- Abstract
The Magazine is one of the outputs of the Saline Water and Food Systems (SWFS) partnership and several of its international partners. In 2022 the Netherlands Food Partnership (NFP) and the Netherlands Water Partnership (NWP) launched the SWFS partnership as a cross-sectoral collaboration to strengthen cooperation between the Dutch water and agrifood sectors to address the challenge of salinity in low and middle-income countries (LMICs). It has been supported by variousorganizations in the Netherlands, including the Dutch Ministry of Agriculture, Nature, and Food Quality.Through the networks of NFP and NWP, we facilitate the collaboration of stakeholders from diverse sectors, including governmental ministries, esteemed knowledge and research institutes, Non-Governmental Organizations,and corporate entities within the water and food sectors.

- Abstract
Background: The optimal treatment of anastomotic leak after rectal cancer resection is unclear. This worldwide cohort study aimed to provide an overview of four treatment strategies applied. Methods: Patients from 216 centres and 45 countries with anastomotic leak after rectal cancer resection between 2014 and 2018 were included. Treatment was categorized as salvage surgery, faecal diversion with passive or active (vacuum) drainage, and no primary/secondary faecal diversion. The primary outcome was 1-year stoma-free survival. In addition, passive and active drainage were compared using propensity score matching (2: 1). Results: Of 2470 evaluable patients, 388 (16.0 per cent) underwent salvage surgery, 1524 (62.0 per cent) passive drainage, 278 (11.0 per cent) active drainage, and 280 (11.0 per cent) had no faecal diversion. One-year stoma-free survival rates were 13.7, 48.3, 48.2, and 65.4 per cent respectively. Propensity score matching resulted in 556 patients with passive and 278 with active drainage. There was no statistically significant difference between these groups in 1-year stoma-free survival (OR 0.95, 95 per cent c.i. 0.66 to 1.33), with a risk difference of -1.1 (95 per cent c.i. -9.0 to 7.0) per cent. After active drainage, more patients required secondary salvage surgery (OR 2.32, 1.49 to 3.59), prolonged hospital admission (an additional 6 (95 per cent c.i. 2 to 10) days), and ICU admission (OR 1.41, 1.02 to 1.94). Mean duration of leak healing did not differ significantly (an additional 12 (-28 to 52) days). Conclusion: Primary salvage surgery or omission of faecal diversion likely correspond to the most severe and least severe leaks respectively. In patients with diverted leaks, stoma-free survival did not differ statistically between passive and active drainage, although the increased risk of secondary salvage surgery and ICU admission suggests residual confounding.

Aim: Crohn's anal fistula (CAF) is a complex condition, with no agreement on which patient characteristics should be routinely reported in studies. The aim of this study was to develop a core descriptor set of key patient characteristics for reporting in all CAF research.Method: Candidate descriptors were generated from published literature and stakeholder suggestions. Colorectal surgeons, gastroenterologists and specialist nurses in inflammatory bowel disease took part in three rounds of an international modified Delphi process using nine-point Likert scales to rank the importance of descriptors. Feedback was provided between rounds to allow refinement of the next ratings. Patterns in descriptor voting were assessed using principal component analysis (PCA). Resulting PCA groups were used to organize items in rounds two and three. Consensus descriptors were submitted to a patient panel for feedback. Items meeting predetermined thresholds were included in the final set and ratified at the consensus meeting.Results: One hundred and thirty three respondents from 22 countries completed round one, of whom 67.0% completed round three. Ninety seven descriptors were rated across three rounds in 11 PCA-based groups. Forty descriptors were shortlisted. The consensus meeting ratified a core descriptor set of 37 descriptors within six domains: fistula anatomy, current disease activity and phenotype, risk factors, medical interventions for CAF, surgical interventions for CAF, and patient symptoms and impact on quality of life.Conclusion: The core descriptor set proposed for all future CAF research reflects characteristics important to gastroenterologists and surgeons. This might aid transparent reporting in future studies.

In recent years, researchers focused on introducing discrete type distributions which satisfy the necessary demand to model the complex performance of the real data sets. In this paper, a discrete inverted Kumaraswamy distribution, which is a discrete version of the continuous inverted Kumaraswamy distribution, is derived using the general approach of discretization of a continuous distribution. The new discrete inverted Kumaraswamy distribution can be applied efficiently in discrete lifetime and count data. Some important distributional and reliability properties of discrete inverted Kumaraswamy distribution such as hazard rate, moments, quantiles, order statistics and some transformations are obtained. Maximum likelihood and Bayesian approaches are applied under Type-II censored samples for estimating the parameters, survival, hazard rate and alternative hazard rate functions. Confidence and credible intervals for the parameters are obtained. A simulation study is carried out to illustrate the theoretical results of the maximum likelihood and Bayesian estimation. Finally, the performance of the new distribution is compared with some distributions using three real data sets to illustrate the suitability and flexibility of the proposed model. 33. Hazardous and Harmful use of Alcohol and / or Other Drugs and Health Status Among South African Patients Attending HIV Clinics

There is growing recognition of the influence of substance use, particularly alcohol use, on HIV disease progression. This study investigated how hazardous/harmful use of alcohol and drugs impacts the health status of 1503 patients attending HIV clinics. Of the sample, 37 % indicated hazardous/harmful drinking and 13 % indicated a drug problem. Hazardous/harmful use of alcohol and drugs was significantly related to health status, with participants using substances more likely to have TB-positive status (χ2 = 4.30, p < 0.05), less likely to be on ARVs (χ2 = 9.87, p < 0.05) and having lower CD4 counts (t = 4.01, p < 0.05). Structural equation modelling confirmed the centrality of hazardous/harmful use of alcohol as a direct and indirect determinant of disease progression. Based on these findings it is recommended that patients attending HIV clinics be routinely screened for problematic alcohol and/or drug use, with strong emphasis on ensuring ARV adherence in those with problematic alcohol use.

