2. Palmoplantar Psoriasis: Epidemiological and Clinical Features and Impact on Quality of Life

Introduction: Palmoplantar psoriasis is a rare variant of psoriasis. Its impact on quality of life have been poorly investigated. Objectives: Our aim was to investigate the prevalence of localized palmoplantar psoriasis (PPP), to assess its severity using the modified-Palmoplantar Pustular Psoriasis Area and Severity Index (m-PPPASI), and to evaluate the correlation with the Dermatology Life Quality Index (DLQI). Methods: We conducted a descriptive study with prospective data-collection from January to June 2021. We enrolled 223 patients with psoriasis. We excluded patients without palmar/plantar involvement, and those with a body surface area (BSA) greater than or equal to 10%. Results: We included 33 patients with PPP. This corresponded to a 14.8% prevalence among all psoriasis phenotypes. The mean age was 45 years. The male to female ratio was 2.3. Pruritus was present in 27 cases. The mean m-PPPASI was 11.77. The mean DLQI was 8.33. A significant correlation was found between DLQI and m-PPPASI: Mean DLQI scores for patients with m-PPPASI less than or equal to 10 and m-PPPASI greater than 10 were 5.6 and 11.3, respectively (p=0.002). Only 11.8% of patients with m-PPPASI less than or equal to 10 had a DLQI greater than 10, whereas 50% of those with m-PPPASI greater than 10 had a DLQI greater than 10 (p=0.026). Conclusions: Based on the present work, we confirm that, although affecting a reduced BSA, PPP is a severe form of psoriasis. We consider the m-PPPASI to be a reliable tool which can be used to assess the severity of PPP.

Drug reaction with eosinophilia and systemic symptoms (DRESS)/drug-induced hypersensitivity syndrome (DIHS) is a life-threatening, multi-organ adverse drug reaction with a mortality rate of approximately 10 %–20 %. The most common culprit drugs are anticonvulsants, some antibiotics such as dapsone and minocycline, salazosulfapyridine, allopurinol and some antiretroviral molecules such as abacavir and nevirapine. Only one case of DRESS induced by sildenafil has been reported in the literature. Here we report a new case.

Abstract SARS‐CoV‐2, the virus that causes coronavirus disease 2019 (COVID‐19), is associated with flares of psoriasis in patients with well‐documented disease. Both viral infection and medications used for treatment, like hydroxychloroquine, were incriminated. Herein, we report the case of a 25‐year‐old male patient who presented a first‐onset guttate psoriasis following a COVID‐19 infection. We have not found previous descriptions of de novo guttate psoriasis post‐COVID‐19.

11. An unusual case of nasal chromoblastomycosis degenerating into squamous cell carcinoma from a nonendemic region

Chromoblastomycosis (CBM) is a granulomatous mycosis rarely described outside tropical countries. Degeneration into squamous cell carcinoma (SCC) is its most serious complication. We report the first case of nasal CBM degenerating into SCC. In 2006, a sixty-year-old male presented himself with an infiltrated plaque on the right thigh. The diagnosis of CBM was confirmed by the presence of fungal elements. In 2019, the patient had developed a mass coming from the right nasal cavity. It had rapidly involved the nasal dorsum. An ulcer-budding nasal tumor and an elevated erythematous and verrucous plaque on the thigh were noted. A biopsy revealed a granulomatous dermis with fungal elements. Other nasal biopsy fragments showed differentiated SCC. A fungal culture inoculated with tissue from both lesions showed dark colonies. The diagnosis of nasal CBM with SCC degeneration was reached. The patient presented asymptomatic endonasal CBM that had slowly evolved and recently degenerated.

Cases of bullous pemphigoid following Covid-19 vaccines have been recently reported. We describe herein an atypical case of acral bullous pemphigoid after Pfizer BionTech vaccine. Clinicians should be aware of this reaction and encourage full vaccination as the disease displays a rapid improvement treatment.

