeti bitince yarışma sonuçları toplanarak, alınan ölçümler ile puanlama arasında ilişkiye bakılmıştır. Çalışmada elde edilen verilerin analiz, SPSS (ver.23.0) programı aracılığı ile gerçekleştirilmiştir. Ölçüm alınan parametrelerin normal dağılım durumunu gösteren basıklık çarpıklık değerleri Skewness Kurtosis testi ile ölçülmüştür. Elde edilen değerlerin normal dağılıma sahip olduğu görülmüştür. Giriş ve Amaç: Bu çalışmanın amacı, nedeni bilinmeyen kanamalarda çift balon enteroskopi için en uygun giriş yönünü belirlemek ve saptanan ince barsak patolojilerin özelliklerini değerlendirmektir. Gereç ve Yöntem: 2006-2009 yılları arasında nedeni bilinmeyen kanama ile kliniğimize başvuran 75 hasta çalışmaya dahil edilmiştir. Çift balon enteroskopi ile ilgili veriler prospektif olarak değerlendirilmiştir. Çift balon enteroskopi, 60 hastada oral, 5 hastada anal ve 10 hastada her iki yönden yapılmıştır. Hastaların ortalama yaşı 50.8 yıl ve %57.3ü erkek idi. Total giriş uzunluğu, total işlem süresi, tanı oranları, lezyonların anatomik lokalizasyonları ve son tanılar değerlendirilmiştir. Bulgular: Hastaların %75inde tanı konulmuştur. Tanı oranı, aşikar kanaması olanlarda anlamlı olarak yüksekti (%91.7). Kanama nedeni 19 hastada saptanamadı. Ortalama işlem süresi, oral, anal ve her iki yön için sırasıyla; 119, 144 ve 154 dk idi. Ortalama giriş uzunluğu oral yol ile 310.65 cm (pilordan itibaren) ve anal yol ile 166.8 cm. (ileoçekal valvden itibaren) idi. En sık rastlanan patoloji vasküler malformasyonlar (n20) ve tümörler (n19) idi. Malign lezyonları n hepsi proksimal ince bağırsak yerleşimliydi. Vasküler malformasyon tüm ince bağırsakta eşit dağılmakta idi. Hastaların %30una endoskopik tedavi uygulandı. Sonuç: Çift balon enteroskopi, nedeni bilinmeyen kanamalarda güvenli uygulanabilir tanı sal ve tedavi edici bir işlemdir. Lezyonların çoğu proksimal ince bağırsak yerleşimlidir. Kapsül endoskopi dahil görüntüleme metodları nın lezyon saptayamadığı durumlarda oral yol ilk tercih olabilir. Background/aims: This study was performed to clarify the best insertion route of double-balloon enteroscopy and to report the characteristics and proportions of small bowel pathologies detected by double-balloon enteroscopy in our patients with obscure gastrointestinal bleeding. Materials and Methods: Between January 2006 and December 2009, 75 patients with obscure gastrointestinal bleeding were enrolled into this study. The procedure was performed by oral route in 60 patients, anal route in 5 patients and both in 10 patients. Mean age of the patients was 50.8 years, and 57.3% of them were male. The main outcome measurements were total length of insertion, total time of double-balloon enteroscopy, diagnostic rates, anatomic location of the lesions, and final diagnosis of lesions detected. Results: Double-balloon enteroscopy was diagnostic in 75% of the patients. This rate was significantly higher in overt bleeding (91.7%). The source of bleeding could not be detected in 19 patients. Mean times of procedures were 119, 144 and 154 minutes for oral route, anal route and both, respectively. The mean insertion length was 310.65 cm (beyond the pylorus) for oral and 166.8 cm (beyond the ileocecal valve) for anal route. The most frequent pathologies were vascular malformations (n20) and tumors (n19). All malignant lesions were detected in the proximal part of the small intestine. Vascular malformations were distributed equally through the small intestine. Endoscopic treatment was performed in 30% of patients. Conclusions: Double-balloon enteroscopy is a safe and feasible examination for obscure gastrointestinal bleeding. Most lesions were localized in the proximal part of the small intestine. The oral route may be preferred as a first choice, if the imaging modalities including capsule endoscopy cannot detect the lesion.

Introduction. Schwannomas are quiet rare in the retroperitoneal region. Here, we describe an incidentally detected retroperitoneal schwannoma in the abdominal computerized tomography (CT) of a patient with acute appendicitis. Case Presentation. A 38-year-old woman was admitted to the emergency service with the complaints of progressive abdominal pain and nausea for the last 24 hours. Abdominal examination was compatible with acute abdomen. Acute appendicitis was diagnosed by CT. During CT evaluation, a round shaped soft-tissue mass at the retroperitoneal area inferior to the right kidney was detected, The mass was resected and histology revealed schwannoma. Conclusion. Rare tumoral lesions with benign course such as schwannoma can be detected incidentally.

