PURPOSE: To determine which patient characteristics influence MRI scan time and how. METHODS: A database search of outpatient MRI liver examinations on 1.5T and 3T scanners from 1/1/2019 to 4/4/2019 was performed using an in-house developed software tool. Mean and median scan times were calculated. Patients who had difficulty following breathing instructions or completing breath-hold sequences were identified. Twenty-one additional patient characteristics were obtained from an Electronic Medical Record (EMR) search. RESULTS: Scan times were significantly increased for patients with breath-holding issues during the exam (N = 43, median = 23.98 min) versus not (N = 179, median = 17.5 min, p < 0.001). Among patients who had difficulty following breathing instructions/completing breath-hold sequences, a significant number were non-native English speakers (23/43, 53%) compared to those whose first language was English (48/179, 27%, p < 0.001). Breath-holding issues were also significantly more frequent for patients requiring a translator during the exam (15/43, 35%) versus those who did not (24/179, 13%, p < 0.001). No other patient characteristics showed a significance difference between those with breathing issues and those without. Patient characteristics that caused a significant number of scan times to be one standard deviation or more above the median were as follows: Breath-holding issues during exam (21/43 ≥ one SD above, 51%, versus 22/189 < one SD above, 12%, p < 0.001); and first language not English (16/71 ≥ one SD above, 23%, versus 55/189 < one SD above, 29%, p = 0.03). CONCLUSION: The ability to follow breathing instructions and complete breath-hold sequences had a significant impact on patient scan time. Patients who were not native English speakers had more frequent breathing issues during scans and significantly longer scans times compared native English speakers.

Contains fulltext : 291426.pdf (Publisher’s version ) (Open Access) PURPOSE: To increase the effectiveness of respiratory gating in radial stack-of-stars MRI, particularly when imaging at high spatial resolutions or with multiple echoes. METHODS: Free induction decay (FID) navigators were integrated into a three-dimensional gradient echo radial stack-of-stars pulse sequence. These navigators provided a motion signal with a high temporal resolution, which allowed single-spoke binning (SSB): each spoke at each phase encode step was sorted individually to the corresponding motion state of the respiratory signal. SSB was compared with spoke-angle binning (SAB), in which all phase encode steps of one projection angle were sorted without the use of additional navigator data. To illustrate the benefit of SSB over SAB, images of a motion phantom and of six free-breathing volunteers were reconstructed after motion-gating using either method. Image sharpness was quantitatively compared using image gradient entropies. RESULTS: The proposed method resulted in sharper images of the motion phantom and free-breathing volunteers. Differences in gradient entropy were statistically significant (p = 0.03) in favor of SSB. The increased accuracy of motion-gating led to a decrease of streaking artifacts in motion-gated four-dimensional reconstructions. To consistently estimate respiratory signals from the FID-navigator data, specific types of gradient spoiler waveforms were required. CONCLUSION: SSB allowed high-resolution motion-corrected MR imaging, even when acquiring multiple gradient echo signals or large acquisition matrices, without sacrificing accuracy of motion-gating. SSB thus relieves restrictions on the choice of pulse sequence parameters, enabling the use of motion-gated radial stack-of-stars MRI in a broader domain of clinical applications.

PURPOSE: This study aimed to (i) develop Magnetization-Prepared Golden-angle RAdial Sparse Parallel (MP-GRASP) MRI using a stack-of-stars trajectory for rapid free-breathing T1 mapping and (ii) extend MP-GRASP to multi-echo acquisition (MP-Dixon-GRASP) for fat/water-separated (water-specific) T1 mapping. METHODS: An adiabatic non-selective 180° inversion-recovery pulse was added to a gradient-echo-based golden-angle stack-of-stars sequence for magnetization-prepared 3D single-echo or 3D multi-echo acquisition. In combination with subspace-based GRASP-Pro reconstruction, the sequence allows for standard T1 mapping (MP-GRASP) or fat/water-separated T1 mapping (MP-Dixon-GRASP), respectively. The accuracy of T1 mapping using MP-GRASP was evaluated in a phantom and volunteers (brain and liver) against clinically accepted reference methods. The repeatability of T1 estimation was also assessed in the phantom and volunteers. The performance of MP-Dixon-GRASP for water-specific T1 mapping was evaluated in a fat/water phantom and volunteers (brain and liver). RESULTS: ROI-based mean T1 values are correlated between the references and MP-GRASP in the phantom (R(2) = 1.0), brain (R(2) = 0.96), and liver (R(2) = 0.73). MP-GRASP achieved good repeatability of T1 estimation in the phantom (R(2) = 1.0), brain (R(2) = 0.99), and liver (R(2) = 0.82). Water-specific T1 is different from in-phase and out-of-phase composite T1 (composite T1 when fat and water signal are mixed in phase or out of phase) both in the phantom and volunteers. CONCLUSION: This work demonstrated the initial performance of MP-GRASP and MP-Dixon-GRASP MRI for rapid 3D T1 mapping and 3D fat/water-separated T1 mapping in the brain (without motion) and in the liver (during free breathing). With fat/water-separated T1 estimation, MP-Dixon-GRASP could be potentially useful for imaging patients with fatty-liver diseases.

To compare dynamic magnetic resonance imaging (MRI) with videofluoroscopy (VFS) regarding image quality and assessment of gap size between soft palate (SP) and posterior pharyngeal wall (PPW) in children and adolescents following surgical correction of velopharyngeal dysfunction (VPD). Twenty-one patients undergoing unenhanced 3-T MRI and contrast-enhanced VFS were included in this IRB-approved prospective study. The MRI scan protocol comprised refocused gradient-echo sequences in transverse and sagittal planes during speech, with TE 1.97 ms, TR 3.95 ms, flip angle 8°, matrix size 128 × 128, and 5-mm slice thickness. Radial k-space sampling and sliding window reconstruction were used to achieve an image acquisition rate of 28 frames per second (fps). VFS with 30 fps was similarly performed in both planes. Closure of the velopharyngeal port during phonation was evaluated by two experienced radiologists. Eleven (52.4%) patients displayed a complete closure, whereas ten (47.6%) patients showed a post-operative gap during speech. VFS and MRI equally identified the cases with persistent or recurrent VPD. Differences in SP-PPW distance between VFS (3.9 ± 1.6 mm) and MRI (4.1 ± 1.5 mm) were not statistically significant (p = 0.5). The subjective overall image quality of MRI was rated inferior (p