13. Assessing the validity and interpretability of the Simplified Psoriasis Index in Tunisian patients

Multiple scores have been developed to assess the severity of psoriasis, but these scores have many limitations. The Simplified Psoriasis Index (SPI) is a summary score with separate components for current severity (SPI-s), psychosocial impact (SPI-p), and past history and interventions (SPI-i). It is available in two similar versions: proSPI and saSPI.To assess the validity of the SPI by studying its correlation to the benchmark scores in Tunisian patients.It was a prospective bicentric study including 80 patients with plaque psoriasis.The median PASI was 7.6 and the median DLQI was 9. The median proSPI-s was 6 and the median saSPI-s was 8. The median SPI-p was 7. The median SPI-i was 2. There was a strong correlation between the proSPI-s and PASI (r=0.87) and between the proSPI-s and saSPI-s (r=0.82). There was a medium correlation between saSPI-s and PASI (r=0.70) and between SPI-p and DLQI (r=0.67). The threshold value for proSPI-s and saSPI-s was 7.25. The threshold value for SPI-p was 6.5.The SPI aims to provide a concise but global measure of the severity and impact of psoriasis on quality of life. The use of SPI has several advantages: the simplicity of use, the additional weight given to critical locations of psoriasis, the possibility for the patient to self-assess his own disease, and the possibility of evaluating all the dimensions of psoriasis at the same time.

Psoriasis is a chronic inflammation of the skin, affecting approx. 120 million people worldwide. The risk of developing skin cancer in psoriatic patients has been stressed, although with a significant amount of conflicting data in the literature. The most consistent findings suggest that psoriatic patients are at an increased risk of non-melanoma skin cancer. An increased risk of melanoma has not been well established. Herein, we report the case of a patient with a history of pustular psoriasis present since the age of ten, who developed an SCC and a melanoma, and discuss the etiopathology of this association.

15. Factors associated with acute and recurrent erysipelas in a young population: a retrospective of 147 cases

erysipelas is a common infection of the superficial layer of the skin, predominantly caused by groups A β-hemolytic streptococci. It is an acute infection of the skin and frequently affects the legs. It is common in the elderly and favoured by the associated comorbidities. Its occurrence in young healthy people is rare.The present study aimed to elucidate factors associated with acute and recurrent erysipelas in a young population.We retrospectively analyzed 147 cases of erysipelas admitted to the dermatology department of the Military Hospital of Tunis, Tunisia, over 18 years, identifying factors associated with recurrence. All patients were aged less than 35 years.During the study period, 147 patients were registered with the diagnosis of erysipelas. There were 125 military soldiers and 22 non-military patients. The prevalence of erysipelas was 2.23%. The median age was 25 years. Almost 86.2% of patients were male. The main favorable factors were: obesity (9%), alcoholism (8%), chronic venous insufficiency (6.5%), chronic lymphedema (3%), leg fracture (2%), and diabetes mellitus (1%). The lesions were mostly located in the lower limbs in 94.9%. According to our multivariate analysis, there was an association between recurrence and diabetes mellitus (p=0.02), female sex (p=0.004), onychomycosis (p=0.004), and plantar dyshidrotic eczema (p0.005).Identifying factors associated with recurrent erysipelas in a young population remains essential for proposing primary and secondary prevention measures.

