1. The Impact of Excluding Adverse Neonatal Outcomes on the Creation of Gestational Weight Gain Charts Among Women from Low- and Middle-income Countries with Normal and Overweight BMI.

Abstract

BACKGROUND: Existing gestational weight gain (GWG) charts vary considerably in their choice of exclusion/inclusion criteria, and it is unclear to what extent these criteria create differences in the charts' percentile values. OBJECTIVES: We aimed to establish the impact of including/excluding pregnancies with adverse neonatal outcomes when constructing GWG charts. METHODS: This is an individual participant data analysis from 31 studies from low- and middle-income countries. We created a dataset that included all participants and a dataset restricted to those with no adverse neonatal outcomes: preterm < 37 wk, small or large for gestational age, low birth weight < 2500 g, or macrosomia > 4000 g. Quantile regression models were used to create GWG curves from 9 to 40 wk, stratified by prepregnancy BMI, in each dataset. RESULTS: The dataset without the exclusion criteria applied included 14,685 individuals with normal weight and 4831 with overweight. After removing adverse neonatal outcomes, 10,479 individuals with normal weight and 3466 individuals with overweight remained. GWG distributions at 13, 27, and 40 wk were virtually identical between the datasets with and without the exclusion criteria, except at 40 wk for normal weight and 27 wk for overweight. For the 10th and 90th percentiles, the differences between the estimated GWG were larger for overweight (∼1.5 kg) compared with normal weight (<1 kg). Removal of adverse neonatal outcomes had minimal impact on GWG trajectories of normal weight. For overweight, the percentiles estimated in the dataset without the criteria were slightly higher than those in the dataset with the criteria applied. Nevertheless, differences were <1 kg and virtually nonexistent at the end of pregnancy. CONCLUSIONS: Removing pregnancies with adverse neonatal outcomes has little or no influence on the GWG trajectories of individuals with normal and overweight.

Purpose. To characterize the physiological changes in 25-hydroxyvitamin D [25(OH)D] and 1,25-dihydroxivitamin D [1,25(OH)2D] throughout pregnancy. Methods. Prospective cohort of 229 apparently healthy pregnant women followed at 5th-13th, 20th-26th and 30th-36th gestational weeks. 25(OH)D and 1,25(OH)2D concentrations were measured by LC-MS/MS. Statistical analyses included longitudinal linear mixed-effects models adjusted for parity, season, education, self-reported skin color and pre-pregnancy BMI. Abstract

Article To study the genotypic variation of cowpeas on plant growth and phosphorus (P) uptake which is a function of different P regimes in the soil rhizosphere , 6 landraces of cowpea commonly found in northern Algeria ( NE4, NE10, NE11, NKT5, NKT7, NKB7 ) and 4 landraces from Sahara in southern Algeria (NAG4, NAG5, NAT2 and ND3) were studied during 2013 – 2014 in greenhouse. They were inoculated with Mesorhizobium sp. (S1), Bradyrhizobium sp. (S2) and co – inoculation (S1 – S2) under three P treatments: no P supply (P0), soluble P (PP) and insoluble P (TCP). Only varieties commonly found in northern Algeria nodulated with soluble P (PP) and inoculation containing Mesorhizobium sp . (S1). As a result of the symbiosis, the use of S1 has significantly increased shoot dry weight by 22%, total P content by 20% and P use efficiency for symbiotic nitrogen fixation by 18% compared to no inoculation (T). The landraces from the northern of Algeria expressed a higher growth than those from Sahara in the south of Algeria, especially three of them (NE4, NE10 and NE11) who showed a high performance under all P regimes. We suggest that these three landraces may be useful for improving symbiotic nitrogen fixation in cowpeas when growth is limited by low – P soils and that they could contribute to sustainable farming systems through reduction of farm er’s dependence on fertilizers.

18. Factors associated with post-partum weight retention in a cohort of women, 2005-2007

Abstract

Objective: to investigate factors associated with post-partum weight retention. Methods: a cohort study with 104 recruited pregnant women in the 8th to 13th week of gestation was carried out at Basic Health Unit in the municipality of Rio de Janeiro and the women were accompanied until around 55 days post-partum. The variables included socio-demographic, economic, biochemical, anthropometric and reproductive data. Post-partum weight retention (the difference between post partum and pre-gestational weight) was used as the dependent variable. Results: the mean weight retention was 3.2 kg ± 3.7 kg. Women with excessive gestational weight gain retained 5.0 kg ± 3.9 in comparison to 3.6 kg ± 3.0 in women with adequate gestational weight gain and 1.4 kg ± 3.1 compared to those with inadequate gestational weight gain. The gestational weight gain and pre-gestational body mass index continued to show a significant association with weight retention in the final model. Primary objective: This article presents data on the secular trend in age at menarche for 1955 women from 16 to 76 years of age born between 1920 and 1979 and studied under the Nutrition and Health Survey conducted in the municipality of Rio de Janeiro, Brazil, in 1996.Methods and procedures: Age at menarche was defined by the retrospective method. 23. Mild but not light or severe food insecurity is associated with obesity among Brazilian women

Abstract

Our aim was to determine whether food insecurity was associated with a higher prevalence of obesity in a large random sample of Brazilian women of reproductive age. The data were derived from the 3rd edition of the Children's and Women's National Demographic and Health Survey conducted in 2006-07. This was a nationally representative cross-sectional study. Obesity (BMI ≥ 30 kg/m(2)) was the outcome variable. Associations were measured using crude and adjusted prevalence ratios (PR) with 95% CI through Poisson regression models taking into account the complex sampling design. The sample included 10,226 women from 18 to 45 y of age. The prevalence of any level of food insecurity measured by the Brazilian Food Insecurity Scale was 40.9%, with 25.5% light, 10.1% mild, and 5.3% severe food insecurities. The prevalence of obesity was 17.4%. We found a borderline effect of light food insecurity and increased prevalence of obesity in Brazil (PR = 1.16; 95% CI = 0.98-1.38; P = 0.08). Women with mild food insecurity had a higher risk of being obese than their food-secure counterparts (PR = 1.49; 95% CI = 1.17-1.90; P = 0.010) after adjustment for skin color/ethnicity, years of schooling, geographical region, income, age, and marital status. In conclusion, this study suggests that mild but not light or severe food insecurity was associated with obesity as assessed by BMI, even after adjusting for various confounding factors in this large cross-sectional survey performed in a middle-income country undergoing the nutrition transition.