Purpose The aim of this study was to demonstrate the value of DCE MRI with high spatiotemporal resolution (GRASP) for differentiating paragangliomas and schwannomas in the head and neck. Methods In a retrospective PACS search of in total 410 patients who had undergone head & neck GRASP-MRI, we identified 6 patients with biopsy proven cervical paragangliomas (n = 3) and schwannomas (n = 3). Conventional MRI features were evaluated, lesion size was determined. Postprocessing in 4D-GRASP datasets was performed (1) based on reconstructions with a temporal resolution (Tres) of 4.1 s, qualitative time-intensity curve classification and semiquantitative parameter (Tpeak, PH, ERmax and Slopemax) analysis, and (2) voxel-based mapping and qualitative and semiquantitative perfusion modeling based on reconstructions with a Tres of 1.6 s. Additionally, GRASP perfusion analysis was performed in another set of 5 patients with presumed cervical paragangliomas (n = 3) and schwannomas (n = 2) based on conventional imaging criteria and was correlated with conventional imaging findings. Due to the small sample size, both groups were compared qualitatively. Results In the time intensity curve classification of 4D GRASP reconstructions (Tres 4.1 s), biopsy proven paragangliomas were consistently characterized by a type-III rapid inflow wash-out pattern, compared to a type-I inflow pattern in the schwannoma group. In both temporal resolutions, semiquantitative analysis of time intensity curves demonstrated rapid wash-in, wash-out, and higher peak signal intensities in paragangliomas compared to schwannomas. In 5 presumed (non-biopsy-proven) paragangliomas and schwannomas, time intensity curves improved diagnostic certainty. Conclusions Visual time intensity curve classification and semi-quantitative analysis of GRASP-MRI were, in this small retrospective series, sufficient to differentiate cervical paragangliomas from schwannomas. Utilization of this technique may further improve diagnostic confidence in lesions lacking conventional imaging features.

PURPOSE To develop a free-breathing hepatic fat and R2∗ quantification method by extending a previously described stack-of-stars model-based fat-water separation technique with additional modeling of the transverse relaxation rate R2∗ . METHODS The proposed technique combines motion-robust radial sampling using a stack-of-stars bipolar multi-echo 3D GRE acquisition with iterative model-based fat-water separation. Parallel-Imaging and Compressed-Sensing principles are incorporated through modeling of the coil-sensitivity profiles and enforcement of total-variation (TV) sparsity on estimated water, fat, and R2∗ parameter maps. Water and fat signals are used to estimate the confounder-corrected proton-density fat fraction (PDFF). Two strategies for handling respiratory motion are described: motion-averaged and motion-resolved reconstruction. Both techniques were evaluated in patients (n = 14) undergoing a hepatobiliary research protocol at 3T. PDFF and R2∗ parameter maps were compared to a breath-holding Cartesian reference approach. RESULTS Linear regression analyses demonstrated strong (r > 0.96) and significant (P ≪ .01) correlations between radial and Cartesian PDFF measurements for both the motion-averaged reconstruction (slope: 0.90; intercept: 0.07%) and the motion-resolved reconstruction (slope: 0.90; intercept: 0.11%). The motion-averaged technique overestimated hepatic R2∗ values (slope: 0.35; intercept: 30.2 1/s) compared to the Cartesian reference. However, performing a respiratory-resolved reconstruction led to better R2∗ value consistency (slope: 0.77; intercept: 7.5 1/s). CONCLUSIONS The proposed techniques are promising alternatives to conventional Cartesian imaging for fat and R2∗ quantification in patients with limited breath-holding capabilities. For accurate R2∗ estimation, respiratory-resolved reconstruction should be used.

BACKGROUND: Hybrid PET/MR imaging has significant potential in cardiology due to its combination of molecular PET imaging and cardiac MR. Multi-tissue-class MR-based attenuation correction (MRAC) is necessary for accurate PET quantification. Moreover, for thoracic PET imaging, respiration is known to lead to misalignments of MRAC and PET data that result in PET artifacts. These factors can be addressed by using multi-echo MR for tissue segmentation and motion-robust or gated acquisitions. However, the combination of these strategies is not routinely available and can be prone to errors. In this study we examine the qualitative and quantitative impacts of multi-class MRAC compared to a more widely available, simple two-class MRAC for cardiac PET/MR. METHODS AND RESULTS: In a cohort of patients with cardiac sarcoidosis, we acquired MRAC data using multi-echo radial gradient-echo MR imaging. Water-fat separation was used to produce attenuation maps with up to 4 tissue classes including water-based soft tissue, fat, lung, and background air. Simultaneously acquired 18F-fluorodeoxyglucose PET data were subsequently reconstructed using each attenuation map separately. PET uptake values were measured in the myocardium and compared between different PET images. The inclusion of lung and subcutaneous fat in the MRAC maps significantly affected the quantification of 18F-fluorodeoxyglucose activity in the myocardium but only moderately altered the appearance of the PET image without introduction of image artifacts. CONCLUSIONS: Optimal MRAC for cardiac PET/MR applications should include segmentation of all tissues in combination with compensation for the respiratory-related motion of the heart. Simple two-class MRAC is adequate for qualitative clinical assessment.

T1-weighted post-contrast MRI is essential in brain protocols. We demonstrate the feasibility and utility of a 3D non-Cartesian radial acquisition in children.To compare bulk motion artifacts, image quality, and lesion conspicuity in 3D T1-weighted post-contrast brain MRI between a new fat-suppressed radial gradient-echo and a traditional non-fat-suppressed inversion-recovery Cartesian gradient-echo sequence.Images from 53 patients acquired at 3 Tesla were compared. Three radiologists rated the images in three categories, including the presence of bulk motion and whether it impacted diagnosis, whether one sequence was preferred over the other in overall image quality and conspicuity of vascular structures and lesions, and whether diagnosis was possible if only the new fat-suppressed radial acquisition was obtained.The Fleiss' kappa for inter-rater agreement was 0.67 for bulk motion and 0.54 for sequence preference. Of the 53 cases, 56% were identified to have significant motion on conventional imaging, while only 13% had motion artifacts on the radial acquisition (p 0.05). There were no cases where motion was seen on the radial acquisition but not on conventional imaging. Both sequences were equally preferred in 87% of the cases. All radiologists agreed that the radial approach had lower gray-white matter contrast than the conventional inversion-recovery method, but preferred the former for making diagnosis in uncooperative patients.We demonstrate the potential utility of a fat-suppressed 3D T1-weighted post-contrast brain gradient-echo sequence in children. The technique is useful in non-sedate pediatric imaging due to its reduced sensitivity to bulk motion.

OBJECTIVE. The purpose of this study is to increase the value of MRI by reengineering the MRI workflow at a new imaging center to shorten the interval (i.e., turnaround time) between each patient examination by at least 5 minutes. MATERIALS AND METHODS. The elements of the MRI workflow that were optimized included the use of dockable tables, the location of patient preparation rooms, the number of doors per scanning room, and the storage location and duplication of coils. Turnaround times at the new center and at two existing centers were measured both for all patients and for situations when the next patient was ready to be brought into the scanner room after the previous patient’s examination was completed. RESULTS. Workflow optimizations included the use of dockable tables, dedicated patient preparation rooms, two doors in each MRI room, positioning the scanner to provide the most direct path to the scanner, and coil storage in the preparation rooms, with duplication of the most frequently used coils. At the new imaging center, the median and mean (± SD) turnaround times for situations in which patients were ready for scanning were 115 seconds (95% CI, 113–117 seconds) and 132 ± 72 seconds (95% CI, 129–135 seconds), respectively, and the median and mean turnaround times for all situations were 141 seconds (95% CI, 137–146 seconds) and 272 ± 270 seconds (95% CI, 263–282 seconds), respectively. For existing imaging centers, the median and mean turnaround times for situations in which patients were ready for scanning were 430 seconds (95% CI, 424–434 seconds) and 460 ± 156 seconds (95% CI, 455–465 seconds), respectively, and the median and mean turnaround times for all situations were 481 seconds (95% CI, 474–486 seconds) and 537 ± 219 seconds (95% CI, 532–543 seconds), respectively. CONCLUSION. The optimized MRI workflow resulted in a mean time savings of 5 minutes 28 seconds per patient.