Les teignes du cuir chevelu (TCC) constituent la principale mycose superficielle de l´enfant avant la puberté. La distribution des dermatophytes en cause change avec le temps et varie d´un pays à l´autre. Le but de ce travail était d´étudier les caractéristiques épidémiologiques et mycologiques des TCC dans la région de Tunis. Notre étude est rétrospective ayant porté sur 474 patients adressés pour prélèvement mycologique du cuir chevelu entre janvier 2012 et décembre 2020. Pour chaque patient, une fiche de renseignement clinique a été remplie et un prélèvement mycologique a été réalisé. L´identification des dermatophytes isolés a reposé sur des critères macroscopiques et microscopiques des colonies. L´identification moléculaire a été réalisée pour 4 isolats parPolymerase Chain Reaction(PCR) en temps réel en utilisant le kit DermaGeniu®2.0. Nous avons colligé 210 prélèvements positifs soit une prévalence de 44,3% (n=210). Les patients de sexe masculin sont plus fréquemment atteints par les TCC (81%, n=170). L´âge moyen des patients est de 6,2±3,4 ans. Les teignes tondantes à grande plaque étaient prédominantes (88%, n=184). La sensibilité de l´examen direct était de 87% (n=182). Il s´agissait d´un parasitisme ectothrix microsporique dans (79%, n=166) des cas et endothrix trichophytique dans (7%, n=14) des cas. La culture était positive dans (98%, n=207) des cas et isolait cinq espèces de dermatophytes:Microsporum canis(87%, n=182),Trichophyton violaceum(9%, n=19).Trichophyton mentagrophytes var mentagrophytes(3%, n=6),Microsporum gypseum(0,5%, n=1) etTrichophyton verrucosum(0,5%, n=1). Notre étude montre l´émergence des dermatophytes zoophiles particulièrementM. canis. L´examen mycologique est essentiel pour la confirmation du diagnostic, la surveillance épidémiologique des dermatophytes selon les régions et la prise en charge thérapeutique.

Onychomatricoma (OM) is a rare benign tumour of the nail matrix characterized by specific clinical and histologic features. The main clinical signs are thickening of the nail plate, xanthonychia, overcurvature of the nail plate, and multiple splinter haemorrhages. The diagnosis is based on clinical, radiological and histopathological findings. Histologically, the tumour is characterized by filiform epithelial projections. The objective of this study is to present the first reported Tunisian case of OM, focusing on the contribution of magnetic resonance imaging to the diagnosis of OM. A review on the subject is also presented.

18. Nail Involvement in Psoriatic Patients and Association with Onychomycosis: Results from a Cross-Sectional Study Performed in a Military Hospital in Tunisia

Nail psoriasis has variable prevalence and heterogeneous aspects. Many of them could mimic onychomycosis (OM). It has been suggested that patients with nail psoriasis are at high risk of OM. The aim of our study was to determine the epidemiological and clinical characteristics of nail psoriasis and to estimate the frequency and the factors associated with OM in psoriatic patients. The studied group included 163 patients with psoriasis aged 18 years or older. Epidemiological and clinical data, as well as the severity of skin and nails disease by evaluating the Psoriasis Area Severity Index (PASI) and Nail Area Psoriasis Severity Index (NAPSI) scores were specified. Mycological testing was performed for patients with nail alterations. Nail involvement was found in 71.2% of patients. The most common nail alterations were subungual hyperkeratosis and onycholysis. The mean NAPSI score was 11.6. Mycological testing was performed in 104 patients with onychodystrophy. OM was diagnosed in 53% of the cases. Dermatophytes were the most isolated pathogens. OM was associated with male gender, but not with age, NAPSI, or PASI score. Psoriasis is one of the dermatoses that most commonly affect the nail. Available data about the association between nail psoriasis and OM are controversial. However, mycological testing should be routinely performed on psoriatic nails.

