Abstract

This case study describes a patient who presented with vague visual complaints in the right eye, decreased visual acuity in the affected eye, and a difficult initial eye evaluation, including fundoscopic and slit lamp examinations, in the emergency department (ED). The preliminary finding included a darkened-appearing area of the retina on fundoscopic exam. The patient subsequently had bedside sonography of the eyes done by an emergency medicine (EM) intern which revealed a thin and serpentine strip appearing as a hyperechoic representation of the retina floating freely into the vitreous from the superior-lateral section of the posterior globe.

2. Subchorionic Hemorrhage Appearing as Twin Gestation on Endovaginal Ultrasound

Abstract

This case study describes a pregnant patient with vaginal bleeding who had a bedside endovaginal ultrasound in the emergency department (ED). The emergency physician identified a live intra-uterine pregnancy (IUP) with another structure that appeared to be a second gestational sac. The patient subsequently had an endovaginal ultrasound in the radiology department 46 minutes later. The attending radiologist described one live IUP and a subchorionic hemorrhage. Comparison of the ED and radiology ultrasound showed that the second structure, identified as a subchorionic hemorrhage, had significantly decreased in size. Endovaginal ultrasound in the evaluation of possible ectopic pregnancy is a useful bedside tool in the ED. We discuss a pitfall that can occur with endocavitary ultrasound when a twin gestation is presumed.

Abstract

Research and experimental development (R&D) is an important driver of economic growth and productivity. Gross expenditure on domestic R&D (GERD) is a country's total expenditure on R&D performed by all sectors of the economy. South Africa's GERD as a percentage of its GDP (GERD/GDP) remains below government targets and has stagnated over the past decade, largely due to declining business sector R&D. This paper aims to identify the drivers of firm-level R&D intensity, defined as the firm's R&D expenditure as a percentage of turnover. It is the first South African study to examine both micro- and macro-economic drivers of firm R&D, with a paucity of literature on this topic for middle-income countries. Using the South African National R&D Survey data, the study utilized the Generalized Method of Moments (GMM) model and found that public financial support for firms, smaller firm-size, firm-level collaboration, political stability, foreign direct investment, and public R&D investment are positively associated with firm R&D intensity. The results highlight the importance of public financial support for smaller firms and investments in higher education and research institutions in promoting firm-level R&D, thereby providing useful policy insights for boosting business sector and economy-wide R&D expenditure and reaching national GERD/GDP targets. 6. A quantitative analysis of obstacles to innovation among small, micro, and medium-sized enterprises: Evidence from South Africa.

Abstract

Firm-level innovation in sub-Saharan Africa remains under-researched. To address this gap, this paper examines the obstacles to innovation faced by small, micro, and medium-sized enterprises (SMMEs) in South Africa. Using the 2018 Business Innovation Survey, we apply the double-hurdle model to investigate factors determining a firm's decision to invest, and the intensity of innovation. Our results show that cost and institutional obstacles were significantly more important as disincentive factors for medium-sized enterprises while knowledge obstacles were significant disincentive factors for large enterprises. In addition, market obstacles hampered investment among large enterprises. Interestingly, none of these factors was found to deter innovation intensity among small and micro enterprises. The study findings suggest that there is no one-size-fits-all approach to innovation policy and intervention programmes to support innovation within firms of different sizes. [ABSTRACT FROM AUTHOR]

Abstract

Bluebird boxes were attached to the back of small road signs and are maintained by the maintenance sign crew with minimal time away from their primary duties. Because locations were chosen where bluebirds had already been seen, success has been 100% since the project started in 2001. In 2002, 120 bluebirds were fledged and approximately 120 tree swallows as well. So far in 2003 there have been 30 pair of nesting bluebirds. We have had more problems this year with English sparrows killing bluebird chicks. Last year two boxes suffered from raccoon predation but that has not been repeated since the sign poles were greased. Costs were for materials only, boxes were built by the winter night crews when they were not plowing.

Abstract

In 1998 The Nature Conservancy (TNC) made the Rochester regional office of the New York State Department of Transportation aware of the need for improved habitat for birds along Lake Ontario. The lake shore is internationally recognized as an important migratory route, nesting and wintering area for many bird species, both songbirds and birds of prey. The Lake Ontario State Parkway, with more than 30 miles of expansive right-of-way along the shoreline, presented an ideal opportunity for the department to respond, while enhancing the aesthetics of the corridor for the traveling public and adjacent land owners. Planning and design began in 1999 with construction in the spring of 2000. The project included restoring and enhancing native vegetation (meadow, shrub and trees) for habitat and food, and adding nest boxes for wood ducks, kestrels and other native cavity dwellers. Nest boxes are monitored through the creation of an ‘adopt a nest box’ volunteer program which has successfully kept European Starling activity to a minimum. Boxes have been used by six different native bird species. Seven hundred native shrubs and 236 fruit or mast bearing trees were planted. Forty-four acres of non-native scrub shrub was removed, with three acres replanted with native woody plants and the rest left open as meadow. Since 2001, the 41-acre R.O.W. area is being managed so that mowing occurs once after September 1, which has improved nesting success for Savannah and Grasshopper Sparrows, Bobolink and Meadowlark. Upland Sandpiper nested there until 1999, and it is hoped they will return. The department is currently revising mowing management practices statewide, in part based on the success of this project. The NYSDOT partnered with TNC, NYS Department of Environmental Conservation, NYS Office of Parks, Recreation, and Historic Preservation, Cornell University, Braddock’s Bay Raptor Research Center, Braddock’s Bay Bird Observatory, Genesee Ornithological Society, Town of Greece, Otetiana Boy Scout Council and DOT staff volunteers through planning and implementation phases of this ongoing project. For a relatively small investment of time and money, the department has made a noticeable improvement in the environment and won accolades from our many partners.