PURPOSE: To develop a protocol for abdominal imaging on a prototype 0.55 T scanner and to benchmark the image quality against conventional 1.5 T exam. METHODS: In this prospective IRB-approved HIPAA-compliant study, 10 healthy volunteers were recruited and imaged. A commercial MRI system was modified to operate at 0.55 T (LF) with two different gradient performance levels. Each subject underwent non-contrast abdominal examinations on the 0.55 T scanner utilizing higher gradients (LF-High), lower adjusted gradients (LF-Adjusted), and a conventional 1.5 T scanner. The following pulse sequences were optimized: fat-saturated T2-weighted imaging (T2WI), diffusion-weighted imaging (DWI), and Dixon T1-weighted imaging (T1WI). Three readers independently evaluated image quality in a blinded fashion on a 5-point Likert scale, with a score of 1 being non-diagnostic and 5 being excellent. An exact paired sample Wilcoxon signed-rank test was used to compare the image quality. RESULTS: Diagnostic image quality (overall image quality score ≥ 3) was achieved at LF in all subjects for T2WI, DWI, and T1WI with no more than one unit lower score than 1.5 T. The mean difference in overall image quality score was not significantly different between LF-High and LF-Adjusted for T2WI (95% CI − 0.44 to 0.44; p = 0.98), DWI (95% CI − 0.43 to 0.36; p = 0.92), and for T1 in- and out-of-phase imaging (95%C I − 0.36 to 0.27; p = 0.91) or T1 fat-sat (water only) images (95% CI − 0.24 to 0.18; p = 1.0). CONCLUSION: Diagnostic abdominal MRI can be performed on a prototype 0.55 T scanner, either with conventional or with reduced gradient performance, within an acquisition time of 10 min or less.

OBJECTIVE To compare the image quality of subtracted and nonsubtracted images obtained using volumetric interpolated breath-hold exam (VIBE) and free breathing T1 weighted Golden-angle Radial Sparse Parallel (GRASP). METHODS We retrospectively evaluated 27 consecutive patients who underwent MRI for the evaluation of renal masses. Contrast enhanced VIBE and free breathing GRASP imaging were performed, and subtraction images generated. Two radiologists performed quantitative and qualitative evaluations of image quality of nonsubtracted and subtracted data sets. Statistical analysis was performed using the Wilcoxon signed-rank test, paired t-test and kappa statistics. Purpose To describe an approach for detection of respiratory signals using a transmitted radiofrequency (RF) reference signal called Pilot-Tone (PT) and to use the PT signal for creation of motion-resolved images based on 3D stack-of-stars imaging under free-breathing conditions. Methods This work explores the use of a reference RF signal generated by a small RF transmitter, placed outside the MR bore. The reference signal is received in parallel to the MR signal during each readout. Because the received PT amplitude is modulated by the subject's breathing pattern, a respiratory signal can be obtained by detecting the strength of the received PT signal over time. The breathing-induced PT signal modulation can then be used for reconstructing motion-resolved images from free-breathing scans. The PT approach was tested in volunteers using a radial stack-of-stars 3D gradient echo (GRE) sequence with golden-angle acquisition. Results Respiratory signals derived from the proposed PT method were compared to signals from a respiratory cushion sensor and k-space-center-based self-navigation under different breathing conditions. Moreover, the accuracy was assessed using a modified acquisition scheme replacing the golden-angle scheme by a zero-angle acquisition. Incorporating the PT signal into eXtra-Dimensional (XD) motion-resolved reconstruction led to improved image quality and clearer anatomical depiction of the lung and liver compared to k-space-center signal and motion-averaged reconstruction, when binned into 6, 8, and 10 motion states. Conclusion PT is a novel concept for tracking respiratory motion. Its small dimension (8 cm), high sampling rate, and minimal interaction with the imaging scan offers great potential for resolving respiratory motion.

MRI methods that have reduced sensitivity to motion are attractive in pediatric applications. In spine imaging, physiologic motion such as respiration and cerebrospinal fluid pulsation can hamper diagnostic image quality. We compare a 3D T1-weighted non-Cartesian radial acquisition with a conventional Cartesian 2D turbo-spin-echo (TSE) acquisition in axial post-contrast spine imaging at 3T. Thirty-two patients (mean age 12.2 ± 5.3 years) scheduled for routine clinical spine exams with contrast were enrolled. Three pediatric neuroradiologists compared the two sequences and assessed the presence of motion, the conspicuity of nerve roots, and whether one of the sequences was preferred in visualizing pathology using Likert scales. The Fleiss’ kappa statistic for inter-rater agreement was 0.29 (95% confidence interval, 0.15–0.43) for the presence of motion, 0.30 (0.21–0.38) for conspicuity, and 0.37 (0.19–0.55) for sequence preference. Purpose Most clinical MR examinations require acquisition of different image contrasts. For abdominal exams, the scans are conventionally performed as separate acquisitions using respiratory gating or repeated breath holding, which can be time-inefficient and challenging for patients. Here, a hybrid imaging approach is described that creates T2 - and T1 -weighted images from a single scan and allows for free-breathing acquisition. Theory and methods T2 -weighted data is collected using 3D fast spin-echo (FSE) acquisition with motion-robust radial stack-of-stars sampling. The wait time between the FSE trains is used to acquire T1 -weighted gradient-echo (GRE) data. Improved robustness is achieved by extracting a respiratory signal from the GRE data and using it for motion-weighted reconstruction. Results As validated in simulations and phantom scans, GRE acquisition in the wait time has minor effect on the signal strength and contrast. Volunteer scans at 1.5T showed that T2 - and T1 -weighted hybrid imaging is feasible during free-breathing. Furthermore, it has been demonstrated in a patient that hybrid imaging with T1 -weighted Dixon acquisition is possible. Conclusion The described hybrid sequence enables comprehensive T2 - and T1 -weighted imaging in a single scan. In addition to free-breathing abdominal examination, it promises value for clinical applications that are frequently affected by motion artifacts.

