2. Assessment of dural invasion in complex cases operated on by collaborative efforts of neurosurgery and otolaryngology for pathologies associated with the frontal sinus.

Purpose: The aim of this study was to evaluate the common interventions performed by neurosurgery and otolaryngology in dural invasion in frontal sinus related pathologies. Materials and Methods: A comprehensive study involving 45 patients who underwent surgical interventions for frontal sinus-associated pathologies between January 2018 and July 2023 was conducted collaboratively by otorhinolaryngology and neurosurgery departments. Patient data, including demographics, pathology types, and surgical approaches, were collected from medical records, surgical reports, imaging studies, and videos. Interdisciplinary surgical teams executed tailored interventions with documented techniques. Results: Among 45 patients, 31 were female (68.8%) with a mean age of 44. Posterior wall defects were detected 13 (28.8%) patients, and intervention was necessary for only 8 of them. Among the 8 (17.7%) patients with identified dural invasion, pathological dural tissue was excised, and duraplasty was performed in 5 (11.1%) cases. There were no neurological complications observed patients during the postoperative period. Conclusion: Despite limitations, the findings contribute to evolving knowledge and highlight the need for prospective studies to optimize patient care strategies. [ABSTRACT FROM AUTHOR]

Objective Music is a very crucial art form that can evoke emotions, and the harmonious presence of the human voice in music is an impactful part of this process. As a result, vocals have had some significant effects on contemporary music. The mechanism behind the cochlear implant (CI) recipients perceiving different aspects of music is clear; however, how well they perceive vocal tuning within music it is not well known. Hence, this study evaluated the mistuning perception of CI recipients and compared their performance with normal-hearing (NH) listeners. Study Design, Setting, and Patients A total of 16 CI users (7 cisgender men, 9 cisgender women) and 16 sex-matched NH controls with an average age of 30.2 (±10.9; range, 19-53) years and 23.5 (±6.1; range, 20-37) years, respectively, were enrolled in this study. We evaluated the mistuning ability using the mistuning perception test (MPT) and assessed self-perceived music perception and engagement using the music-related quality-of-life questionnaire. Test performance was measured and reported on the item-response theory metric with a z score ranging from -4 to +4. Results A significant difference in the MPT scores was found between NH and CI recipients, whereas a significant correlation was noted between the music-related quality-of-life questionnaire-frequency subscale and MPT scores. No significant correlations were found between age, CI age, and CI usage duration and MPT performance. Conclusions This study revealed that musical mistuning perception is a limitation for CI recipients, similar to previously evaluated aspects of music perception. Hence, it is important to consider this aspect in the assessment of music perception, enjoyment, and music-based auditory interventions in CI recipients, as vocals are paramount in music perception and recreation. The MPT is a convenient and accessible tool for mistuning assessment in CI and hearing-aid users.

Introduction: Malignant tumors of the skin include basal cell carcinoma, squamous cell carcinoma, malignant melanoma and tumors of the skin appendages. Skin lesions in the head and neck region are frequently encountered in the geriatric population. Although these lesions are generally benign, it is widely known that the prevalence of malignant lesions increases with age. In adults, 80% of the masses in the neck, except benign thyroid diseases, are of neoplastic origin and this rate reaches 90% after the 5th decade. About 70% of these neoplastic masses are malignant, and three-quarters of these are metastatic and one-fourth is primary malignancies. Objectives: Increasing world population, increasing survival time and increasing geriatric patient population create some differences in healthcare. We planned this study for this purpose. Methods: The data of 73 patients aged 65 and over were retrospectively analyzed. Patients were divided into two groups as 65-75 years old (Group 1), > 75 years old (Group 2), by age. Results: Premalignant lesions were found to be statistically significantly higher in Group 1 (p<0.01). It was observed that malignant lesions were statistically significantly more common in the skin compared to the subcutaneous tissue. (p=0.015) Malignant skin lesions were observed statistically significantly more frequently in the neck region compared to other regions (p=0.004). Conclusions: The risk of malignancy in the patient group over 75 years is approximately doubled. It is necessary to approach the neck and subcutaneous masses carefully in terms of malignancy in the geriatric patient group [ABSTRACT FROM AUTHOR]

The severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was isolated as the causative agent following the first case reports of pneumonia with unknown etiology in China. The 2019 novel coronavirus disease (COVID-19) significantly affected the health care systems in the world. The disease was accepted as COVID-19 pandemic by WHO. Diagnosis and treatment of COVID-19 infection requires multidisciplinary management and there are currently many ongoing studies for possible vaccination and antiviral therapies for the treatment. The major symptoms are known to be high fever and dry cough. Moreover, new symptoms like olfactory and gustatory dysfunction are defined. The epithelium of upper respiratory tract and related cavities (ear, paranasal sinuses) are known to be reservoir for COVID19 virus. Therefore, otorhinolaryngology physicians are one of the highest risk groups for nosocomial transmission. The pandemic altered standard daily working schedule of otorhinolaryngology physicians in the world. Elective operations such as rhinologic and otological surgeries are suspended, and novel prevention strategies were developed to prevent nosocomial transmission during physical examination and surgery. However, the treatment of emergency diseases like deep neck infections, epistaxis, tracheostomy, and head and neck cancers are still going on as expected. COVID-19 associated ARDS is known to require prolonged intubation and mechanical ventilation. But at the moment, the postitive effects of performing tracheostomy is not clearly identified on the outcomes of COVID-19-affected patients. In this study we aimed to review the literature regarding COVID-19 from an otorhinolaryngology point of view. We wanted to show how the pandemic affected daily otorhinolaryngology practice including outpatient clinic, head and cancer treatment, tracheostomy, and otologic manifestations along with preventive measures. PurposeTo investigate the role of non-echo planar diffusion weighted imaging (DWI) using \"periodically rotated overlapping parallel lines with enhanced reconstruction\" (PROPELLER) sequence for the diagnosis of cholesteatoma compared to surgical and histopathological results in an attempt to determine the factors causing false negative and false positive diagnoses.MethodsPatients who had PROPELLER DWI before ear surgery were retrospectively reviewed. The presence of a lesion with diffusion restriction on PROPELLER DWI was accepted as positive for cholesteatoma, and the results were compared to the intraoperative and histopathological findings.ResultsA total of 112 ears in 109 patients were reviewed. On PROPELLER DWI, a lesion with diffusion restriction was found in 101 (90.2%) ears, while in 11 (9.8%) of the patients, no diffusion restriction was found. Surgery and histopathological analysis revealed a cholesteatoma in 100 (89.3%) ears, while in 12 (10.7%) ears, no cholesteatoma was found surgically. There were 96 (85.7%) true positives, 7 (6.2%) true negatives, 5 (4.5%) false positives and 4 (3.6%) false negatives. The accuracy, sensitivity, specificity, positive predictive and negative predictive values of non-echo planar DWI were calculated to be 91.96%, 96%, 58.33%, 95.05%, and 63.64%, respectively.ConclusionNon-echo planar DWI using PROPELLER sequence has high accuracy, sensitivity and positive predictive value and can be used for the detection of cholesteatoma. The external auditory canal, postoperative ears and small lesions should be evaluated with caution to avoid false results.

© 2021The aim of this study was to investigate the effect various mediators in synovial fluid (SF) on the pathogenesis of temporomandibular disorders (TMD) and to evaluate the relationship between clinical and radiological features of temporomandibular joint (TMJ) diseases. Patients who had received SF sample during arthrocentesis because of TMD were included in this study. Clinical and radiological records were evaluated retrospectively. Enzyme-Linked ImmunoSorbent Assay (ELISA) method was used for analysis of aggrecan, adiponectin, resistin, apelin, Vascular Endothelial Growth Factor (VEGF) and Prostaglandin E2 (PGE2) in SFs. 59 joints of 41 patients were included in the study. Anterior disc displacement with reduction (ADDwR) was detected in 22 joints, anterior disc displacement without reduction (ADDwoR) was detected in 29 joints and osteoarthritis (OA) in 8. In OA group, PGE2 level was significantly higher than the other groups (p = 0.029). Aggrecan and PGE2 levels were statistically higher in joints with localized pain (p = 0.030, p = 0.029). The aggrecan level was statistically significant higher in patients who had degenerative changes in radiological examinations (p = 0.044). Resistin was correlated with PGE2 and aggrecan (p = 0.011), and apelin showed positive correlation with VEGF (p˂0.001). The detection of aggrecan and adipokines in SF may be a precursor of degenerative joint disease and it should be taken into account that the presence of localized pain in the joint area may be an early sign of degenerative changes.

Background: Most of the studies on salivary gland cancers are limited for various reasons such as being single-center, small number of patients, including only major or minor SGCs, or only including epidemiological data. Methods: A total of 37 medical oncology clinics from different regions of Turkey participated in this retrospective-multicenter study. The analyzed data included clinical and demographical features, primary treatment, metastasis localizations, and treatments and includes certain pathologic features. Results: The study included data from a total of 443 SGCs. 56.7% was in major salivary glands and 43.3% was in minor salivary glands. Distant metastasis in the major SGCs was statistically significantly more common than in the minor SGCs, locoregional recurrence was statistically significantly more common in the minor SGCs than in the major SGCs (p = 0.003). Conclusions: Epidemiological information, metastasis and recurrence patterns, treatment modalities, and survival analysis of the patients over 20 years of follow-up are presented.