Traditional optimizers fail to pick good execution plans, when faced with increasingly complex queries and large data sets. This failure is even more acute in the context of XQuery, due to the structured nature of the XML language. To overcome the vulnerabilities of traditional optimizers, we have previously proposed ROX, a Run-time Optimizer for XQueries, which interleaves optimization and execution of full tables. ROX has proved to be robust, even in the presence of strong correlations, but it has one limitation: it uses full materialization of intermediate results making it unsuitable for pipelined systems. Therefore, this paper proposes ROX-sampled, a variant of ROX, which executes small data samples, thus generating smaller intermediates. We conduct extensive experiments which proved that ROX-sampled is comparable to ROX in performance, and that it is still robust against correlations. The main benefit of ROX-sampled is that it allows the large number of pipelined databases to import the ROX idea into their optimization paradigm.

We demonstrate ROX, a run-time optimizer of XQueries, that focuses on finding the best execution order of XPath steps and relational joins in an XQuery. The problem of join ordering has been extensively researched, but the proposed techniques are still unsatisfying. These either rely on a cost model which might result in inaccurate estimations, or explore only a restrictive number of plans from the search space. ROX is developed to tackle these problems. ROX does not need any cost model, and defers query optimization to run-time intertwining optimization and execution steps. In every optimization step, sampling techniques are used to estimate the cardinality of unexecuted steps and joins to make a decision which sequence of operators to process next. Consequently, each execution step will provide updated and accurate knowledge about intermediate results, which will be used during the next optimization round. This demonstration will focus on: (i) illustrating the steps that ROX follows and the decisions it makes to choose a good join order, (ii) showing ROX’s robustness in the face of data with different degree of correlation, (iii) comparing the performance of the plan chosen by ROX to different plans picked from the search space, (iv) proving that the run-time overhead needed by ROX is restricted to a small fraction of the execution time.

With the increasing need for manipulating and exchanging XML data, the topic of processing XML documents and optimizing XML queries is being studied by many researchers. There are still, however, many open issues in query optimization in the context of XML database systems. In this paper, we give an overview on the state of XML query optimization by reviewing the different optimization techniques developed by previous research and employed by existing XML database systems. We subsequently enumerate some of the still unresolved problems in the optimization process. We conclude this paper with a description of our current work in which we focus on improving the evaluation of XPath expressions by rewriting the path at the algebraic level.

Fifty-six patients were diagnosed to have mitral valve prolapse (MVP) syndrome by auscultatory criteria, confirmed by cross-sectional echocardiography. Complete physical examination and x-ray of the bony thorax revealed bony deformities which were subdivided into four main groups according to the predominant deformity. Shallow chest with pectus excavatum, straight back, kyphoscoliosis and elliptical chest in marfanoid patients were reported in the cases studied. It is concluded that musculoskeletal abnormalities have to be considered as nonauscultatory features of MVP. Therefore, any patient with musculoskeletal deformity has to be screened for MVP by cross-sectional echocardiography to prevent life-threatening complications.

Background: A performance measurement system is planned for South African substance abuse treatment services. Provider-level barriers to implementing these systems have been identified in the United States, but little is known about the nature of these barriers in South Africa. This study explored the willingness of South African substance abuse treatment providers' to adopt a performance measurement system and perceived barriers to monitoring service quality that would need to be addressed during system development. Methods: Three focus group discussions were held with treatment providers from two of the nine provinces in South Africa. These providers represented the diverse spread of substance abuse treatment services available in the country. The final sample comprised 21 representatives from 12 treatment facilities: eight treatment centres in the Western Cape and four in KwaZulu-Natal. Content analysis was used to extract core themes from these discussions. Results: Participants identified barriers to the monitoring of service quality that included outdated modes of collecting data, personnel who were already burdened by paperwork, lack of time to collect data, and limited skills to analyse and interpret data. Participants recommended that developers engage with service providers in a participatory manner to ensure that service providers are invested in the proposed performance measurement system. Conclusion: Findings show that substance abuse treatment providers are willing to adopt a performance measurement system and highlight several barriers that need to be addressed during system development in order to enhance the likelihood that this system will be successfully implemented. © 2014 Myers et al.; licensee BioMed Central Ltd.

Optimization of complex XQueries combining many XPath steps and joins is currently hindered by the absence of good cardinality estimation and cost models for XQuery. Additionally, the state-of-the-art of even relational query optimization still struggles to cope with cost model estimation errors that increase with plan size, as well as with the effect of correlated joins and selections. In this research, we propose to radically depart from the traditional path of separating the query compilation and query execution phases, by having the optimizer execute, materialize partial results, and use sampling based estimation techniques to observe the characteristics of intermediates. The proposed technique takes as input a Join Graph where the edges are either equi-joins or XPath steps, and the execution environment provides value- and structural-join algorithms, as well as structural and value-based indices. While run-time optimization with sampling removes many of the vulnerabilities of classical optimizers, it brings its own challenges with respect to keeping resource usage under control, both with respect to the materialization of intermediates, as well as the cost of plan exploration using sampling. Our approach deals with these issues by limiting the run-time search space to so called “zeroinvestment” algorithms for which sampling can be guaranteed to be strictly linear in sample size. All operators and XML value indices used by ROX for sampling have the zero-investment property. We perform extensive experimental evaluation on large XML datasets that shows that our run-time query optimizer finds good query plans in a robust fashion and has limited run-time overhead.