Purpose of this study was to compare the quality of perfusion maps obtained from prototypical free-breathing magnetic resonance imaging (MRI) with continuous golden-angle radial sampling and iterative reconstruction (GRASP) to conventional acquisition using time-resolved angiography with interleaved stochastic trajectories (TWIST) in patients with rectal cancer.Forty cases were included for retrospective analysis. Twenty of the patients received routine multiparametric MRI at 3 T for rectal cancer staging, including perfusion measurement with GRASP or TWIST (10 patients for each technique, including 5 prechemoradiation and 5 postchemoradiation). Twenty patients without history of rectal disease served as control group (10 GRASP, 10 TWIST). GRASP images were reconstructed at temporal resolution of 3.45 seconds (21 spokes/frame). A voxel-by-voxel deconvolution approach was used to determine rectal plasma flow (mL/100 mL per minute). Regions of interest were placed at 3 levels within the tumor and normal rectum (lower, middle, and upper part). The quality of morphologic images, perfusion maps, and arterial input function were scored by 2 blinded radiologists. Independent t tests were applied.Three patients of the TWIST control group had to be excluded due to technical failure of the sequence. Significantly higher scores for the perfusion maps and arterial input functions (total cohort) were obtained using GRASP (P0.05). Artifacts in the perfusion maps were rated significantly lower than for TWIST (P0.05). In the healthy rectum cohort, the average plasma flow of normal rectal wall was 31.78 ± 7.39 mL/100 mL per minute with GRASP, compared with 77.62 ± 34.08 mL/100 mL per minute with TWIST, indicating much lower variance for GRASP. Plasma flow values obtained with both methods enabled distinguishing between normal rectal wall and rectal cancer, both before and after chemoradiation. Morphologic image quality was generally higher with GRASP (P0.01).GRASP perfusion imaging can distinguish between normal rectum and rectal cancers with higher image quality and less variance than TWIST. Additional morphologic assessment with high spatial resolution from the GRASP acquisition may increase the accuracy and diagnostic confidence of the examination.

OBJECTIVES The aim of this study was to assess the applicability of Dixon radial volumetric encoding (Dixon-RAVE) for comprehensive dynamic contrast-enhanced 3D magnetic resonance imaging (MRI) of the breast using a combination of radial sampling, model-based fat/water separation, compressed sensing, and parallel imaging. MATERIALS AND METHODS In this Health Insurance Portability and Accountability Act-compliant prospective study, 24 consecutive patients underwent bilateral breast MRI, including both conventional fat-suppressed and non-fat-suppressed precontrast T1-weighted volumetric interpolated breath-hold examination (VIBE). Afterward, 1 continuous Dixon-RAVE scan was performed with the proposed approach while the contrast agent was injected. This scan was immediately followed by the acquisition of 4 conventional fat-saturated VIBE scans. From the comprehensive Dixon-RAVE data set, different image contrasts were reconstructed that are comparable to the separate conventional VIBE scans.Two radiologists independently rated image quality, conspicuity of fibroglandular tissue from fat (FG), and degree of fat suppression (FS) on a 5-point Likert-type scale for the following 3 comparisons: precontrast fat-suppressed (pre-FS), precontrast non-fat-suppressed (pre-NFS), and dynamic fat-suppressed (dyn-FS) images. RESULTS When scores were averaged over readers, Dixon-RAVE achieved significantly higher (P < 0.001) degree of fat suppression compared with VIBE, for both pre-FS (4.25 vs 3.67) and dyn-FS (4.10 vs 3.46) images. Although Dixon-RAVE had lower image quality score compared with VIBE for the pre-FS (3.56 vs 3.67, P = 0.490), the pre-NFS (3.54 vs 3.88, P = 0.009), and the dyn-FS images (3.06 vs 3.67, P < 0.001), acceptable or better diagnostic quality was achieved (score ≥ 3). The FG score for Dixon-RAVE in comparison to VIBE was significantly higher for the pre-FS image (4.23 vs 3.85, P = 0.044), lower for the pre-NFS image (3.98 vs 4.25, P = 0.054), and higher for the dynamic fat-suppressed image (3.90 vs 3.85, P = 0.845). CONCLUSIONS Dixon-RAVE can serve as a one-stop-shop approach for comprehensive T1-weighted breast MRI with diagnostic image quality, high spatiotemporal resolution, reduced overall scan time, and improved fat suppression compared with conventional imaging.

Background and purpose Preoperative localization of the pituitary gland with imaging in patients with macroadenomas has been inadequately explored. The pituitary gland enhancing more avidly than a macroadenoma has been described in the literature. Taking advantage of this differential enhancement pattern, our aim was to evaluate the role of high-resolution dynamic MR imaging with golden-angle radial sparse parallel reconstruction in localizing the pituitary gland in patients undergoing trans-sphenoidal resection of a macroadenoma. Materials and methods A retrospective study was performed in 17 patients who underwent trans-sphenoidal surgery for pituitary macroadenoma. Radial volumetric interpolated brain examination sequences with golden-angle radial sparse parallel technique were obtained. Using an ROI-based method to obtain signal-time curves and permeability measures, 3 separate readers identified the normal pituitary gland distinct from the macroadenoma. The readers' localizations were then compared with the intraoperative location of the gland. Statistical analyses were performed to assess the interobserver agreement and correlation with operative findings. Results The normal pituitary gland was found to have steeper enhancement-time curves as well as higher peak enhancement values compared with the macroadenoma (P Conclusions This study confirms our ability to consistently and accurately identify the normal pituitary gland in patients with macroadenomas with the golden-angle radial sparse parallel technique with quantitative permeability measurements and enhancement-time curves.

Retrospective electrocardiogram-gated, 2D phase-contrast (PC) flow MRI is routinely used in clinical evaluation of valvular/vascular disease in pediatric patients with congenital heart disease (CHD). In patients not requiring general anesthesia, clinical standard PC is conducted with free breathing for several minutes per slice with averaging. In younger patients under general anesthesia, clinical standard PC is conducted with breath-holding. One approach to overcome this limitation is using either navigator gating or self-navigation of respiratory motion, at the expense of lengthening scan times. An alternative approach is using highly accelerated, free-breathing, real-time PC (rt-PC) MRI, which to date has not been evaluated in CHD patients. The purpose of this study was to develop a 38.4-fold accelerated 2D rt-PC pulse sequence using radial k-space sampling and compressed sensing with 1.5 × 1.5 × 6.0 mm3 nominal spatial resolution and 40 ms nominal temporal resolution, and evaluate whether it is capable of accurately measuring flow in 17 pediatric patients (aortic valve, pulmonary valve, right and left pulmonary arteries) compared with clinical standard 2D PC (either breath-hold or free breathing). For clinical translation, we implemented an integrated reconstruction pipeline capable of producing DICOMs of the order of 2 min per time series (46 frames). In terms of association, forward volume, backward volume, regurgitant fraction, and peak velocity at peak systole measured with standard PC and rt-PC were strongly correlated (R2 > 0.76; P < 0.001). Compared with clinical standard PC, in terms of agreement, forward volume (mean difference = 1.4% (3.0% of mean)) and regurgitant fraction (mean difference = -2.5%) were in good agreement, whereas backward volume (mean difference = -1.1 mL (28.2% of mean)) and peak-velocity at peak systole (mean difference = -21.3 cm/s (17.2% of mean)) were underestimated by rt-PC. This study demonstrates that the proposed rt-PC with the said spatial resolution and temporal resolution produces relatively accurate forward volumes and regurgitant fractions but underestimates backward volumes and peak velocities at peak systole in pediatric patients with CHD.