© 2023 Elsevier B.V.Objective: This longitudinal study aimed to investigate the maturational development of P1 latency and the effects of a short inter-implant delay in children who received bilateral sequential cochlear implants. Materials and methods: Fourteen children (first CI mean age: 1.4 ± 0.4 years and Second CI mean age: 3.3 ± 0.5 years) who had received sequential bilateral cochlear implants during the sensitive period for auditory maturation participated in our study. The speech-evoked cortical P1 response was recorded after the activation of the second CI at four intervals (implant activation, 3 months, 6 months, 12 months) under three listening conditions (first CI, second CI, binaural). Results: Our results showed that the P1 latencies of the second CI reached normative values within 3 months, but did not reach the P1 latency of the first CI until 12 months. We found a strong negative correlation between the inter-implant delay and the P1 latencies of the second CI when the second CI was activated. Conclusion: Changes in cortical auditory responses over time resulted in normal auditory maturation in children with sequential bilateral cochlear implants during the sensitive period. The results also provide evidence that the timing of auditory experience in the first ear during the sensitive period may influence the speed of compensation in children receiving sequential cochlear implants.

Introduction: Otorhinolaryngology (ENT) physicians may be effective in the early diagnosis of autism spectrum disorder (ASD). The aim of this study was to investigate the awareness of ENT residents in İstanbul about recognizing ASD. Methods: The type of the study was cross-sectional. The study population consisted of 97 ENT residents, and 83 (85%) of these residents were included in the study. The questionnaire consisted of questions about the sociodemographic data of the participants and Autism Spectrum Disorder Awareness Questionnaire. Autism Spectrum Disorder Awareness Questionnaire contains the findings of ASD reported in the literature. This questionnaire includes observable findings of possible ASD in a child during routine examination of the ENT physician. Results: In our study, the most known finding of childhood autism was found to be difficulty in making eye contact (97.5%). Regarding work experience, "difficulty in making eye contact" (100%) and "lack of age-appropriate language development" (91.5%) findings were well-known findings among residents with less than four years of experience. On the other hand, "very sensitive vision, hearing and touch senses" (95.8%) and "seem unable to hear even if hearing tests are normal" (91.7%) findings were more prominent among residents with four or more years of experience. Conclusion: The ASD awareness of the ENT residents participating in our study can be considered as relatively good. The "Autism Spectrum Disorder Awareness Questionnaire" developed by the researchers is planned to be validated with the studies to be carried out later and developed to be used for ENT specialists. PAX8/PPARG chromosomal rearrangement is frequently seen in thyroid cancer, and PPARG overexpression has been shown in the follicular variant of papillary thyroid carcinoma, but not in papillary thyroid carcinoma other than the follicular variant. The main aim of this study was to investigate the frequency of PPARG overexpression among papillary thyroid carcinoma and if there were any variants of papillary thyroid carcinoma with PPARG overexpression other than the follicular variant. Methods. Immunohistochemical analysis of PPARG overexpression was performed using a PPARG monoclonal antibody in a series of 111 paraffin-embedded blocks of thyroid tumours. Of the patients in our study, 100 were diagnosed with papillary thyroid carcinoma, 9 with follicular adenoma and 2 with follicular carcinoma. Results. PPARG staining was detected in 19 of the 111 cases. Sixteen patients with PPARG overexpression had papillary thyroid carcinoma and 3 had follicular adenoma. Conclusion. PPARG overexpression was detected mainly in follicular-variant papillary thyroid carcinoma. Vascular invasion, lymphatic invasion, thyroid capsule invasion and lymph node positivity were lower in patients with PPARG overexpression.

Objective (1) To compare anxiety between parents of newborns with cleft lip and palate (CLP), isolated cleft palate (CP), and healthy newborns and (2) to evaluate anxiety between parental dyads within these groups. Design A cross-sectional study. Setting University Hospital. Participants Surveys were completed by 20 mothers and 20 fathers of newborns with CLP, 21 mothers and 21 fathers of newborns with CP, and 23 mothers and 23 fathers of healthy newborns (controls). Main Outcome Measure The State-Trait Anxiety Inventory (STAI) assessed parental anxiety. Mothers of newborns with a cleft reported on concerns regarding cleft-related issues and facial appearance. Results State and trait anxiety were generally in the moderate range for parents of newborns with a cleft, while control parents had low state anxiety and moderate trait anxiety. Mothers of newborns with CP and CLP had significantly higher state and trait anxiety levels than control mothers ( p Conclusions Parents of children with a cleft may need psychological support in the early postnatal period. It is important for neonatal cleft team providers to help reduce parental anxiety and educate families about cleft care, with a focus on feeding.