Methods: The childhood malnutrition (z-score < -2 standard deviation) was made considering anthropometric indicators in children < 5 years; height for age, weight for height and weight for age. Prevalence was estimated based on sex and type of household (urban and rural). Maps with malnutrition distribution were drawn. The World Health Organization database for malnutrition and growth was employed. Statistical analysis included correlation and simple regression. Results: the countries were organized according to quintiles of malnutrition distribution: 0-10%: Argentina, Brazil and Costa Rica; 11-20%: Chile, Colombia, Republican Dominican, Uruguay and Venezuela; 21-30%: Ecuador, El Salvador, Haiti, México and Nicaragua; 3140%: Bolivia, Honduras and Peru; > 40% Guatemala. The relation between malnutrition and socioeconomic indicators was always inverse except for infant mortality. Conclusions: Haiti, Guatemala, Honduras and Nicaragua were the countries with the highest malnutrition prevalence. The relation between malnutrition and socioeconomic was inverse for IDH, birth expectancy and schooling. The relation with infant mortality was positive.

Abstract

Avaliou-se a capacidade que a Circunferência da Cintura (CC) tem em identificar valores de IMC ≥25 (sobrepeso) e ≥30 (obesidade) a partir de níveis de ação recomendados internacionalmente. Setecentas e noventa e uma mulheres entre 15-59 anos foram recrutadas. Foram calculadas a sensibilidade e a especificidade no diagnóstico de sobrepeso e obesidade e a sensibilidade e especificidade na predição da hipertensão, a partir de valores de CC. As associações foram testadas por análise de regressão linear e regressão logística controlando para o efeito de fatores de confusão. A CC ≥80 e ≥88cm discriminou corretamente 89,8% de mulheres com IMC ≥25 e 88,5% com IMC ≥30. A obesidade abdominal (CC ≥88cm) esteve associada significativamente com a hipertensão na análise multivariada (OR = 2,88, IC 95%: 1,77-4,67). A hipertensão foi identificada com sensibilidade de 63,8 e 42,8% e especificidade de 68,0 e 83,3% para CC ≥80 e ≥88, respectivamente. A obesidade abdominal nos pontos de corte propostos pode discriminar adequadamente indivíduos em risco de se tornarem obesos, no entanto, apresenta um poder apenas moderado para discriminar indivíduos com níveis pressóricos altos. _______________________________________________________________________________ ABSTRACT This study examined the capacity of waist circumference (WC) to identify subjects with overweight (BMI ≥25) and obesity (BMI ≥30), in agreement with internationally recommended levels of action. Data were obtained from 791 women, 15-59 years old. After identifying overweight and obesity according to WC values, sensitivity and specificity were calculated to verify whether WC could be a good risk predictor for hypertension. Associations were tested by linear regression and logistic regression, controlling for confounding.WC cut-off points of 80cm and 88cm correctly identified 89.8% and 88.5% of women with overweight and obesity, respectively. Abdominal obesity (WC ≥88cm) was statistically associated with hypertension in the multivariate analysis (OR = 2.88; 95% CI: 1.77-4.67). Hypertension was identified with a sensitivity of 63.8% and 42.8%, and with a specificity of 68.0% and 83.3%, for WC ≥80 and ≥88, respectively. Abstract

Objective: To evaluate the effect of leptin and other selected variables on gestational weight gain (GWG) according to pre-gestational body mass index (BMI).Design: Prospective cohort.Setting: Public Health Center, Rio de Janeiro, Brazil.Sample: Two hundred and twenty-eight pregnant women.Methods: Women were followed at the 5-13, 20-26 and 30-36th gestational weeks. The effects of independent variables on GWG in normal weight (BMI 18.5-24.9 kg/m(2) ), overweight (BMI 25.0-29.9 kg/m(2) ) and obese (BMI ≥ 30.0 kg/m(2) ) women were assessed using longitudinal linear mixed-effects models.Main Outcome Measure: Maternal body weight (kg) throughout pregnancy.Results: Leptin concentrations were associated with GWG in normal weight (β = 0.048, P < 0.001) and overweight (β = 0.023, P = 0.048) women, but not in obese ones (β = 0.011, P = 0.308). Additionally, the number of hours slept per night decreased the effect of leptin on GWG in OW women (β = -0.013, P = 0.002). The effect of other maternal characteristics on GWG was different depending on the BMI category.Conclusions: Leptin concentrations were positively associated with GWG in normal weight and overweight women, but not in obese ones. Maternal height was associated with GWG in all BMI categories, but other variables such as sleep duration, QUICKI values, HDL-c, smoking habit and marital status presented differential effects according to BMI. We encourage further studies to investigate the association between leptin and gestational weight gain, taking into account the pre-pregnancy weight and sleep duration, in order to compare and confirm our results.Tweetable Abstract: Leptin is associated with weight gain in normal weight and overweight pregnant women, but not in obese ones.