Purpose: To study the effects of magnetization transfer (MT, in which a semisolid spin pool interacts with the free pool), in the context of magnetic resonance fingerprinting (MRF). Methods: Simulations and phantom experiments were performed to study the impact of MT on the MRF signal and its potential influence on T1 and T2 estimation. Subsequently, an MRF sequence implementing off-resonance MT pulses and a dictionary with an MT dimension by incorporating a two-pool model were used to estimate the fractional pool size in addition to the B1+, T1, and T2 values. The proposed method was evaluated in the human brain. Results: Simulations and phantom experiments showed that an MRF signal obtained from a cross-linked bovine serum sample is influenced by MT. Using a dictionary based on an MT model, a better match between simulations and acquired MR signals can be obtained (NRMSE 1.3% versus 4.7%). Adding off-resonance MT pulses can improve the differentiation of MT from T1 and T2. In-vivo results showed that MT affects the MRF signals from white matter (fractional pool-size ~16%) and gray matter (fractional pool-size ~10%). Furthermore, longer T1 (~1060 ms versus ~860 ms) and T2 values (~47 ms versus ~35 ms) can be observed in white matter if MT is accounted for. Conclusion: Our experiments demonstrated a potential influence of MT on the quantification of T1 and T2 with MRF. Purpose To develop a free‐breathing hepatic fat and R2∗ quantification method by extending a previously described stack‐of‐stars model‐based fat‐water separation technique with additional modeling of the transverse relaxation rate R2∗. Methods The proposed technique combines motion‐robust radial sampling using a stack‐of‐stars bipolar multi‐echo 3D GRE acquisition with iterative model‐based fat‐water separation. Parallel‐Imaging and Compressed‐Sensing principles are incorporated through modeling of the coil‐sensitivity profiles and enforcement of total‐variation (TV) sparsity on estimated water, fat, and R2∗ parameter maps. Water and fat signals are used to estimate the confounder‐corrected proton‐density fat fraction (PDFF). Two strategies for handling respiratory motion are described: motion‐averaged and motion‐resolved reconstruction. Both techniques were evaluated in patients (n = 14) undergoing a hepatobiliary research protocol at 3T. PDFF and R2∗ parameter maps were compared to a breath‐holding Cartesian reference approach. Results Linear regression analyses demonstrated strong (r > 0.96) and significant (P ≪ .01) correlations between radial and Cartesian PDFF measurements for both the motion‐averaged reconstruction (slope: 0.90; intercept: 0.07%) and the motion‐resolved reconstruction (slope: 0.90; intercept: 0.11%). The motion‐averaged technique overestimated hepatic R2∗ values (slope: 0.35; intercept: 30.2 1/s) compared to the Cartesian reference. However, performing a respiratory‐resolved reconstruction led to better R2∗ value consistency (slope: 0.77; intercept: 7.5 1/s). Conclusions The proposed techniques are promising alternatives to conventional Cartesian imaging for fat and R2∗ quantification in patients with limited breath‐holding capabilities. For accurate R2∗ estimation, respiratory‐resolved reconstruction should be used.

BACKGROUND AND PURPOSE: Conventional imaging frequently shows overlapping features between benign and malignant parotid neoplasms. We investigated dynamic contrast-enhanced MR imaging using golden-angle radial sparse parallel imaging in differentiating parotid neoplasms. MATERIALS AND METHODS: For this retrospective study, 41 consecutive parotid neoplasms were imaged with dynamic contrast-enhanced MR imaging with golden-angle radial sparse parallel imaging using 1-mm in-plane resolution. The temporal resolution was 3.4 seconds for 78.2 seconds and 8.8 seconds for the remaining acquisition. Three readers retrospectively and independently created and classified time-intensity curves as follows: 1) continuous wash-in; 2) rapid wash-in, subsequent plateau; and 3) rapid wash-in with washout. Additionally, time-intensity curve–derived semiquantitative metrics normalized to the ipsilateral common carotid artery were recorded. Diagnostic performance for the prediction of neoplasm type and malignancy was assessed. Subset multivariate analysis (n = 32) combined semiquantitative time-intensity curve metrics with ADC values. RESULTS: Independent time-intensity curve classification of the 41 neoplasms produced moderate-to-substantial interreader agreement (κ = 0.50–0.79). The time-intensity curve classification threshold of ≥2 predicted malignancy with a positive predictive value of 56.0%–66.7%, and a negative predictive value of 92.0%–100%. To evaluate potential confounding factors in the quantitative assessment of liver fibrosis and cirrhosis using T1 relaxation times.The study population is based on a radiology-information-system database search for abdominal MRI performed from July 2018 to April 2019 at our institution. After applying exclusion criteria 200 (59 ± 16 yrs) remaining patients were retrospectively included. 93 patients were defined as liver-healthy, 40 patients without known fibrosis or cirrhosis, and 67 subjects had a clinically or biopsy-proven liver fibrosis or cirrhosis. T1 mapping was performed using a slice based look-locker approach. A ROI based analysis of the left and the right liver was performed. Fat fraction, R2*, liver volume, laboratory parameters, sex, and age were evaluated as potential confounding factors.T1 values were significantly lower in healthy subjects without known fibrotic changes (1.5 T MRI: 575 ± 56 ms; 3 T MRI: 857 ± 128 ms) compared to patients with acute liver disease (1.5 T MRI: 657 ± 73 ms, p 0.0001; 3 T MRI: 952 ± 37 ms, p = 0.028) or known fibrosis or cirrhosis (1.5 T MRI: 644 ± 83 ms, p 0.0001; 3 T MRI: 995 ± 150 ms, p = 0.018). T1 values correlated moderately with the Child-Pugh stage at 1.5 T (p = 0.01, ρ = 0.35).T1 mapping is a capable predictor for detection of liver fibrosis and cirrhosis. Especially age is not a confounding factor and, hence, age-independent thresholds can be defined. Acute liver diseases are confounding factors and should be ruled out before employing T1-relaxometry based thresholds to screen for patients with liver fibrosis or cirrhosis.