Objectives: To develop a reproducible experimental animal model for sulcus vergeture in rabbits. Study Design: Experimental animal study. Material Method: We evaluated three methods of inducing sulcus in twelve New Zealand white rabbits to produce a sulcus model. Experimental groups comprised: group 1 (n = 4) underwent submucosal injury following endolaryngeal epithelial incision; group 2 (n = 4) received submucosal injury via thyrotomy; group 3 (n = 4) received submucosal injury via thyrotomy, followed with horizontal mucosal lateralization suture. Right vocal folds of the rabbits underwent surgery to produce sulcus vocalis and left vocal folds were used for the comparison. In the sixth week after the procedure, the rabbits were sacrificed and the larynxes were harvested and analyzed histopathologically. Results: No animals in group 1 or 2 developed sulcus vocalis. Sulcus formation was observed in all rabbits in group 3, under endoscopic examination and microscopic sulcus formation was demonstrated for the first, second, and third rabbits. An epithelial depression area was seen at the glandular ductal opening zone for the fourth rabbit, but it could also be accepted as a sulcus formation. Conclusion: We successfully developed a reproducible survival model for sulcus using a submucosal injury via thyrotomy, followed with a horizontal mucosal lateralization suture. This model provides the groundwork for future research into the applicability of new approaches for sulcus management.

© 2022 Associação Brasileira de Otorrinolaringologia e Cirurgia Cérvico-FacialObjective: In this study, we created an animal model to demonstrate the effects of thiamine on the hearing pathways of new-borns during pregnancy and lactation by inducing a dietary thiamine deficiency in the mother. Methods: The study included 16 female Wistar albino rats. The animals were separated into four groups and provided the appropriate amounts of dietary thiamine according to their groups during pre-pregnancy, pregnancy, and lactation periods. Three pups from each mother were included in the study, and 12 pups were selected from each group. On the fortieth day after birth, the auditory pathways of 48 pups in the 4 groups were examined electro physiologically and ultra-structurally. Results: In Group N-N, morphology of hair cells stereocilia degeneration was not obtained in all turns of cochlea. In Group N-T, Inner Hair Cells (IHCs) and Outher Hair Cells (OHCs) stereocilia didn\"t show degeneration in all turns of cochlea but had rupture inrows of HCs stereocilia. In group T-N IHCs stereocilia less degeneration was observed in all turns of cochlea. OHC stereocilia partial loss was observed only in basal turn of cochlea. In Group T-T IHCs stereocilia was observed less degeneration and rupture in all turns of cochlea. Conclusion: Thiamine is vital for the development of cochlear hair cells during both prenatal and postnatal periods. Even partial deficiency of thiamine causes significant degeneration to the auditory pathway. Level of evidence: The level of evidence of this article is 5. Purpose We investigated the relationship between hearing aid or cochlear implants usage and body image, and quality of life. Methods Of the 96 participants studied, 26 were cochlear implant users, 20 hearing aid users, 20 non-device users with hearing loss, and 30 normal hearing. The inclusion criteria for the study for all groups were subjects 18 years or older with no other physical complaints. The average age of groups and gender distributions were similar. The Body Image Scale (BIS) and World Health Organization Quality of Life Scale were applied to all participants, whereas the International Outcome Inventory for Hearing Aids, Turkish Version, was applied only to device users. Results A significant difference in body image scores was identified between non device users with hearing loss group and normal hearing group, device users group (HA/CI), and cochlear implant users group. In contrast, no significant difference in body image scores between normal hearing group and the device users group was identified. Finally, a positive correlation between body image scores and quality of life scores in all groups was identified. Conclusions Non-device users with hearing loss have a lower body image and quality of life than those in the other groups, and device usage has a positive effect on body image and quality of life.

Each individual included in the study WHOQOL–BREF Quality-of-Life Scale, Rosenberg Self-Esteem and Speech, Spatial Perception and Qualities of Hearing Scale was applied. Results: We found that the self-esteem level of the group with hearing loss, but without a device, was significantly lower than that of the group with a cochlear implant/hearing aid. There was a moderately statistically significant relationship between self-esteem, quality of life, speech perception, and hearing quality. Conclusions: Using hearing aid and cochlear implants increases self-esteem among people with hearing impairments. Self-esteem is associated with quality of life, speech perception, and hearing quality.