24. Syndrome de Graham Little-Piccardi-Lassueur : une variante rare du lichen plan pilaire induit par le nilotinib

Introduction Le nilotinib est un inhibiteur specifique de l’activite de la tyrosine kinase de 2e generation. Il constitue une alternative therapeutique efficace pour la leucemie myeloide chronique (LMC) en cas d’intolerance ou resistance a l’imatinib. Les toxidermies constituent son principal effet indesirable. Nous rapportons le cas d’une eruption a type de syndrome de Graham Little-Piccardi-Lassueur (SGLPL) chez une patiente traitee par nilotinib. Observation Une patiente âgee de 49 ans est suivie depuis 2 ans pour LMC. Elle a ete initialement traitee par imatinib avec une mauvaise reponse therapeutique. Elle a ete mise alors en septembre 2020 sous Nilotinib. Six semaines apres, elle a presente une eruption cutanee prurigineuse generalisee. L’examen clinique a montre des petites papules folliculaires brunes et keratosiques, diffuses touchant le visage, le cou, le tronc et les extremites. L’examen du cuir chevelu revelait une plaque alopecique cicatricielle de 3 cm de grand axe au niveau du vertex, d’extension centrifuge, avec erytheme et squames peripilaires en peripherie de la plaque. Par ailleurs, elle presentait une hypotrichose des aisselles et du pubis sans atrophie. La biopsie cutanee, faite au niveau du cuir chevelu, etait en faveur d’un lichen plan pilaire. Devant ce tableau, le diagnostic du SGLPL etait porte. La correlation chronologique entre l’introduction du traitement et l’apparition de l’eruption a permis d’incriminer le nilotinib. La substitution du nilonitib n’etait pas possible. Une legere amelioration du prurit etait obtenue sous traitement keratolytique. Discussion Les toxidermies sous nilotinib sont tres frequentes, benignes mais genantes. Elles touchent specifiquement le follicule pileux. On a decrit dans la litterature des cas de keratose pilaire, de lichen plan pilaire avec erytheme et squames perifolliculaires et alopecie cicatricielle du cuir chevelu, des cas d’alopecie des sourcils, de rarefaction de la pilosite et de prurit localise ou diffus. Ces manifestations cutanees n’ont pas ete observees sous imatinib. Cela pourrait etre explique par le spectre d’action plus large du nilotinib inhibant les autres tyrosines kinases qui seraient presentes dans les follicules pileux. Notre cas est particulier car il decrit la forme clinique la plus rare du lichen plan folliculaire, le SGLPL defini par la triade : alopecie cicatricielle du cuir chevelu associee a une keratose pilaire et une alopecie non cicatricielle des aisselles et du pubis avec prurit. Il se voit surtout chez la femme adulte. Sa pathogenie est mal elucidee. Notre cas differe de la forme idiopathique par l’apparition premiere de la keratose pilaire. Conclusion Une meilleure connaissance de la physiopathologie de l’atteinte folliculaire specifique et frequente sous nilotinib permettrait d’expliquer l’etiopathogenie du lichen plan pilaire.

27. Chromomycose nasale compliquée de carcinome épidermoïde

Introduction La chromomycose (CM) est une infection fongique cutanee d’evolution lente rarement decrite en dehors des pays tropicaux qui peut se compliquer de carcinome epidermoide (CE). Nous rapportons un cas tunisien exceptionnel de CM nasale compliquee de CE. Observations Un homme de 59 ans, masseur dans un bain turc, nous a consulte en 2006 pour une plaque infiltree de la cuisse droite. Le diagnostic de CM a ete porte devant la presence de cellules fumagoides a l’examen histologique et mycologique. Il n’y avait pas de notion de sejour en zone d’endemie. Il etait traite par antifongique avec bonne evolution mais etait perdu de vue. En 2019, il developpait une masse charnue au niveau de la cavite nasale droite. Devant l’extension rapide de la tumeur infiltrant le dorsum nasal il nous consultait. L’examen montrait une tumeur nasale ulcero-bourgeonnante a bords irreguliers surmontee de telangiectasies. L’ancienne lesion de la cuisse apparaissait comme une plaque infiltree de 9 cm erythemateuse keratosique avec des zones atrophiques. Cliniquement, on evoquait les diagnostics de CE nasal et de CM nasale. L’examen histologique des prelevements du nez et de la cuisse montrait une inflammation dermique polymorphe avec reaction granulomateuse. Les cellules fumagoides etaient identifiees seulement au niveau de la cuisse. D’autres fragments endonasaux et du dorsum nasal revelaient un CE moyennement differencie. L’etude mycologique des pieces d’origine nasale et de la cuisse objectivait des corps fumagoides a l’examen direct et isolait Fonsecaea pedrosoi a la culture. Le diagnostic de CM nasale degeneree en CE etait retenu. Le bilan d’extension etait negatif. Le patient etait traite par terbinafine et itraconazole et adresse au service maxillo-facial pour prise en charge carcinologique. Discussion La CM est presente essentiellement dans les regions tropicales et subtropicales. En Tunisie, 4 cas seulement ont ete rapportes. Elle est contractee par inoculation de champignons pigmentes du groupe des dematies lors d’un traumatisme par des eclats de bois contamines. Elle siege au niveau des zones decouvertes principalement la jambe et le pied. La pratique du sauna a aussi ete incriminee comme source de contamination. C’est le cas probablement de notre patient masseur dans un bain turc. Ceci explique le siege de sa lesion initiale au niveau d’une zone couverte. Quant a l’atteinte nasale, elle est exceptionnellement decrite comme site de CM. Dans notre cas elle pourrait etre expliquee par l’auto-inoculation liee au grattage. L’evolution de la CM est lente et asymptomatique. Initialement decrite en 1968, on compte aujourd’hui une vingtaine de cas de CM compliquee de CE. Notre patient aurait presente une CM nasale asymptomatique ayant lentement evolue compliquee de CE.