Abstract

BackgroundThe World Trade Center (WTC) attacks on 11 September 2001 created a hazardous environment with known and suspected carcinogens. Previous studies have identified an increased risk of prostate cancer in responder cohorts compared with the general male population.ObjectivesTo estimate the length of time to prostate cancer among WTC rescue/recovery workers by determining specific time periods during which the risk was significantly elevated.MethodsPerson-time accruals began 6 months after enrolment into a WTC cohort and ended at death or 12/31/2015. Cancer data were obtained through linkages with 13 state cancer registries. New York State was the comparison population. We used Poisson regression to estimate hazard ratios and 95% CIs; change points in rate ratios were estimated using profile likelihood.ResultsThe analytic cohort included 54 394 male rescue/recovery workers. We observed 1120 incident prostate cancer cases. During 2002–2006, no association with WTC exposure was detected. Beginning in 2007, a 24% increased risk (HR: 1.24, 95% CI 1.16 to 1.32) was observed among WTC rescue/recovery workers when compared with New York State. Comparing those who arrived earliest at the disaster site on the morning of 11 September 2001 or any time on 12 September 2001 to those who first arrived later, we observed a positive, monotonic, dose-response association in the early (2002–2006) and late (2007–2015) periods.ConclusionsRisk of prostate cancer was significantly elevated beginning in 2007 in the WTC combined rescue/recovery cohort. While unique exposures at the disaster site might have contributed to the observed effect, screening practices including routine prostate specific antigen screening cannot be discounted.

16. Changes in Cancer Incidence Patterns among a Northeastern American Indian Population: 1955-1969 versus 1990-2004

Abstract

Purpose: This manuscript examines shifts in patterns of cancer incidence among the Seneca Nation of Indians (SNI) for the interval 1955-1969 compared to 1990-2004. Methods: A retrospective cohort design was used to examine cancer incidence among the SNI during 2 time intervals: 1955-1969 and 1990-2004. Person-years at risk were multiplied by cancer incidence rates for New York State, exclusive of New York City, over 5-year intervals. A computer-aided match with the New York State Cancer Registry was used to identify incident cancers. Overall and site-specific standardized incidence ratios (SIRs = observed/expected x 100), and 95% confidence intervals (CIs), were calculated for both time periods. Results: During the earlier interval, deficits in overall cancer incidence were noted among males (SIR = 56, CI 36-82) and females (SIR = 71, CI 50-98), and for female breast cancers (SIR = 21, CI 4-62). During the more recent intervals, deficits in overall cancer incidence persisted among both genders (males SIR = 63, CI 52-77; females SIR = 67, CI 55-80). Deficits were also noted among males for cancers of the lung (SIR = 60, CI 33-98), prostate (SIR = 51, CI = 33-76) and bladder (SIR = 17, CI = 2-61) and among females for breast (SIR = 33, CI = 20-53) and uterus (SIR = 36, CI = 10-92). No cancer sites demonstrated increased incidence. Persons ages 60-69 years, 70-79 years, and ages 80+ years tended to exhibit deficits in overall incidence. Conclusions: Despite marked changes over time, deficits in overall cancer incidence have persisted between the time intervals studied. Tribal-specific cancer data are important for the development and implementation of comprehensive cancer control plans which align with local needs.

