Konversiyon bozukluğu (KB) herhangi bir organik bir sebep olmadan bireyin motor ve/veya duyu fonksiyonlarında kayıp ile karakterizedir. Burada sol alt ekstremitede kuvvetsizlik, yürümede zorluk ve parestezi şikayetleri olan 39 yaşında bir erkek hasta sunulmuştur. Nörolojik bulgularını açıklayacak olası hastalıklar dışlandıktan sonra KB tanısı konmuştur. Sitalopram 20 mg/gün dozunda başlanmış ve psikiyatri kliniğinde takibe alınmıştır. Üç ay sonraki takibinde, hastanın şikayetleri düzelmiştir. Bu yazıda, tanıda ayrıntılı bir fizik ve nörolojik muayene ile laboratuar değerlendirmenin yapılması gerektiği vurgulanmıştır. Conversion disorder (CD) is characterized by deficit in motor and/or sensory functions of an individual without any organic reason. Herein, a 39 years old male patient who had symptoms of weakness in left lower extremity, difficulty in walking and parestesia is presented. CD is diagnosed after the exclusion of the possible disorders that explain the neurologic signs. Twenty mg/day citalopram is started and taken to follow up in psychiatry clinic. After 3 months follow-up, complaints of patients is improved. In this paper, the necessity of a detailed physical and neurologic examination with laboratory evaluation during diagnose is emphasized.

11. Omuz Ağrılı Hastalarda Farklı Analjezik Akımların Etkinliğinin Karşılaştırılması

Amaç: Bu çalışmanın amacı; ağrılı omuz şikayeti ile başvuran hastalarda, Transkutan Elektrik Sinir Stimülasyonu (TENS) ve interferansiyel akımın etkinliklerinin karşılaştırılmasıdır.Gereç ve Yöntem: Çalışmaya omuz ağrısı olan 44 hasta dahil edildi. Hastalar rastgele iki gruba ayrıldı. Birinci gruba (n=22) hotpack, ultrason, TENS, codman ve germe egzersizleri uygulanırken, ikinci gruba (n=22) hotpack, ultrason, interferansiyel akım, codman ve germe egzersizleri 15 seans olarak uygulandı. Hastaların tedavi öncesi ve sonrası ağrı düzeyleri görsel ağrı skala (GAS, 0-10) ve likert ağrı skalası ile değerlendirildi. Omuz hareket genişliği gonyometre ile ölçüldü. Bulgular: Çalışmaya katılan 10'u erkek, 34'ü kadın toplam 44 omuz ağrısı olan hasta randomize olarak iki gruba ayrıldı. Hastaların tedavi bitiminde TENS tedavisi alan grup ve interferans tedavisi alan grup kendi içinde ağrı değerleri ve eklem hareket açıklığı açısından istatiksel olarak anlamlı düzelme saptandı (p=0.000). Ancak gruplar arası karşılaştırmalarda tedavi ile GAS, Likert ağrı düzeyleri ve eklem hareket açıklığındaki değişim açısından istatiksel olarak anlamlı fark yoktu (p>0.05). Sonuçlar: Omuz ağrısı olan durumlarda yüzeyel ve derin ısıtıcı ile kombine edilen fizik tedavi modalitelerinden TENS ve Interferansın her ikisi de benzer olarak ağrı şiddetini azaltma ve eklem hareket açıklığını arttırmada etkilidir.Anahtar Kelimeler: Omuz ağrısı, TENS, İnterferansiyal akımAstractComparison of Efficacy of Different Analgesic Currents in Patients with Shoulder PainObjective: The aim of this study is to compare the effectiveness of Transcutaneous Electrical Nerve Stimulation (TENS) and interferential current in patients with shoulder pain.Material-Methods: Fourty four patients with shoulder pain were included in the study. The patients were randomly assigned to one of the two groups. In first group; hotpack, ultrasound, TENS, codman and streching exercises and in second group; hotpack, ultrasound, interferential current, codman and streching exercises were applied for 15 sessions. The pain severity was evaluated with visual analog scale (VAS, 0-10cm) and likert scale before and after the treatment. The range of motion of shoulder was assessed by goniometry.Results: Fourty four patients, 10 male, 34 female, with shoulder pain were randomly assigned in two groups. After treatment, there was statistically significant improvement in pain severity and range of motion in both groups (p=0.000). But there were no significant differences between two groups in pain severity and range of motion (p>0.05) after treatment. Conclusion: Both of TENS and interferential current therapy combined with superficial and deep heating methods are found to be similarly effective in decreasing pain severity and increasing range of motion of shoulders in patients with shoulder pain. ÖZET