Bu çalışmada, beyin anevrizması nedeniyle cerrahi tedavi uygulanan bir hasta grubunda İndosiyanin Yeşili Videoanjiyografi (ICG-VA) yönteminin sonuçlarının değerlendirilmesi amaçlanmıştır. Bu intraoperatif değerlendirme yönteminin güvenilirliğini saptamak için, ICG-VA bulguları postoperative anjiyografi ile karşılaştırılmıştır ve bu yöntemin faydaları ve kısıtlılıkları tartışılmıştır. Bu çalışmaya, 75 anevrizma saptanan altmış bir ardışık hasta dahil edilmiştir. Anevrizmanın kliplenmesini takiben; anevrizmayı, ana arterleri ve perforan arterleri göz lemlemek için intravenöz yoldan ICG uygulandı. Ameliyat sonrasında tüm hastalara taburculuk öncesi anjiyografi yapıldı. Anevrizmala rın %86,6'sında, ICG-VA tatmin edici klipleme sağlandığını gösterdi ve ICG-VA bulguları postoperatif anjiyografi ile uyumluydu. Anevriz maların %6.6'sında, anormal ICG-VA bulguları saptandı ve kliplerin değiştirilmesi veya düzeltilmesi gerekli oldu. Anevrizmaların bir di ğer % 6.6'sında ise, ICG-VA herhangi bir patolojik bulgu göstermezken postoperatif anjiyografide anormal bulgular saptandı. ICG-VA ameliyat esnasında kan akımının değerlendirmesinde altın standart yöntem olan intraoperatif anjiyografinin yerini almamakla birlikte, beyin anevrizmalarının cerrahi tedavisinde hasta güvenliğine büyük katkıda bulunmaktadır. ICG-VA, iyi görüntü kalitesiyle cerrahi alanın gerçek zamanlı değerlendirilmesini mümkün kılmaktadır. İşlemi gerçekleştirmek ve değerlendirmek kolaydır. This study evaluates results of Indocyanine Green Videoangiography (ICG-VA) in a patient group with intracerebral aneurysms. To assess reliability of this technique, ICG-VA findings were compared with postoperative cerebral angiography. Furthermore, benefits and limitations of this intraoperative evaluation method were discussed. This study includes 75 aneurysms in 61 consecutive patients. Following clip ligation of the aneurysm, ICG was administrated intravenously to observe the aneurysm, parent arteries, and perforating arteries. All patients under went postoperative angiography before discharge. In 86.6% of aneurysms, ICG-VA confirmed satisfactory clip application and was concor dant with postoperative angiography. In 6.6% of aneurysms, ICG-VA showed pathological findings intraoperatively leading to readjustment of clips immediately. In another 6.6% of aneurysms, ICG-VA did not reveal any pathological findings intraoperatively while postoperative angiography was not normal in these cases. ICG-VA cannot replace the intraoperative angiography yet as the gold standard method of int raoperative blood assessment. However, this technique adds greatly to the safety of the surgical treatment of intracranial aneurysms. ICG-VA promoted real time assessment of the surgical field with good image quality. It is easy to perform and evaluate the procedure. This technique has the potential to be considered as a standard step of this surgical intervention.

Craniosynostosis is a craniofacial malformation in which one or more sutures of the cranial vault are fused prematurely. It is estimated that craniosynostosis affects 1 in 2,000 to 2,500 live births worldwide. Early and accurate diagnosis of craniosynostosis is very important since premature suture closure causes not only a deformity of skull but also can directly affect the development of brain. Craniosynostosis occurs in all racial groups and more than 70% of all cases are non-syndromic. Until now, mutations in genes such as TWIST, EFNB1, FGFR1, FGFR2 and FGFR3 have been shown to play a role in craniosynostosis, whereas geneticfactors of non-syndromic cases have not been fully identified yet.The aim of this study is to identify novel genes and gene regions for non-syndromic craniosynostosis cases by utilizing a genetic approach and a high resolution aCGH. Out of 10 patients included in this study, two patients were diagnosed with Craniofrontonasal syndrome and Apert syndrome, respectively. For these two patients, molecular analysis of EFNB1 and FGFR2 genes were carried out using Sanger sequencing. The patient diagnosed with Craniosynostosis syndrome has been shown to carry a novel mutation. The other patient carried one of the most common mutations of FGFR2; c.755 C>G. For the remaining 8 nonsyndromic cases, aCGH analysis was performed by using microarray chips at 5.3KB resolution (SurePrint G3 Human CGH Microarray Kit, 2x400K, Agilent Technologies), and various deletions and duplications were detected. Overall, this study provides insights into the genetic determinants of craniosynostosis, which will contribute to the future studies in the field.

Although folic acid deficiency is known to be one of the factors in the development of spina bifida and other neural tube defects (NTD) the exact pathophysiology still remains unclear. Progesterone is an endogenous hormone which increases significantly during pregnancy.We aimed to study the possible negative effects of high dose progesterone on neural tube development in early chick embryos. In order to test our hypothesis, early chick embryos were exposed to physiological saline, normal and high doses of progesterone.160 fertile, specific pathogen free white leghorn eggs (Gallus gallus), all at stage eight of development were divided into four equal groups.The first group was incubated without any operation. The second group was injected with physiological saline. The third and fourth groups were injected with two and twenty times more than physiologic doses of progesterone respectively. After 48 hours of incubation, all embryos were analyzed for the presence of NTDs under light microscopy.None.At 48 hours of incubation, 84% (135/160) of the embryos passed characteristics of Stage 12 development and were included to the study. None of the eggs in the first three groups showed NTDs, whereas 81.8% (27/33) of the eggs in the fourth group showed NTDs.Our study showed that progesterone at levels twenty times more than its physiologic level might cause NTDs. Further studies are needed to explain the mechanisms of this teratogenic effect.

ÖZET Nöroşirurjide bugüne kadar tarif edilen tüm yaklaşımlar uniportal kraniotomi fikri çevresinde gelişmiştir. Yeni gelişen ` anahtar deliği ` ve minimal invazif nöroşirurji kavramları doğrultusunda yapılan çalışmada biportal endo - mikroşirürjikal yaklaşım tekniği ile bazal sisternaların disseksiyonu, özellikle Arteria Basilaris tepe bölgesine ulaşılması ve klip aplikasyonunun uygulanabilirliği irdelendi. Ortopedi, genel cerrahi, üroloji gibi cerrahi branşlarda kullanılmakta olan biportal yaklaşım, nöroşirurjide intrakranial olarak ilk kez bu çalışmada uygulanmış oldu. Çalışmada 22 taze, fikse edilmemiş ve 1 Fikse edilmiş kadavra kullanıldı. Planlanan 5 yaklaşım kombine edilerek 12 farklı biportal kombinasyon, 30 biportal endo - mikroşirürjikal disseksiyon uygulandı. Uygulanan teknikde bir çalışma kanalından endoskop ile görün tüleme sağlanırken, diğerinden mikroşirürjikal enstrümanlar hedefe yön lendirilip endoskopik kontrol altında manipule edildi. Çalışmada elde edilen veriler değerlendirildiğinde 7 farklı biportal kombinasyonun güvenilir bir şekilde kullanılabileceği belirlendi. Biportal endo - mikroşirürjikal teknik ile parasellar, suprasellar ve rostral beyin sapı bölgeler ine rahatlıkla ulaşılabildiği ve disseksiyon. klip aplikasyonunun yapılabildiği tesbit edildi. Bu tekniğin daha verimli kullanılması için yeni mikroşirürjikal enstrümanların geliştirilmesi gerekmektedir.