Laringomalazi yenidoğan ve infantlarda inspiratuar stridor en sık nedenidir, bu çocukların %10\"unda cerrahi girişim gerektiren ciddi semptomlar mevcuttur. Laringomalazide supraglottik larinksin kollabe olması sonucunda inspiratuar stridor görülür. Tanıda öncelikle fleksibl fiberoptik laringoskop ile değerlendirilme önerilmektedir. Senkronize havayolu lezyonlarının tespiti için bronkoskopi de yapılmalıdır. Laringomalazi şiddeti; hafif, orta ve ağır olarak değerlendirilir. Hafif, orta ve şiddetli olgular için ise asit süpresyon tedavisi, yutma terapisi ve yutmanın değerlendirilmesi tavsiye edilmiş olup; şiddetli laringomalazide laringoskopi/bronkoskopi sonrasında supraglottoplasti tavsiye edilmiştir. Supraglottoplasti laringomalazili çocukların çoğunluğunda başarı ile sonuçlanır. Ancak eşzamanlı havayolu patolojileri ve komorbid hastalıkların varlığı olumsuz sonuçlara neden olabilir ve trakeostomi gerektirebilir. Laryngomalacia is the most common cause of inspiratory stridor in neonates and infants, with 10% of these children having severe symptoms requiring surgical intervention. Inspiratory stridor is seen as a result of collapse of the supraglottic larynx in laryngomalacia. In the diagnosis, evaluation with a flexible fiberoptic laryngoscope is recommended. Bronchoscopy should also be performed to detect synchronous airway pathologies. Laryngomalacia severity; rated as mild, moderate and severe. Acid suppression therapy, swallowing therapy and evaluation of swallowing were recommended for mild, moderate and severe cases; Supraglottoplasty is successful in the majority of children with laryngomalacia. However, the presence of syncronous airway pathologies and comorbid conditions may result worse outcomes and may require tracheostomy.

Subglottik stenoz (SGS), çocukluk çağında kronik üst solunum yolu obstrüksiyonunun en sık nedenlerinden biridir. Konjenital veya edinsel nedenlerle oluşabilir. En sık endotrakeal tüp veya trakeostominin oluşturduğu hasara bağlı anormal yara iyileşmesi sonucu oluşmaktadır. Bu nedenle; tanısında ayrıntılı anamnez önemlidir. Risk faktörleri varlığında SGS araştırılmalıdır. Stridor en sık semptomdur ancak diğer solunum sistemi bulguları ile de karşımıza çıkabilir. Tanıda öncelikle laringoskop ile değerlendirilme önerilmektedir. Laringoskopi ile tanı koyulamadığında bronkoskopi ikinci basamak tanı yöntemi olarak tercih edilmelidir. Darlığın şiddetine göre farklı tedavi protokolleri uygulanır. Hafif darlığı olan vakaların çoğunda konservatif yaklaşım yeterli iken, orta ve şiddetli subglottik stenoz tedavisinde genellikle cerrahi gereklidir. Pediatrik hastalarda subglottik stenozun (SGS) değerlendirilmesi ve yönetimi karmaşık ve teknik olarak zor olduğundan uzmanlaşmış bir ekip ile multidisipliner yaklaşım gerektirir. Subglottic stenosis (SGS) is one of the most common causes of chronic upper airway obstruction in childhood. It can occur due to congenital or acquired reasons. It most commonly occurs as a result of abnormal wound healing due to damage to the endotracheal tube or tracheostomy. Therefore, detailed anamnesis is important in the diagnosis. In the presence of risk factors, SGS should be investigated. Stridor is the most common symptom, but it may also present with other respiratory system findings. In the diagnosis, it is recommended to evaluate with laryngoscope first. When the diagnosis cannot be made by laryngoscopy, bronchoscopy should be preferred as a second-line diagnostic method. Different treatment protocols are applied according to the severity of the stenosis. While the conservative approach is sufficient in most cases with mild stenosis, surgery is usually required for the treatment of moderate to severe subglottic stenosis. The assessment and management of subglottic stenosis (SGS) in pediatric patients is complex and technically challenging, requiring a multidisciplinary approach with a specialized team.

Aim: Majority of the health problems that family physicians encounter are associated with otorhinolaryngology (ORL) diseases. It was aimed in this study to determine how many of the patients who admitted to ORL out-patients clinics at a training and research hospital can be managed by primary care physicians, the attitudes of patients about this issue and their thoughts about family medicine practice. Materials and Methods: A total of 300 patients who were attended to Keçiören Training and Research Hospital Otorhinolaryngology-Head and Neck Surgery out patient clinics between April-May 2016, were included into the study and sociodemographic and clinic characteristics, compliants of referrral, family physician visits for the same complaints, frequency of visits to family physicians and satisfaction status of patients using primary care, diagnosis after the examination of ORL specialist were recorded. Results: Mean age of patients was 49,04± 16,17 years, 64% of them were female, 36% were male. Most common admission complaints were sore throat (%24.1), otalgia (%13.3), voice in the ear (13,0%), hear loss (12,3%) and nasal/postnasal drainage (10,3%); most common diagnoses were impact serumen (16,7%), acute pharyngitis (10,0%), tinnitus (8,3%), benign paroxysmal vertigo (7,7%) and acute otitis media (7,02%), respectively. Of all the patients 70,7% reported that they did not visit their family physicians for these complaints. Of the patients 18% were admitted ORL outpatient clinics with the referral of family physicians, One hundred eighty-seven patients (62,3%) reported that they have never visited their family doctors. Admission rates to primary care were higher in married patients than those single (p<0.05). Satisfaction rates were high in patients who admitted to family physicians (p<0.05). Conclusions: Our findings suggested that %70 of diagnoses of patients admitted to ORL out-patient clinics can be managed in primary care. For the correct use of secondary and tertiary health care facilities, a working referral system, qualified family physicians and sufficient public consciousness are essential. [ABSTRACT FROM AUTHOR]