Verrucous carcinoma (VC) of the nail bed is a rare variant of squamous cell carcinoma that is often misdiagnosed as a benign condition. The clinical presentation of this tumor is very similar to that of warts or onychomycosis hence the delayed appropriate treatment. Its association with human papillomavirus (HPV) infection has rarely been reported. The treatment of VC of the nail unit depends on the extent of the lesion and the presence or the absence of bone. We here report an unusual case of VC of the nail bed of the left big toe in a man associated with HPV53 infection that had been mistaken for a wart for 1 year. The condition was treated by ray amputation.

30. Intérêt du Simplified Psoriasis Index dans l’évaluation de la sévérité du psoriasis : résultats d’une étude prospective tunisienne

Introduction L’indice simplifie du psoriasis ou « Simplified Psoriasis Index » (SPI) est un score composite qui comprend trois variables distinctes : la severite actuelle du psoriasis (SPI-s), son impact psychosocial (SPI-p), et l’historique de la maladie (SPI-i). Le SPI est decline en deux versions similaires : l’une est remplie par le medecin (proSPI) et la deuxieme par le patient (saSPI). Le but de notre etude etait d’evaluer la validite du SPI en etudiant sa correlation aux scores de reference (le Psoriasis Area and Severity Index (PASI) et le Dermatology Life Quality Index (DLQI)) chez des patients tunisiens. Materiel et methodes Etude prospective bi-centrique, menee entre avril 2018 et avril 2019, incluant des patients adultes atteints de psoriasis en plaques. Les formulaires PASI et proSPI ont ete remplis par le medecin dans leurs versions en langue francaise. Les patients ont ensuite ete invites a completer les formulaires DLQI et saSPI dans leurs versions validees en langue arabe. Les valeurs seuils du SPI ont ete determinees en se referant aux valeurs seuils de severite du PASI (PASI > 10) et du DLQI (DLQI > 10). Resultats Quatre-vingt patients ont ete inclus, d’âge moyen 45,8 ± 14,8 ans. Une predominance masculine etait notee (sex ratio H/F a 1,35). La duree d’evolution du psoriasis etait de 9,3 ± 8,5 ans. Le PASI moyen etait de 9,4 ± 7,2. Dans 40 % des cas, celui-ci etait superieur a 10. Le DLQI moyen etait de 10,2 ± 5,8. Dans 47,5 % des cas, le DLQI etait superieur a 10. Le proSPI-s moyen etait de 8 ± 6,7 et le saSPI-s moyen etait de 10,2 ± 7,7. Le SPI-p moyen etait de 6,2 ± 2,7. Le SPI-i moyen etait de 1,9 ± 1,6. Il existait une correlation forte entre les scores proSPI-s et PASI (rho = 0,87 ; p Discussion Les versions traduites en langue francaise et en langue arabe du SPI ont recemment ete validees. Nos resultats sont compatibles avec ceux des series internationales evaluant le SPI. Dans notre etude, nous avons determine pour la premiere fois les valeurs seuils du SPI permettant de definir un psoriasis severe. Au vu de nos resultats et des donnees de la litterature, le SPI est un outil pratique qui presente plusieurs avantages : la simplicite d’utilisation, la ponderation supplementaire accordee aux localisations critiques du psoriasis sur le plan fonctionnel et psychosocial, la possibilite pour le patient de s’autoevaluer, et la possibilite d’evaluer a la fois toutes les dimensions du psoriasis dispensant de la necessite d’utiliser plusieurs outils differents. D’autres etudes sont necessaires afin d’evaluer la concordance inter-observateurs du SPI.