Nozokomiyal santral sinir sistemi enfeksiyonları, tüm nozokomiyal enfeksiyonların %0.4'ünden sorumludur. Nozokomiyal menenjit etkenleri toplum kökenli menenjit etkenlerinden farklı olup, morbidite ve mortalite oranları oldukça yüksektir. Prognozu etkileyen en önemli faktör ise etkene yönelik doğru antibakteriyel tedavi seçimidir. Bu raporda, lomber disk hernisi operasyonu sonrasında, Klebsiella pneumoniae ve Acinetobacter spp.'nin birlikte etken olduğu nozokomiyal menenjit gelişen 64 yaşında bir kadın hasta sunulmaktadır. Olgunun üç kez nöroşirürjik operasyon geçirmesi ve uzun süreli (29 gün) eksternal ventriküler drenaj (EVD) kateteri olması, risk faktörleri olarak değerlendirilmiştir. Olguya, postoperatif beşinci günde klinik ve laboratuvar bulgularına dayanılarak nozokomiyal menenjit tanısı konulmuş ve ampirik olarak meropenem (3 x 2 g, IV) ve vankomisin (2 x 1 g, IV) tedavisi verilmiştir. Aynı gün alınan beyin omurilik sıvısı (BOS) ve kan kültürlerinde, genişlemiş spektrumlu beta-laktamaz (GSBL) üreten K. pneumoniae (amikasin, imipenem, meropenem, sefoksitin, siprofloksasin, piperasilin-tazobaktam ve trimetoprim/sülfametoksazole duyarlı) üremiştir. Meropenem tedavisinin sekizinci gününde hastanın kliniğinde bir değişiklik olmamış; BOS incelemesinde yüksek lökosit (1300/mm3) ve gram-negatif basil varlığı saptanmış, BOS kültüründe de GSBL pozitif K. pneumoniae (antibiyotik duyarlılığı bir önceki suş ile aynı) üremesi devam etmiştir. Buruın üzerine tedaviye siprofloksasin (3 x 400 mg, IV) eklenmiş ve EVD kateteri değiştirilmiştir. Meropenem tedavisinin on üçüncü, siprofloksasin tedavisinin beşinci gününde, BOS ve kan kültüründe GSBL pozitif K. pneumoniae (antibiyotik duyarlılığı bir önceki suş ile aynı) ve Acinetobacter spp. (gentamisin, tobramisin, netilmisin, siprofloksasin, levofloksasin ve sefepime duyarlı) üremesi nedeniyle tedaviye intraventriküler gentamisin (15 mg/gün) ve IV gentamisin (3 x 120 mg) eklenmiştir. Olgu üç haftalık intraventriküler antibiyotik tedavisi sonunda primer hastalığının sekelleri dışında yeni bir sekel gelişmeksizin iyileşmiştir. Bu olgu, GSBL pozitif K. pneumoniae ve Acinetobacter spp.'nin birlikte etken olduğu, yerli ve yabancı literatür taramalarında ulaşabildiğimiz kadarıyla ilk nozokomiyal menenjit olgusu olması nedeniyle sunulmuştur., Nosocomial central nervous system infections constitute 0.4% of all nosocomial infections. The responsible pathogens of nosocomial meningitis are quite different from community-acquired meningitis with high rates of morbidity and mortality. The most important prognostic factor is the appropriate choice of pathogen-specific antibacterial therapy. In this report, a 64 years old woman with nosocomial meningitis caused by Klebsiella pneumoniae and Acinetobacter spp. after lumbar disc hernia operation, has been presented. The risk factors were detected as recent history of neurosurgical operation for three times and long term (29 days) use of external ventricular drainaige (EVD) catheter. Empirical meropenem (3 x 2 g, IV) and vancomycin (2x1 g, IV) therapy was initiated upon the diagnosis of nosocomial meningitis based on the clinical and laboratory findings on the postoperative fifth day. Extended-spectrum beta-lactamase (ESBL) producing K. pneumoniae (susceptible to amikacin, imipenem, meropenem, cefoxitine, ciprofloxacin, piperasillin-tazobactam and trimethoprim/sulfamethoxazole) was recovered from cerebrospinal fluid (CSF) and blood samples obtained on the same day. There was no change in the status of the patient on the eighth day of meropenem therapy, with high leukocyte number (1300/mm3) and presence of gram-negative bacilli in CSF, and ESBL positive K. pneumoniae (antibiotic susceptibility pattern same with the previous isolate) growth in CSF culture. Thereupon intravenous ciprofloxacin (3 x 400 mg) was added to the therapy and her EVD has been changed. However, ESBL positive K. pneumoniae (antibiotic susceptibility pattern same with the previous isolate) together with Acinetobacter spp. (susceptible to gentamycin, tobramycin, netilmicin, ciprofloxacin, levofloxacin and cefepime) were isolated from CSF and blood cultures obtained on the 13th day of meropenem and fifth day of ciprofloxacin therapy. Therefore intraventricular and intravenous gentamicin (15 mg/days and 3 x 120 mg, respectively) were added to the therapy. The patient recovered at the end of three weeks treatment without any additional sequela other than her primary illness. This case was the first case of nosocomial meningitis due to ESBL positive K. pneumoniae together with Acinetobacter spp. in the available literature.