The introduction of compressed sensing for increasing imaging speed in magnetic resonance imaging (MRI) has raised significant interest among researchers and clinicians, and has initiated a large body of research across multiple clinical applications over the last decade. Compressed sensing aims to reconstruct unaliased images from fewer measurements than are traditionally required in MRI by exploiting image compressibility or sparsity. Moreover, appropriate combinations of compressed sensing with previously introduced fast imaging approaches, such as parallel imaging, have demonstrated further improved performance. The advent of compressed sensing marks the prelude to a new era of rapid MRI, where the focus of data acquisition has changed from sampling based on the nominal number of voxels and/or frames to sampling based on the desired information content. This article presents a brief overview of the application of compressed sensing techniques in body MRI, where imaging speed is crucial due to the presence of respiratory motion along with stringent constraints on spatial and temporal resolution. The first section provides an overview of the basic compressed sensing methodology, including the notion of sparsity, incoherence, and nonlinear reconstruction. The second section reviews state-of-the-art compressed sensing techniques that have been demonstrated for various clinical body MRI applications. Purpose Conventional fat/water separation techniques require that patients hold breath during abdominal acquisitions, which often fails and limits the achievable spatial resolution and anatomic coverage. This work presents a novel approach for free-breathing volumetric fat/water separation. Methods Multiecho data are acquired using a motion-robust radial stack-of-stars three-dimensional GRE sequence with bipolar readout. To obtain fat/water maps, a model-based reconstruction is used that accounts for the off-resonant blurring of fat and integrates both compressed sensing and parallel imaging. The approach additionally enables generation of respiration-resolved fat/water maps by detecting motion from k-space data and reconstructing different respiration states. Furthermore, an extension is described for dynamic contrast-enhanced fat-water–separated measurements. Results Uniform and robust fat/water separation is demonstrated in several clinical applications, including free-breathing noncontrast abdominal examination of adults and a pediatric subject with both motion-averaged and motion-resolved reconstructions, as well as in a noncontrast breast exam. Furthermore, dynamic contrast-enhanced fat/water imaging with high temporal resolution is demonstrated in the abdomen and breast. Conclusion The described framework provides a viable approach for motion-robust fat/water separation and promises particular value for clinical applications that are currently limited by the breath-holding capacity or cooperation of patients. Magnetic resonance imaging (MRI) has become an unrivalled medical diagnostic technique able to map tissue anatomy and physiology non-invasively. MRI measurements are meticulously engineered to control experimental conditions across the sample. However, residual radiofrequency (RF) field inhomogeneities are often unavoidable, leading to artefacts that degrade the diagnostic and scientific value of the images. Here we show that, paradoxically, these artefacts can be eliminated by deliberately interweaving freely varying heterogeneous RF fields into a magnetic resonance fingerprinting data-acquisition process. Observations made based on simulations are experimentally confirmed at 7 Tesla (T), and the clinical implications of this new paradigm are illustrated with in vivo measurements near an orthopaedic implant at 3T. These results show that it is possible to perform quantitative multiparametric imaging with heterogeneous RF fields, and to liberate MRI from the traditional struggle for control over the RF field uniformity., Magnetic resonance fingerprinting (MRF) requires a uniform B1 + radiofrequency field. Here the authors present plug-n-play MRF, a technique that enables multiparametric imaging with heterogeneous B1 + fields, and demonstrate its utility in the presence of RF distortion caused by a metallic orthopaedic implant.

OBJECTIVE This study aimed to demonstrate feasibility of free-breathing radial acquisition with respiratory motion-resolved compressed sensing reconstruction [extra-dimensional golden-angle radial sparse parallel imaging (XD-GRASP)] for multiphase dynamic gadolinium ethoxybenzyl diethylenetriamine pentaacetic acid (Gd-EOB-DTPA)-enhanced liver imaging, and to compare image quality to compressed sensing reconstruction with respiratory motion-averaging (GRASP) and prior conventional breath-held Cartesian-sampled data sets [BH volume interpolated breath-hold examination (VIBE)] in same patients. SUBJECTS AND METHODS In this Health Insurance Portability and Accountability Act-compliant prospective study, 16 subjects underwent free-breathing continuous radial acquisition during Gd-EOB-DTPA injection and had prior BH-VIBE available. Acquired data were reconstructed using motion-averaging GRASP approach in which consecutive 84 spokes were grouped in each contrast-enhanced phase for a temporal resolution of approximately 14 seconds. Additionally, respiratory motion-resolved reconstruction was performed from the same k-space data by sorting each contrast-enhanced phase into multiple respiratory motion states using compressed sensing algorithm named XD-GRASP, which exploits sparsity along both the contrast-enhancement and respiratory-state dimensions.Contrast-enhanced dynamic multiphase XD-GRASP, GRASP, and BH-VIBE images were anonymized, pooled together in a random order, and presented to 2 board-certified radiologists for independent evaluation of image quality, with higher score indicating more optimal examination. RESULTS The XD-GRASP reconstructions had significantly (all P < 0.05) higher overall image quality scores compared to GRASP for early arterial (reader 1: 4.3 ± 0.6 vs 3.31 ± 0.6; reader 2: 3.81 ± 0.8 vs 3.38 ± 0.9) and late arterial (reader 1: 4.5 ± 0.6 vs 3.63 ± 0.6; reader 2: 3.56 ± 0.5 vs 2.88 ± 0.7) phases of enhancement for both readers. The XD-GRASP also had higher overall image quality score in portal venous phase, which was significant for reader 1 (4.44 ± 0.5 vs 3.75 ± 0.8; P = 0.002). In addition, the XD-GRASP had higher overall image quality score compared to BH-VIBE for early (reader 1: 4.3 ± 0.6 vs 3.88 ± 0.6; reader 2: 3.81 ± 0.8 vs 3.50 ± 1.0) and late (reader 1: 4.5 ± 0.6 vs 3.44 ± 0.6; reader 2: 3.56 ± 0.5 vs 2.94 ± 0.9) arterial phases. CONCLUSION Free-breathing motion-resolved XD-GRASP reconstructions provide diagnostic high-quality multiphase images in patients undergoing Gd-EOB-DTPA-enhanced liver examination.