32. La gale en milieu militaire : profil épidémio-clinique sur dix ans

Introduction La gale est une ectoparasitose prurigineuse contagieuse qui sevit selon un mode endemique dans les collectivites. Le but de ce travail est de decrire les caracteristiques epidemiologiques, cliniques et evolutives de la gale en milieu militaire. Materiel et methodes Etude retrospective sur dix ans (janvier 2008 a decembre 2017) portant sur tous les patients militaires consultant pour une gale a l’hopital militaire de Tunis. Le diagnostic etait clinique et anamnestique. Resultats Nous avons collige 432 patients (430 hommes et 2 femmes). L’âge moyen etait de 24,74 ans (19 ans a 53 ans). La repartition durant l’annee etait variable : automne : 142 cas, hiver : 120, ete : 98 et printemps : 72 cas. Tous les patients avaient un prurit. La recrudescence nocturne etait notee chez 60 % des cas. La topographie du prurit etait variable, tronc (90 %), dos (8 %) et genital (2 %). Les symptomes evoluaient depuis 29 jours en moyenne (1 a 195 jours). L’examen objectivait des lesions de grattage dans 56 % des cas, des nodules scabieux (46 %), des sillons scabieux (5 %), des vesicules perlees (17 %) et un chancre scabieux (8 %). Des symptomes similaires dans l’entourage etaient rapportes dans 14 % des cas. Le traitement etait deux badigeons de benzoate de benzyle (BB) 25 %, a 24 heures d’intervalle, recu en milieu hospitalier pour 312 malades. L’evolution etait marquee par la guerison chez 92 % des patients ; 8 % n’ont pas repondu au traitement. Les complications etaient a type d’impetiginisation (22 cas) et d’eczematisation (39 cas). La recidive etait notee dans 14 % des cas (1 a 4 episodes). Les jours totaux d’absenteisme etaient estimes a 2,6 jours/malade. Discussion La gale est frequente en milieu militaire, la promiscuite en constituant le premier facteur de risque. L’incidence trouvee pourrait etre sous-estimee en rapport avec une prise en charge en dehors du milieu hospitalier. La nette predominance masculine s’explique par la forte proportion des hommes exercant en milieu militaire. Nos resultats confirment les donnees de la litterature montrant un pic de frequence pendant les saisons froides et humides. Le prurit generalise de topographie anterieure, epargnant le dos, est le maitre symptome. La recrudescence nocturne, habituellement decrite, est trouvee dans notre etude. La faible specificite du tableau clinique est a l’origine de difficultes diagnostiques et d’un retard de prise en charge. Le BB, restant le seul traitement disponible dans notre pays, a montre son efficacite et son innocuite. L’echec therapeutique pourrait s’expliquer par une mauvaise application ou par une insuffisance de prise en charge des sujets contacts. Les recidives, a distance de l’episode initial, sont dues une nouvelle contamination. Conclusion La prise en charge precoce de la gale reduit la duree et la frequence d’absenteisme. Dans notre experience, deux badigeons de BB a 24 heures d’intervalles semblent etre aussi efficaces que 3 badigeons et permettent d’eviter un prurit de duree prolonge.