18. Early COVID-19 Hospitalizations Among New York State Residents with a History of Invasive Cancer

BACKGROUND: Individuals with a history of cancer may be more susceptible to severe COVID-19 due to immunosuppression, comorbidities, or ongoing treatment. We linked inpatient claims data on COVID-19 hospitalizations to cancer diagnoses from the New York State Cancer Registry (NYSCR) to examine associations between prior cancer diagnoses and hospitalizations for COVID-19, and factors associated with death at discharge after COVID-19 hospitalization. METHODS: New York State (NYS) residents diagnosed with invasive cancer before July 1, 2021, who were alive on January 1, 2020, were identified from NYSCR data. We obtained claims data for discharge year 2020 and the first half of 2021 from NYS's Statewide Planning and Research Cooperative System (SPARCS), and we linked inpatient records with COVID-19 as the primary diagnosis to cancer data from the NYSCR using deterministic matching methods. We calculated descriptive statistics and conducted multivariable-adjusted logistic regression analyses to examine associations of cancer case characteristics with COVID-19 hospitalization and with vital status at discharge among patients with a history of cancer. All analyses were conducted in SAS 9.4. RESULTS: Our analysis included 1,257,377 individuals with a history of cancer, 10,210 of whom had a subsequent primary COVID-19 hospitalization. Individuals with a history of cancer were 16% more likely to be hospitalized with COVID-19, compared to the general population of NYS, after adjusting for age and sex (95% CI, 14%-19%). Factors independently associated with COVID-19 hospitalization among cancer patients included older age, male sex, non-Hispanic Black race or Hispanic ethnicity, diagnosis with late-stage cancer or with multiple tumors, more recent cancer diagnosis, and New York City (NYC) residency at the time of cancer diagnosis. Factors independently associated with death at discharge among individuals with COVID-19 hospitalization and a prior cancer diagnosis included older age, male sex, non-Hispanic Black or non-Hispanic Asian/Pacific Islander race or Hispanic ethnicity, residence in NYC at the time of COVID-19 hospitalization, and an active cancer diagnosis claim code at the time of COVID-19 hospitalization. CONCLUSION: This claims-based study identified higher risks of COVID-19 hospitalization and death at discharge among individuals with a history of cancer, and particularly those in certain demographic and diagnostic groups.

34. Analysing the role of language in the context of education, employment and income in South Africa

Although most of the South African population speak an African language as their home language, English remains the lingua franca and continues to dominate economic and political life. This thesis explores the role of language in the context of the labour market, the schooling system and survey data collection primarily using data from the National Income Dynamics Study (NIDS) survey. Firstly, I assess the impact of English proficiency among African home language speakers on employment probabilities and wages. After accounting for endogeneity and measurement error in the employment and wage models, I estimate that being proficient in English is associated with a 23-25 percentage point increase in the probability of employment, and a wage premium of 33 percent. These results contribute to the limited research on this topic in South Africa. Secondly, I explore the extent to which English and Afrikaans dominate as the Languages of Learning and Teaching (LoL T's) in the foundation phase of schools where a share of the learners speak an African language as their home language. Despite overwhelming evidence that mother-tongue instruction in the early years of schooling is superior pedagogically, parental preferences towards it remain low and I find that most schools in urban areas still choose to teach in English or Afrikaans. Mother-tongue instruction tends to occur in rural areas, or poorer urban communities, and where learners within a school come from relatively similar language backgrounds. Thus, when devising implementation plans to assist schools in adopting mother-tongue instruction, there may be a need for different strategies across different areas and schools. Finally, l investigate the role of language in determining the quality of the data that are used to investigate the phenomena in the first two sections. Through a multilevel analysis of the NIDS survey data, I find that when the interviewer and respondent are from the same language group, the respondent is more likely to participate in the survey. This is an important methodological finding as it implies that matching interviewers and respondents according to language may have a positive impact on survey response and hence the representativity of the data.

Background: Statistically significantly increased cancer incidence has been reported from 3 cohorts of World Trade Center (WTC) disaster rescue and recovery workers. We pooled data across these cohorts to address ongoing public concerns regarding cancer risk 14 years after WTC exposure.Methods: From a combined deduplicated cohort of 69 102 WTC rescue and recovery workers, a sample of 57 402 workers enrolled before 2009 and followed through 2015 was studied. Invasive cancers diagnosed in 2002-2015 were identified from 13 state cancer registries. Standardized incidence ratios (SIRs) were used to assess cancer incidence. Adjusted hazard ratios (aHRs) were estimated from Cox regression to examine associations between WTC exposures and cancer risk.Results: Of the 3611 incident cancers identified, 3236 were reported as first-time primary (FP) cancers, with an accumulated 649 724 and 624 620 person-years of follow-up, respectively. Incidence for combined FP cancers was below expectation (SIR = 0.96, 95% confidence interval [CI] = 0.93 to 0.99). Statistically significantly elevated SIRs were observed for melanoma-skin (SIR = 1.43, 95% CI = 1.24 to 1.64), prostate (SIR = 1.19, 95% CI = 1.11 to 1.26), thyroid (SIR = 1.81, 95% CI = 1.57 to 2.09), and tonsil (SIR = 1.40, 95% CI = 1.00 to 1.91) cancer. Those arriving on September 11 had statistically significantly higher aHRs than those arriving after September 17, 2001, for prostate (aHR = 1.61, 95% CI = 1.33 to 1.95) and thyroid (aHR = 1.77, 95% CI = 1.11 to 2.81) cancers, with a statistically significant exposure-response trend for both.Conclusions: In the largest cohort of 9/11 rescue and recovery workers ever studied, overall cancer incidence was lower than expected, and intensity of WTC exposure was associated with increased risk for specific cancer sites, demonstrating the value of long-term follow-up studies after environmental disasters.