Lyme hastalığı Borrelia burgdorferi enfeksiyonunun neden olduğu infiamatuar bir hastalıktır. Bu vaka sunumunda, intrauterin ola¬rak Lyme hastalığını geçirip yaşayan ve daha sonra triventriküler hidrosefali ve aquaductus serebri stenozu geliştiren nadir bir ol¬gu sunulmuştur. Daha önce yayınlanmış olan vakalarda modern görüntüleme teknikleri kullanılmamıştı. Bizim hastamızda, intra¬uterin manyetik rezonans görüntüleme tekniği hidrosefalinin tanısında ve tedavinin planlanmasında yardımcı olmuştur., Lyme disease is an inflammatory disorder caused by infection with Borrelia burgdorferi. In this report a rare case of a girl sur¬viving intrauterine Lyme disease, who subsequently developed triventricular hydrocephalus and aquaductus cerebri stenosis was presented. In earlier cases modern imaging techniques have not been used. In our patient, intrauterine magnetic reso¬nance imaging technique has been helpful in the diagnosis and management of the therapy. The presence of aneurysm remnant after incomplete or unsuccessful surgical clipping is associated with persistent risk of regrowth and rupture, and additional treatment is generally recommended. Advances in aneurysm coiling techniques and technology have allowed for more remnants to be treated safely. We present a 47-year-old woman with residual internal carotid artery aneurysm after surgical clipping with coils and balloon remodeling technique.

AMAÇ Lomber disk hastalığı tanısında, konvansiyonel MRG ile karşılaştırarak üç boyutlu MR myelografinin (3B-MRM) yerini değerlendirmek. GEREÇ VE YÖNTEM Lomber disk herniasyonu semptomları olan ardışık 50 hasta çalışmaya dahil edildi. 1.5 T MR sistemde konvansiyonel MRG ve koronal 3B-MRM uygulandı. Konvansiyonel MRG ve 3B-MRM ile her bir disk düzeyinde disk herniasyonu ve sinir kökü basısı varlığı ve lokalizasyonu değerlendirildi. Çok seviyede disk herniasyonu ve dar lomber spinal kanal saptanan hastalarda en önemli disk düzeyini belirlemede 3B-MRM’nin tanıya ek katkısı not alındı. BULGULAR Konvansiyonel MRG ile 60, 3B-MRM ile 54 seviyede disk herniasyonu saptandı. 3B-MRM’nin disk herniasyonu saptamadaki duyarlığı %90, özgüllüğü %100 ve doğruluğu %96 idi. Konvansiyonel MRG ile 91, 3B-MRM ile 98 sinir kökü basısı saptandı. 3BMRM’nin sinir kökü basısını saptamada duyarlılığı %100, özgüllüğü %97 ve doğruluğu %98 idi. Çok seviyede disk herniasyonu ve dar lomber spinal kanal saptanan 13 hastanın hepsinde 3B-MRM ek katkı sağladı. SONUÇ 3B-MRM’nin çekim süresi kısadır. Kolayca rutin MRG’ye eklenebilir. Lomber disk hastalığı tanısında değerli bir yöntem olabilir. Yalancı-negatif ve yalancı-pozitif sonuçların varlığı 3B-MRM görüntülerini yorumlarken dikkatli olmamız gerektiğini göstermektedir. Bununla birlikte, 3B-MRM özellikle çok seviyede disk herniasyonu veya dar lomber spinal kanal saptadığımız olgularda semptomlara sebep olan en önemli disk düzeyini hızla ve kolayca belirlememizi sağlayarak yararlı olabilir., PURPOSE: To evaluate the role of the three dimensional (3D) MR myelography (MRM) in the diagnosis of the lumbar discogenic disease compared with conventional MR imaging. MATERIALS AND METHODS: Fifty consecutive patients with clinical symptoms of lumbar disc herniation were enrolled for the study. Conventional MR imaging and coronal 3D MRM were performed on a 1.5 T MR system. At each disc space level, the presence and the location of disc herniation and nerve root compression were evaluated. Note was also made if MRM had additional contribution to the determination of most significant level in the cases of multilevel disc disease and lumbar spinal stenosis. RESULTS: Disc herniation was found in 60 disc space levels on conventional MR imaging and in 54 disc space levels on 3D-MRM imaging. As regards to disc herniation, the sensitivity, specificity, and accuracy of 3D-MRM was 90%, 100%, and 96%, respectively. Nerve root compression was seen in 91 levels on conventional MR images, as opposed to 98 on 3D-MRM. Regarding nerve root compression, the sensitivity, specificity, and accuracy of 3D-MRM was 100%, 97%, and 98%, respectively. In all of 13 patients with multilevel disc disease and lumbar spinal stenosis, 3D-MRM had additional contribution. CONCLUSION: Scan time of 3D-MRM is short. It can be easily added to routine lumbar MR imaging. It may be a valuable modality in the diagnosis of the lumbar discogenic disease. The presence of false-negative and false-positive examinations necessitates caution in interpreting 3D-MRM images. 3D MRM technique may be useful in selected cases such as patients with multilevel disc abnormalities or lumbar spinal stenosis where it allows a rapid and easy appreciation of the level most likely to account for the pathology.