Introduction Une prevalence elevee du syndrome metabolique (SM) et sa manifestation hepatique, la steatose hepatique non alcoolique (SHNA) pouvant evoluer vers la fibrose hepatique (FH), ont ete rapportees au cours du psoriasis. Des etudes recentes ont montre qu’en plus du SM et de la prise de methotrexate (MTX), d’autres facteurs, lies au psoriasis, sont impliques dans la survenue de la FH. Le but de notre etude etait de comparer les caracteristiques des patients psoriasiques avec et sans FH. Materiel et methodes Il s’agissait d’une etude transversale colligeant les patients consultant pour psoriasis dans le service de dermatologie de l’hopital militaire de Tunis entre septembre 2016 et mai 2017. Chaque patient a ete examine au plan dermatologique et anthropometrique ; ont ete realisees egalement une echographie hepatique et une elastometrie hepatique. Etaient exclus les malades ayant une hepatopathie chronique, hepatite auto-immune ou hepatite B/C, les malades sous traitement hepatotoxique autre que le MTX et les alcooliques. Etaient considerees comme formes severes, le psoriasis en plaques avec PASI > 10, le psoriasis pustuleux generalise, les psoriasis, erythrodermique et arthropathique. La FH etait definie par un degre d’elasticite du foie (E) > 7 kPa. Les facteurs associes a un E > 7 kPa au seuil p ≤ 0,05 ont ete retenus comme facteurs de risque de FH au cours du psoriasis. Resultats Nous avons inclus 83 patients (58 hommes/25 femmes), d’âge moyen de 46,4 ans. L’indice de masse corporelle et le tour de taille moyenne etaient respectivement de 26,3 kg/m2 et 96 cm. Le psoriasis etait severe chez 45 % des malades. Le diabete, l’hypertension arterielle, la dyslipidemie, le SM et la steatose hepatique etaient presents chez, respectivement, 20 %, 9,6 %, 28 %, 37 % et 35 % des patients. Le MTX etait prescrit chez 44,5 % des malades avec une dose cumulee moyenne de 1,26 g. E mesure par l’elastometrie hepatique etait en moyenne de 5,4 kPa. La prevalence de la FH etait de 22 %. L’âge et la menopause etaient significativement associes a la FH. Le surpoids, l’obesite abdominale et le SM etaient significativement plus frequents chez les malades ayant une FH. La severite du psoriasis, le PASI, le psoriasis arthropathique, la duree d’evolution et l’âge de debut n’etaient pas associes a une atteinte hepatique. La dose cumulee de MTX etait plus elevee chez les patients avec FH sans difference significative (p = 0,055). Discussion Notre etude confirme la forte prevalence de fibrose et de steatose hepatique chez les psoriasiques. L’âge et la menopause augmentent ce risque, explique par la frequence du SM dans la population tunisienne. Contrairement aux caracteristiques cliniques du psoriasis, les facteurs metaboliques semblent impliques dans la survenue de la FH. Conclusion Le depistage d’une FH chez les psoriasiques semble necessaire, surtout en cas de comorbidites metaboliques.

Necrobiosis lipoidica is an idiopathic dermatological condition that is strongly associated with diabetes mellitus. It is more commonly seen in women than men. The average age of onset is 30-40 years. Necrobiosis Lipoidica diabeticorum is an extremely rare finding in childhood diabetes. We describe the case of a 13-year-old girl who has had type 1 diabetes mellitus since she was 8 years old. The patient presented with 2 well-defined, persistent plaques with a depressed central area and elevated purple peripheral ring, one on the right thigh and the other over the lateral left leg. Histopathologic evaluation of the patient's biopsy confirmed the diagnosis of necrobiosis lipoidica with transfollicular elimination. Our patient is the second pediatric case described with perforating necrobiosis lipoidica. We review the literature and discuss clinical features, several complications, and the most recent treatment options for necrobiosis lipoidica in diabetic children.

Buschke-Löwenstein tumor (BLT), or giant condyloma acuminatum, is a rare sexually transmitted disease. The virus responsible for condyloma is human papillomavirus, usually serotype 6 or 11. A BLT is always preceded by condyloma acuminatum and may occur at any age after puberty. It is characterized by invasive growth and recurrence after treatment, and malignant transformation is possible. We report the case of a 44-year-old male patient with a 2-year history of a penoscrotal Buschke-Löwenstein tumor.
