Newborn screening (NBS) dramatically improves outcomes in severe childhood disorders by treatment before symptom onset. In many genetic diseases, however, outcomes remain poor because NBS has lagged behind drug development. Rapid whole-genome sequencing (rWGS) is attractive for comprehensive NBS because it concomitantly examines almost all genetic diseases and is gaining acceptance for genetic disease diagnosis in ill newborns. We describe prototypic methods for scalable, parentally consented, feedback-informed NBS and diagnosis of genetic diseases by rWGS and virtual, acute management guidance (NBS-rWGS). Using established criteria and the Delphi method, we reviewed 457 genetic diseases for NBS-rWGS, retaining 388 (85%) with effective treatments. Simulated NBS-rWGS in 454,707 UK Biobank subjects with 29,865 pathogenic or likely pathogenic variants associated with 388 disorders had a true negative rate (specificity) of 99.7% following root cause analysis. In 2,208 critically ill children with suspected genetic disorders and 2,168 of their parents, simulated NBS-rWGS for 388 disorders identified 104 (87%) of 119 diagnoses previously made by rWGS and 15 findings not previously reported (NBS-rWGS negative predictive value 99.6%, true positive rate [sensitivity] 88.8%). Retrospective NBS-rWGS diagnosed 15 children with disorders that had been undetected by conventional NBS. In 43 of the 104 children, had NBS-rWGS-based interventions been started on day of life 5, the Delphi consensus was that symptoms could have been avoided completely in seven critically ill children, mostly in 21, and partially in 13. We invite groups worldwide to refine these NBS-rWGS conditions and join us to prospectively examine clinical utility and cost effectiveness.

Theoretical analyses of thermal elimination reaction kinetics of ethyl formate, ethyl xanthate, and ethyl phosphinate reveal that the transition structure exhibits a greater length for the C(sub alpha)-O bond than the C(sub beta)-O bond. The degree of bond lengthening is correlated to the reaction rates. C(sub alpha)-O dissociation precedes C(sub beta)-O bond dissociation in this elimination reaction. An increase in C(sub alpha)-O bond length increases elimination rate.

DNA from four human esophageal carcinoma cell lines and 50 primary esophageal carcinomas, taken from various countries for expansion of the cyclin D1 gene, were examined. About 36 of the 50 carcinomas were examined by immunohistochemistry for cyclin D1 and retinoblastoma (RB) protein expression. Approximately 16 tumors and 2 cell lines exhibited a three to ten fold expansion in cyclin D1 gene. During evolution of tumors through reduced RB gene expression or through enhanced cyclin D1 gene expression, the restrictive influence of RB on cell cycle development can be removed.

- Abstract
We propose that the expression of Sex Hormone-Binding Globulin (SHBG) by breast cancer cells is biologically regulated and that this SHBG functions to alter the effects of estrogens within the breast cancer cell. The expression of SHBG by breast cancer cells raises the important question of how the local regulation of SHBG synthesis and secretion affects both the sequestration of steroid hormones within the breast, and estrogen induced signal transduction at the cell membrane. The local synthesis of SHBG is consistent with an autocrine/paracrine role for this protein in breast cancer. We have made significant progress setting up our in vitro system for studying the effects of SHBG expression on breast cancer growth. We have generated candidate clones, derived from the human MCF-7 breast cancer cell line, that can be induced to either overexpress SHBG or to silence endogenous SHBG expression. These clones, along with a vector control cell line, will serve as vehicles to study the effects of SHBG expression on the regulation of steroid based signaling in human breast cancer.

- Abstract
Sex Hormone Binding Globulin (SHBG), in addition to regulating free concentrations of steroid sex hormones in plasma, also participates in membrane-based steroid signaling in breast and prostate cells. In this study, we address whether the breast and prostate can synthesize their own SHBG. SHBG mRNA and protein were detected in human breast and prostate cancer cell lines, as well as in normal and cancerous breast and prostate tissue where it was prominent in epithelial cells. In cultured human LNCaP prostate cancer cells, SHBG was found both in the cytoplasm, and on the outer cell membrane where it was presumably bound to its high affinity receptor (RSHBG). When LNCaP cells were treated with 2-methoxyestradiol (2MeOE2), an antagonist of SHBG binding to RSHBG, membrane-associated SHBG was eliminated. These results demonstrate that the breast and prostate can synthesize SHBG locally, and that SHBG can bind to, and perhaps participate in autocrine and/or paracrine signaling through RSHBG. Therefore, perturbations in SHBG expression in breast and prostate tumors may affect the regulation of steroid signaling through RSHBG and target SHBG-mediated biologic properties at the cellular level.