Background To evaluate the influence of temporal sparsity regularization and radial undersampling on compressed sensing reconstruction of dynamic contrast-enhanced (DCE) MRI, using the iterative Golden-angle RAdial Sparse Parallel (iGRASP) MRI technique in the setting of breast cancer evaluation. Methods DCE-MRI examinations of the breast (n = 7) were conducted using iGRASP at 3 Tesla. Images were reconstructed with five different radial undersampling schemes corresponding to temporal resolutions between 2 and 13.4 s/frame and with four different weights for temporal sparsity regularization (λ = 0.1, 0.5, 2, and 6 times of noise level). Image similarity to time-averaged reference images was assessed by two breast radiologists and using quantitative metrics. Temporal similarity was measured in terms of wash-in slope and contrast kinetic model parameters. Results iGRASP images reconstructed with λ = 2 and 5.1 s/frame had significantly (P 5%), in agreement with the assessment of two breast radiologists. Higher undersampling (temporal resolution 23% between λ = 2 and 0.5). The difference between the kinetic-model transfer rates of benign and malignant groups decreased as temporal resolution decreased (82% between 2 and 13.4 s/frame). Conclusion This study demonstrates objective spatial and temporal similarity measures can be used to assess the influence of sparsity constraint and undersampling in compressed sensing DCE-MRI and also shows that the iGRASP method provides the flexibility of optimizing these reconstruction parameters in the postprocessing stage using the same acquired data. BACKGROUND AND PURPOSE: The pituitary gland is located outside of the blood-brain barrier. Dynamic T1 weighted contrast enhanced sequence is considered to be the gold standard to evaluate this region. However, it does not allow assessment of intrinsic permeability properties of the gland. Our aim was to demonstrate the utility of radial volumetric interpolated brain examination with the golden-angle radial sparse parallel technique to evaluate permeability characteristics of the individual components (anterior and posterior gland and the median eminence) of the pituitary gland and areas of differential enhancement and to optimize the study acquisition time. MATERIALS AND METHODS: A retrospective study was performed in 52 patients (group 1, 25 patients with normal pituitary glands; and group 2, 27 patients with a known diagnosis of microadenoma). Radial volumetric interpolated brain examination sequences with golden-angle radial sparse parallel technique were evaluated with an ROI-based method to obtain signal-time curves and permeability measures of individual normal structures within the pituitary gland and areas of differential enhancement. Statistical analyses were performed to assess differences in the permeability parameters of these individual regions and optimize the study acquisition time. RESULTS: Signal-time curves from the posterior pituitary gland and median eminence demonstrated a faster wash-in and time of maximum enhancement with a lower peak of enhancement compared with the anterior pituitary gland ( P < .005). Time-optimization analysis demonstrated that 120 seconds is ideal for dynamic pituitary gland evaluation. In the absence of a clinical history, differences in the signal-time curves allow easy distinction between a simple cyst and a microadenoma. CONCLUSIONS: This retrospective study confirms the ability of the golden-angle radial sparse parallel technique to evaluate the permeability characteristics of the pituitary gland and establishes 120 seconds as the ideal acquisition time for dynamic pituitary gland imaging. The aims of this study were to observe the pattern of transient motion after gadoxetic acid administration including incidence, onset, and duration, and to evaluate the clinical feasibility of free-breathing gadoxetic acid-enhanced liver magnetic resonance imaging using golden-angle radial sparse parallel (GRASP) imaging with respiratory gating.In this institutional review board-approved prospective study, 59 patients who provided informed consents were analyzed. Free-breathing dynamic T1-weighted images (T1WIs) were obtained using GRASP at 3 T after a standard dose of gadoxetic acid (0.025 mmol/kg) administration at a rate of 1 mL/s, and development of transient motion was monitored, which is defined as a distinctive respiratory frequency alteration of the self-gating MR signals. Early arterial, late arterial, and portal venous phases retrospectively reconstructed with and without respiratory gating and with different temporal resolutions (nongated 13.3-second, gated 13.3-second, gated 6-second T1WI) were evaluated for image quality and motion artifacts. Diagnostic performance in detecting focal liver lesions was compared among the 3 data sets.Transient motion (mean duration, 21.5 ± 13.0 seconds) was observed in 40.0% (23/59) of patients, 73.9% (17/23) of which developed within 15 seconds after gadoxetic acid administration. On late arterial phase, motion artifacts were significantly reduced on gated 13.3-second and 6-second T1WI (3.64 ± 0.34, 3.61 ± 0.36, respectively), compared with nongated 13.3-second T1WI (3.12 ± 0.51, P0.0001). Overall, image quality was the highest on gated 13.3-second T1WI (3.76 ± 0.39) followed by gated 6-second and nongated 13.3-second T1WI (3.39 ± 0.55, 2.57 ± 0.57, P0.0001). Only gated 6-second T1WI showed significantly higher detection performance than nongated 13.3-second T1WI (figure of merit, 0.69 [0.63-0.76]) vs 0.60 [0.56-0.65], P = 0.004).Transient motion developed in 40% (23/59) of patients shortly after gadoxetic acid administration, and gated free-breathing T1WI using GRASP was able to consistently provide acceptable arterial phase imaging in patients who exhibited transient motion.

PURPOSE: Most clinical MR examinations require acquisition of different image contrasts. For abdominal exams, the scans are conventionally performed as separate acquisitions using respiratory gating or repeated breath holding, which can be time-inefficient and challenging for patients. Here, a hybrid imaging approach is described that creates T2- and T1-weighted images from a single scan and allows for free-breathing acquisition. THEORY AND METHODS: T2-weighted data is collected using 3D fast spin-echo (FSE) acquisition with motion-robust radial stack-of-stars sampling. The wait time between the FSE trains is used to acquire T1-weighted gradient-echo (GRE) data. Improved robustness is achieved by extracting a respiratory signal from the GRE data and using it for motion-weighted reconstruction. RESULTS: As validated in simulations and phantom scans, GRE acquisition in the wait time has minor effect on the signal strength and contrast. Volunteer scans at 1.5 Tesla showed that T2- and T1-weighted hybrid imaging is feasible during free-breathing. Furthermore, it has been demonstrated in a patient that hybrid imaging with T1-weighted Dixon acquisition is possible. CONCLUSION: The described hybrid sequence enables comprehensive T2- and T1-weighted imaging in a single scan. In addition to free-breathing abdominal examination, it promises value for clinical applications that are frequently affected by motion artifacts.

Magnetic resonance (MR)/PET scanners provide an imaging platform that enables simultaneous acquisition of MR and PET data in perfect spatial and temporal registration. This feature allows improving image quality for the MR and PET images obtained during the course of an examination. In this work the authors demonstrate the use of prospective MR-based motion tracking information for removing motion blur in MR/PET images of small pulmonary nodules. The theoretical basis for the algorithms is presented alongside clinical examples of its use.

Estimation of the Discrete-Time Fourier Transform (DTFT) at points of a finite domain arises in many imaging applications. A new approach to this task, the Golden Angle Linogram Fourier Domain (GALFD), is presented, together with a computationally fast and accurate tool, named Golden Angle Linogram Evaluation (GALE), for approximating the DTFT at points of a GALFD. A GALFD resembles a Linogram Fourier Domain (LFD), which is efficient and accurate. A limitation of linograms is that embedding an LFD into a larger one requires many extra points, at least doubling the domain's cardinality. The GALFD, on the other hand, allows for incremental inclusion of relatively few data points. Approximation error bounds and floating point operations counts are presented to show that GALE computes accurately and efficiently the DTFT at the points of a GALFD. The ability to extend the data collection in small increments is beneficial in applications such as Magnetic Resonance Imaging. Experiments for simulated and for real-world data are presented to substantiate the theoretical claims. The mathematical analysis, algorithms, and software developed in the paper are equally suitable to other angular distributions of rays and therefore we bring the benefits of linograms to arbitrary radial patterns.