Kronik Bel Ağrısında Üç Farklı Tedavi Yönteminin Karşılaştırılması Kronik bel ağrısı en sık özürlülük yapan nedenlerden biridir ve psikolojik durum üzerine olumsuz etkileri vardır. Kronik bel ağrılı hastalar için çeşitli tedavi yöntemleri önerilmektedir ve tedavilerin etkinlikleri ile ilgili çelişkili sonuçlar rapor edilmektedir. Bu çalışmada kronik bel ağrısında üç farklı tedavi yönteminin ağrı, spinal mobilite, özürlülük, psikolojik durum ve aerobik kapasite üzerine etkileri araştırıldı. Kronik bel ağrısı olan 60 hasta 3 gruba randomize edildi: I. aerobik egzersiz + ev egzersiz programı, II. fizik tedavi (hot pack, ultrason, TENS) + ev egzersiz programı, III. sadece ev egzersiz programı. Hastalar spinal mobilite ölçümleri, ağrı şiddeti, hastaların genel değerlendirimi, özürlülük ve psikolojik durum açısından tedavi öncesi, tedavi soması ve tedaviden 1 ay somasında değerlendirildi. Aerobik kapasite ölçümleri ise tedavi öncesi ve tedavi soması treadmilde Bruce protokolü ile yapılan ergospirometrik egzersiz testi ile ölçüldü. Tüm gruplar tedavi soması ve tedaviden 1 ay somasında ağrı şiddetinde benzer azalma gösterdiler ve gruplar arasında anlamlı farklılık yoktu. II. grupta (fizik tedavi + ev egzersiz programı) tedavi somasında Beck Depresyon skorlarında anlamlı azalma oldu. Tedaviden 1 ay somasında I. (aerobik egzersiz + ev egzersiz programı) ve II. grupta Genel Sağlık Anketi puanlarında anlamlı azalma oldu. II. grupta ayrıca Roland Morris Özürlülük puanlarında azalma oldu. Üç grupta da aerobik kapasite ölçümlerinde artış gözlendi. Sonuç olarak, kronik bel ağrısında üç tedavi edici uygulama ağrı azaltılmasında ve aerobik kapasitenin arttırılmasında etkili olarak bulundu. Ancak fizik tedavi + ev egzersiz programı özürlülük ve psikolojik durumu iyileştirmede daha etkilidir. Anahtar Sözcükler: Aerobik egzersiz, ev egzersizi, fizik tedavi, kronik bel ağrısı, özürlülük. SUMMARY Comparison Of Three Different Approaches In The Treatment Of Chronic Low Back pain Chronic low back pain is one of the most common disorder leading to disability and has negative effects on physchological state. Various therapeutic approaches have been proposed for patients with chronic low back pain and contradictory results have been reported about the effectivity of the treatments. The effects of three therapeutic approaches in chronic low back pain in terms of pain, spinal mobility, disability, physchological state and aerobic capacity were investigated in this study. 60 patients with chronic low back pain were randomised to three groups: I. aerobic exercise + home exercise programme, II. physiotherapy (hot pack, ultrasound, TENS) + home exercise programme, III. only home exercise programme. Patients were assessed with spinal mobility measurements, pain severity, patients global assessment, disability and physchological status pre-posttreatrment and 1 month follow-up. Aerobic capacity meaurements were performed with ergospyrometric exercise test using Bruce protocol on treadmill pre-posttreatment. All the groups showed similar decrease in pain in posttreatment and 1 month follow-up and there was no significant difference between the groups. In the second group (physiotherapy + home exercise programme) showed a significant decrease in Beck Depression Inventory scores on posttreatment. At 1 month follow-up group one (aerobic exercise + home exercise programme) and group two showed significant decrease in General Health Assessment Questionnaire scores. The second group also showed significant improvement in Roland Morris Disability scores. There were similar improvements in aerobic capacity measurements in all of the three groups. In conclusion, all of the three therapeutic approaches were found to be effective in diminishing pain and increasing aerobic capacity in patients with chronic low back pain. But physiotherapy + home exercise programme was found to be more effective in improving disability and physchological status. Amaç: Bu çalışmada betahistin dihidroklorid, transkütaneal elektriksel sinir stimülasyonu ve saf ses maskeleme-tinnitus eğitim terapisi TET yöntemleri arasında hastaların yaşam kalitesi ve septomların tedavisi üzerindeki etkiler açısından fark olup olmadığı