© 2022 Elsevier LtdObjectives: The thyroid imaging reporting and data system (TI-RADS) and 2015 American Thyroid Association (ATA) guidelines are two well-known risk stratification systems for classifying thyroid nodules based on cancer risk. This study aims to evaluate the diagnostic efficacy of these two systems in predicting malignancy in patients undergoing thyroid surgery. Methods: We studied data on 120 individuals who were scheduled to undergo surgery for benign or malignant nodular diseases of the thyroid gland between October 2017 and October 2019. The TI-RADS category and ultrasound pattern based on ATA guidelines were assigned to dominant thyroid nodule categories by two experienced radiologists blinded to patients’ previous thyroid ultrasonography and fine-needle aspiration biopsy results. A pathologist with experience in thyroid diseases blinded to patients’ sonographic and clinical data reviewed the thyroidectomy specimens. Results: A total of 120 patients, 88 women and 32 men, were included in our study. Final histopathological results were as follows: 50% (n=60) papillary thyroid carcinoma, 36.6% (n=44) benign nodular thyroid diseases, 4.1% (n=5) follicular adenoma, 2.5% (n=3) hurtle cell adenoma, 1.7% (n=2) follicular thyroid carcinoma, 1.7% (n=2) medullary thyroid carcinoma, 1.7% (n=2) hurtle cell carcinoma, and 1.7% (n=2) follicular tumor of uncertain malignancy potential. The sensitivity, specificity, positive predictive value, and negative predictive value for TI-RADS were 80%, 56%, 72%, and 67%, respectively, and that for ATA were 80%, 64%, 76%, and 69%, respectively. Conclusion: The TI-RADS and ATA showed similar rates of sensitivity, specificity, NPV, and PPV. Our observed risk of malignancy was higher than expected for the ACR TI-RADS 3–5 categories and the very low, low, and intermediate suspicion risk strata in the ATA guidelines. We found no difference between observed and expected malignancy risk for the ACR TI-RADS 2’s and ATA\"s high suspicion categories.

The majority of the tissues and organs that fall into the area of expertise of ear, nose and throat (ENT) specialists are adjacent to the orbit, and therefore to the globe. As a result of this contiguity, ENT diseases can spread to the orbit and present with various orbital complications. While these complications can be mild, they can also manifest as severe morbidities that lead to permanent loss of vision. In addition, ENT diseases can also spread to the cranium as well as the orbit, and lead to severe intracranial complications that may result in mortality. Such intracranial complications may also manifest with findings in an ophthalmological examination, as many cranial nerves (CNs) are also associated with the orbital structures. This is why ENT specialists commonly encounter ophthalmological findings as a result of both the orbital and cranial complications of infectious ENT diseases. © 2022 Elsevier Inc.Objective: The aim of this study is to propose a new approach in crooked nose deformity with key-stone plasty and asymmetric hump resection. Method: Twelve patients with crooked nose deformities were operated using the open rhinoplasty technique. Our method, unlike other methods, has two different steps. Following asymmetric hump resection, cartilaginous and osseous septum were cut separated at the key stone area or more caudally and fix the septum with sutures again by allowing them to slide over each other in a way that directs the septum to the midline. If there is an inability to reveal the septum, we apply a longer spreader graft to the cartilage septum side. Results: The mean ages were 27.4 years. The mean follow-up time of the patients was 19.1 months. No complications were observed due to this technique. This technique was effective in the treatment of all our patients with crooked nose deformities. Conclusion: A novel surgical approach with key-stone plasty and asymmetric hump resection method was proposed in crooked nose deformity with a video animation.

Objective: This study aimed to investigate the articles regarding rhinology published in national otorhinolaryngology journals between 2010 and 2015 according to the manuscript type, headline, citation city, and institution, as a reference for studies that will be designed in the future. Methods: Articles published on rhinology between 2010 and 2015 in national otorhinolaryngology journals were searched through websites of journals. The names of the journals, subjects of the manuscripts, manuscript type, citation city and institution, and manuscript language were recorded. Results: In this study, 890 articles were reviewed in 6 national otorhinolaryngology journals. Of these, 274 were regarding rhinology and 129 (47%) and 117 (42.7%) were original articles and case reports, respectively. Of the 274 articles, 21 (7.7%) and 5 (1.9%) were review and animal studies, respectively, and 2 (0.7%) articles defined surgical technique. Most frequently studied topics were benign tumors and inflammatory diseases of the paranasal sinuses. Conclusion: The ratio of case reports was relatively high in national rhinology publications. In future, smell and taste disorders, skull base surgery, and nasal physiologyrelated to Rhinology may be innovative research areas. [ABSTRACT FROM AUTHOR]

