Platelet-rich plasma has been used to support fat graft retention, but it may include inflammatory mediators such as interleukin-1β. Autologous conditioned serum also contains high levels of various anti-inflammatory cytokines. The authors hypothesized that combining autologous conditioned serum with fat graft would increase fat graft survival more than platelet-rich plasma. Methods: Twenty-seven adult, male, Sprague-Dawley rats were divided into three groups of nine. Ten nonstudy rats were used to prepare platelet-rich plasma, autologous conditioned serum, and fat grafts. Next, 0.7-ml fat graft with a combination of 0.2 ml of autologous conditioned serum, platelet-rich plasma, or phosphate-buffered saline was applied to their dorsa. Fat graft volume was assessed on postoperative day 2 and on the day of euthanization at 1, 3, and 5 months postoperatively. Histopathologic analysis was performed to measure integrity, inflammation, fibrosis, and vascularization. Results: The median volume percentages and interquartile ranges at 1 month postoperatively were 97.3 percent (77.3 to 119.6 percent), 40.4 percent (30.9 to 46.9 percent), and 72.1 percent (53.6 to 84.9 percent) in autologous conditioned serum plus fat graft, phosphate-buffered saline plus fat graft, and platelet-rich plasma plus fat graft, respectively (p < 0.05); at 3 months postoperatively, values were 82.3 percent (70.3 to 88.3 percent), 36.6 percent (29.4 to 43.1 percent), and 48.3 percent (31.4 to 57.9 percent) (p < 0.001); and at 5 months postoperatively, values had increased to 83.9 percent (58.3 to 102.4 percent), 40.3 percent (20.1 to 50.6 percent), and 56.3 percent (37.7 to 74.9 percent), respectively (p < 0.05). Conclusions: Autologous conditioned serum and platelet-rich plasma improved fat graft outcomes compared to saline, whereas autologous conditioned serum was associated with less inflammation, greater fat viability, and more integrity. Clinical Relevance Statement: Combining fat graft with autologous conditioned serum may be a better option to minimize resorption rate and improve graft survival.

Besides, the main advantage of the technique is the reconstruction of the eyelashes with their exact equivalents and the ability of primary closure of the donor area., Kirpik bölgesi ve göz kapağı marjinal defektleri kendine has yapısı nedeniyle eksikliği durumunda rekonstrüksiyonu oldukça problemli olabilen bir alandır. Kirpiklerin rekonstrüksiyonu, sadece estetik değil aynı zamanda fonksiyonel sonuçlara da katkı sağlar. İki aşamada kirpiklerin rekonstrüksiyonunu amaçlayan switch flap tekniği, oldukça güvenilir vaskülariteye sahip bir doku sağlayabilmektedir. Bunun yanında, tekniğin asıl avantajı kirpiklerin birebir eşdeğerleri ile rekonstrüksiyonunu ve donör alanın primer kapanması avantajlarını beraberinde getirmesidir.

Purpose: Head and neck injuries may end up with physical,functional and psychological problems besides persistent facial deformities.This study aimed to review the management of patients who were admitted to ourdepartment due to animal attack to head and neck regionMaterialand Method: Sixteen patients whowere admitted to Ankara Training and Research Hospital, Plastic, Reconstructiveand Aesthetic Surgery Department between 2006-2018 due to animal attack to headand neck region and managed by surgical intervention were included in thisstudy. Demographic features, type of injuries and type of surgical managementsare reviewed in here. Results: Twenty four lesions of sixteen patients were operated.Mean age of the patients was 18.8 (3-64) and female/male ratio was 2.2/1. Mostof the patients were injured due to dog attack (10/16) while others were due todonkey, bear and horse attack in order of frequency. The most injured part was perioral region(8/24) while scalp (5/24), malar (5/24), periorbital (2/24), nasal (2/24), ear(1/24) and neck (1/24) region were the others. There were fractures in threepatients and fractures were found to be statistically significantly higher inadult patients compared to children (p=0.036). Seventeen of the injuries weremanaged by direct suturation, while six were treated with local flaps and oneby free flap. Conclusion: Current management strategy in animal attacks to headand neck region consists of early management, irrigation and debridement of thewound followed by primary repair of the lesion, as prefered in the currentstudy. The patients attacked especially by large animals should be evaluatedfor the presence of fractures. Primary repair, rational use of antibiotics,tetanus and rabies prophylaxis, all together seem to be effective with theleast complication rates in this group of patients., Amaç: Baş ve boyun bölgesinin yaralanmaları, fiziki, işlevselve psikolojik problemlere ve kalıcı yüz deformitelerine neden olabilir. Buyazıda, farklı hayvan saldırılarına bağlı baş ve boyun bölgesinde defekt nedeniile kliniğimize başvuran hastaların değerlendirilmesi ve deneyimlerinpaylaşması amaçlanmıştır. Gereç veYöntem: Bu çalışmaya, Ankara Eğitim ve AraştırmaHastanesi, Plastik Rekonstrüktif ve Estetik Cerrahi Kliniği’ ne 2006-2018tarihleri arasında hayvan saldırısına bağlı baş ve boyun bölgesi yaralanmasınedeniyle başvuran ve rekonstrüksiyon için cerrahi müdahale ihtiyacı olan 16hasta dahil edilmiştir. Hastaların demografik özellikleri, yaralanma biçimlerive uygulanan cerrahi tedaviler değerlendirilmiştir. Bulgular:Çalışmaya dahil edilen 16 hastada 24 bölgeonarımı yapıldı. Hastaların yaş ortalaması 18,8 (3-64 yaş) ve Kadın / Erkekoranı 2,2/1 idi. Kliniğimize en sık köpek saldırısına bağlı hastalarbaşvururken (10/16), eşek, ayı ve at saldırısına maruz kalmış hastalar azalansıklıkla bu grubu izlemekteydi. En fazlaonarım yapılan bölge perioral (8/24), sonrasında azalan sıklık oranlarına göre skalp (5/24), malar (5/24), periorbital (2/24),burun (2/24), kulak (1/24) ve boyun (1/24) bölgesi idi. Hastaların 3 tanesindefraktür izlenmiş ve erişkin hastalarda çocuk hastalara göre daha fazla fraktürolduğu görülmüştür (p=0.036). Tüm yaralanmaların 17 si direkt sütürasyonlayapılırken, 6 sı lokal fleplerle ve 1 tanesi de serbest flep ile onarıldı.Sonuç: Hayvan saldırılarına bağlı baş boyun bölgesiyaralanmalarında güncel yaklaşım, bu çalışmada da tercih edildiği gibi erkenmüdahale edilmesi ve irrigasyon ve debridmanını takiben primer onarım yönündedir. Büyük hayvanyaralanmalarında hastalar fraktür açısından değerlendirilmelidir. Komplikasyonoranları da dikkate alındığında, primer kapama, akılcı antibiyotik kullanımı,kuduz ve tetanoz profilaksisi bu grup hastada etkin gözükmektedir.

Maksillofasyal travmalar ve sonrasında oluşan orbital yabancı cisimler problemli yaralanmalardandır. Bu tarz yaralanmalar kolaylıkla atlanabilir ve ciddi komplikasyonlara neden olabilmektedir. Bu vaka takdiminde, 20 yıl önce oluşmuş maksillofasyal travmaya sekonder intraorbital yabancı cisim (cam) nedeniyle opere edilen bir hastanın sunulması amaçlandı.