belirlendi.Hastalar ve Yöntemler: Haziran 2009 - Haziran 2010 tarihleri arasında Ufuk Üniversitesi Kulak Burun Boğaz Kliniğine subjektif tinnitus yakınması ile başvuran ve işitme kaybı olmayan toplam 91 hasta 42 kadın, 49 erkek; ort. yaş 49.3±8.3 yıl; dağılım 30-70 yıl çalışmaya dahil edildi. Bu çalışmada iki taraflı subjektif tinnitus yakınması olan hastalarda bu üç tedavi yönteminin iyileşme ve yaşam kalitesi üzerine etkisi Tinnitus Değerlendirme Anketi Skoru THIS , görsel analog ölçek GAÖ ve odyolojik parametreler kullanılarak karşılaştırılmalı olarak değerlendirildi. Değerlendirmeler tedaviden hemen önce, tedaviden hemen sonra ve tedaviden üç hafta sonra gerçekleştirildi. Verilerin normal dağılımını test etmek için Kolmogorov-Smirnov analizi, değişik tedavi yöntemleri arasındaki tedavi öncesi, tedaviden hemen sonra ve tedaviden üç hafta sonraki farkı göstermek için Wilcoxon signed rank testi kullanıldı. Gruplar arası farklılıkları göstermek için Mann-Whitney U ve Kruskal-Wallis H testi kullanıldı.Bulgular: Gruplar arası analizlerde saf ses maskeleme-TET yönteminin başarı oranı diğer yöntemlerle karşılaştırıldığında çok daha yüksek idi. Üç aylık periyodun sonunda yapılan değerlendirmelerde de tedavi etkinliğinin devam ettiği görüldü.Sonuç: Bu sonuçlara göre saf ses maskeleme-TET’nin diğer yöntemlerle karşılaştırıldığında en iyi tedavi yöntemi olduğu bulundu ve bu tedavinin idiyopatik tinnituslu hastalarda ilk tedavi seçeneği olarak değerlendirilebileceği sonucuna varıldı, Objectives: This study aims to detect whether any differences were present between betahistine dihydrochloride, transcutaneal electrical nerve stimulation and pure tone masking-tinnitus retraining therapy TRT methods in the effects on quality of life and treatment of the symptoms of the patients. Patients and Methods: A total of 91 patients 42 females, 49 males; mean age 49.3±8.3 years; range 30 to 70 years who admitted to the Otorhinolaryngology Clinic of the Ufuk University between June 2009 and June 2010 with a complaint of subjective tinnitus and who had no hearing loss were included in the study. In this study, the effects of these three treatment methods on healing and quality of life in patients suffering from bilateral subjective tinnitus were comparatively evaluated using Tinnitus Handicap Inventory Score THIS , visual analog scale VAS and audiological parameters. The evaluations were made immediately before the treatment, immediately after the treatment and three weeks after the treatment. KolmogorovSmirnov analysis was used to test the normal distribution of the data and Wilcoxon signed rank test was used to show the differences between the different treatment methods before the treatment, immediately after the treatment and three weeks after the treatment. Mann-Whitney U and Kruskal-Wallis H tests were used to show the inter-group differences. Results: In the inter-group analyzes, success rate of the pure tone masking-TRT was much higher when compared to the other treatment methods. In the evaluations performed at the end of the three-month period, it was seen that the efficacy of the treatment was continuing. Conclusion: According to these results, pure tone maskingTRT was found to be the best treatment method when compared to other methods and it was concluded that this treatment may be considered as the first choice in patients with idiopathic tinnitus.

Stroke is the second most common cause of mortality and disability worldwide. Most of the patients cannot regain their walking ability after a stroke. Impaired gait and mobility negatively affect the activities of daily living and quality of life of stroke survivors. Restoring gait and mobility are the most important targets of the rehabilitation approaches. Advances in computers and engineering have enabled robotics to be used in many areas of rehabilitation medicine. One of them is gait training. High-intensity, repetitive task training is crucial for neural plasticity and motor learning. Robot-assisted gait training may be a promising method leading to functional recovery in patients with stroke. 19. Why do manuscripts submitted to the Turkish Journal of Physical Medicine and Rehabilitation get rejected?