29. Opinions of Otorhinolaryngology Residents about Their Education Process.

Objective: Our study was planned to get the views of residents about the Otorhinolaryngology (ORL) education process and to enlighten the studies to make this process more effective. Methods: A questionnaire was sent to the residents who were still in the residency program in all education clinics via "Google Drive". Seventy-four of 354 residents responded and the answers were evaluated electronically. Results: Fifty residents (67.56%) gave an affirmative answer to the question about the use of "Resident Log Book" and no difference was seen among the clinics. While 9 residents (12.16%) were reporting that they did not read any scientific papers, 43 (58.1%) reported they read less than three per month. Forty-one residents thougt that they were having a good and sufficient education. Seventeen residents (51.51%) who thought they were not having a sufficient education reported that the education period should be longer. When they were wanted to evaluate the education process, while 66 of them (89.18%) said "Exhausting", 52 (70.27%) said "Stressful", it was seen that the ones who said "Instructive" and "Rewarding" were 26 (35.13%) and 17 (22.97%) respectively. Further, 43 of 48 residents (89.58%) who were over the third year of their residency program indicated that they were unable to perform at least one procedure listed in the questionnaire after finishing their education. Conclusion: This study is important because it is the first study about the opinion of ORL residents and will help determine the current status in Turkey. Objective: The aim of the study was to analyze the 100 most cited publications with Turkish origin in the Web of Science Otorhinolaryngology (ORL) journals. Methods: The Web of Science database was searched in terms of citations for publications originating from Turkey in ORL journals since 1983. After the identification of the 100 most cited articles, analysis was performed for the first author, institution, city, publication type, subject related to subspecialty, and journals having the most cited articles. Moreover, the number of ORL publications and citations of countries was determined in descending order using the same database. Results: A total of 3948 ORL articles with Turkish origin was identified. The number of citations was 181 for the first and 28 for the last in the 100 most cited articles. As there was more than one article with 28 citations, 101 articles were analyzed. The number of the articles was 76, 22, and 3 for the university, education/research, and state hospitals, respectively. Hacettepe University, Ankara Numune Hospital, and Gazi University were the three leading institutions having the most cited articles, and Ankara was the first city. While 98 of 101 articles were original research, the number of case reports and review articles were 2 and 1, respectively. Thirty-five articles were related to otology, 23 to pediatric ORL, 20 to rhinology and head and neck surgery, and 3 to facial plastic surgery. Laryngoscope, Otolaryngology-Head and Neck Surgery, and International Journal of Pediatric Otorhinolaryngology were the leading 3 journals with the most cited articles coming from Turkey. The evaluation of countries revealed that Turkey was among the first 10 countries in terms of number of ORL articles but fell behind for the number of citations. Conclusion: This bibliometric study is the first one regarding the contribution of Turkish authors and institutions to ORL literature. In this study, we evaluated the cochlea, medial olivocochlear system, and brainstem function in autistic children using evoked otoacoustic emissions (OAEs) and a noninvasive contralateral suppression (CLS) test. In total, we included 21 autistic children with normal hearing (study group) and 11 healthy children (control group). Transient-evoked OAEs (TEOAEs) and CLS of TEOAE were evaluated in the left and right ears of all patients. In a silent room, spontaneous, transient, and dP ILO292 were evaluated. The mean age of the study and control group was 9.1 years (range: 6-13 and 6-12 years, respectively). For the study group, there was no statistically significant difference between the OAE and CLS values of the right ear ( P > .05). However, for the left ears, OAE values were statistically significantly higher than the CLS values ( P < .05). In the control group, the OAE values of both ears were statistically significantly higher than the CLS values ( P < .05). In autistic children with normal hearing, the medial olivocochlear system functions more effectively in the right ear than the left ear. Asymmetry between the ears is likely responsible for the peripheral auditory lateralization and independence in auditory function between the left and right ears.

