Abstract

BACKGROUND: The appropriate use of pulmonary artery catheters (PACs) in critically ill cardiac patients remains debated. OBJECTIVES: The authors aimed to characterize the current use of PACs in cardiac intensive care units (CICUs) with attention to patient-level and institutional factors influencing their application and explore the association with in-hospital mortality. METHODS: The Critical Care Cardiology Trials Network is a multicenter network of CICUs in North America. Between 2017 and 2021, participating centers contributed annual 2-month snapshots of consecutive CICU admissions. Admission diagnoses, clinical and demographic data, use of PACs, and in-hospital mortality were captured. RESULTS: Among 13,618 admissions at 34 sites, 3,827 were diagnosed with shock, with 2,583 of cardiogenic etiology. Abstract

Background: Because of advances in medical treatment of heart failure, patients are living longer than in previous eras and may approach the need for advanced therapies, including heart transplantation, at older ages. This study assesses practices surrounding heart transplant in older adults (> 70 years) and examines short‐ and medium‐term outcomes. Methods and Results: This study is a retrospective analysis using the United Network for Organ Sharing (UNOS) database from 2010 to 2021. The absolute number of older adults being transplanted is increasing. Older adults were more likely to have had a prior malignancy or ischemic cardiomyopathy and less likely to be on extra‐corporeal membrane oxygenation or have a high UNOS status prior to transplant. Mortality at 1‐year was higher for older adults (27.8% vs. 23.4%), but at 5 years there was no significant difference (22.3% vs. 19.4%.). Older adults were more likely to die of malignancy or infection. Adults under 70 were more likely to die of cardiovascular causes or graft failure. There was less rejection in older adults. Mortality has not changed for older adults transplanted before versus after the 2018 UNOS allocation change. Conclusions: Carefully selected older adults may be considered for heart transplantation, given similar intermediate‐term mortality. [ABSTRACT FROM AUTHOR]

Neprilysin is a neutral endopeptidase and its inhibition increases bioavailability of natriuretic peptides, bradykinin, and substance P, resulting in natriuretic, vasodilatatory, and anti-proliferative effects. In concert, these effects are prone to produce a powerful ventricular unloading and antihypertensive response. LCZ696 (Valsartan/sacubitril) is a first-in-class angiotensin II-receptor neprilysin inhibitor. LCZ696 is a novel drug not only for the treatment of heart failure but it is also likely to be a useful antihypertensive drug and may have a preferential effect on systolic pressure. This review discusses (i) the mechanism of action, pharmacokinetics, and pharmacodynamics of this novel drug, (ii) the efficacy, safety, and tolerability of LCZ696 in treatment of hypertension from the available trials, (iii) evidence from other contemporary trials on combined Neprilysin inhibitors, (iv) future trials and areas of research to identify hypertensive patient populations that would most benefit from LCZ696

Abstract

Introduction: Implantable cardioverter defibrillators (ICD) and cardiac resynchronization therapy (CRT) for the treatment of heart failure with reduced ejection fraction (HFrEF) should only be considered after 3 to 6 months of optimal guideline-directed medical therapy (GDMT). This includes heart failure approved beta-blockers (HF-BB), ace inhibitors (ACE-I) or angiotensin II receptor blockers (ARB), and mineralocorticoid receptor antagonists (MRA) for select patients. Our study attempts to characterize the rates of GDMT delivery prior to device implantation in an urban academic hospital and identify potential quality improvement targets. Hypothesis: Prescription rates of HF-BB, ACE-I or ARB, and MRAs will be low prior to device implantation. Methods: We conducted a retrospective analysis of patients who underwent de novo ICD or CRT device implantation at our institution from January 2015 to July 2017. Charts were reviewed to obtain demographics, comorbidities, laboratory, imaging, and medication reconciliation data. We evaluated the rates of any GDMT use on admission as well as rates of two-drug and three-drug regimens. Results: Among 144 patients scheduled for ICD or CRT (mean age 60; 67% male; mean LVEF 27%), 83% were on a HF-BB, 83% were on an ACE-I/ARB, and 31% were on an MRA. Two-drug regimens (HF-BB and ACE-I /ARB) were seen in 69%, and three-drug regimens (HF-BB, ACE-I/ARB, and MRA) in 22%. Only 42% of HF-BBs and 26% of ACE-I/ARB were prescribed at ≥ 50% of the maximal dose. There was no significant difference in GDMT rates between men and women. In addition, the rate of GDMT did not differ from admission to discharge. Conclusion: The data indicate that patients admitted for heart failure device therapy are not receiving optimal GDMT. 48. Abstract 13196: Use of Pulmonary Artery Catheters in Cardiac Intensive Care Units: Analysis From the Critical Care Cardiology Trials Network (CCCTN) Registry

Abstract

Background:Whether there is clinical benefit of hemodynamic monitoring using pulmonary artery catheters (PAC) in cardiogenic shock (CS) remains unknown and concerns over risks vs. benefit have resulted in a decline in use. Current drivers of PAC and right heart catheterization (RHC) use are unknown.Aim:To characterize current use of PAC/RHC in cardiac intensive care units (CICUs).Methods:The CCCTN is a multicenter network of tertiary CICUs in North America. Between 9/2017 and 9/2018, each center (n=16) contributed 2-months of all consecutive medical CICU admissions. Data were submitted to the coordinating center (TIMI Study Group, Boston, MA). Rates of PAC/RHC use were stratified by site CS volume and by CS severity, classified using the IABP-SHOCK II Risk Score. Multivariate analysis of the probability of PAC/RHC use included 14 variables: clinical site, age, sex, creatinine, Sequential Organ Failure Assessment score, maximum # of pressors, cardiac arrest, ICU respiratory insufficiency, invasive coronary procedures, CS, and history of chronic diseases such as heart failure.Results:Among 2985 pts, 533 (18%) underwent PAC/RHC. PAC/RHC were used for 48.4% of pts with CS and 59.3% of CS pts with mechanical circulatory support; however, use was highly variable across sites without any relationship to CS volume (Fig-Top). Most pts with CS who underwent PAC/RHC had a low risk profile (Fig-Bottom, light blue) and shock risk profile did not meaningfully change with increasing PAC/RHC use (Fig-Bottom, moving left to right). Multivariable regression demonstrated the clinical site to be the only independent predictor of use of a PAC/RHC. In-hospital mortality for CS pts with PAC/RHC was 30.2%.Conclusions:There is wide variability between institutions in the use of PAC/RHC for CS that is not explained by volume, severity, or other patient or presenting characteristics. These findings underscore the need for robust evidence to guide the use of PAC/RHC in CICUs.