- Abstract
Objectives: The study aimed to examine the reasons for the rejection of manuscripts, considering the increased rejection rates of our journal of up to 73% in 2022, and help authors realize what the editors and referees are paying attention to while assessing the manuscript., Materials and Methods: In this retrospective study, original articles, case reports, systematic reviews, and meta-analyses submitted and rejected to the Turkish Journal of Physical Medicine Rehabilitation were searched between January 1, 2016, and June 30, 2022. After reviewing the referee's evaluations and editorial opinions for all rejected articles, the reasons for rejection were classified under three main headings: journal, manuscript, and ethical issues. The manuscript issues were detailed under 11 subheadings., Results: A total of 1,293 rejected submissions were reviewed. Of these, 35% were rejected at the editorial stage, while 65% were rejected after peer review. Thirty-three submissions were rejected for ethical reasons, 168 were out of the journal's field of interest, and 1,092 (84%) submissions were rejected for reasons related to the manuscript. 20. Applying the WHO ICF framework to long COVID patients with persistent respiratory symptoms.

- Abstract
Objectives: The aim of this study was to evaluate long COVID patients with persistent respiratory symptoms through the application of the World Health Organization (WHO) International Classification of Functioning, Disability and Health (ICF) framework., Patients and Methods: This national, prospective, multicenter, cross-sectional study was conducted with 213 patients (118 females, 95 males; median age 56 years; range, 20 to 85 years) with long COVID between February 2022 and November 2022. The ICF data were primarily collected through patient interviews and from the acute medical management records, physical examination findings, rehabilitation outcomes, and laboratory test results. Each parameter was linked to the Component Body Functions (CBF), the Component Body Structures (CBS), the Component Activities and Participation (CAP), the Component Environmental Factors (CEF), and Personal Factors according to the ICF linking rules. Analysis was made of the frequency of the problems encountered at each level of ICF category and by what percentage of the patient sample., Results: In the ICF, 21 categories for CBF, 1 category for CBS, and 18 categories of CAP were reported as a significant problem in a Turkish population of long COVID patients with persistent respiratory symptoms. Furthermore, eight categories for CEF were described as a facilitator, and four as a barrier., Conclusion: These results can be of guidance and provide insight into the identification of health and health-related conditions of long COVID patients with persistent respiratory symptoms beyond the pathophysiological aspects, organ involvement, and damage of COVID-19. - Abstract
Objectives: This study aimed to investigate the physical and emotional effects of the coronavirus disease 2019 pandemic in patients with fibromyalgia syndrome (FMS) and chronic low back pain (CLBP) patients., Patients and Methods: The cross-sectional controlled study was performed with 1,360 participants (332 males, 1,028 females; mean age: 42.3±12.5 years; range, 18 to 65 years) between September 2020 and February 2021. The participants were evaluated in three groups: the FMS group (n=465), the CLBP group (n=455), and the healthy control group (n=440). Physical activity, pain levels, and general health status before and during the pandemic were evaluated in all participants. Stress levels were analyzed with the perceived stress scale (PSS) in all groups, and disease activity was analyzed with the fibromyalgia impact questionnaire (FIQ) in patients with FMS., Results: Patients with FMS had worsened general health status and pain levels during the pandemic compared to the other groups (p<0.01). The FMS group showed significantly higher PSS scores than those in other groups (p<0.01). There was a weak-positive correlation between FIQ and PSS parameters in patients with FMS (p<0.05, r=0.385)., Conclusion: The general health status, pain, and stress levels of the patients with FMS and CLBP tended to worsen during the pandemic. 22. Restless legs syndrome in children with allergic rhinitis: A comparative study on frequency, severity and sleep quality.

- Abstract
Objectives: This study aims to investigate the frequency and severity of restless legs syndrome (RLS) and its relationship with sleep quality in children with allergic rhinitis (AR) with the hypothesis that comorbid RLS may be an additional causative factor of sleep disturbances in pediatric AR., Patients and Methods: A total of 143 children with AR (aged 8-18 years) and 144 healthy control subjects (aged 8-18 years) were included. The diagnosis of AR was established on history, clinical examination and skin prick test according to the Allergic Rhinitis and its Impact on Asthma guidelines. Presence of RLS was determined using the International RLS study group (IRLSSG) criteria. The severity of RLS was assessed using the IRLSSG rating scale. Sleep quality was evaluated by Pittsburgh Sleep Quality Index (PSQI)., Results: Thirteen patients (9.1%) in AR group, and six children (4.2%) in control group had RLS (p=0.159). The frequency of RLS in AR group was higher than two folds when compared to the control group; however, the difference was not statistically significant. Restless legs syndrome severity score was significantly higher in AR group than control group (15.00 [11-20] and 11.00 [10-16] respectively, p=0.046). Total PSQI scores were similar between groups. Also, no significant differences were observed in total PSQI scores of AR patients with or without RLS., Conclusion: Restless legs syndrome was not more common but was more severe in children with AR.
