Abstract

Backgrounds: Remote cardiac rehabilitation has proven useful in patients with cardiovascular disease; however, the methodology had not been fully validated. This study aimed to investigate the efficacy and safety of remote cardiac rehabilitation (RCR) with real-time monitoring and an ergometer using a bidirectional communication tool during the recovery phase of cardiovascular diseases. Methods: This multicenter, nonrandomized, interventional study was conducted at 29 institutions across Japan and enrolled patients with cardiovascular diseases who met indications for cardiac rehabilitation (CR) after receiving in-hospital treatment. The RCR group exercised at home using an ergometer and was monitored in real-time using interactive video and monitoring tools for 2–3 months. Educational instructions were provided concurrently through e-learning approaches. The safety of the RCR protocol and the improvement in peak oxygen consumption (VO2) were compared with those of the historical control group that participated in center-based CR. Results: Fifty-three patients from the RCR group were compared with 103 historical controls having similar background characteristics. No patients in RCR experienced significant cardiovascular complications while engaging in exercise sessions. After 2–3 months of RCR, the peak VO2 improved significantly, and the increases in the RCR group did not exhibit any significant differences compared to those in the historical controls. During follow-up, the proportion of patients whose exercise capacity increased by 10% or more was also evaluated; this finding did not indicate a statistically significant distinction between the groups. Conclusions: RCR during the recovery phase of cardiovascular diseases proved equally efficient and safe as center-based CR.

44. 日本人男女におけるパノラマX 線写真上の指標と骨粗鬆症診断および骨粗鬆症性骨折との関係

Abstract

【目的】パノラマX 線写真による骨粗鬆症スクリーニング指標は腰椎や大腿骨頚部などの骨密度、あるいは骨代謝マーカーと密接に関係することが世界中で多数報告されている。一方、骨粗鬆症のアウトカムである骨折との関係については、海外の2 つの報告では「関係あり」、国外の1 例および国内の1 例では「なし」とされており、未だ真偽が明確になってはいない。本研究では、病院を受診してパノラマX 線写真を撮影した患者をベースとした病院ベース多目的コホートを作成し、パノラマX 線写真による骨粗鬆症スクリーニング指標と骨粗鬆症診断歴（未骨折者）および骨粗鬆症性骨折歴との関係について検討を行った。【対象および方法】松本歯科大学病院を2007～2013 年に受診し、歯科治療のためパノラマX線写真を撮影した40 歳以上の患者2186 名に対して、骨粗鬆症を初めとする種々の全身疾患や栄養摂取状況、生活習慣に関する質問表と質問に関する同意書を送付し、回答が得られた1021 名（男性371 名、女性650 名） を分析した。本研究は松本歯科大学倫理委員会の承認（0152 号） を受けて行われた。書類を送付した全ての患者のパノラマX 線写真について、経験年数24年の歯科放射線専門医が骨粗鬆症スクリーニングの皮質骨形態指標をこれまでの報告に従い、3 型（ 1 型：正常、2 型：軽度～中等度粗鬆、3 型：高度粗鬆） に分類した。骨粗鬆症診断歴（ 未骨折） および骨粗鬆症性骨折歴を各々独立変数として、近年WHO で開発されたFRAXⓇ の骨折リスク因子を考慮して、年齢、性別、体格指数、喫煙歴、関節リウマチの有無、糖尿病の有無および現在歯数を共変量として、二項ロジステイック解析により、骨粗鬆症スクリーニング指標との関係を評価した。また１型を正常として、２ ， ３ 型をスクリーニングリスク指標とした場合の感度、特異度、陽性予測率（ PPV ） 、陰性予測率（NPV） 、尤度比（LR） を評価した。加えてROC 解析により両者におけるスクリーニング能力を評価した。【結果】同意により質問表の回答が得られたのは1021 名、非同意（死亡含む）及び未回収により回答が得られなかったのは1165 名であった。同意者のうち男性は371 名、女性は650 名であり、未回答者よりも男性が有意に多かった（ P ＜0 .001） 。同意者の平均年齢（±標準偏差）は64. 6 （ ±10. 6 ）歳であり、未回答者よりも有意に高かった（ P ＜ 0.001 ） 。両群の平均現在歯数および皮質骨形態指標分布に有意差は見られなかった。未骨折の骨粗鬆症診断歴を有する88 名の被験者では、皮質骨形態指標のオッズ比は１型に比して、2 型で1 .40（95％信頼区間[ C I ]、0 .76－ 2.58） 、3 型で2.64（95％CI、1 .38－ 5 .03） であった。一方で、骨粗鬆症性骨折歴を有する55 名の被験者では、2 型で0 .83（95％CI、0 .41－ 1.66） 、3 型で1 .13（95％CI、0 .51－ 2 .49） であった。未骨折の骨粗鬆症診断を有する被験者のスクリーニング能力の感度は75. 0 ％ と比較的高かったが、骨粗鬆症性骨折を有する被験者の感度と特異度は58％ 前後と低い値であった。骨粗鬆症診断を有する被験者のPPV は14. 9 ％ 、NPV は96. 2 ％ 、LR（ ＋ ） は1 .86 およびLR（ － ） は0.42 であった。一方で、骨粗鬆症性骨折を有する被験者については、PPV が7 .2％ 、NPV が96. 0 ％ 、LR（ ＋ ） が1.37 およびLR（ － ） 0 .73 となっていた。骨粗鬆症診断を有する被験者のROC 解析における曲線下面積（AUROC） は0 .71（P＜0.001）であったが、骨粗鬆症性骨折を有する被験者のAUROC は0 .60（P＝ 0 .015）であった。【考察】病院ベースコホートには選択バイアスが存在するものの、本結果からは、パノラマX 線写真における下顎骨皮質骨形態指標は未骨折の骨粗鬆症診断歴とは関係を有するが、骨粗鬆症性骨折既往とは関係を有さない可能性が示された。日本人を対象にした場合、パノラマX 線写真による皮質骨形態指標は、骨粗鬆症と診断される患者をスクリーニングするのには適するかもしれないが、骨粗鬆症性骨折のリスクを有する患者はスクリーニングできないかもしれない。, 2014

49. Distraction osteogenesis assisted by tissue engineering in an irradiated mandible: a case report.

Abstract

Distraction osteogenesis (DO) can provide predictable bone regeneration without grafting procedures but requires long treatment time and forms less bone transverse to the direction of distraction. To promote 3-dimensional bone formation and shorten the consolidation period, tissue-engineered osteogenic material (injectable bone) was applied in a patient who was being treated with vertical DO with an osteocutaneous fibular flap to reconstruct the mandible. The material, which comprised autologous mesenchymal stem cells culture-expanded then induced to be osteogenic in character and platelet-rich plasma (PRP) activated with thrombin and calcium chloride, was infiltrated into the distracted tissue at the end of distraction and injected into a space created labially with a titanium mesh at implant placement. The infiltration contributed to full consolidation of the regenerate for 3 months, and the injection thickened the regenerated ridge and bridged a gap between the native mandible and distracted fibula. The reconstructed mandible was expanded from 10 mm to 25 mm in height despite a lacerated and opened labial periosteum in the distracted area. Six implants 18 mm in length were placed and subsequently achieved osseointegration. The cutaneous flap covering the implants was trimmed, and the palatal mucosa was transplanted to the regenerated ridge for vestibuloplasty. These raw surfaces were covered with PRP; within 3 weeks, they had attained an epithelium. The implants have supported a fixed prosthesis with adequate surrounding bone and attached mucosa. DO was assisted by tissue engineering and became effective in restoring the compromised mandible. [ABSTRACT FROM AUTHOR]