Seröz otitis mediada medikal tedaviye cevap vermeyen durumlarda ventilasyon tüpü uygulanmaktadır. Böylece orta kulak ventilasyonu sağlanmaktadır. Orta kulak ventilasyonun 2-3 hafta yeterli olduğu düşünüldüğünde ventilasyon tüpü uygulamasının getirdiği riskler ve komplikasyonlar nedeniyle alternatif yöntemler araştırılmaktadır. Bu alternatif yöntemler arasında termal miringotomiler, lokal Mitomisin C ve 5 Floururasil uygulamaları sayılabilir. Bu çalışmada, daha önceki çalışmlarda perforasyon süresini arttırdığı tespit edilmiş olan Mitomisin C ile bu konuda hakkında çok az çalışma yapılmış olan Kolşisin karşılaştırılmış ve Kolşisin'in Mitomisin C' nin alternatifi olup olmadığı incelenmiştir.Çalışmamızda 30 adet Wistar cinsi sıçan kullanıldı. Hayvanlar 10'ar adet 3 gruba ayrıldı. Bir grubun sağ ve sol kulaklarına Mitomisin C, diğer gruba Kolşisin lokal olarak perforasyona uygulandı. 3. grup ise kontrol grubu olarak ayrıldı ve serum fizyolojik sürüldü. Hayvanlar haftalık olarak mikroskop altında kontrol edildi ve perforasyonların kapanma süreleri Kaplan-Meier Survival istatistik tekniği kullanılarak karşılaştırıldı.Perforasyonların kapanma süreleri Mitomisin C grubunda 4,9 hafta, Kolşisin grubunda 3,1 hafta, kontrol grubunda 1,1 hafta olarak tespit edildi. Mitomisin C ve Kolşisin grupları kontrol grubuyla karşılaştırıldığında istatistiksel olarak perforasyon kapanmasını anlamlı derecede uzattığı tespit edildi. Bu iki grup arasında Mitomisin C'nin Kolşisin'e oranla istatistiksel olarak anlamlı deredece daha çok uzattığı tespit edildi. Mitomisin C grubuna ait kulaklardan birinde perforasyon kapanmadı.Mitomisin'in perforasyon kapanma sürelerine olan etkisi Kolşisin'e oranla daha çok etkili olmasına rağmen Kolşisin'in de etkili ve yeterli olduğu saptandı. Böylece ventilasyon tüpüne alternatif daha ucuz, daha güvenilir bir madde olabileceği görüldü.Anahtar Sözcükler: Miringotomi, Mitomisin C, Kolşisin Myringotomy and ventilation tube insertion is most effective surgical treatment modality of serous otitis media that is unresponsive to medical treatment. So middle ear ventilation provided. 2 or 3 weeks of continuous aeration of middle ear is thought to be suffcient in treating serous otitis media. However, high incidence of complications of ventilation tube insertion is the reason fort he searching of alternative surgical procedures. Recent studies have showed that application of topical Mitomycin C or 5- Flourouracil to myringotomies can lengthen the closure time of perforations and termal myringotomies. The aim of the present study was to compare the effects of topically applied Mitomycin C and Cholchisine on potency time of perforations and investigate whether this technique can be an alternative precedure to ventilation tube insertion by the information gathered from this study.This study was performed on 30 healty Wistar rats. Myringotomies were performed under general anesthesia. Mitomycin C pledgets were applied to the right and left ears of 10 rats and Cholchicine were applied to the right and left ears of 10 rats. 10 rats were selected as a control group. Animal were monitonied using otomicroscopy weekly and Kaplan - Meier survival tecnique were used to compare patency time between groups.The mean patency time was 4,9 weeks in Mitomycin C, 3,1 weeks in Cholchicine group and 1,1 weeks in control group. The test showed the study groups to have significantly longer patency then the control group. Allthrough the patency rate of Mitomycin C group was longer than Cholchicine group, the difference between these groups was statistically significant. One permanent perforation occured in mitomycin group.Mitomycin C and Cholchicine application to myringotomies prolongs myringotomy patency time and be proposed as an alternative to the ventilation tube insetion technique especially for indications that need short ventilation time .Keywords: Myringotomy, Mitomycin C, Cholchicine 49

Bu konuda bir model oluşturan Hacettepe Üniversitesi Tıp Fakültesi Kulak-Burun-Boğaz Baş Boyun Cerrahisi Kliniği yeni gelişmeler, bilimsel araştırmalar ve yurt dışı temaslar ışığında kendi uygulamalarında da bazı değişikliklere gitmiştir. Anatomik, fizyolojik ve sosyal ozellikleri nedeni ile yuzy~l~a~k~n bir sureden beri hekimlerin ugra~ kaynaklar~ndan biriniolu~turan RLS paralizilerine ili~kin bir cozum aray~~~ ileduzenlenen bu cal~emada, RLS kesildikten sonra distal ucu vagustakibir centige dikilmietir. Kontrol grubunu olueturan dorts~can d~~~nda 17 s~canda da anastomoz olueturulmu~tur. Histopatolojikincelemelerin sonucunda ise anastomoz alan~nda siniraleyhine onemli geli~imler gozlenmekle birlikte, posteriorcricoarytenoid kaslnda innervasyon bulgular~n~n saptanm~e olmas~ve bu sonuclar~n istatistik korrelasyonlar~ bu yontemin arzulanansonucu verdigini gostermektedir. 61