Purpose To develop a fast and flexible free-breathing dynamic volumetric MRI technique, iterative Golden-angle RAdial Sparse Parallel MRI (iGRASP), that combines compressed sensing, parallel imaging, and golden-angle radial sampling. Methods Radial k-space data are acquired continuously using the golden-angle scheme and sorted into time series by grouping an arbitrary number of consecutive spokes into temporal frames. An iterative reconstruction procedure is then performed on the undersampled time series where joint multicoil sparsity is enforced by applying a total-variation constraint along the temporal dimension. Required coil-sensitivity profiles are obtained from the time-averaged data. Results iGRASP achieved higher acceleration capability than either parallel imaging or coil-by-coil compressed sensing alone. It enabled dynamic volumetric imaging with high spatial and temporal resolution for various clinical applications, including free-breathing dynamic contrast-enhanced imaging in the abdomen of both adult and pediatric patients, and in the breast and neck of adult patients. Conclusion The high performance and flexibility provided by iGRASP can improve clinical studies that require robustness to motion and simultaneous high spatial and temporal resolution. Non-Cartesian magnetic resonance imaging (MRI) sequences have shown great promise for abdominal examination during free breathing, but break down in the presence of bulk patient motion (i.e. voluntary or involuntary patient movement resulting in translation, rotation or elastic deformations of the body). This work describes a data-consistency-driven image stabilization technique that detects and excludes bulk movements during data acquisition. Bulk motion is identified from changes in the signal intensity distribution across different elements of a multi-channel receive coil array. A short free induction decay signal is acquired after excitation and used as a measure to determine alterations in the load distribution. The technique has been implemented on a clinical MR scanner and evaluated in the abdomen. Six volunteers were scanned and two radiologists scored the reconstructions. To show the applicability to other body areas, additional neck and knee images were acquired. Data corrupted by bulk motion were successfully detected and excluded from image reconstruction. An overall increase in image sharpness and reduction of streaking and shine-through artifacts were seen in the volunteer study, as well as in the neck and knee scans. The proposed technique enables automatic real-time detection and exclusion of bulk motion during MR examinations without user interaction. It may help to improve the reliability of pediatric MRI examinations without the use of sedation.

We investigated a novel sequence with radial k-space sampling, gridding and sliding window reconstruction with bSSFP contrast that allows for true real-time functional cardiac evaluation independent from respiration and ECG triggering. 12 healthy volunteers underwent 1.5 T cardiac MRI. Single-shot short axis views were acquired with a) standard retrospectively ECG-gated segmented breath-hold (bh) bSSFP and with the real-time radial bSSFP sequence with a nominal temporal resolution of b) 16 fps (frames per second) and c) 40 fps. Radial bSSFP were acquired during free breathing without ECG synchronization. Left ventricular functional parameters (EDV, ESV, SV and EF) were compared and quality of wall motion depiction was assessed. Contrast-to-noise-ratio (CNR) of myocardium/blood pool in the left ventricle was calculated. EF showed excellent correlation (Bland-Altman r = 0.99; Lin rho = 0.91) between bh-bSSFP (65 %) and 40 fps radial (64 %) and moderate correlation (r = 0.84, rho = 0.20) with 16 fps radial bSSFP (56 %). While EDV was in good agreement for all three sequences, ESV was significantly overestimated with 16 fps radial bSSFP. Despite lower CNR, image quality for wall motion assessment was rated significantly better for 40 fps compared to 16 fps radial bSSFP due to the faster temporal resolution. Left ventricular functional analysis with fast true real-time radial bSSFP is in good agreement with standard ECG-gated bh-bSSFP. The independency from ECG synchronization and breathing promises a robust method for patients with impaired cardiopulmonary status in whom breath-hold and good quality ECG cannot be achieved.

Background Interest in MR-only treatment planning for radiation therapy is growing rapidly with the emergence of integrated MRI/linear accelerator technology. The purpose of this study was to evaluate the feasibility of using synthetic CT images generated from conventional Dixon-based MRI scans for radiation treatment planning of lung cancer. Methods Eleven patients who underwent whole-body PET/MR imaging following a PET/CT exam were randomly selected from an ongoing prospective IRB-approved study. Attenuation maps derived from the Dixon MR Images and atlas-based method was used to create CT data (synCT). Treatment planning for radiation treatment of lung cancer was optimized on the synCT and subsequently copied to the registered CT (planCT) for dose calculation. Planning target volumes (PTVs) with three sizes and four different locations in the lung were planned for irradiation. The dose-volume metrics comparison and 3D gamma analysis were performed to assess agreement between the synCT and CT calculated dose distributions. Results Mean differences between PTV doses on synCT and CT across all the plans were −0.1% ± 0.4%, 0.1% ± 0.5%, and 0.4% ± 0.5% for D95, D98 and D100, respectively. Difference in dose between the two datasets for organs at risk (OARs) had average differences of −0.14 ± 0.07 Gy, 0.0% ± 0.1%, and −0.1% ± 0.2% for maximum spinal cord, lung V20, and heart V40 respectively. In patient groups based on tumor size and location, no significant differences were observed in the PTV and OARs dose-volume metrics (p > 0.05), except for the maximum spinal-cord dose when the target volumes were located at the lung apex (p = 0.001). Gamma analysis revealed a pass rate of 99.3% ± 1.1% for 2%/2 mm (dose difference/distance to agreement) acceptance criteria in every plan. Conclusions The synCT generated from Dixon-based MRI allows for dose calculation of comparable accuracy to the standard CT for lung cancer treatment planning. The dosimetric agreement between synCT and CT calculated doses warrants further development of a MR-only workflow for radiotherapy of lung cancer.

The introduction of compressed sensing for increasing imaging speed in magnetic resonance imaging (MRI) has raised significant interest among researchers and clinicians, and has initiated a large body of research across multiple clinical applications over the last decade. Compressed sensing aims to reconstruct unaliased images from fewer measurements than are traditionally required in MRI by exploiting image compressibility or sparsity. Moreover, appropriate combinations of compressed sensing with previously introduced fast imaging approaches, such as parallel imaging, have demonstrated further improved performance. The advent of compressed sensing marks the prelude to a new era of rapid MRI, where the focus of data acquisition has changed from sampling based on the nominal number of voxels and/or frames to sampling based on the desired information content. This article presents a brief overview of the application of compressed sensing techniques in body MRI, where imaging speed is crucial due to the presence of respiratory motion along with stringent constraints on spatial and temporal resolution. The first section provides an overview of the basic compressed sensing methodology, including the notion of sparsity, incoherence, and nonlinear reconstruction. The second section reviews state-of-the-art compressed sensing techniques that have been demonstrated for various clinical body MRI applications. Conventional fat/water separation techniques require that patients hold breath during abdominal acquisitions, which often fails and limits the achievable spatial resolution and anatomic coverage. This work presents a novel approach for free-breathing volumetric fat/water separation.Multiecho data are acquired using a motion-robust radial stack-of-stars three-dimensional GRE sequence with bipolar readout. To obtain fat/water maps, a model-based reconstruction is used that accounts for the off-resonant blurring of fat and integrates both compressed sensing and parallel imaging. The approach additionally enables generation of respiration-resolved fat/water maps by detecting motion from k-space data and reconstructing different respiration states. Furthermore, an extension is described for dynamic contrast-enhanced fat-water-separated measurements.Uniform and robust fat/water separation is demonstrated in several clinical applications, including free-breathing noncontrast abdominal examination of adults and a pediatric subject with both motion-averaged and motion-resolved reconstructions, as well as in a noncontrast breast exam. Furthermore, dynamic contrast-enhanced fat/water imaging with high temporal resolution is demonstrated in the abdomen and breast.The described framework provides a viable approach for motion-robust fat/water separation and promises particular value for clinical applications that are currently limited by the breath-holding capacity or cooperation of patients.
