Abstract

Objectives:The ICD-10 coding system is commonly used in organizations providing health services in many countries. This coding system used for diagnosis of diseases is standardized for national and international diagnoses with the aim of determining incidence rates for diseases and acting as a guide for determination of health policies.AIMThe aim of the study was to determine the distribution of ICD-10 codes used for all patients in a 1-year period. Materials and Methods:From 15/04/2018 to 15/04/2019, a total of 3,764,124 ICD-10 codes were recorded for patients admitted to all departments of the hospital. The distribution of these codes were analyzed and the most used code for urogenital diseases were extracted.Results:The physicians who work at a governmental hospital, used mostly ICD-10 code for urogenital infections. The most used ICD-10 code was N30, cystitis. Another important results of this study was the narrow dispersion of the used ICD-10 codes. Forty-three (%22) of all ICD-10 codes for urogenital disorders was not entered by physicians and the most used ICD-10 codes were general codes. Conclusion: The usage of the ICD-10 codes for urogenital system infections have an important place in clinical practice of urology., Arka Plan: ICD -10 sağlık hizmeti sunan kuruluşlarda yaygın olarak kullanılmaktadır. Hastalıkların tanısında kullanılan bu kodlama sistemi bize ulusal ve uluslararası tanıları standardize etmemize olanak tanıyarak, hastalıkların görülme oranlarının kolayca belirlenmesine olanak tanıyarak ilgili sağlık politikaları için yön gösterici olabilmesidirAmaç: 1 yıl içinde hastanemize başvuran tüm hastalar için kullanılan ICD-10 tanı kodlarının miktarı, ürogenital sistem hastalıklarla ilgili ICD-10 oranları , ürogenital sistem hastalık dağılımını belirlemektir.Gereç ve Yöntemler: 15/04/2048-15/04/2019 tarihleri arasında Kanuni Sultan Süleyman Eğitim ve Araştırma Hastanesinde tüm birimlerinde tedavi olan ve bunların tanı ve tadavisinde kullanılmış olan toplam 3.764.124 ICD -10 Kodları kayıt altına alındı. Ürogenital sistem için kullanılan 174.448(%5) ICD-10 kod ve ürogenital hastalıkalrı için belirlenmiş olan ve hiç kullanılmayan kodlar kayıt altına alındı. Sonuç: Ürogenital hastalıklar ile ilgili 196 ICD-10 kodlama sisteminden 43(%22) tanesinin kullanılmadığı, 1 yıllık süreçte toplam kodlama sisteminin %5 i Ürogenital sistem hastalıklarla ilgili olduğu ve bunun %50 oranında enfeksiyon hastalıklarıyla ilgiliydi. SONUÇÜrogenital sistem enfeksiyonları en sık kullanılan tanı olup ICD-10 kod sisteminde N30 sistit tanısı en çok girilen koddur.

Abstract

This study was aimed to identify the most frequently observed pathogens in uncomplicated urinary tract infections from outpatient urinary isolates obtained across seven different geographical regions in Türkiye and to determine whether the antibiotic resistance rates of these pathogens differ significantly between these regions. The study included patients aged 18 to 65 years who were diagnosed with uncomplicated urinary tract infections and had positive urine cultures from March 2021 to August 2022, across 37 different centers in Türkiye. The participating centers were selected based on their use of the disk diffusion method, in line with the European Committee on Antimicrobial Susceptibility Testing (EUCAST) guidelines, to ensure standardization of urine culture data. A total of 1850 patients who met the inclusion criteria were included in the study. The analysis of the distribution of antibiotic resistance rates in Escherichia coli isolates revealed statistically significant differences in resistance to ampicillin, fosfomycin and nitrofurantoin across different regions (p< 0.05, p< 0.05, p< 0.05, respectively). The Southeastern Anatolia region was identified as having the highest resistance rates to fosfomycin and trimethoprim-sulfamethoxazole (27.4% and 35.3%, respectively). Additionally, the region with the highest nitrofurantoin resistance was determined as the Eastern Anatolia Region with a rate of 35.7% and the region with the highest ciprofloxacin resistance was determined as the Central Anatolia Region with a rate of 51%. Our study demonstrated that antibiotic resistance in the treatment of uncomplicated urinary tract infections varies by geographical region. We believe this comprehensive, national prospective study will provide valuable insights for clinicians planning empirical treatment for uncomplicated urinary tract infections.
